Stability of unilateral sagittal split ramus osteotomy for correction of facial asymmetry: long-term case series and literature review

  • Bilateral sagittal split ramus osteotomy is considered a standard technique in mandibular orthognathic surgeries to reduce unexpected bilateral stress in the temporomandibular joints. Unilateral sagittal split ramus osteotomy (USSO) was recently introduced to correct facial asymmetry caused by asymmetric mandibular prognathism and has shown favorable outcomes. If unilateral surgery could guarantee long-term postoperative stability as well as favorable results, operation time and the incidence of postoperative complications could be reduced compared to those in bilateral surgery. This report highlights three consecutive cases with long-term follow-up in which USSO was used to correct asymmetric mandibular prognathism. Long-term postoperative changes in the condylar contour and ramus and condylar head length were analyzed using routine radiography and computed tomography. In addition, prior USSO studies were reviewed to outline clear criteria for applying this technique. In conclusion, patients showing functional-type asymmetry with predicted unilateral mandibular movement of less than 7 mm can be considered suitable candidates for USSO-based correction of asymmetric mandibular prognathism with or without maxillary arch surgeries.

  • Background: We evaluated early and long-term results after heart transplantation (HTPL). Methods: One hundred five consecutive patients (male:female=80:25) who underwent HTPL between 1994 and 2013 were enrolled. Based on the changes in immunosuppressive regimen, the study patients were divided into two groups. Early and long-term clinical outcomes were evaluated and compared between the patients who underwent HTPL before (group E, n=41) and after July 2009 (group L, n=64). The group L patients were older (p<0.001), had higher incidence of hypertension (p=0.001) and chronic kidney disease (p<0.001), and more frequently needed preoperative mechanical ventilation (p=0.027) and mechanical circulatory support (p=0.014) than the group E patients. Results: Overall operative mortality was 3.8%, and postoperative morbidities included acute kidney injury (n=31), respiratory complications (n=16), reoperation for bleeding (n=15) and wound complications (n=10). There were no significant differences in early results except acute kidney injury between group E and group L patients. Overall survival rates at 1, 5, and 10 years were 83.8%, 67.7%, and 54.9%, respectively, with no significant difference between the two patient groups. Rejection-free rates at 1 and 5 years were 63.0% and 59.7%, respectively; rates were significantly higher in group L than in group E (p<0.001). Conclusion: Despite increased preoperative comorbidities, group L patients showed similar early and long-term outcomes and significantly higher rejection-free rates when compared with group E patients.

Studies on the Change of Components with Long-Term Storage of paddy (장기저장 미곡의 성분변화 특성)

  • In order to investigate the changes of rice qualities during 4 years storage of paddy stored in ware-house of normal temperature condition. Temperature in warehouse was changed more than 3$0^{\circ}C$ under the influence of average temperature outside of a warehouse. Water content of paddy was not increased over 15% But as average temperature in warehouse was gone up 18$^{\circ}C$ from June to September every years it was supposed that these periods were to be deterioration of rice quality. On investigation of change in paddy it was supposed that these periods were to be deterioration of rice quality. On investigation of change in paddy components during the long term storage, reducing, sugar, amylose, crude protein were increased 0.24%, 19.23%, 7.02% at enterance time to 0.5%, 20.31% 7.46% 4 years later respectively. Max viscosity final visocity and set back value by amylograph were increased 449B,U 610B.U, 161B.U to 493B.U, 715B,U 222B.U but breakdown was decreased 125B.U to 76B.U with the increase of storage period. Gel consistency of rice stored was decreased 44.7mm at enterance time to 39,9mm 4years later. Fatty acid was increased remarkably 4.5KOHmg/100g to 24.4KOHmg/100g. Germination ratio and germ activity of paddy during long-term storage were decreased 97%, 100% to 0%, 0.4% respectively, With the increase of storage period contaminated paddy by molds increased and its by bacteria decreas-ed.

