Factors Influencing Stroke in Community-dwelling Adults : Focusing on Health-related Quality of Life (지역사회거주 성인의 뇌졸중 영향 요인 : 건강관련 삶의 질을 중심으로)

  • Objective : The purpose of this study was to investigate the factors influencing stroke in community-dwelling adults. Methods : This study used raw data from the community health survey in 2016. Among the 228,452 subjects who participated in the survey, 225,003 (98.5%) of them were included in the analysis of this study. The sociodemographic characteristics were selected as gender, age, region, income, marital status, and comorbidity was selected as osteoporosis, hypertension, diabetes, dyslipidemia, myocardial infarction and arthritis. Health-related quality of life was assessed by EuroQol-5 Dimension(EQ-5D) and the subcategory of EQ-5D (mobility, self-care, usual activities, pain/discomfort, anxiety/depression) were included in the analysis. Dependent variables were stroke, and the independent variables were the 16 variables described above. Statistical analysis was performed using binomial logistic regression analysis. Results : In sociodemographic variables, stroke was predicted by men, aging, and lower income levels. In comorbidity variables, stroke was predicted by hypertension, diabetes mellitus, hyperlipidemia, and myocardial infarction. In sub-domains of health-related quality of life, stroke was predicted by self-care, usual activities, mobility, anxiety/depression, and pain/discomfort. Conclusion : These finding suggest that it is needed development of a customized health promotion program for the improvement of self-care and activities of daily living in community-dwelling stroke survivors.

Life Satisfaction of Older Adults using Hierarchical Model Analysis focused on Individual and Community Factors (다층모형을 활용한 노인의 삶의 만족도 분석: 개인적 요인과 지역적 요인의 특성을 중심으로)

  • This study aims to observe the effect of age friendliness of cities on life satisfaction and to suggest ways to improve quality of life of older people. The secondary data sets were used in this study, which were '2014 Survey of Living Conditions and Welfare Needs of Korean Older People.' It's a nationwide data collected by the Korean Institute of Health and Social Affairs. A multilevel analysis model was used to analyze the data because the level of age friendliness has a hierarchical data structure. Results showed as follows: First, life satisfaction of older adults is affected by the level of age-friendliness of cities in which they live. Second, on the personal level, older people showed low life satisfaction when they are more older and have more chronic diseases and more depressed. On the contrary, life satisfaction of older adults increased when they have higher education and income. Third, on the city level, older people showed higher life satisfaction when they live in high employment rate area and participation rate of lifelong education. Cautions should be placed when interpret the result because the variables that represent the characteristics of age friendless of cities were constituted arbitrary. Based on the results, suggestions for improving the city environment age-friendly and implications for social welfare practice were provided.

A Study of Social Network Type among Korean Older Persons: Focusing on Network Size, Frequencies of Contact, and Closeness (한국 노인의 사회적 연계망 유형: 연계망 크기, 접촉 빈도, 친밀도를 중심으로)

  • This study examines 2014 National Survey on Older Koreans (10,279 persons aged 65 or above) to understand social networks among older Koreans. In order to classify the types of social relationships, the latent profile analysis is conducted based on such structural aspects of social networks as network size, frequencies of contact, and closeness. The results show that older Koreans can be categorized into 5 distinct social network types: disengaged (11.6%), ritual-family-focused (17.7%), close-family-focused (23.6%), close-restricted (28,4%), and diverse (18.8%). Characteristics by each social network type are compared and multinominal logistic regression analyses are applied to figure out the influencing factors of social network type. Older persons with disengaged social network ties tend to be overwhelmingly female, living alone, old-old, and of low socioeconomic status. On the other hand, older persons with diverse social network ties tend to be young-old and had highly-ranked jobs in their mid-life. Spouse/children are the focal point of social relationships in both ritual-family-focused and close-restricted social network. However, the proportion of men is higher in the close-family-focused type than in the ritual-family-focused. Older Koreans with close-restricted social network ties tend to be female, uneducated, and have engaged in agricultural and allied activities. This study discusses the implications of its findings and how research in this area should develop in the future.

