쪼그려 들기와 허리 굽혀 들기 시 하지관절 움직임과 요추 전만에 관한 생체역학적 분석 (Biomechanical Analysis of Lower Limb Joint Motions and Lumbar Lordosis during Squat and Stoop Lifting)

  • In this study, lower extremity joint kinematics and kinetics and lumbar lordosis were investigated for two different symmetrical lifting techniques(squat and stoop) using the three-dimensional motion analysis. Twenty-six male volunteers lifted boxes weighing 5, 10 and 15kg by both squat and stoop lifting techniques. There were not significant differences in maximum lumbar joint moments between the two techniques. The hip and ankle contributed the most part of the support moments during squat lifting, and the knee flexion moment played an important role in stoop lifting. The hip, ankle and lumbar joints generated power and only the khee joint absorbed power in the squat lifting. The knee and ankle joints absorbed power, the hip and lumbar joints generated power in the stoop lifting. The bi-articular antagonist muscles' co-contraction around the knee joint during the squat lifting and the eccentric co-contraction of the gastrocnemius and semitendinosus were found to be important for straightening up during the stoop lifting. At the time of lordotic curvature appearance in the squat lifting, there were significant correlations in all three lower extremity joint moments with the lumbar joint. Differently, only the hip moment had significant correlation with the lumbar joint in the stoop lifting. In conclusion, the knee extension which is prominent kinematics during the squat tilling was produced by the contributions of the kinetic factors from the hip and ankle joints(extensor moment and power generation) and the lumbar extension which is prominent kinematics during the stoop lifting could be produced by the contributions of the knee joint kinetic factors(flexor moment, power absorption, bi-articular muscle function).

상지의 움직임과 저항 적용에 따른 Formetric 4D를 이용한 척추의 3차원적 분석 (Three-dimensional Analysis of the Spine using Formetric 4D according to Upper Limb Movement and Resistance Application)

  • PURPOSE: The aim of this study was to measure changes in spine inclination and thoracolumbar structure and morphology according to upper-extremity movements with and without resistance in order to evaluate the spine stability in workers. METHODS: Forty-eight middle-aged male workers (mean age, 40.48 ± 6.27 years) participated in this study. Using the spine analysis system, changes in the inclination of the spine and structure as well as shape of the thoracolumbar spine were measured. For posture measurement, the postures of standing, lifting the right and left arms (shoulder joint 90° flexion), and lifting with both arms were measured in random order. In addition, variables were measured using a resistance of 3 kg for each posture. The statistical significance level was set at α = .05 for all variables. RESULTS: There were statistically significant differences between the front and back inclinations of the spine, kyphotic curve of the thoracic spine, lordotic curve of the lumbar spine, rotation changes in the thoracolumbar spine, and rotation changes in the T4 vertebra (p < .05). However, there was no significant difference in the left and right tilts of the spine. In the post-hoc analysis, rotation changes in the T4 vertebra showed a significant difference in posture when resistance was applied to the left and right sides CONCLUSION: Causes of musculoskeletal diseases include excessive thoracic spine rotation, torsion, and hyperlordosis of the lumbar spine. Therefore, it is necessary to improve the working environment in order to ensure a healthy posture and prevent musculoskeletal diseases that can reduce the ability to carry various and/or excessive loads.

Midline-Splitting Open Door Laminoplasty Using Hydroxyapatite Spacers : Comparison between Two Different Shaped Spacers

  • Objective : Although hydroxyapatite (HA) spacer has been used for laminoplasty, there have been no reports on factors associated with fusion and on the effects of HA shape. Methods : During January 2004 and January 2010, 45 patients with compressive cervical myelopathy underwent midline-splitting open door laminoplasty with winged (33 cases) and wingless (12 cases) HAs by a single surgeon. Minimal and mean follow up times were 12 and 28.1 months, respectively. Japanese Orthopedic Association (JOA) score was used for clinical outcome measurement. Cervical X-rays were taken preoperatively, immediately post-operatively, and after 3, 6, and 12 months and computed tomography scans were performed preoperatively, immediately post-operatively and after 12 months. Cervical lordosis, canal dimension, fusion between lamina and HA, and affecting factors of fusion were analyzed. Results : All surgeries were performed on 142 levels, 99 in the winged and 43 in the wingless HA groups. JOA scores of the winged group changed from $10.4{\pm}2.94$ to $13.3{\pm}2.35$ and scores of the wingless group changed from $10.8{\pm}2.87$ to $13.8{\pm}3.05$. There was no significant difference on lordotic and canal dimensional change between two groups. Post-operative 12 month fusion rate between lamina and HA was significantly lower in the winged group (18.2 vs. 48.8% p=0.001). Multivariate analysis showed that ossification of the posterior longitudinal ligament, male gender, and wingless type HA were significantly associated with fusion. Conclusion : Clinical outcome was similar in patients receiving winged and wingless HA, but the wingless type was associated with a higher rate of fusion between HA and lamina at 12 months post-operatively.

