Objective : The purpose of this study is to quantitatively analyze the role of occupational therapy in long - term care insurance for the elderly using text mining, one of the big data analysis techniques. Method : For the analysis of newspaper articles, "Long - Term Care Insurance for the Elderly + Occupational Therapy for the Elderly" was collected after the period from 2007 to 208. Naver, which has a high share of the domestic search engine, utilized the database of Naver News by utilizing Textom, a web crawling tool. After collecting the article title and original text of 510 news data from the collection of the elderly long term care insurance + occupational therapy search, we analyzed the article frequency and key words by year. Result : In terms of the frequency of articles published by year, the number of articles published in 2015 and 2017 was the highest with 70 articles (13.7%), and the top 10 terms of the key word analysis showed the highest frequency of 'dementia' (344) In terms of key words, dementia, treatment, hospital, health, service, rehabilitation, facilities, institution, grade, elderly, professional, salary, industrial complex and people are related. Conclusion : In this study, it is meaningful that the textual mining technique was used to more objectively confirm the social needs and the role of the occupational therapist for the dementia and rehabilitation in the related key keywords based on the media reporting trend of the elderly long - term care insurance for 11 years. Based on the results of this study, future research should expand research field and period and supplement the research methodology through various analysis methods according to the year.

This study was performed to determine the levels of MMSE-K among the elderly people affiliated with long-term care insurance services in rural areas, and to reveal its association with related variables. The interviews were performed, during the period from March 1st, to May 31th, 2009, to 410 elderlies in rural areas. As a results, The prevalence of cognitive functions among all subjects were 80.5% of severe cognitive impairment, 11.7% of mild cognitive impairment, and 7.8% of normal. The prevalence of cognitive functions were lower according to the higher grade of long-term care insurance. And the prevalence of cognitive functions were lower in the group of lower educational level, unemployed, live alone, lower monthly income, bear for living expenses by sons and daughters and medical aid, lower visual acuity, with urinary incontinence, with amnesia, without regular exercise and lower frequency of going out than their respective counterparts. The hierarchial multiple regression analysis was used to reveal the explanatory powers of factors influencing on the cognitive functions level. Such factors as age, bear for living expenses, visual acuity, urinary incontinence, amnesia, regular exercise and grade of long-term care services were significantly influenced with cognitive functions. especially the factors of health status were significantly influenced with cognitive functions.
Backgrounds/Aims: Central pancreatectomy (CP) is associated with a higher rate of postoperative pancreatic fistula (POPF), and it is less preferred over distal pancreatectomy (DP). We compared the short- and long-term outcomes between CP and DP for low-grade pancreatic neck and body tumors. Methods: This was a propensity score-matched case-control study of patients who underwent either CP or DP for low-grade pancreatic neck and body tumors from 2003 to 2020 in a tertiary care unit in southern India. Patients with a tumor >10 cm or a distal residual stump length of <4 cm were excluded. Demographics, clinical profile, intraoperative and postoperative parameters, and the long-term postoperative outcomes for exocrine and endocrine insufficiency, weight gain, and the 36-Item Short Form Survey (SF-36) quality of life questionnaire were compared. Results: Eighty-eight patients (CP: n=37 [cases], DP: n=51 [control]) were included in the unmatched group after excluding 21 patients (meeting exclusion criteria). After matching, both groups had 37 patients. The clinical and demographic profiles were comparable between the two groups. Blood loss and POPF rates were significantly higher in the CP group. However, Clavien-Dindo grades of complications were similar between the two groups (p = 0.27). At a median follow-up of 38 months (range = 187 months), exocrine sufficiency was similar between the two groups. Endocrine sufficiency, weight gain, SF-36 pain control score, and general health score were significantly better in the CP group. Conclusions: Despite equivalent clinically significant morbidities, long-term outcomes are better after CP compared to DP in low-grade pancreatic body tumors.