  • The key to the long-term preservation of electronic records is the long-term retention of the bitstream that contains the original functional attributes and appearances of the electronic records that were originally created and utilized. Considering economic and technical aspects, most of the major domestic and foreign archives have adopted a migration strategy that allows conversion to a new format depending on the stability and reliability of the format containing the original bitstream. However, as the type of electronic records varies and its scope continuously expands, several problems arise if a single migration strategy is observed. Therefore, the applicability of the emulation strategy is examined in this paper to reproduce the system and application environment where electronic records are created and utilized. As such, electronic records can be maintained without any changes in the bitstream by utilizing cloud computing technology.

Long-term Outcomes of Augmentation Cystoplasty in a Pediatric Population With Refractory Bladder Dysfunction: A 12-Year Follow-up Experience at Single Center

  • Purpose: Augmentation cystoplasty (AC) is a surgical procedure used in adults and children with refractory bladder dysfunction, including a small bladder capacity and inadequate bladder compliance, and in whom conservative and medical treatment has failed. This study was aimed to determine the long-term outcomes of AC in children. Methods: A retrospective analysis was conducted of 42 patients (31 males; mean age, $14.2{\pm}6.2years$) who underwent AC for neurogenic and nonneurogenic bladder dysfunction, with a median $12.0{\pm}1.5years$ of follow-up. All patients underwent AC using the ileum with or without continent reconstruction. Pre-AC, concurrent, and post-AC procedures and complications were analyzed. Patients who underwent ureterocystoplasty, were lost to follow-up, or had less than 10 years of follow-up were excluded. The primary outcomes were the complication and continence rates, the post-AC linear rate of height and weight gain, and renal function. The Student t-test was used to evaluate between-group differences and the paired t-test was used to evaluate longitudinal changes in measured variables. Results: Renal function was stable or improved in 32 of 42 patients (76.2%), with a post-AC continence rate of 88.1%. Thirty patients (71.4%) required 72 procedures post-AC. There was no statistically significant difference in the mean percentile of height (P=0. 212) or weight (P=0.142) of patients in the pre- and post-AC periods. No cases of bladder perforation or malignancy were detected. Conclusions: We consider AC to be a safe and effective procedure that does not negatively affect future physical growth, while achieving a good rate of stable renal function. Patients need long-term follow-up to address long-term complications.

  • This study grasped long-term changing tendency of shoreline during lately about 30 years in five region of South East coast, and analyzed long-term changing tendency of East coast shoreline and the factors that synthesized studies of Central and South East coast. As a result of calculating of shoreline variations using DSAS, each shoreline of Mangyangjeong and Josa region regressed mean 28.9m and 6.4m, but each shoreline of Goraebul, Daejin and Bonggil region progressed mean 25.0m, 10.6m and 18.8m. Synthesizing changing tendency of East coast shoreline, 1) progressive and regressive zones of shoreline in all regions seem to repeat. 2) looking at shoreline of south zone adjacent to lately constructed or extended breakwater progressed, because it is thought due to effect of a longshore current flowing north. 3) zones using beach relatively tends to regress shorelines. 4) progress and regress of shoreline in zones including estuary of stream are various features as change of deposit supply from a upstream region.

Long-Term Trend of Surface Wind Speed in Korea: Physical and Statistical Homogenizations (한반도 지상 풍속의 장기 추세 추정: 관측 자료의 물리적 및 통계적 보정)

  • The long-term trend of surface wind speed in Korea is estimated by correcting wind measurements at 29 KMA weather stations from 1985 to 2019 with physical and statistical homogenization. The anemometer height changes at each station are first adjusted by applying physical homogenization using the power-law wind profile. The statistical homogenization is then applied to the adjusted data. A standard normal homogeneity test (SNHT) is particularly utilized. Approximately 40% of inhomogeneities detected by the SNHT match with the sea-level-height change of each station, indicating that an SNHT is an effective technique for reconciling data inhomogeneity. The long-term trends are compared with homogenized data. Statistically significant negative trends are observed along the coast, while insignificant trends are dominant inland. The mean trend, averaged over all stations, is -0.03 ± 0.07 m s-1 decade-1. This insignificant trend is due to a trend change across 2001. A decreasing trend of -0.10 m s-1 decade-1 reverses to an increasing trend of 0.03 m s-1 decade-1 from 2001. This trend change is consistent with mid-latitude wind change in the Northern hemisphere, indicating that the long-term trend of surface wind speed in Korea is partly determined by large-scale atmospheric circulation.