A study on the factors influence on the family function of persons with mental illness - Comparison of the persons with recent onset and chronic mental illness - (정신질환자 가족기능에 영향을 미치는 요인연구: 발병초기와 만성 정신질환자의 비교)

  • The primary objective of this study is to examine differences between the persons with recent onset and chronic mental illness in their family function; and analyze factors influence on the family function. The independent variables are divided into four sets: socio-economic, clinical, familial, and social characteristics. The subjects of this study are the 628 with mental disorders, and the sample was selected in Pusan and Gyeong-nam area. The findings of this study can be summarized as followed: The family with chronic mental illness showed higher family function in comparison with recent onset. Result of hierarchical multiple regression analysis show that age of onset, difference between ideal and reality in family function variable only influenced family function for the recent onset group, and activities of daily living, difference between ideal and reality in family function, caregiver's health, community support variables influenced family function for the chronic group. The results suggest a need for using different treatment approaches for recent onset versus chronic mentally disabled families. Implications for mental health social work practice for family with disabled are suggested.

A Qualitative Study on Coping strategies of Older adults with Depression: Focused on the Experience of Coping with Depression in Older Adults Living in Seoul (노인의 우울 대처 전략에 관한 질적 연구: 서울시 거주 도시 노인의 우울 대처 경험을 중심으로)

  • The purpose of this study is to explore qualitatively what kind of coping strategy the Korean older adults use when they are depressed and why. Participants were users, older than 60, of an elderly welfare center in Seoul, and answers of 34 respondents who experienced depression were included in the analysis. The collected data were analyzed through content analysis. The results of the analysis showed that strategies to cope with depression used by participants were composed of 6 domains and 11 sub-domains: health behavior (medical approach / exercise and diet), family and social contact (social interaction / going out and going on a trip / communication with family), Religious activities (Religious activities), lifelong education (hobbies / educational activities), productive activities (labor), health risk behaviors (drinking and gambling / resignation). The depression coping strategies of the older adults and their characteristics are as follows. First, older adults used diverse problem-focused coping strategies to cope with depression. Second, older adults considered various coping strategies together and used them simultaneously. Third, the facility space for older adults functions as a shelter. Fourth, although there were cases where medical approach was used, the intention to reuse was very low. Through this study, the following suggestions were made to help older adults cope with depression more successfully. First, access to various coping strategies should be promoted. Second, more places where older adults can spend their time comfortably need to be provided. Third, resistance to mental health care should be resolved.

Depression as a Mediator in the relation between The Socioeconomic Deprivation Life Satisfaction in Older Adults (노인의 사회경제적 박탈과 삶의 만족도의 관계연구: 우울의 매개효과 중심으로)

  • This research aims to examine whether depression has mediating effects in the effects of socioeconomic deprivation, especially of lower variables of socioeconomic deprivation on life satisfaction of the elderly. For this purpose, the 12th year(2017) data of the Korea Welfare Panel Study were used. The mediated effect analysis method of Baron and Kenny(1986) were used, and mediating effect of depression was validated through Sobel Test. The results of this research are as follows. Firstly, the lower variables of socioeconomic deprivation of the elderly -deprivation of housing, deprivation of dietary life, social deprivation, deprivation of employment, and deprivation of healthcare- had partial mediating effects of depression in life satisfaction. It was verified that as the elderly experienced deprivation of housing, deprivation of dietary life, social deprivation, deprivation of employment and deprivation of healthcare, depression increased and life satisfaction was lowered. Therefore, it was suggested that to enhance the living satisfactions of the elderly, policies and services reflecting the personal characteristics of socioeconomic deprivation and depression must be made, and that criteria to identify the multidimensional poverty and deprivation experienced in everyday life must be prepared in addition to poverty focusing on income.

Familism Values and Social Welfare Policy Implications of Young Adults' (청년층의 가족주의 가치관과 사회복지정책적 제언)

  • This study examines the familism values of the young generation. This study was conducted on 436 university students living in Daejeon and Chungcheong area. The results of the study are as follows. First, as the socio-demographic characteristics of the subjects, by gender, There were more female students (56.0%) than male students (44.0%). 34.4% of the respondents were under age 20, followed by 21 ~ 22 years old (30.7%) and 23 ~ 24 years old by 17.0%. As for the grade, 61.9% of the first and second graders were more than the third and fourth graders. In the case of religion, 42.7% of the respondents said that they had no religion, followed by Buddhism (29.1%) and Christianity (19.5%). 69.7% of the respondents said that they had two siblings. Second, when looking at the level of familism values perceived by the survey subjects, the Filial piety (4.35) was the highest, followed by Equality Consciousness of husband-wife (3.51) and Sense of Community in Blood Relationship (2.87), Family-First Consciousness (2.64), and succession of Patrilineal Family (2.41). Third, the overall Familism Values according to socio-demographic variables showed a significant difference between the age groups (F = 11.08, P <.001) and the grade (F = 4.70, P <.01).

