도시하천은 급속한 경제성장과 개발 등 여건변화로 본연의 모습과 가치를 잃고 인공적인 모습으로 변해왔다. 이로 인해 도시민의 삶의 질과 만족도 또한 하락되고 있다. 본 연구에서는 부산의 대표적 도시하천인 수영강을 대상으로 도시하천 수질 개선을 위한 도시민의 지불의사액을 추정하였다. 비시장가치 추정을 위해 조건부가치측정법(Contingent Valuation Method, CVM)을 적용하여 수질개선을 위한 지불의사액(Willingness To Pay, WTP)를 파악하고 경제적 가치를 추정하였다. 가상시나리오는 EPA, 수질사다리(waterquality ladder)의 개념을 이용하여 현재 수영강 수질이 오염된 물(4등급)이라고 가상시장을 설정하고 낚시가 가능한 수준(2등급)까지 개선을 시키고자 할 때의 지불의사를 조사하였다. 이때 지불수단은 기부금의 형태로 지불기간은 연 1회 향후 5년간으로 설정하였다. WTP는 한국개발연구원(Korea Development Institute, KDI)에서 제시하고 있는 로지스틱 분포와 스파이크 모형을 사용하여 추정하였다. 지불의사 추정결과 수영강 인접 지역주민의 지불의사가 상대적으로 높게 추정되었다. 또한 부산시민의 절반 이상(56.5%)이 '자연 및 생태보전'을 수영강의 주요 기능으로 인지하고 있음을 고려할 때, 하천환경개선을 위한 지불의사의 상승은 도시하천의 가치 향상을 위한 정책적 의사결정 기준으로 작용할 것으로 판단된다. 또한 도시민의 삶의 질 향상 기대수준 충족과 더불어 도시하천 경제적 가치상승을 위한 재원 조달 방안을 마련할 수 있을 것이다.
최근 소득수준 향상, 유연근무제 확산 등 다양한 이슈로 인하여 산림휴양복지에 대한 수요 및 이용 패턴의 변화가 발생하고 있다. 이에, 산림휴양·복지 시장 전반에 대한 세분화를 통해 각 유형별로 어떤 특성이 있는지 규명하고, 이에 따른 유형별 차별화된 정책 마련이 필요한 시점이다. 본 연구는 잠재계층분석을 통하여 산림휴양·복지 활동 경험 유형을 4개의 그룹(소극적 이용형, 보통형, 적극적 애호가형, 무관심형)으로 세분화하고, 다항로짓모형을 이용하여 그룹 간 차이의 원인을 인구사회경제학적 특성을 통해 살펴본다. 연구 결과에 기반하여 3가지 정책적 시사점을 도출하였다. 첫째, 산림휴양 복지를 경험하는 그룹이 세분화되어있다는 것이다. 둘째, 산림활동 경험 유형별 그룹을 구분 짓는 사회경제적 특성이 규명되었다는 것이다. 셋째, 산림휴양복지 경험을 높일 수 있는 정책적 대상 및 특성을 규명하였다는 것이다. 본 연구는 그룹별 차별화된 정책을 제시하고, 바람직한 그룹으로 이동할 수 있도록 정책수단을 제시한다는 점에서 의의가 있다.
항만의 역할이 변화하고 그 기능이 다양화되면서 항만배후부지의 역할과 기능은 부가가치 창출뿐만 아니라 효율적인 SCM구축을 위한 국제물류의 거점으로서 그 기능과 역할이 확대되었다. 부가가치 물류활동에 대한 항만배후단지 입주 및 항만배후부지 내 부가가치물류 활동을 통한 이익 창출형 사업모델(Biz-model) 개발에 대한 기업의 관심이 증가하고 있다. 따라서 본 연구는 항만배후단지 내 국제물류센터의 입지경쟁력 평가요인을 도출하였다. 또한 요인분석 및 AHP 방법론 등 기존 연구들의 문제점을 보완하기 위해 IPA 기법을 적용하여 각 속성의 중요도와 만족도를 동시에 비교하고 분석한다. 분석 결과 19개의 평가요인은 지원정책, 성장잠재력, 시설활용, 시장접근성, 연결성 등을 포함하는 5개의 상부요인으로 분류되었다. IPA 기법을 통한 분석 결과 사분면에 속해있는 세부내용들은 항만과 항만의 배후단지를 연결하는 연결성, 시장접근성, 지원정책 등이 입주결정에 중대한 요인으로 작용하였으며, 정책적인 지원들은 우선 시정 노력이 필요하다고 밝혀졌다. 추가적으로, 연구결과는 사분면 내 세부 내용들에서 중요도는 높지만 만족도가 낮은 영역인 복합운송체계 구축 수준 및 효율성, 그리고 지원정책을 구성하는 인센티브 프로그램, 홍보 및 마케팅 지원, 입주 시 금융지원정책, 자유무역 시스템 및 관련 법규 등에 대한 집중적인 우선 시정 노력이 필요하다는 점을 시사한다.