Purpose: Recently, Neurofeedback training system that based on biofeedback of brain wave was introduced. This study was performed to identify the effects of the improvement of brain function by Neurofeedback training on elders(the 2nd or 3rd grade of long-term care insurance services). Methods: A quasi-experimental design using a nonequivalent control group, pre-post test was used. Total 11 elderly were enrolled in this study (experimental group 5, control group 6). The intervention was conducted 3 times a week for 30 minutes from January to June, 2012 (total 60 times). Chi-square test and Mann-Whitney U-test were used to analyze the data. Results: After the Neurofeedback intervention, attention quotient (AQ), anti-stress quotient (ASQ), emotion quotient (EQ) and brain quotient (BQ) of the experimental group were significantly better than those of the control group. Conclusion: The findings indicate that the Neurofeedback training program was effective in reducing fatigue by AQ, increasing the physical and mental stress resistance by ASQ, emotional balance by EQ and improving of total brain function by BQ. Therefore Neurofeedback training be used as an effective training intervention for the health of elderly in geriatric facility.
This study develops an indicator to manage regional variations of approval rates for long-term care (LTC) service. We used LTC insurance data for grade assessment that include 433,155 applicants from 227 LTC centers across Korea in 2015. The approval rate for each center was defined as the proportion of the numbers of approved applicants out of all applicants. We assumed that the approval rates depended on the characteristics of applicants. We estimated the 'standard' approval rates from a multiple linear regression analysis using the characteristics of applicants as independent variables. The difference between the observed and the standard rates was then defined as an indicator for deviation. A center having a large difference could be considered as a center with a potential error in grade assessment. We also examined if the characteristics of investigators affected the approval rates. We found that the socio-demographic characteristics of applicants and reapplication rate for LTC grade were independent factors affecting the approval rates. Centers having the management indicator values falling outside the middle 95% of the distribution were identified as centers with an error in grading. We expect that this study will contribute to enhancing reliability and equity in LTC grading.

Objectives: This research has executed a new oral health promotion program among the elderly residents of a long-term care center, which purpose was to verify its effectiveness of oral health promotion through the improvement of their oral function. Methods: This study has selected the elderly over the age of 65, capable of communication, who use a long-term care center over the period of two months between July and September 2014. The subjects who remained until the final analysis numbered 50 excluding the dropouts during the program session (experimental: 33, control : 17). The oral stretching program was exercised two days a week, for total of two months. Each function was assessed by the standardized methods and measurement equipment. Also the sum of each function was converted into the oral health grade. Results: The oral function score of the experimental group also showed a statistically significant difference after the execution of the program, where the oral function score of experimental group increased $6.70{\pm}1.30$ from $4.95{\pm}0.89$ after the execution of the program (p<0.05), while the comparison group showed no valid statistical difference with the score result of $5.00{\pm}0.87$ down from $5.11{\pm}0.93$ after the execution of the program (p>0.05). Conclusions: Therefore if the oral health promotion program is reflected to the welfare policy in the future, it can be said that it contributes to the improved health status of the elderly who reside in the long-term care centers.
Between May 1991 and August 1993, 16 patients underwent repair of complete atrioventricular septal defect without another major anomaly at Cardiovascular Center,Yonsei University College of Medicine. Ages of the patients ranged from 3 months to 38 years with a mean of 42 months. Among 16, 10 patients[63%] are associated with Down`s syndrome. All patients underwent primary repair except and one who received had been repaire of coactation of aorta and patent ductus arteriosus 2 month before. Preoperative mitral valve regurgitation [MR] was evaluated with Doppler echocardiography and angiography which revealed absent or grade I in 1, grade II in 8, grade III in 4, and grade IV in 3. Operative technique was performed under the moderate hypothermic cardiopulmonary bypass with crystalloid cardioplegia. Intraoperative echocardiography was performed epicardial approach [n=7] in the operative table or transthoracic approach [n=9] at intensive care unit. In all patients except 3, MR were improved. But in 3 patients, was not improved or exagerated comparing preoperative one. All of them were died.One patient was showed MR grade IV in intraoperative echocardiography, we re-repaired atriventricular valve with cardiopulmonary bypass. During follow-up period [at a mean of 11 months after repair], doppler echocardiography was performed in all patients. The follow up echocardiography revealed that the degree of MR in immediate postoperative period was not changed except in two patients in whom it was aggravated. Thus it seems that intraoperative and early postoperative echocardiography was employed important role of survival and can be predictable for long term results.

