Living Experiences of Indian Adult Cancer Survivors - a Brief Report

  • Background: This pilot study looked into the physical, social, psychological and economic issues of Indian adult cancer survivors. Materials and Methods: Assumed cancer free patients, after cancer directed therapy (CDT), were assessed on the basis of a questionnaire developed by the investigators. The mental status of the survivors was elicited by modified MINI international neuro-psychiatric interview. This cross sectional assessment was conducted as a direct interaction with each patient for 30 to 45 minutes at a cancer hospital in 2012. Results: Thirty one adult cancer survivors participated in this study. Median age was 53 years with a median follow up duration of 21.8 months (Range 2.3-194.1 months). The majority (68%) did not receive financial support for treatment. Median interval after CDT to start of activity of daily living was 1.5 months (range: 0-24 months). Fatigue and loss of appetite were reported by 52% and 29% respectively. The cancer diagnosis and its treatment adversely impacted the financial condition in 42% of patients. Nineteen percent each showed social anxiety and post-traumatic stress disorder and another 13% patients reported depression. Conclusions: This prospective assessment highlights survivorship issues and the need to address those issues particularly in the context of developing countries where resources and manpower are limited.

Analyzing the Effects of MEA Designs on Cold Start Behaviors of Automotive Polymer Electrolyte Fuel Cell Stacks (자동차용 고분자전해질형연료전지 스택에서의 막-전극접합체 설계인자가 저온시동에 미치는 영향성 연구)

  • This paper presents a three-dimensional, transient cold-start polymer electrolyte fuel cell (PEFC) model to numerically evaluate the effects of membrane electrode assembly (MEA) design and cell location in a PEFC stack on PEFC cold start behaviors. The cold-start simulations show that the end cell experiences significant heat loss to the sub-freezing ambient and thus finally cold-start failure due to considerable ice filling in the cathode catalyst layer. On the other hand, the middle cells in the stack successfully start from $-30^{\circ}C$ sub-freezing temperature due to rapid cell temperature rise owing to the efficient use of waste heat generated during the cold-start. In addition, the simulation results clearly indicate that the cathode catalyst layer (CL) composition and thickness have an substantial influence on PEFC cold-start behaviors while membrane thickness has limited effect mainly due to inefficient water absorption and transport capability at subzero temperatures.

Numerical Analysis of a Turbine Rotor Cascade with Unsteady Passing Wakes (비정상 후류를 지나는 터빈 동익 주위의 유동장 수치해석)

  • A turbine stage consists of a stator and rotor. A stator provides the required inlet flow conditions so that a rotor can produce the necessary power. Passing wakes generated at the trailing edge of a stator make an interaction with a rotor. In the present study, this interaction flow mechanism is investigated using the numerical analysis. In case of the large gap distance between the stator and rotor, the stator and rotor flow analysis can be separated. First, only the stator flow field is solved. Second, the rotor flow field is solved including the passing wake information from the stator analysis. The passing wake experiences the shearing as it approaches to the rotor leading edge. And it is chopped when it strikes the rotor body. After that, the chopped wakes becomes the prolongation as it goes downstream. Also, the aerodynamic characteristics with the variation of the gap distance between a stator and rotor was investigated. Pressure jumps due to the passing wakes result in the pressure and lift loss and it gets stronger with the closer gap distance. This unsteady effect proves to be directly related to the fatigue and noise in turbomachinery and this study would be helpful to investigate such fields.

Time-Efficient Event Processing Using Provisioning-to-Signaling Method in Data Transport Systems Requiring Multiple Processors

  • In connection-oriented data transport services, data loss can occur when a service experiences a problem in its end-to-end path. To resolve the problem promptly, the data transport systems providing the service must quickly modify their internal configurations, which are distributed among different locations within each system. The configurations are modified through a series of problem (event) handling procedures, which are carried out by multiple control processors in the system. This paper proposes a provisioning-to-signaling method for inter-control-processor messaging to improve the time efficiency of event processing. This method simplifies the sharing of the runtime event, and minimizes the time variability caused by the amount of event data, which results in a decrease in the latency time and an increase in the time determinacy when processing global events. The proposed method was tested for an event that required 4,000 internal path changes, and was found to lessen the latency time of global event processing by about 50% compared with the time required for general methods to do the same; in addition, it reduced the impact of the event data on the event processing time to about 30%.

  • Excessive generation of BOG during the LNG transportation not only causes the severe financial loss but also leads to the unexpected disaster. Therefore, the carrier cargo insulating interior LNG should be carefully designed based upon an accurate heat transfer analysis. However. it is not simple to analyze heat transfer of LNG cargo, because it is in a complex insulation structure and LNG carrier experiences a complicated heat transfer according to various kinds of voyage conditions. In this paper, we carried out the transient finite element heat transfer analysis for a cargo of Mark-111 membrane-type LNG carrier during laden voyage, and we compared heat transfer rates between considering natural convection and considering conduction. For this goal, we developed a PCL program incorporating with a commercial MSC/NASTRAN FEM code.