Background: Since November 2019, long-term care hospitals have been able to provide patients with discharging programs to support the elderly in the community. This study aimed to identify both patient- and hospital-level factors that affect successful community discharge from long-term care hospitals. Methods: A multilevel logistic regression model was performed using hospitals as a clustering unit. The dependent variable was whether a patient stayed in the community for at least 30 days after discharge from a long-term care hospital. As for the patient-level independent variables, an agreement between a patient and the family about discharge, length of hospital stay, patient category, and residence at discharge were included. The number of beds and the ratio of long-stay patients were selected for the hospital-level factors. The sample size was 1,428 patients enrolled in the discharging program from November 2019 to December 2020. Results: The number of patients who were discharged to the community and stayed at least for 30 days was 532 (37.3%). The intraclass correlation coefficient was 22.9%, indicating that hospital-level factors had a significant impact on successful community discharge. The odds ratio (OR) of successful community discharge increased by 1.842 times when the patients and their families agreed on discharge. The ORs also increased by 3.020 or 2.681 times, respectively when the patients planned to discharge to their own house or their child's house compared to those who didn't have a plan for residence at discharge. The ORs increased by 1.922 or 2.250 times when the hospitals were owned by corporate or private property compared to publicly owned hospitals. The ORs decreased by 0.602 or 0.520 times when the hospital was sized over 400 beds or located in small and medium-sized cities compared to less than 200 bedded hospitals or located in metropolitan cities. Conclusion: The results of the study showed that the patients' and their family's willingness for discharge had a great impact on successful community discharge and the hospital-level factors played a significant role in it. Therefore, it is important to acknowledge and support long-term care hospitals to involve active in the patient discharge planning process.

BACKGROUND/OBJECTIVES: This study examined the relationship between famine exposure in early life and the risk of type 2 diabetes in adulthood during the 1959-1961 Chinese Famine. SUBJECTS/METHODS: A total of 3,418 individuals aged 35-74 years free of diabetes from two studies in 2006 and 2009 were followed up prospectively in 2009 and 2012, respectively. Famine exposure was classified as unexposed (individuals born in 1962-1978), fetal exposed (individuals born in 1959-1961), child exposed (individuals born in 1949-1958), and adolescent/adult exposed (born in 1931-1948). A logistic regression model was used to assess the relationship between famine exposure and diabetes after adjustment for potential covariates. RESULTS: During a three-year follow-up, the age-adjusted incidence rates of type 2 diabetes were 5.7%, 14.5%, 12.7%, and 17.8% in unexposed, fetal-exposed, child-exposed, and adolescent/adult-exposed groups, respectively (P < 0.01). Relative to the unexposed group, the relative risks (95% confidence interval) for diabetes were 2.15 (1.29-3.60), 1.53 (0.93-2.51), and 1.65 (0.75-3.63) in the fetal-exposed, child-exposed, and adolescent/adult-exposed groups, after controlling for potential covariates. The interactions between famine exposure and obesity, education level, and family history of diabetes were not observed, except for the urbanization type. Individuals living in rural areas with fetal and childhood famine exposure were at a higher risk of type 2 diabetes, with relative risks of 8.79 (1.82-42.54) and 2.33 (1.17-4.65), respectively. CONCLUSIONS: These findings indicate that famine exposure in early life is an independent predictor of type 2 diabetes, particularly in women. Early identification and intervention may help prevent diabetes in later life.
