The Effects of Neurofeedback Training on Brain Function Quotient of Elderly with Long-term Care Insurance Service (뉴로피드백 훈련이 장기요양시설노인의 뇌기능지수에 미치는 효과)

  • Purpose: Recently, Neurofeedback training system that based on biofeedback of brain wave was introduced. This study was performed to identify the effects of the improvement of brain function by Neurofeedback training on elders(the 2nd or 3rd grade of long-term care insurance services). Methods: A quasi-experimental design using a nonequivalent control group, pre-post test was used. Total 11 elderly were enrolled in this study (experimental group 5, control group 6). The intervention was conducted 3 times a week for 30 minutes from January to June, 2012 (total 60 times). Chi-square test and Mann-Whitney U-test were used to analyze the data. Results: After the Neurofeedback intervention, attention quotient (AQ), anti-stress quotient (ASQ), emotion quotient (EQ) and brain quotient (BQ) of the experimental group were significantly better than those of the control group. Conclusion: The findings indicate that the Neurofeedback training program was effective in reducing fatigue by AQ, increasing the physical and mental stress resistance by ASQ, emotional balance by EQ and improving of total brain function by BQ. Therefore Neurofeedback training be used as an effective training intervention for the health of elderly in geriatric facility.

  • After elderly long term care insurance implementation (July 1, 2008), the elderly medical and welfare facilities covered by medical insurance continuously increases 30 to 63% sanction annually. Rapid growth of senior citizen welfare facilities arises in a poor residential environment issues. In most cases, it is the legal minimum criteria tailored for 4 persons accommodated in nature, such as the hospital. Therefore, 'Hometown-style traditional Korean House' is needed to rest comfortable for elderly people. Research goal is to provide direction and design materials by developing a type of architectural planning for the elderly welfare facilities unit care of the atmosphere of a traditional Korean House. The following two elements are applied to the elderly welfare facilities. Senior citizen welfare facilities in the main living space are applied to modern residential space of the elderly-friendly atmosphere of a traditional Korean House. It is applied for the Korean lifestyle and residential culture are through an analysis of Japan's leading development unit care system.

A Study on the Development of the Healing Environment Evaluation Criteria for Elderly Care Hospital focusing on the Elderly Inpatient Characteristics (노인환자의 특성을 고려한 노인요양병원 치유환경 평가지표)

  • This paper investigates the evaluation criteria in order to manage healing environment of long-term care elderly hospital. Elderly hospital evaluation tool developed by Korean Government is used to assess elderly hospital facilities to check the hospital facility and maintain its quality. However, the evaluation indicators and questions mainly focusing on safety indicators. Some questions are too vague for precise evaluation. In this paper, we discuss the advantages and disadvantages of the present evaluation criteria to establish new assessment tool for precise evaluation. The literature research was conducted tp set up the new evaluation criteria. From this research, we developed an elderly focusing on healing environment checklist for elderly care hospital which contains 7 factors as the primary hierarchy structure (Safety, Accessibility, Amenity, Sensibility, Friendly to nature, Territory, Interaction) and 23 factors as the secondary hierarchy structure. This evaluation criteria will help healthcare facility designers and healthcare organizations to build the healthcare facilities.

Assessment of Basic Physical Activity for Elderly Women in Long-Term Care Facilities (요양시설 거주 여성노인의 기초신체활동 측정 평가)

  • This study was to investigate basic level of physical activity test that reflects activity of daily living(ADL) for institutionalized elderly women. The subjects were 105 elderly women over than 65 years living in long-term care facilities located in Daejeon and Chungnam. The measured eight items related to ADL were tested and the data was analyzed by descriptive statistics, one-way ANOVA, factors analysis and correlation. The results were statistically significant differences. Among eight test items, two factors(muscle strength and mobility) were extracted by principal component factor analysis(PCA). The two factors are appropriate domains for assessment of basic physical activity of institutionalized elderly women. By using Pearson's correlation, between two factors and age, the first principal component scores were significantly decreased with age increasing. Degradation of ADL must be prevented for institutionalized elderly women through maintain muscle strength and mobility.

The Effects of Leadership Style on Service Quality in Long-Term Care Facility: the Mediating Effect on Organizational Commitment (노인요양시설 리더십 유형이 서비스 품질에 미치는 영향에 관한 연구: 조직몰입의 매개효과)

  • The purpose of this study is to increase the long term care service quality. This study examines how leadership style and organizational commitment can effect service quality. The basic data has been made by 318 long-term care workers. Major findings are reported: transformational leadership effects affective commitment, normative commitment, continuance commitment and service quality. And transactional leadership effects continuance commitment, too. Espicially, the mediating effects of normative commitment in the relationship between transformational leadership and service quality were verified. These results have implications for the applications of transformational and transactional leadership and on human resource management and suggest on the support policy.

