A case report and literature review of osseous choristoma on the tongue. (혀에서 골성 이소종(osseous choristoma)에 대한 증례 보고 및 문헌 고찰)

  • Choristoma is a growth of normal tissue that occurs at an abnormal region. In the oral cavity, osseous choristoma that has a normal bone feature mainly has been appeared. It is mainly occurred on the tongue. So, It can be found in dental practice or in discomfort in the mouth. Osseous choristoma in the oral cavity is extremely rare. But It can be treated excisional biopsy under local anesthesia. The prognosis is very good. This case is a report of osseous choristoma on the dorsal surface of the tongue. A 17-year-old female patient presented with a foreign body sensation of the tongue. A diameter of the mass is 0.7cm, and there is no pain. Panoramic radiographs showed a radiopaque image in the form of a double image. An excisional biopsy was performed and diagnosed as an osseous choristoma. 12 months after the resection, there were no signs of recurrence. Intraoral osseous choristoma is very rare, but it is clinically very simple and can be found in dental radiographs. We present this case because of Dentists who examine the oral cavity of the patient can easily diagnose the lesion and treat it by simple resection.

  • The Neurofibroma is a benign neoplasm originating from neural tissue such as Schwann cell, fibroblast and peripheral fibroblast, which rarely converts to malignancy. About 25-45% of neurofibroma are located in head and neck, and the most common sites are lateral cervical region and mouth. Because of its high vascularity and invasion to the adjacent organs, surgical resection may be confronted and limited, moreover large neurofibroma can cause large defect that needs various kinds of flap. The factors related to clinically suspected malignancy are painlessness, abrupt increase in size, firm fixation, and central ulceration, although the malignancy rate of neurofibroma is low. The complete resection was failed several times at local clinic, the patient was transferred to our hospital. We have successfully removed the retroauricular neurofibroma using transposition flap & free skin graft on its large defects.

  • Matching spots between two sets of 2-dimensional electrophoresis can make it possible to find out the generation, extinction and change of proteins. Generally protein spots are separated by 2-dimensional electrophoresis. This process makes the position of the same protein spot a little different according to the status of the tissue or the experimental environment. Matching the spots shows that the relation of spots is non-uniform and non-linear transformation. However we can also find that the local relation preserves the topology. This study proposes a matching method motivated by the preservation of the topology. To compare the similarity of the topology, we compared the distance and the angle between neighbour spots. Experimental result shows that the proposed method is effective.

  • The vascular changes in periodontal tissues cause local hypoxia which seems to affect the periodontal tissue cells. Abrupt changes in oxygen availability within the periodontium have been suggested to have a regulatory role in alveolar bone remodeling during tooth movement, bone growth or fracture healing. The purpose of this study was to study the effects of hypoxia on formation of osteoclast responsible for bone resorption, in vitro. Primary mouse bone marrow cells were cultured in normoxic (20% $O_2$) and hypoxic (1% $O_2$) conditions and assayed for cellular proliferation. The results obtained were as follows : 1. Reducing oxygen tension increased the formation of multinucleated osteoclasts. 2. Hypoxic stimulus increased the size of mature osteoclasts.

  • Synovial chondromatosis (SC) in temporomandibular joint is a rare benign disorder characterized by cartilaginous metaplasia of the mesenchymal remnants of the synovial tissue. The etiology of the disease is unclear but may be associated with trauma, overuse, local infection, and embryologic disturbance. SC does not spontaneously resolve and respond to nonsurgical treatment. SC should be differentially diagnosed with other temporomandibular joint disorders such as arthralgia or osteoarthritis because surgery should be done for managing it. However, primary diagnosis of SC is not easy because of nonspecific symptoms and signs. For the patients with unsuccessful conservative treatment response, especially accompanied by crepitus, preauricular swelling or posterior open bite, computed tomography/cone-beam computed tomography or magnetic resonance imaging should to be performed to exclude SC. We discussed the importance of the early diagnosis and surgical treatment of SC from this case.

