The Need for Rehabilitation Day Care Program Service of Stroke Survivor's Family (재가 뇌졸중 환자 가족의 주간재활간호 서비스 요구와 관련요인)

  • This study was carried out to find out the basic data required to plan and develop Rehabilitation Day Care Program for the stroke survivor's family in Korea. The subjects comprised of 92 stroke survivor's family who discharged from 4 hospitals in Seoul during the past 2 years. The data were collected from August 3, 1998 to September 18, 1998, through interviews with questionnaires about general characteristics, activities of daily living, depression and service need of rehabilitation day care program at the outpatient clinics by trained nursing graduates. Data were analyzed with descriptive analysis, Pearson's correlation analysis, and Stepwise multiple linear regression analysis using SPSS/WIN 10.0 program. The results obtained are as follows; 1. The mean score of the general need of rehabilitation day care program of stroke survivor's family was 3.10(range 1-4). The highest need among the service categories of the rehabilitation day card program was self-care and restorative activities category(3.30), and health services referral category, recreation category, psychosocial activities category in order. The needs of each category are as follows. In the health services referral category, the need for dental examination and medical examination were highest, followed by the need for physical therapy and occupational therapy. In the psychosocial activities category, the need for family counselling was highest. In the self-care and restorative activities category, the need for ROM exercise training was highest, followed by bowel training, and ambulation training. 2. The need of family for rehabilitation day care program service displayed a correlation with the level of education, ADL, and the level of depression, and a reverse correlation with age, illness intrusiveness, depression, knowledge, subject and object burden and relationship with stroke survivors. 3. The stepwise multiple linear regression analysis revealed following results. For the need for rehabilitation day care program service, 22.6% of the variance was initially explained by level of family's knowledge about caring method for stroke survivors, 8.8% was the level of subjective burden and 5.4% was relationship with stroke survivors. In conclusion, above characteristics should be considered to develop stroke survivors' rehabilitation day care program.

The Medicare Service Facility Location Problem (의료서비스시설 입지문제)

  • Health has been seen as a resource necessary for both maintaining oneself and for living in a society pursuing high quality of life. Therefore, the medicare provision takes place of one of the most significant indexes for evaluating the regional welfare level. The purpose of this study is to investigate the locational characteristics of medicare service facilities. For the purpose, the distribution patterns of the medicare resources are analyzed both at the regional and local scales. The medicare resources are concentrated at the Metropolitan Seoul area in the regional scale, and they are also concentrated at the economic core areas in the local scale. Especially, higher level medicare resources, such as the general hospitals and medical specialists, show more concentration at the economic core areas than lower level services. This means that access to opportunities to receive higher level medicare is poorer in a more socially disadvantaged area. Since medicare service facilities are invested and operated by both private and public sectors, whose locational mechanisms are deferent, we analyzed the distribution patterns. Most medicare facilities are invested and managed by private sector in Korea, which is operated by market mechanism. This is related with the fact that the medicare facilities are concentrated at the economic core areas. The proportion of private sector is higher in the Metropolitan Seoul area, and thus this region shows stronger spatial disparity of medicare provision. In general, the medicare service facilities are located in the hierarchical structure, and thus the services levels and facility scales are differentiated by the hierarchy. We examined the hierarchical structure in the medicare service facilities in Korea. The referral medicare system has been applied with the assumption of the three level hierarchical structures in Korea. However, we could not find any distinct hierarchical structure in the scales of the medical service facilities.

  • The purpose of this study is to offer fundamental data for classification of somatotype for boys of elementary school age. The subject were 458 elementary school boys aged from 7 to 12 living in Pusan, Data were collected by 57 anthropometric and 11 photographic measurements and analyzed by factor analysis according to SAS package 1. Through the factor analysis by each period of school ages 6-7 factor were obtained in upper half and they are as followings: 1) Factor 1 is horizontal size of upper half in every period 2) Factor 2 is vertical size of upper half in every period 3) Factor 3 is shoulder shape in the first period and length of upper half in the middle and latter period 4) Facto 4 sis length of upper half in the first period and shoulder shape in the middle and latter period 5) Factor 5 is angle shape of the breast and back in the first period angle shape of the lower breast and back in the middle of period and angle shape of the upper breast and back in the latter of period 6) Factor 6 is angle of shoulder in the first period angle shape of the upper breast and back in the middle of period and angle shape of the lower breast and back in the latter of period 7)Factor 7 is angle of shoulder in the latter of period 2. The Relationship among Life-satisfaction, Self-neglect, Psychological Well-Being of the Elderly (노인의 생활만족도, 자기방임, 심리적 행복감에 관한 연구)

  • The purpose of this study is to verify the relationship between the elderly's life satisfaction, self-neglect, and psychological well-being and to provide the basis for the development of mediation program to improve the psychological well-being of the elderly. The subjects of this study were 143 elderly persons who living in J city of G province. For data analysis, SPSS WIn 21.0 program was used to calculate real numbers, percentages, the mean values, and standard deviation values, and to conduct t-test, ANOVA, Pearson correlation, and multiple regression analysis. The subjects' psychological well-being was analyzed according to their general characteristics. As a result, their subjective well-being had a positive correlation with their life satisfaction (r=.841, p<.001), and had a negative correlation with their self-neglect (r=-.501, p<.001). As a result of stepwise regression analysis, the factors affecting the psychological well-being of the elderly were life satisfaction (β=.787, p<.001), income source (β=-.114, p=.028), and the explanatory power of these variables was 71.3%. In order to improve the psychological well-being of the elderly through the results of this study, it is necessary to prepare various social systems suitable for the characteristics of the elderly.

