Free Toe Tissue Transfer using Infiltration Method of Local Anesthetic Agent (국소 침윤마취법을 이용한 족지조직 유리 피판술)

  • This study was designed to introduce free toe tissue transfer using infiltration method of local anesthetic agent. Four toe pulp tissues were transferred to reconstruct finger tip defect in four patients who were not suitable for general anesthesia. Two flaps taken from the lateral side of the great toe was used for reconstruction of thumb defect and two flaps from the medial side of the second toe for resurfacing of the index and fifth finger. Flap sizes were various from $2.0{\times}2.0\;cm^2$ to $1.6{\times}4.0\;cm^2$. Anesthesia was induced by infiltration of 2% lidocaine hydrochloride (with 1:100,000 epinephrine) with dilution of normal saline in same volume unit, as like as in ordinary digital block. All vessels were anastomosed within 2 cm of distance from the proximal margin of the defect. Whole operative procedures were carried out by one team. All flaps were successfully taken without complication. The average operation time was 4 hours 10 minutes. The amount of anesthetic agent used in whole operative procedures was roughly 4 mL in the toe, 8 mL in the finger, and 12 mL totally. In conclusion, free toe tissue transfer using infiltration method of local anesthetic agent would be good strategy for finger tip reconstruction in the patient not suitable for general anesthesia.

  • This paper explains an error process for multimedia collaboration works with session management running on vehicle network environment. This system consists of an FDA and FRA. FDA is an agent that detects an error by hooking techniques for multimedia system based on vehicle network environment with session management. FRA is a system that is suitable for recovering software error for multimedia system with session management based on vehicle network environment. This paper describes only FRA. When multiple local sessions is opened, each local session manager sends information of participant to global session manager and take current information about session of processing in network.

  • This paper describes the development of the pilot plant of distributed intelligent management system for a microgrid. For optimal control and management of microgrids, intelligent agents area applied to the microgrid management system. Each agent includes intelligent algorithms to make decisions on behalf of the corresponding microgrid entity such as distributed generators, local loads, and so on. To this end, each agent has its own resources to evaluate the system conditions by collecting local information and also communicating with other agents. This paper presents key features of the data communication and management of the developed pilot plant such as the construction of mesh network using local wireless communication techniques, the autonomous agent coordination schemes using plug-and-play functions of agents and contract net protocol (CNP) for decision-making. The performance of the pilot plant and developed algorithms are verified via real-time microgrid test bench based on hardware-in-the-loop simulation systems.

  • Lidocaine is the most commonly used local anesthetic (LA) agent in various dental as well as oral and maxillofacial procedures. Although rare, adverse effects and allergic reactions to lidocaine have been reported. In patients with suspected allergy to LA or a history of such reaction, careful history-taking and allergy testing should be performed to choose an alternative LA agent to avoid any adverse effects. Here, we present two cases of delayed hypersensitivity reaction to lidocaine, wherein the patients presented with erythema, edema, and itching. Intradermal testing confirmed allergic reaction to lidocaine, and the patients underwent successful dental treatment using an alternative LA agent. This report highlights the importance of allergy testing prior to LA use considering the serious consequences of allergy to these agents and describes the management of such patients using an alternative LA agent.

  • Fourth generation mobile communication network have the technology of extensive form include basic service technology and it has been developed from the radio access technology and network topology. Not only fourth generation mobile communication network have basically done new highspeed radio access technology which is suitable to high and low speed environment of transfer, but also it is possible that they have been made for freely vertical handover. ETRI also has made fourth generation mobile communication network which is WiNGS(Wireless Initiative for Next Generation Service) satisfied that demand. This paper is made by lossless handover method through the local retransmission ARQ agent that is one of the main technology of fourth generation mobile communication network. Lossless handover method through local retransmission ARQ agent has been basically made by WiNGS and it was better than original local retransmission of layer by simulation.

  • This paper proposes efficient methods that integrate and execute local plan rules of task agents in a multi-agent environment. In these methods, each agent's plan rules are represented in a network structure, and these networks are then collected by a single task agent to build a integrated domain network, which is exploited to achieve the goal. Agent problem solving by using the domain network enables a concurrent execution of plan rules that are sequential in nature.