Pilot Prospective Study for Validation of Korean Medication Review Tool for Nursing Home Residents in Older Adults Living in Long-term Care Facilities (장기요양시설 거주 노인을 대상으로 한국형 요양원 입소 노인 약물검토 도구의 타당성 검증을 위한 전향적 선행연구)

  • Background: To implement medication management service in Korean nursing home (NH), medication review tool for residents in Korean long-term care facilities was developed. This prospective pilot study aimed to verify the applicability of this newly developed tool and to evaluate the drug related problems (DRPs) identified by pharmacists' medication review in NH setting. Methods: This study was carried out in two NHs in Korea. The elderly residents (65 or older) using 5 or more medications were eligible. Pharmacists conducted medication review and identified the DRPs and potentially inappropriate medications (PIMs) based on the newly developed tool. Results: Among 43 NH residents, 27 residents agreed to participate. The median age was 87 and about 55.6% of them were taking 10 or more medications. Pharmacists identified a total of 37 PIMs in 18 NH residents (66.7%) and 54 DRPs in 22 residents (81.5%). The most frequent PIM was general-PIM (26 cases) followed by PIM under specific diseases or conditions (7 cases). Out of 77 items contained in the tool, 15 items were detected in study participants. 'Continued use or regular daily use of hypnotics' was the most highly detected item (9 cases) followed by 'multiple prescriptions within each class of hypnotics/sedatives' (5 cases). Among 54 DRPs identified, pharmacist intervened 39 cases (72.2%) and interventions were accepted in 18 cases (46.1%). Conclusion: This pilot study demonstrated that newly developed tool is feasible for the nursing home residents. However, further studies with larger population are warranted.

Factors Related to Long-term Hospital Length of Stay and Opinions on Discharge-related Community-based Medical and Welfare Service on Elderly Patients with Chronic Diseases in Korean Veterans Hospitals

  • Purpose: This study aims to investigate factors related to long-term length of stay (LOS) of patients with chronic diseases in Korean veterans hospitals. Methods: The subjects were 196 elderly patients with chronic disease staying in the hospital for more than 10 days, Data were collected by the survey of patients with structured questionnaires and medical records review by nurses from July 15 to August 10, 2019. Collected data were analyzed using t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. Results: The present and desired LOS were 37.78±32.66 days and 60.87±45.95 days, respectively. Factors affecting hospital LOS were found to be main disease (genitourinary) (p<.001), assistance in activities of daily living (p<.001), area of hospital (p<.001), payment of medical fees (p=.026), hospital satisfaction (p=.036) and the explanatory power of these variables was 26.4%. The most common health problems that need to be solved after discharge were symptom alleviation and health promotion. These problems can be solved using community-based facility services or visiting medical-welfare services (especially home care nursing). Conclusion: In order to reduce hospital LOS, the following measures are required: personalized self-management education, provision of transportation services for dialysis therapy of inactive patients, linking patients with visiting medical-welfare services including home care nursing and mobile healthcare services, operation of the case management system including the notice of the discharge date at admission, interim check of patient status, and connecting the patient with community resources or transferring the patient to long-term care facilities at discharge.

Developing a Performance Information Integration Method (PIIM) (성과 정보 통합 방법의 개발)

  • Several performance measurement systems have been proposed and are currently being used in many organizations. However, no performance measurement system has presented a mechanism for integrating all of the information of performance measures in a way that considers the relationships between them. This paper proposes the performance information integration method (PIIM) which synthesizes all useful information of performance measures with consideration of the relationships and generates a single, global, organizational performance score. The PIIM showed good performance in the implementation in one long-term care facility. This paper provides performance measurement systems with an integration mechanism for multiple performance measures.

A Study of Social Workers' Understanding of Elderly Patients' and Family Caregivers' Rights to End-of-Life Care Decisions and of Their Own Roles in the Process (노인환자와 가족의 임종의료결정 권리 및 사회복지사 역할 이해도 - 장기요양 입소 시설 사회복지사를 대상으로 -)

  • Purpose: This study was aimed to analyze how social workers understand the rights for elderly patient and family caregiver to make end-of-life (EOL) care decisions and their roles the decision making process. Methods: The study employed a quantitative research method of collecting data from a structured questionnaire that was filled out by 334 social workers at long-term care facilities. Data were analyzed by descriptive statistics, mean differences, correlation between variables, using SPSS 20.0 program. Results: The mean score for the understanding the rights to an EOL care decision was $3.46{\pm}0.69$ and of their own roles $3.48{\pm}0.84$. The level of understanding significantly differed by social workers' experience of assisting a process to make an EOL care decision such as advance directives and life sustaining treatment, work experience, and the number of beds. Positive correlation was observed between the level of understanding of the rights for EOL care decisions and of social workers' roles (Pearson r=0.329, P<0.001). Conclusion: This study proposes development of an education program for social workers and devising standards for the EOL care decision making process to protect elderly patients, family caregivers as well as social workers in a long term care facility.

Research Trends in the Journal of Korean Academic Society of Home Health Care Nursing from 2010 to 2019: Using the Keyword Home Health Care (가정간호학회지 게재 논문 분석: 2010년부터 2019년까지 가정간호분야를 중심으로)

  • Purpose: The purpose of this study was to analyze research trends, using the keyword home health care, in articles published in the Journal of Korean Academic Society of Home Health Care Nursing over the past 10 years. Methods: An analysis was conducted of 50 home health care-based studies chosen from among the 206 studies published in the Journal of Korean Academic Society of Home Health Care Nursing from 2010 to 2019. The analysis focused on research methodology and keyword. Descriptive statistics were used to examine the frequency distribution of research methods and keywords. Results: Study participation was mainly focused on nurses (52.0%). Most of the studies used quantitative methods (96.0%), and 43 studies (86.0%) used self-report structured questionnaires. The most commonly used data analyses methods were descriptive statistics, t-test, analysis of variance, correlation, and regression. Major keywords were home health nursing, elderly care facility, visiting nurse, home care service, home healthcare nurse, home care agencies, long-term care, and home care. Conclusion: The results of this study identified current trends and interests in the Journal of Korean Academic Society of Home Health Care Nursing. This study suggests that future studies include a variety of research methods and maintain appropriate standards of research ethics.