본 연구의 목적은 한국 암환자들의 근력운동 빈도, 근력, 그리고 건강관련 삶의 질과의 관계를 조사하는 것이다. 이를 위해 2014~2016년 국민건강영양조사 자료를 통합하여, 총 647명의 암환자들의 주당 근력운동 빈도, 악력, 건강관련 삶의 질의 관계를 복합표본 일반선형모형과 로지스틱 회귀분석 방법을 이용하여 분석하였다. 분석결과, 근력운동빈도가 주당 0~1일인 암환자들은 5일 이상인 암환자들에 비해(0.852±0.016 vs. 0.890±0.020 ; p=0.006), 상대 악력이 낮은 암환자들은 높은 그룹에 비해 (0.850±0.018 vs. 0.911±0.016; p<0.001) 건강관련 삶의 질 지수 (EQ-5D index) 가 유의하게 낮게 나타났다. 또한, 주당 근력운동 빈도가 0~1일인 암환자들은 5일 이상인 암환자들에 비해 운동능력에 문제가 생길 확률이 4.07배 (95% CI=1.31-12.63), 상대 악력이 낮은 그룹은 높은 그룹에 비해 운동능력에 문제가 생길 확률이 4.94배 (95% CI=2.14-11.41), 일상생활에서는 5.18배 (95% CI=1.56-17.14), 통증/불편에서는 2.46배 (95% CI=1.33-4.55) 증가했다. 본 연구결과를 통해 암환자들의 근력운동 빈도와 근력이 건강관련 삶의 질과 유의한 관계에 있는 것으로 나타났으며, 꾸준한 근력운동으로 인한 근력향상은 암환자들의 삶의 질 관리에 큰 도움이 될 것으로 사료된다.
노인우울은 유병율이 높을 뿐만 아니라 상당히 심각한 신체적, 심리사회적, 경제적 부담을 초래함에도 불구하고, 노화의 과정으로 또는 신체질환의 일부로 여겨지는 가운데 개입이 지연되고 있다. 건강문제를 해결하기 위한 시도가 일반적으로 당사자나 주변인들의 문제 인식에서 비롯되는 점에 비추어 볼 때, 우울증상에 대한 정확한 식별은 자기 자신은 물론 주위 노인들의 증상에 대한 조기발견과 개입을 촉진하는데 도움이 될 수 있다. 이에 본 연구에서는 대도시에 거주하는 노인 104명에 대한 면접 설문조사를 통해 우울증상에 대한 노인들의 식별력과 증상의 개념화 방식을 탐색하고 이에 영향을 미치는 요인들에 대한 분석을 실시했다. 연구결과, 우울증상을 식별할 수 있었던 노인은 전체 응답자의 14%에 불과했으며, 그 밖에는 외로움, 정서적 문제, 경제적 문제 등으로 인식했다. 75세 이상의 나이, 주된 정신건강정보원이 대중매체인 경우 식별력이 감소하는 것으로, 정신건강 지식이나 정보의 주된 원천이 정신건강 관련교육인 경우 식별력 증진에 도움이 되는 것으로 나타났다. 이와 같은 결과에 기초할 때, 노인의 우울증상 식별력을 증진시키기 위한 전략으로 정보지 비치나 일회성 행사위주의 정보전달보다 면대면의 구체적 정보제공과 상담 방식이 효과적일 것으로 보이며, 대중매체의 정신건강 및 질환 콘텐츠에 대한 면밀한 검토에 초점을 둔 후속 연구도 필요할 것이다.
빈곤에는 인적자본 및 가족구조 등의 물질적 결핍 뿐 아니라 심리사회적 측면이 주요하게 관련되어 있음이 널리 제기됨에도 불구하고, 국내에서는 이에 대한 관련 연구성과들은 미흡한 실정이다. 이에 주목하여, 본 연구에서는 빈곤층의 심리사회적 측면이 빈곤지위의 결정 및 이의 변화에 어떠한 관련성을 지니는지를 정신건강 변수들을 중심으로 분석하고자 하였다. 분석에서는 한국복지패널 1~4차년도 자료를 활용하여, 우울 및 자아존중감 등의 정신건강 요인과 빈곤지위의 관계를 다양한 측면에서 분석하였는데, 주요 분석결과들은 다음과 같다. 첫째, 빈곤층과 비빈곤층의 정신건강 수준은 유의미한 차이가 있었다. 빈곤층 내부에서도 빈곤이 심각할수록 정신건강이 좋지 못한 것으로 나타나, 빈곤과 정신건강의 부적(-) 관계를 확인할 수 있었다. 둘째, 빈곤탈출 집단과 빈곤지속 집단의 정신건강 수준도 유의미한 차이를 보여, 정신건강이 빈곤지위 변화와도 밀접한 관련성을 지니고 있음을 확인할 수 있었다. 셋째, 빈곤경험 양상은 정신건강에 따라 상당한 차이가 있는 것으로 분석되었는데, 정신건강수준이 양호하지 못할수록 빈곤 경험율 및 빈곤지속의 경험이 높고, 빈곤경험기간도 긴 것으로 확인되었다. 또한, 이들의 빈곤 탈출율은 낮은 반면 빈곤 지속율은 높은 것으로 나타났다. 넷째, 여타 관련 변수들을 통제한 모델에 대한 분석에서는 정신건강이 빈곤지위 및 빈곤지위 변화에의 주요 결정 요인의 하나로 확인되었다. 본 연구 결과는 빈곤지위 및 이의 변화에 정신건강의 주요한 영향을 보여주는 바로, 우리사회의 빈곤문제에 효과적으로 대처하기 위해서는 물질적 결핍에 대한 지원에만 치중된 정책적 접근에서 벗어나 빈곤층의 정신건강 문제에 대한 적극적인 개입 및 지원이 요청됨을 정책적 함의로 시사하여 준다.