  • Topical anesthetics are commonly used in oral & maxillofacial surgery to control pain in the oral cavity mucosa before local anesthetic injection. These anesthetic agents come in many forms, developed for different usages, to minimize adverse reactions, and for optimal anesthetic efficiency. Earlier studies have revealed that these agents may also limit the growth of microorganisms in the area of anesthetic application. Many topical anesthetic agents show different levels of antimicrobial activity against various bacterial strains and Candida. The dosage of local anesthetic agent used in some clinical preparations is too low to show a significant effect on microbial activity. Efficiency of antimicrobial activity depends on the local anesthetic agent's properties of diffusion within the bloodstream and binding efficiency with cytoplasmic membrane, which is followed by disruption of the bacterial cell membrane. The antimicrobial properties of these agents may extend their usage in patients to both control pain and infection. To develop the topical local anesthetic optimal usage and antimicrobial effect, a collaborating antiseptic agent may be used to benefit the local anesthetic. However, more research is required regarding minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of topical local anesthetic agents with drug interaction between anesthetics and antiseptic agents.

Mutual Verification of an Analytic Model of a Complex System and Space Syntax Using Network Analyses (네트워크 분석방식 선택에 따른 복잡계 모형과 공간구문론의 상호검증)

  • A social phenomenon that occurs in a physical space is said to be a complex system. However, space syntax, which is commonly employed by researchers to identify such social phenomena, has various limitations in interpreting their complexity. On the other hand, agent-based modeling considers a variety of factors including the personality of the agent, objective-oriented work flows, estimation according to time flows and better prediction of space use through diverse parameters depending the situation, as well as the characteristics of the space. The agent-based method thus has the potentials to be developed as an alternative to space syntax techniques. In particular, discrete event driven simulation(DEVS), which is part of the agent-based modeling method, embraces the concept of networks just like space syntax, which allows a possible theoretical linkage in the future. This study suggests a procedural model of agent-based DEVS reflecting two different connection methods, i.e. connections between adjacent areas and those of the entire space, and attempts to identify the relationship between the local and regional indices of space syntax. A number of spaces were selected as examples-one for a preliminary experiment and eight modified for the main experiment-and space syntax and DEVS were applied to each of them. The comparative analysis of the results led to the conclusions as follows: 1) Adjacent connections were closely related to local indices, while the whole-space approach to regional indices. Local integration shows both characteristics. 2) Observation of the time flow model indicated a faster convergence with the range of 1 to 3-fold of the total time of one lap, with the error of less than 10%. 3) The heat map analysis showed more obvious characteristics of using the space for the entire space rather than adjacent connections. 4) Space syntax shows higher eligibility than ABM.

An Energy Efficient Distributed Approach-Based Agent Migration Scheme for Data Aggregation in Wireless Sensor Networks

  • The use of mobile agents for collaborative processing in wireless sensor network has gained considerable attention. This is when mobile agents are used for data aggregation to exploit redundant and correlated data. The efficiency of agent-based data aggregation depends on the agent migration scheme. However, in general, most of the proposed schemes are centralized approach-based schemes where the sink node determines the migration paths for the agents before dispatching them in the sensor network. The main limitations with such schemes are that they need global network topology information for deriving the migration paths of the agents, which incurs additional communication overhead, since each node has a very limited communication range. In addition, a centralized approach does not provide fault tolerant and adaptive migration paths. In order to solve such problems, we have proposed a distributed approach-based scheme for determining the migration path of the agents where at each hop, the local information is used to decide the migration of the agents. In addition, we also propose a local repair mechanism for dealing with the faulty nodes. The simulation results show that the proposed scheme performs better than existing schemes in the presence of faulty nodes within the networks, and manages to report the aggregated data to the sink faster.

Construction of roadbed with environmental friendly soil amendment agent (친환경 토질개량제를 이용한 도로노반 건설공사에 관한 연구)

  • The purpose of this paper is to study on the construction of roadbed with environmental friendly soil amendment agent. The special amendment agent used in this study is mainly composed of inorganic metal salts such as sodium chloride, magnesium chloride, potassium chloride, calcium chloride,, thus is friendly to the environment, and has a function of soil-cement-agent solidification. Various components of this agent weaken the negative function of humic acid and decompose humic acid itself. Then, the calcium cation of the cement can now be made contact directly to the soil surface. The project of local road demonstration of roadbed construction with special soil treatment agent was peformed in Northeast Thailand on August 1999 by the sponsor of Highway Department of Thailand. A series of field experiments including unconfined compressive strength were carried out to investigate the physical and mechanical characteristics of solidified roadbed treated by this solidifying agent. The results of this research showed that the roadbed using poor soil could be efficiently constructed by treatment of this amendment agent.