Axial Neck Pain after Cervical Laminoplasty

  • Objective : It has been demonstrated that cervical laminoplasty is an effective and safe method of treating multi-level cervical spondylotic myelopathy and ossification of the posterior longitudinal ligament. However, recent reports have suggested that axial neck pain is frequently encountered after cervical laminoplasty. The aim of the present study was to determine clinical significance of the C7 spinous process on axial neck pain after cervical laminoplasty. Methods : A total of 31 consecutive patients that underwent cervical laminoplasty between March 2002 and December 2008 were reviewed. The authors evaluated and compared axial neck pain and lordotic angle in patients that underwent C7 spinous process preserving surgery (group 1, n=16) and in patients in which the C7 spinous process was sacrificed (group 2, n=15). Results : Severe or moderate early axial pain occurred in 56.2% of patients in group 1 and in 86.6% in group 2. Severe or moderate late axial pain occurred in 12.5% in group 1 and in 73.3% in group 2. Eighty-Six percent of patients in group 2 and 43% in group 1 experienced aggravation of their axial neck pain during the early postoperative period. Aggravation of axial neck pain during early postoperative period was less common in group 1 but not statistically significant (p=0.073). Sixty-six percent of patients in group 2 and 12% in group 1 had aggravated axial neck pain at late postoperative period and aggravation of late axial neck pain was significantly less common in group 1 (p=0.002). Conclusion : The present study demonstrates that C7 spinous process preserving laminoplasty decreases the incidence of aggravated axial neck pain after cervical laminoplasty.

영상검사 소견으로 관찰된 후만곡을 동반한 경추 전만각에 대한 영향인자 분석 (Analysis of Related Factors on Cervical Angle with Kyphosis Observed by X-ray)

  • Objectives : The purpose of this study is to find out the factors affecting the cervical angle with kyphosis. Methods : We investigated 340 cases of patients who had cervical kyphosis. We used cervical angle, flexion malposition angle, odontoid process cline angle, axis-atlas cline angle, odontoid process-atlas cline angle and occiput-atlas cline angle. Correlations between cervical angle and flexion malposition angle, odontoid process cline angle, axis-atlas cline angle, odontoid process-atlas cline angle, occiput-atlas cline angle were analyzed by pearson correlation analysis. Results : The correlation between cervical angle and flexion malposition angle was p-value 0.007, and odontoid process cline angle was p-value 0.002, and axis-atlas cline angle was p-value 0.000, and odontoid process-atlas cline angle was p-value 0.000, and occiput-atlas cline angle was p-value 0.012. Conclusions : Flexion malposition angle, odontoid process cline angle are inversely proportional to cervical angle. And correlation is statistically significant(p<0.01). Axis-atlas cline angle, odontoid process-atlas cline angle are proportional to cervical angle. And correlation is statistically significant(p<0.01). Occiput-atlas cline angle is proportional to cervical angle. And correlation is statistically significant(p<0.05).

한방병원에 내원한 악관절 환자 52명에 관한 임상적 고찰 (The Clinical Study of 52 Patients with Temporomandibular Disorder who Visited Oriental Medical Hospital)

  • Objectives : The propose of this study is to analyze characteristics with sex, age, motivation, symptom, Impression, treatment effect about temporomandibular disorder patients. Method : We performed a retrospective study of 52 patients who visited the Jaseng Hospital of Oriental Medicine, from August 2008 to October 2008. Result : Women occupied the most part. And 27 persons in the 20th are the most distribution of age. The majority of patients occured the symptom for no reason. In the distribution of symptoms, TMD pain(88.46%), Click sound(82.69%), neck pain(52.92%), headache(38.46%), lumbago(21.15%) happened. In the impression through radiological diagnoses, The majority of patients diagnosed Straightening of cervical lordotic curvature. Oriental medical treatment showed a very valuable effect on improvement of TMD symptom. Conclusion : The results showed a valuable treatment effect of temporotandibular disorders on Oriental medical treatment. Therefore this work have the necessity of continuous study and observation and will help us to understand the features of patients who visiting the TMD clinic of Jaseng Hospital of Oriental medicine.