롤 모델은 개인의 직업 혹은 경력선택에 영향을 주기도 한다고 알려져 있다. 창업을 선택함에 있어서 롤 모델의 긍정적 영향은 많은 연구를 통해 이미 밝혀진 바가 있다. 롤 모델로는 혈연으로 연결된 가족 구성원인 부모형제 및 친척뿐만 아니라 사회적 관계로 만난 지인을 롤 모델로 선정한다고 한다. 본 연구에서는 자기이외에 롤 모델이 없는 창업자들과 롤 모델이 있는 창업자들로 구분하였다. 그리고 롤 모델이 있는 창업자들과 롤 모델이 없는 창업자들 간에 개인속성, 기업가정신 요인들, 학습지향성의 차이를 검증하고자 하였다. 또, 부모형제 및 친척 롤 모델을 강한 유대의 롤 모델로 지인 롤 모델을 약한 유대의 롤 모델로 구분하였다. 강한 유대 롤 모델을 가진 창업자들과 약한 유대 롤 모델을 가진 창업자들 간에 개인속성, 혁신성, 진취성, 위험감수성, 학습지향성의 차이를 검증하고자 하였다. 이를 위해 요인분석, t-검증 및 로지스틱 회귀분석을 실시하였다. 본 연구를 통해 밝혀진 실증분석 결과는 다음과 같다. 첫째, 롤 모델이 없는 창업자들 중 여성의 비율이 높은 것으로 나타났다. 둘째, 약한 유대의 롤 모델을 가진 창업자들이 강한 유대의 롤 모델을 가진 창업자들에 비해 큰 규모의 창업기업을 운영한다는 것이 밝혀졌다. 셋째, 약한 유대의 롤 모델을 가진 창업자들이 강한 유대의 롤 모델을 가진 창업자들에 비해 학습지향성이 더 높은 것으로 나타났다. 넷째, 로지스틱 회귀분석을 통해 살펴본 결과 롤 모델의 유무에 대한 영향은 성별, 학습지향성, 위험감수성, 창업규모 순인 것으로 나타났다. 다섯째, 로지스틱 회귀분석 결과에서 롤 모델의 유형에 대한 영향은 학습지향성, 창업규모의 순인 것으로 나타났다.

Objective: To extract pulmonary and cardiovascular metrics from chest CTs of patients with coronavirus disease 2019 (COVID-19) using a fully automated deep learning-based approach and assess their potential to predict patient management. Materials and Methods: All initial chest CTs of patients who tested positive for severe acute respiratory syndrome coronavirus 2 at our emergency department between March 25 and April 25, 2020, were identified (n = 120). Three patient management groups were defined: group 1 (outpatient), group 2 (general ward), and group 3 (intensive care unit [ICU]). Multiple pulmonary and cardiovascular metrics were extracted from the chest CT images using deep learning. Additionally, six laboratory findings indicating inflammation and cellular damage were considered. Differences in CT metrics, laboratory findings, and demographics between the patient management groups were assessed. The potential of these parameters to predict patients' needs for intensive care (yes/no) was analyzed using logistic regression and receiver operating characteristic curves. Internal and external validity were assessed using 109 independent chest CT scans. Results: While demographic parameters alone (sex and age) were not sufficient to predict ICU management status, both CT metrics alone (including both pulmonary and cardiovascular metrics; area under the curve [AUC] = 0.88; 95% confidence interval [CI] = 0.79-0.97) and laboratory findings alone (C-reactive protein, lactate dehydrogenase, white blood cell count, and albumin; AUC = 0.86; 95% CI = 0.77-0.94) were good classifiers. Excellent performance was achieved by a combination of demographic parameters, CT metrics, and laboratory findings (AUC = 0.91; 95% CI = 0.85-0.98). Application of a model that combined both pulmonary CT metrics and demographic parameters on a dataset from another hospital indicated its external validity (AUC = 0.77; 95% CI = 0.66-0.88). Conclusion: Chest CT of patients with COVID-19 contains valuable information that can be accessed using automated image analysis. These metrics are useful for the prediction of patient management.

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
