Microsurgical Foraminotomy via Wiltse Paraspinal Approach for Foraminal or Extraforaminal Stenosis at L5-S1 Level : Risk Factor Analysis for Poor Outcome

  • Objective : The purpose of this study was to present the outcome of the microsurgical foraminotomy via Wiltse paraspinal approach for foraminal or extraforaminal (FEF) stenosis at L5-S1 level. We investigated risk factors associated with poor outcome of microsurgical foraminotomy at L5-S1 level. Methods : We analyzed 21 patients who underwent the microsurgical foraminotomy for FEF stenosis at L5-S1 level. To investigate risk factors associated with poor outcome, patients were classified into two groups (success and failure in foraminotomy). Clinical outcomes were assessed by the visual analogue scale (VAS) scores of back and leg pain and Oswestry disability index (ODI). Radiographic parameters including existence of spondylolisthesis, existence and degree of coronal wedging, disc height, foramen height, segmental lordotic angle (SLA) on neutral and dynamic view, segmental range of motion, and global lumbar lordotic angle were investigated. Results : Postoperative VAS score and ODI improved after foraminotomy. However, there were 7 patients (33%) who had persistent or recurrent leg pain. SLA on neutral and extension radiographic films were significantly associated with the failure in foraminotomy (p<0.05). Receiver-operating characteristics curve analysis revealed the optimal cut-off values of SLA on neutral and extension radiographic films for predicting failure in foraminotomy were $17.3^{\circ}$ and $24^{\circ}s$, respectively. Conclusion : Microsurgical foraminotomy for FEF stenosis at L5-S1 level can provide good clinical outcomes in selected patients. Poor outcomes were associated with large SLA on preoperative neutral (>$17.3^{\circ}$) and extension radiographic films (>$24^{\circ}$).

A Study on Relationship between Lumbosacral Curvature and Neck-Waist Circumference on College Students in Seoul (서울지역 대학생들의 요천추 만곡과 목-허리둘레의 관계에 대한 연구)

  • Objectives To investigate correlation between anthropometric data (neck circumference (NC), waist circumference (WC), body mass index (BMI), and body shape indexes) and radiological parameters of lumbosacrum. Methods The data of college students living in Seoul (n=24) were analyzed retrospectively. Anthropometric data of NC, WC, and BMI were measured. Lumbar spine X-ray film was taken to measure lumbar lordotic angle, Ferguson's angle. To evaluate body shape of participants, three indexes of neck-to-waist ratio (NWR), neck-to-height ratio (NHR), and waist-to-height ratio (WHR) were used. Anthropometric data's correlations with radiological parameters of lumbosacrum were investigated. Results Anthropometric data of NC, WC, and BMI had no significant correlation with radiological parameters of lumbosacrum. NWR had significant positive correlation with lumbar lordotic angle and Ferguson's angle. NHR and WHR had no significant correlation with radiological parameters of lumbosacrum. Conclusions The results suggest that NWR-related fat distribution in neck has significant correlation with radiological parameters of lumbosacrum regardless of obesity.

A Study on Characteristics and Correspondence of Men's Back and Chest Types (20대 남성의 등면 및 가슴부위 형태특성과 대응에 관한 연구)

  • The purpose of this study is to classify the types of men's back and chest through the criteria which represent the characteristic of men's back and chest. We have the following conclusions based on our sample size of 291 men's back and chest. The result of factor analysis concerned with men's back indicates that six factors are extracted and they are consist of 80.6% of total variance. We divide the shape of back into three categories: bent, protrusion of the scapula, and breadth. Each category is divided into subcategories. According to the frequency based on our data entries of 291 men's back, we introduce five new types of men's back. 83.8% of examined men's back belong to one of these five types: (i) 15.5% of protrudent and lordotic type; (ii) 6.5% of flat and lordotic type; (iii) 22.7% of protrudent and normal type; (iv) 34.4% of flat and normal type; (v) 4.8% of flat and bent type. The result of factor analysis related to with men's chest shows that six factors are extracted and they are consist of 76.0% of total variance. We divide the shape of chest into three categories: thickness, breadth, and protrusion of the breast. Each category is divided into subcategories. According to the frequency based on our data entries of 291 men's chest, we introduce four new types of men's chest. 65.0% of examined men's chest belong to one of these four types: (i) 7.2% of full and well-developed type; (ii) 24.1% of full and developed type; (iii) 25.8% of flat and developed type; (iv) 7.9% of flat and under-developed type. In order to consider the shape of the upper body as a whole, the back and the chest types should be corresponded.