Needs for Integrated Care for Older Adults in Seoul (서울특별시 지역사회 거주 노인의 통합돌봄 요구)

  • The purpose of this study was to classify the care needs of the older adults aged 65 and over and to identify characteristics of care need groups. This was a secondary analysis study using data from 2017 National Survey of Older Persons in Seoul. There were 50.4% in the general group without any support needs, 17.9% in the medical needs group, 14.2% in the welfare needs group with support needs of daily living or social activity, and 17.5% in the complex needs group with both medical and welfare needs. Significant differences were shown in most variables of the general characteristics, grading of long-term care or disability, financial burden and caregiving, health behaviors, health status, and life satisfactions among groups (p<.001). The complex care need group should be provided with integrated care service for medical and welfare through multidisciplinary team approach.

Factors Affecting Smoking Attitude and Intention among Preschool Children (유치원 아동의 흡연태도 및 흡연의도에 미치는 영향요인)

  • Parental smoking status and smoking environment do not only affect children's health through exposure to nicotine, but are also related to children's smoking attitude, intention and knowledge of smoking. Since this study was conducted to examine a relationship between family members smoking and smoking attitude, intention and knowledge of their children, little is known about psychosocial factors influencing smoking among Korean preschool children. This study aimed to determine the psychosocial factors (sociodemographic factors, education for smoking cigarette, knowledge about smoke cigarette) influencing smoking attitude and smoking intention, with a purpose of providing directions for future education programs for Korean preschool children. A cross sectional sample of 840 children, ages 4-7 years, living in a rural district were interviewed during August 6 - November 14, 2003. Among them, 834 children whose missing data were less than $10\%$ were included the analysis. The independent variables included sociodemographic characteristics and education for smoking cigarette, while the dependent variables were knowledge about smoking, the children's smoking attitude and their intention of smoking. The data on sociodemographic characteristics were analyzed using descriptive statistics including frequency, percentage, mean, and standard deviation. The correlations between the independent variables and attitude and intention of smoking were analyzed using chi-square test, t-test, and logistic regression. The results of this study were s follows. The factors that were found to be significantly correlated to smoking attitude include mother's smoking $(x^2=4.86,\;p=.03)$, grandmother's smoking $(x^2=5.56,\;p=.02)$, and knowledge of smoking cigarette $(x^2=-5.13 p<.01)$. The factors that were significantly correlated to the intention of smoking include gender $(x^2=.08,\; p=.02)$, education for smoking$(x^2=17.65,\;p<.01)$, mother's smoking $(x^2=19.05,\; p<.01)$, grandmother's smoking $(x^2=17.67,\; p<.01)$, and knowledge$(x^2=-2.17,\;p=.03)$. The children who had non smoking mother were more likely to have negative attitude (OR=2.29). The children who had a higher knowledge about smoking were more likely to have negative attitude (OR=1.58). The findings suggest that a smoking prevention program specific for preschool children is necessary to prevent early onset of smoking. The findings also suggest that the program should involve children's family members who influence their smoking attitude and intention.

Psychosocial Stress Levels and Related Factors in some Rural Residents (일부 농촌지역 주민에서 사회심리적 스트레스 수준과 관련요인)

  • Objectives: This study was performed to investigate the levels of psychosocial stres and to identify its Methods: The study subjects were 1,806 adults aged 40-70 years living in Naju City, Jeollanamdo. Sociodemographic characteristics, health-related behavior, social suport and personality were collected for statistical analysis. The measurement instrument of psychosocial stress was psychosocial well-being index. The regression analysis. Results: The results showed that 16.6% of subjects were categorized as high stress, 63.4% as moderate, and 20.0% as normal. Overall, the mean value of PWI was 17.29.8 and significantly diferent by socioeconomic characteristics, health-related behavior, social suport and personality. In the case of men, the income, decreased positive social suport, increased negative social suport, type A behavior pattern, decrease of internal locus of control or poorer self-esteem. In the case of women, the PWI was significantly deteriorated in the subjects with no job, no spouse, decreased positive social suport, increased negative social support, type A behavior pattern, decrease of internal locus of control or poorer self-esteem.Conclusions: The psychosocial stres for rural residents was related with social suport and personality. Thus, these results should be considered to reduce stres levels in rural adults.