  • The advent of free bone flaps has made successful replacement of extensive areas of bone loss in the upper and lower extremities. The microvascular free bone flaps have faster healing without bony absorption or atrophy and can heal in the hostile environment of scarred bed or infection. Since the fibula free flap introduced by Taylor and colleague in 1975, it has been used extensively for skeletal reconstruction of extremities. In 1988, the folded vascularized fibula free flap was first described as a technique to reconstruct significant long bone defect of upper and lower extremities. During the same time, the fibular free flap has evolved to become most preferred choice of mandibular reconstruction. Up to present day, few reports have been made on the fibular free flap used for reconstruction of injured hand containing metacarpal bone and soft tissue defect. We present here our new and unique experiences with vascularized fibular osteocutaneous free flap as useful and satisfactory one for reconstruction of hand with composite defects.

  • The gag reflex is a physiologic reaction which safeguards the airway from foreign bodies. But, an exaggerated gag reflex can be a severe limitation to a patient's ability to accept dental care and for a clinician's ability to provide it. The overactive gag reflex can be due to psychological factors or physiological factors, or both. Psychological factors can include fear of loss of control and past traumatic experiences. A 58-year-old man, scheduled for extraction of left upper second molar, left lower second and third molar and implantation of left upper second molar, and left lower second molar had no specific underlying medical problems. He had exaggerated gag reflex. Dental treatment was successfully performed using intravenous sedation. Intravenous sedation with midazolam and propofol was a useful management technique for reflex control during dental treatment extended to the posterior regions in the oral cavity.

  • There is a need to define the concept of suffering more appropriate in the context of Korean culture. This research is an attempt to reanalyze the concept of suffering by applying Walker & Avant's process of concept analysis. The steps of concept analysis are : 1. Select a concept. 2. Determine the aims or purpose of the analysis. 3. Identify all uses of the concept that you can discover. 4. Determine the defining attributes. 5. Construct model, contrary, borderline, related and invented cases. 6. Identify antecedents and consequences. 7. Define empirical referents. The concept of suffering can be defined as follows: Suffering is a fundamental and inevitable experience of all human beings. When each individual experiences loss, damage and pain which threaten one's personal integrity, suffering is perceived differently among each individual depending on their personal inner factors, one's significant others, exterior circumstances and stimuli, and the ultimate meaning of life. Suffering brings severe and unendurable distress which accompany despair, powerlessness, anxiety, bitterness, fear, anguish, guilt, depression, withdrawal and anger. The attributes of suffering were analyzed. Model case, contrary case, borderline case, related case, and invented case were described. Antecedents, consequences and empirical referents were explored. This concept analysis provides data for new insights of suffering. These findings suggests that appropriate nursing interventions aimed at relieving suffering of patients need to be developed and implemented more widely.

  • A numerical analysis of axisymetric backward facing step main nozzle flow in air jet loom has been accomplished. To obtain basic design data for an optimum main nozzle for an air-jet loom and to predict the transonic/supersonic flow, a characteristic based upwind flux difference splitting compressible Navier-Stokes method has been used. The wall static pressure of the main nozzle and the flow velocity changes in the nozzle tube were analyzed by changing air tank pressures and acceleration tube lengths. The flow inside the nozzle experiences double choking one at the needle tip and the other at the acceleration tube exit at tank pressures over $4kg_f/cm^2$. The tank pressure $P_t$ leading to the critical condition depends on the acceleration tube length; i.e, $P_t$ is higher for longer acceleration tubes. The $P_t$ value required to bring the acceleration tube exit to the critical condition is nearly constant regardless of acceleration tube length. The round needle tip shape might lead to less total pressure loss when compared with step shape.

Plus-size Women and Appearance Management with a Focus on Clothing -Grounded Theory Based Exploratory Study- (근거이론에 기초한 플러스 사이즈 여성 소비자의 의류를 중심으로 한 외모관리에 관한 탐색적 연구)

  • This study explores various issues of appearance management behavior for plus-size women in Korea with a focus on clothing. In-depth interviews and focused group interviews were conducted with 24 plus-size women. The interviews were recorded and the transcripts were analyzed based on grounded theory. Discomfort was the main phenomenon involving the experience of plus-size women related to appearance management. Psychological as well as physiological/physical discomfort, unmet needs (regarding clothing) and inconvenient shopping experiences were frequently mentioned. Causal conditions for discomfort were obesity, social stigma, and an underdeveloped clothing market for plus-size consumers. Interviewees developed strategies to cope with discomfort (suppressing clothing need, loss of interest in clothing, diversion from clothing needs, sole focus on physical comfort, dress-up and increase in shopping channels, and change in shopping patterns) that depended on contextual conditions (such as duration of obesity and attitudes of people) close to the interviewees. The discomfort of interviewees decreased or continued depending on if they became ambivalent about their obese condition, lost weight, or utilized plus-size specialty stores.