Clinical Experience of Anterior Cervical Interbody Fusion with the PCB System (전방 경추 유합술에서의 PCB System의 임상적 경험)

  • Objective : The purpose of the study was to evaluate the clinical and radiological results after discectomy and Lubboc bone graft in the surgical management of the cervical diseases with a new titanium interbody implant and integrated screw fixation(PCB) by anterior approach. Methods : The authors retrospectively analyzed 28 cases of anterior cervical fusion with PCB system and Lubboc bone(xeno graft) from september 1998 to december 2000. Twenty-eight patients with cervical diseases underwent decompression cervical lesion and followed from 5 to 27 months with a mean follow-up of 14 months. There patients were evaluated with clinically and radiologically at immediate postoperative period and at 3, 6, 9, and 12 months. Result : The authors investigated the pre- and postoperative intervertebral disc space, clinical outcomes, radiography fusion rate, and Cobb angle in the fixed segments by anterior approach. The lordotic angles and height of disc space were increased after the operation. The clinical outcome of patients follow-up was good or excellent result based on Odom's criteria with improvement of clinical symptom in about 92.9% of the cervical diseases. Two patients showed loosening of the lower and upper cervical screw of PCB instruments, and two patients showed swallowing difficulty and wound infection Conclusion : The PCB system is a new implant for anterior cervical interbody fusion in the degenerative cervical disease and disc herniations. It provides immediate stability and segment distraction. The results of this study indicate that the PCB system is safe, easy handling of hardware, less complications, high fusion rate, and has provide the keeping the intervertebral disc space height and lordotic angles.

Finding of Relation Between 4th Lumbar Rotational Malposition and Scoliosis (요추 4번의 회전변위와 척추측만증과의 상관성에 관한 연구)

  • Objectives : The purpose of this study is to find out the relation between 4th lumbar rotational malposition and scoliosis. Methods : We investigated 22 cases of patients who were diagnosed as scoliosis. We used AP & Lateral view X-ray for patients. And we analysed the relation between 4th lumbar rotational malposition and scoliosis. Results : P-value was 0.436 between 4th lumbar rotational amount and lordotic angle, and was 0.758 between 4th lumbar rotational amount and wedge angle(p>0.05). And p-value was 0.022 between 4th lumbar rotational amount and scoliotic apex rotational amount(p<0.05), but was 0.286 between 4th lumbar rotational amount and Cobb's angle(p>0.05). Conclusions : The results suggest that 4th lumbar rotational malposition was statistically correlation with scoliotic apex rotational malposition, was not Cobb's angle, 4th lumbar lordotic & wedge angle.

Effects of Therapeutic Exercise on Hip Joint Range of Motion, Manual Muscle Test, Functional Movement Screen and Radiological Evaluation in a Youth Football Player with Football-specific Anterior Pelvic Tilt: A Case Report (치료적 운동이 축구 특이적 골반 앞 기울임을 가진 유소년 축구선수 엉덩관절의 관절가동범위, 도수근력, 기능적 움직임 및 방사선학적 평가에 미치는 영향: 증례보고)