  • Urticaria is a common disease and a well known reaction pattern of the skin characterized by erythema or wheal and edema, that is result of a local leakage of plasma from minute vessel into the connective tissue of the dermis. The urticaria that lasts for more than six of eight weeks, is categorized as chronic urticaria. In Oriental medicine, urticaria is classified as Yeun-jin(慇疹) and similar to Poong-yeun-jin(風慇疹), Poong-so-yeun-jin(風瘙慇疹), Poong-yum-leu, Poong-dan(風丹) and Poong-jin-geu(風疹塊). We report four improved cases of chronic urticaria that had lasted for more than eight weeks. We regarded their chronic urticaria as wind-heat(風熱) and prescribed Bangpungtongsungsan-gami(防風通聖散加味) and Seunggaltang-gami(升葛湯加味) to these patients. These patients showed improvement of chronic urticaria.

  • Kimura's disease is uncommon and chronic inflammatory disease of unknown etiology. It manifests as solitary or multiple subcutaneous nodule, primarily located on the cervical area with local adenopathies and salivary gland hypertrophy. Its clinical course is benign nature. Histologic findings are hyperplastic lymphoid tissue, inflammatory infiltrates rich in eosinophils and proliferations of capillary venules. Treatment options are steroid therapy, radiation therapy and surgical excision. We report two cases of Kimura's disease on medial aspect of upper arm.

  • The epithelioid sarcoma is a rare high grade soft tissue sarcoma that affects young usually male, adults or adolescents. The most common localization is the hand and the forearm, followed by the leg and the foot. In most cases, the sarcoma grows slowly, involves the dermis, subcutis, or deeper soft tissues in the distal extremities. Epithelioid sarcoma is a slowly growing tumor with a high propensity for local recurrences and lymph node metastases. This neoplasm is likely to be confused with a variety of benign and malignant conditions. The treatment consists of wide surgical excision, chemotherapy and radiotherapy. We report the cases of 16-year-old girl with an epithelioid sarcoma on the lower extremity.

  • The purpose of this study was to investigate the neurophysiological mechanisms, assessment of muscle spasm and treatments that could apply to clinical field. Spasm is a painful pattern of contraction of muscle caused by chronic or acute trauma, excessive tension, or organic disorders. Aside from pain, muscle spasm creates shortenning of muscles and limits motion. Untreated spasm and protective immobility due to pain lead to decreased local blood flow in the muscles and result in a vicious cycle of muscle spasm and paul. The assessment of muscle spasm involve muscle tone assessment, tissue compliance, and joint ROM. Each of these aessessments utilize as a part of the patient's condition Clinical managements involve drug management and physical therapy. Expecially, physical therapy is one of the most important techniques to reduce muscle spasm. Physical therapy includes applying heat and cold, electrical stimulation, massage, and traction. This investigation should entourage phisical therapists to experiment further with various techniques to reduce muscle spasm.

Treatments of Phyllodes Tumor in Breast and Immediate Breast Reconstruction (유방에 발생한 엽상종양의 치료 및 즉시 유방재건)

  • Purpose: There have been limited reports on breast reconstruction after excision of phyllodes tumor. This paper reports four patients who had immediate reconstruction of the breast following excision of phyllodes tumor. Methods: We retrospectively reviewed the medical records of 14 patients from March 2000 to March 2008. Clinical data were analyzed including age, presenting symptoms and signs, type of surgery and metastasis. Results: The mean age was 38.6 years. The mean follow-up period was 40.6 months. Reconstruction was performed with latissimus dorsi musculocutaneous flap in 3 patients and transverse rectus abdominis musculocutaneous flap in 1 patient. Other cases were covered with skin graft or primary repair. 2 local recurrent cases were noted. Conclusion: The breast affected by phyllodes tumor must undergo complete excision. Followed by mastectomy, immediate reconstruction of breast improved cosmetic results, and allowed a wider surgical excision margin of tumor.