Patients with disabilities are a significant segment of Korea population. However, they encounter more challenges when seeking dental care. So, dentists and students who are future dental providers should have a responsibility to provide oral care services for them. In this study, we hypothesize that the education of special care dentistry may influence positive attitudes of the disabled. The attitudes are influenced by demographic variables such as gender, religion, experience of volunteer activities and marital status. Therefore, the objective of this study was to explore the change of attitudes of students towards people with disabilities between before and after class of special care dentistry. 72 students enrolled at the Kyung Hee University were surveyed. All participants attended class of Special Care dentistry in the third grade. The survey was conducted two times, just before class and after class. The attitudes of students were assessed using the well-documented ATDP (Attitude Toward Disabled Person)-O(Original) scale and MAS (Multidimensional Attitudes Scales Toward Persons with Disabilities) scale through the methodology of questionnaires. The results indicated that gender, experience of volunteer activities and marital status did not affect their ATDP and MAS score. In religious affiliation, only, there were statistically significant difference in student attitudes between before and after class of Special care dentistry. It means that habitual religious activity may offer stronger effects on student's attitude than short-term education. Therefore, long-term and specific educational experiences are needed to promote positive attitudes for people with disability.
Korea currently has the world's lowest birth rate but a rapidly inreasing number of preterm infants. The Korean Neonatal Network (KNN), launched by the Korean Society of Neonatology under the support of Korea Centers for Disease Control, has collected population-based data for very low birth weight infants (VLBWIs) born in Korea since 2013. In terms of the short-term outcomes of VLBWIs born from 2013 to 2016 registered in the KNN, the survival rate of all VLBWIs was 86%. Respiratory distress syndrome and bronchopulmonary dysplasia were observed in 78% and 30% of all VLBWIs, respectively. Necrotizing enterocolitis occurred in 7%, while 8% of the VLBWIs needed therapy for retinopathy of prematurity in the neonatal intensive care unit (NICU). Sepsis occurred in 21% during their NICU stay. Intraventricular hemorrhage (grade ≥III) was diagnosed in 10%. In terms of the long-term outcomes for VLBWIs born from 2013 to 2014 registered in the KNN, the post-discharge mortality rate was approximately 1.2%-1.5%, mainly owing to their underlying illness. Nearly half of the VLBWIs were readmitted to the hospital at least once in their first 1-2 years of life, mostly as a result of respiratory diseases. The overall prevalence of cerebral palsy was 6.2%-6.6% in Korea. Bilateral blindness was reported in 0.2%-0.3% of VLBWIs, while bilateral hearing loss was found in 0.8%-1.9%. Since its establishment, the KNN has published annual reports and papers that facilitate the improvement of VLBWI outcome and the formulation of essential healthcare policies in Korea.
The purpose of this study was to analyze factors affecting malnutritional status of elders with dementia in facilities. The subjects were 140 elders with dementia residing in three nursing facilities in Chungnam, South Korea. Data were collected from May 30 to September 30 in 2016, using questionnaires. The collected data were tested by t-test, ANOVA, correlation, regression analysis. The malnutritional status of elders with dementia in facilities was high in the high-risk group (84, 60.0%), and the nutritional status was appeared to be related to sex, long-term care grade, Korean version-Activities of Daily Living and feeding difficulty and cognitive function. As a result of multiple regression analysis, significant influence variables on malnutritional status were Activities of Daily Living(${\beta}=0.379$, p=.001), feeding difficulty(${\beta}=0.264$, p=.001), cognitive function(${\beta}=-0.187$, p=.014) and these variables showed an explanatory power of 35.9% on malnutritional status. The results of this study are expected to be used as basic data for the development of malnutritional status improvement program for elders in facilities with dementia.