Long-term effects of maxillary skeletal expander treatment on functional breathing

  • Objective: To investigate the long-term effects of maxillary skeletal expander (MSE) treatment on functional breathing. Methods: Objective measures of breathing, the peak nasal inspiratory flow (PNIF), and peak oral inspiratory flow (POIF), and subjective measures of breathing, the visual analog scale (VAS) and nasal obstruction symptom evaluation (NOSE) survey, were used to investigate the long-term effects of MSE in functional breathing. Seventeen patients, mean age 19.4 ± 3.9 years treated at the UCLA Orthodontics Clinic were assessed on their functional breathing at 3 timepoints: pre-expansion (T0), post-expansion (T1), and post-orthodontic treatment (T2). Results: Immediately after expansion (T1), all the objective functional breathing values were significantly increased in comparison to T0 (P < 0.05). The VAS total, VAS right and VAS left were significantly lower at T1 in comparison to T0 (P < 0.05). At 26.8 ± 3.9 months after MSE expansion (T2), PNIF total, PNIF right, PNIF left, and POIF were significantly higher when compared to T0 (P < 0.05). Also, VAS total, VAS right and VAS left were significantly lower at T2 when compared to T0 (P < 0.05). Additionally, there was a positive correlation between PNIF and the magnitude of expansion at anterior nasal spine and zygomaticomaxillary point (ZMA). There was a positive correlation between total VAS and the magnitude of expansion at the ZMA. There were no significant changes for the NOSE subjective breathing measurement at all time comparisons. Conclusions: Overall, MSE treatment produces an increased objective and subjective airway improvement that continues to remain stable in the long-term post expansion.

  • The purpose of this study is to evaluate the hydrological impact due to temporal land cover changes by gradual urbanization of a watershed. WMS HEC-1 was adopted, and DEM with 200m resolution and hydrologic soil group from 1:50,000 soil map were prepared. Land covers of 1986, 1990, 1994 and 1999 Landsat TM images were classified by maximum likelihood method. By applying the model, watershed average CN value was affected in the order of paddy, forest and urban/residential, respectively.

Evaluation of Coastal Sediment Budget on East Coast Maeongbang Beach by Wave Changes (파랑 변화에 따른 동해안 맹방 해수욕장 연안 표사수지 파악)

  • Numerical simulation of the sediment by the Delft3d model was conducted to examine the changes in the sediment budget transport caused by long-term wave changes at the Maengbang beach. Representative waves were generated with input reduction tools using NOAA NCEP wave data for about 40 years, i.e., from January 1979 to May 2019. To determine the adequacy of the model, wave and depth changes were compared and verified using wave and depth data observed for about 23 months beginning in March 2017. As a result of the error analysis, the bias was 0.05 and the root mean square error was 0.23, which indicated that the numerical wave results were satisfactory. Also, the observed change in depth and numerical result were similar. In addition, to examine the effect due to long-term changes in the waves, the NOAA wave data classified into each of the representative wave grades, and then the annual trend of the representative wave was analyzed. After deciding the weight of each wave class considering the changed wave environment in 2100, the amounts of sedimentation, deposition, and the sediment transport budget were reviewed for the same period. The results indicated that the sedimentation pattern did not change significantly compared to the current state, and the amount of the local sediment budget shown in the present state was slightly less. And there has been a local increase in the number of sediment budget transport, but there is no significant difference in the net and amount of sediment movements.