Bitcoin(Gold)'s Hedge·Safe-Haven·Equity·Taxation (비트코인(금)의 헷지·안전처·공평성·세제 소고)

  • Btcoin has made a big progress through anonymity, decentralized authority, sharing economy, multi-ledger book-keeping, block-technology and the convenient financial vehicle. Bitcoin has the characteristics of mining and supply by decentralized suppliers, limited supply quantity and the partial money-like function as well as gold. The paper studies the hedge and safe-haven of Bitcoin and gold on daily frequency data over the period of July 20, 2010-Dec. 27, 2017 employing Asymmetric Vector GARCH. It finds that gold has the hedge and safe-haven against inflation and capital markets while Bitcoin has the weak hedge and the weak safe-haven. It shows insignificant effects of inflations of US and Korea on the volatilities of Bitcoin and gold. It also suggests the necessity of clearing of vagueness behind the anonymity for fair and transparent trade through the law application in the absence or fault in law (Lucken im Recht). following the spirit of the living constitution (lebendige gutes Recht oder Vorschrift). The relevant institutions are hoped to be given some of obligations such as registration, minimum required capital. report, disclosure, explanation, compliance and governance with autonomous corresponding rights. The study also suggests the reestablishment of the relevant financial law and taxation law. The hedge would not be successfully accomplished without the vigilant cautions of investors.

Factors Affecting the Quality of Life of Patients With Dementia: A Systematic Review of Literature Focusing on the Subjects Admitted to an Overseas Long-term Care Facility (치매 환자의 삶의 질에 미치는 요인 분석: 국외 요양시설에 입소한 대상자를 중심으로 체계적 문헌고찰)

  • Objective: The purpose of this study was to perform a systematic literature review focusing on international studies to identify various factors affecting the quality of life(QOL) of dementia patients admitted to long-term care facilities. Methods: From January 2000 to July 2018, the articles published in foreign journals were searched through CINAHL and MEDLINE databases. The main search terms were'dementia or Alzheimer's'. 'quality of life', 'long-term care', 'care home', 'nursing home', 'care institution', 'residential care', 'small- scale setting'. The first 1706 articles were searched, but 10 studies were selected using the selection and exclusion criteria. Results: Analysis of ten cross-sectional studies showed that factors such as physical functioning status, ability to perform activities of daily living, and cognition showed a positive correlation with QOL. Depression and anxiety, severity of dementia, and neuropsychiatric symptoms were found to be negatively correlated with QOL. In addition, two longitudinal studies have confirmed that factors affecting QOL are affected by individual functional status and social environmental factors rather than the size and form of care facilities. Conclusion: This study summarized 10 papers and analyzed them through a systematic review of literature. We found that factors such as individual characteristics and social environment determine QOL of patients with dementia. In this study, various intervention methods to improve QOL of patients with dementia should be developed and used in long-term care facilities by identifying the factors affecting QOL of dementia patients and using them.

A Study on Self-reliance and Residential Service Needs of Patients with Chronic Mental Illness (만성정신질환자의 자립 및 주거 서비스에 대한 요구도 조사)

  • Objectives : Residential services are provided to help patients with chronic mental illness, but those have some problems because of provider focused services and uniformized training. So the purpose of the study was to explore residential needs of patients with the chronic mental illness such as chronic schizophrenia. Methods : A survey was conducted on 139 mental illness patients using mental health facilities at Ansan city area in South Korea. We investigated their demographic and social characteristics, the degree of self- reliance and residential service needs. Results : More than half of them had the desire for self-reliance, and they needed job support mostly. They were positive about the use of residential facilities and needed daily living skill and social skill training. Also they preferred day rehabilitation in type of facility. And they wanted to operate in a way that they returned home at the weekend after group home. Conclusions : For facilitating the recovery of patients with chronic mental illness, it would be necessary to provide individualized residential rehabilitation services and to improve existing residential facilities and programs reflected on the patients' needs.