VDT 증후군을 가진 사무직 근로자를 위한 심부경부굴곡 운동의 효과 연구 (The Effects of a Deep Cervical Flexion Exercise for Sedentary Workers with VDT Syndrome)

  • The aims of this study were to compare and analyze two types of deep cervical flexion exercises, craniocervical flexion and cervical flexion, and to ascertain their relative effects on neck and shoulder pain and functional improvement. The participants of the study were individuals who work in sedentary jobs in an office environment. 54 appropriate subjects were chosen and randomly divided into two groups: one group underwent a craniocervical flexion exercise program and the other a cervical flexion exercise program. The six week exercise program consisted of home exercises performed by each subject five times a week and education once a week. Neck-shoulder pain, muscle strengthening, cervical alignment were measured prior to commencement of the exercise programs and again after six weeks. At completion of the six week, both exercise programs decreased neck pain (p<.05). Forward head postures were reduced, and the craniocervical flexion exercise program was more effective in reducing forward head posture (p<.05). The maximum muscle strength and 50% of maximum muscle strength maintaining time of the deep cervical flexor muscles were significantly increased. There were no significant changes of the cervical lordotic curve. The results of this study showed deep cervical flexion exercise was effective in the treatment of neck and shoulder pain, however craniocervical flexion exercise was more effective than cervical flexion exercise.

Anterior Cervical Instrumentation Using Intradiscal Cage with Integrated Plate

  • Objective : The retrospective study is undertaken to report clinical results of anterior cervical interbody fusion with an intradiscal cage with an integrated plate [PCB cervical plating system]. Methods : 38 patients underwent anterior cervical interbody fusion with PCB cervical plating system and followed $6{\sim}24\;months$. The authors investigated overall surgical results; clinical outcome, fusion rate, change of interspace height & lordotic angle, and complications. Results : No complication was observed during the operation. Clinical improvement was identified in 34 cases [89.5%]. Bone fusion observed in 44 out of 49 sites [90.7%]. After operation, the interspace height increased from $5.4{\pm}1.3mm$ to $7.8{\pm}1.5mm$ and maintained $7.4{\pm}1.1mm$ and, interspace angle went up from $4.2{\pm}0.7^{\circ}$ to $4.8{\pm}1.1^{\circ}$ and maintained $4.6{\pm}$0.9^{\circ}. The loosening of screw was observed in 6 cases, one of which had reoperation because of the expulsion of the device accompanied. Conclusion : PCB cervical plating system could restore interbody height and lordosis in anterior cervical interbody fusion. But, if the insertion of the spacer is not precise, the frequencies of hardware failure are relatively high. It is considered necessary for the operator to be careful in the procedure.

T1 Slope and Cervical Sagittal Alignment on Cervical CT Radiographs of Asymptomatic Persons

  • Objective : We performed a retrospective analysis of medical records and radiographic images of patients who never underwent spinal treatment including diagnosis. The objective of this study is to explain the biomechanical and physiologic characteristics of cervical alignment related to thoracic inlet angle including T1 slope changes in each individual. Methods : We reviewed the cervical CT radiographs of 80 patients who visited ENT outpatient clinic without any symptom, diagnosis and treatment of cervical spine from January 2011 to September 2012. All targeted people were randomized without any prejudice. We assessed the data-T1 slope, Cobb's angle C2-7, neck tilt, sagittal vertical axis (SVA) C2-7 and thoracic inlet angle by the CT radiographs. Results : The relationships between each value were analyzed and we concluded that Cobb's angle C2-7 gets higher as the T1 slope gets higher, while the SVA C2-7 value decreases. Conclusion : We propose that the T1 slope is background information in deciding how much angle can be made in the cervical spinal angle of surgical lordotic curvature, especially severe cervical deformity.

The Effects of Modified Chin Tuck Exercise on the Cervical Curvature, the Strength and Endurance of the Deep Cervical Flexor Muscles in Subjects with Forward Head Posture

  • Purpose : The purpose of this study was to investigate the effects of chin tuck exercise (CTE) on the craniovertebral angle (CVA), strength and endurance of deep cervical flexor (DCF) muscles in subjects with forward head posture(FHP). This study was performed on 30 subjects with FHP. Method : Thirty subjects were divided into two groups; modified CTE (n=15), conventional CTE (n=15). Both of the group performed the exercise 4 times a week for 6 weeks. The subjects performed CTE in two different methods; modified CTE, with device designed that help keep cervical lordois curve, and coventional CTE, without using device. The CVA was measured using Image software version. A pressure biofeedback unit was used to measure the strength and endurance of the DCF muscles. The data was analyzed by the paired t-test for comparing before and after changes of variables in each group and the independent t-test for comparing the between groups. Result : There was statistically significant difference of before and after strength and endurance of DCF muscles in modified CTE (p<0.05). There was statistically significant difference of before and after only endurance of DCF muscles in conventional CTE (p<0.05). There was statistically significant difference of between the two group in strength of DCF muscles (p<0.05). Conclusion : Muscle strength to stabilize the spine plays an important role in maintaining a good posture. Therefore, we suggest that the application of CTE with a device designed to maintain the lordotic curvature in the neck is likely to yield better outcomes in FHP subjects in future studies.