  • PURPOSE: This study was conducted to determine the effects of therapeutic exercise on range of motion (ROM), the manual muscle test (MMT), functional movement screen (FMS) and radiological evaluation in a youth football player with football-specific anterior pelvic tilt (APT). METHODS: The subject of this case report was a 12-year-old youth football player, who presented with football-specific APT. Therapeutic exercise consisted of hamstring stretch, prone hip extension, abdominal crunch, bridging with isometric hip abduction, plank exercise with posterior tilt and posterior pelvic tilting exercise using a swiss-ball for 40 min/day, twice a week for 8-weeks. ROM, MMT, FMS (deep squat, hurdle step, inline lunge, shoulder mobility, active straight-leg raise, rotary stability and trunk stability push-up) and radiographs (lumbar lordotic and sacral horizontal angle) were analyzed before and after week 8 of therapeutic exercise. RESULTS: The ROM, MMT, and FMS increased and the lumbar lordotic angle and sacral horizontal angle improved after 8-weeks of therapeutic exercise. CONCLUSION: The results of this case report suggest that therapeutic exercise improves ROM, MMT and radiography associated parameters in youth football players with football-specific APT. These findings have clinical implications for therapeutic exercise in youth football players with football-specific APT.

A Study on the Structure of Three Dimentional Spine, Pelvic Deviation and Foot Pressure in Golf Players (골프선수의 3차원적 척추구조, 골반변위 및 족압에 관한 연구)

  • The purpose of this study was to investigate variables of significantly difference as the structure of the spine, pelvic deviation and foot pressure between undergraduates and golf player subjects. The subjects of study were composed of 20 male golf players and 20 male undergraduates. Both groups were right handed persons. The measurement tools of this study were Formetric 4D(Diers, Germany) which is a three dimensional measure. The result are the follows: there were a significant difference between golf players and general students of trunk imbalance, pelvic tilt, pelvic torsion, pelvic rotation, surface rotation, lumbar lordotic curve, foot pressure(fore & behind foot), weight distribution(right & left foot). In conclusion, golf players might cause transform of spine and foot pressure due to golf exercise for several years. Such as imbalance affect to induce functional impairment and pain of musculoskeletal system, and appropriate evaluation and treatment were necessary for golf players.

A Clinical Study of Cervical Radiculopathy in Patients with Shoulder Pain (견통을 주소로 한 경추부 신경근병변의 임상적 고찰)

  • The purpose of this study were to identify the correlation of shoulder pain to Cervical Pathology and effectiveness of Cervical treatment on the neck and shoulder. Sixty patients complaining of shoulder pain only without neck pain were evaluated by physical examination. Cervical X- rays and electrodiagnostic Study. The results were as follows: 1) Forty nine Cases were abnormal in plain Cervical spine X-ray with straightening of Cervical lordotic curve in 41 cases, Cervical spondybsis 16 cases and disc space narrowing in 4 cases. 2) Cervical radiculopathies were diagnosed 52 cases which showed abnormal spontaneous activities in needle. EMG : 26 cases in paraspinal muscles and 26 cases in both paraspinal and upper extremity muscles. 3) The results of treatment were excellent in 6 cases, good in 6 cases and fair in 32 cases. 4) Treatment side were effective with shoulder and conical in 36 cases.

Comparison of the Body Alignment during Standing on Level and Wedge Board (평지와 쐐기 발판(wedge board) 위에 기립 시 신체정렬 비교)

  • The purpose of this study was to compare the body alignment during standing on level and wedge board. Twenty healthy college students (8 females, 12 males) were evaluated in this study. Diagnostic contourline potographic imaging system (Model JTC-1, Jodang Trading Co.) was used to measure body alignment. Sagittal and frontal plane images were used to analyze the body alignment. The result showed that the cervical and lumbar lordotic curve significantly decreased during standing on wedge board when compared with standing on level. On the other hand, thoracic kyphosis significantly increased during standing on wedge board. There was no significant difference in body alignment according to gender, weight, and height. Clinically, patients with low back pain and severe lordosis may be affected by heel wedge. Further study is needed to identify whether the standing on wedge board can change the body alignment in patients with low back pain and spinal deformity.

  • Objectives : To study the cervical lordosis and roentgenographic analysis of cervical disc space in neck pain with or without temporomandibular disorder(TMD). Methods : Radiographic measures of cervical lordosis, cervical disc space narrowing were collected, statiscally analyzed. Results : TMD group showed a significant increase in cervical lordotic angle as compared with non-TMD group, whereas no significant change in cervical disc degeneration. Conclusions : The findings from this study suggest that the curvature of the cervical spine is related to the subject's TMD.