Factors Influencing Perceived Health Status and Life Satisfaction in Rural Residents (일부 농촌 주민들의 주관적 건강상태와 삶의 만족도 관련요인)

  • Objectives: This study was performed to compare rural residents' perceived health status and life satisfaction according to their general and health related characteristics and to identify factors related to perceived health status and life satisfaction in this population. Methods: Subjects were 1,091 adults aged 20 years or older living in Geochang Gun, South Korea. A survey was conducted from June 28 to August 25, 2009. We used the health self rating scale by Lawton et al. (1982) and the overall life satisfaction scale by Wrosch et al. (2000). The collected data were analyzed using SPSS WIN 18.0. Result: Sex, age, education, spouse, medical diagnosis, currently taking medication, main health maintenance behavior, alcohol consumption, frequency of alcohol consumption, exercise, family harmony, and friend relationships were significantly related to perceived health status. Age, education, spouse, main health maintenance behavior, smoking, exercise, family harmony, and friend relationships were significantly related to life satisfaction. Further, 29.3% of the variance in perceived health status could be explained by medical diagnosis, exercise, family harmony, education, currently taking medication, and friend relationships, while 11.1% of the variance in life satisfaction could be explained by family harmony, perceived health status, main health maintenance behavior, and friend relationships. Conclusion: Health management programs must be designed so as to incorporate the general and health related characteristics of rural residents if they aim to improve the perceived health status and life satisfaction in that population.

Some Determinants of manager Style and household Work Organization on the Urban Homemakers in Korea (도시주부의 가사노동 조직화 및 관리자 유형 분석)

  • The main objective of this dissertation is to find out the measurement methods of household work organization and manger style and the quantitative analysis methods of influential factors on household work organization and manager style. Subjects were homemakers living in metropolitan , medium and small cities. Questionnaires were sent to subjects and usable responses were 1139 out of 1239 received questionnaires. Data collected through both preliminary and main survey were analyzed by using frequencies, percentage, analysis of variance, chi-square test and pearson's correlation analysis. Results are as follows; 1) The degree of household work organization was distributed on upper middle level exceeding middle score. Among subsystems of household work such as task assignment , test regularization, task arrangement and task standardization. subjects show the highest positive attitude in task standardization. 2) The characteristics of management in household work have task centered orientation. By the prority of task centered orientation, the order was integrated style. dedicated style. It means that the most ideal style is an integrated style which accounts for one-third of the total homemakers. 3) Household organization's subsystems and household work manager style are related with socio-demographic variables. Integrated style is significantly related with homemaker's employment and having helpers for household work. homemakers who are employed and have helpers for household work exhibit high scores in task assignment and task arrangement. Dedicated style is significantly related with homemaker's age and the number of children. Homemakers who are older and have many children exhibit high scores in task assignment. Related style is significantly related with marriage duration, education level and income level. Homemakers who have a long marriage duration exhibit high scores in task assignment and who have high education and income level exhibit high scores in task regularization. Seperates style is significantly related with homemaker's age m marriage duration an family life cycle. homemakers who ar older, have a long marriage duration and are of late stage in family life cycle exhibit seperated style and high scores in organization. To summarize research findings, household work manger and household work organization are related with family life experiences, and task assignment is an influential variable on manager style. in conclusion, it is evident that household work organization emphasizes efficiency as cognitive component and such organization. Finally it is necessary to broaden this research considered social psychological variables. And also it needs to build household manger style models appropriate fro the characteristics of each style.

Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (5) Meal Patterns (서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (5) 끼니별 섭취)

  • This study was performed to investigate the characteristics of meal patterns among Koreans of different age groups. The study subjects consisted of preschool children (PC), elementary school children (EC), high school students (HS), college students (CS), and adults over 30 years old living in Seoul and the surrounding areas. A dietary survey was conducted using 24-hour recall method from 1996-1998 and data were collected from 2392 subjects. The subjects over 30 years old were classified as 30∼49 years, 50∼69 years, and over 70 years. EC, HS, and CS groups consumed less than 20% of their total energy intake from breakfast. PC and EC consumed over 30% of their energy from snacks. In all age groups, the proportion of subjects who skipped a meal was highest for breakfast and lowest for dinner. Especially, 31.4% of the CS group skipped breakfast. Breakfast and snacks provided higher quality meals than the others when each meal was assessed by the index of nutritional quality (INQ). Especially, lunch of the HS group, each meal of the EC group and subjects over 70 years of age had a lower diet quality than the others did. The major snack items for PC, EC, HS, and CS groups were milk, ice cream, and snack cakes, but those for subjects over 30 years of age were fruits. The PC group obtained 35.5% of their energy from snacks, but the proportion of snack energy decreased with increasing age. PC and EC groups obtained a great proportion of calcium, vitamin B2, fat, and cholesterol intake from snacks. Snack foods provided 53.2%, 42.4%, 45.4%, and 24.6% of those nutrients, respectively, for the PC group and 54.2%, 43.5%, 37.4%, and 26.8%, respectively, for the EC group. These results suggested that each age group had specific meal patterns. Especially, breakfast and snack intake seemed to complement overall diet quality. However, more intervention is needed to improve overall dietary nutrition, e.g., efforts to decrease fat and cholesterol proportion and increase breakfast consumption. These dietary characteristics among different age groups present foundation data for the development of methods to resolve the nutritional problems of each age group.
