Is higher dose always the right answer in stereotactic body radiation therapy for small hepatocellular carcinoma?

  • Purpose: This study was conducted to compare clinical outcomes and treatment-related toxicities after stereotactic body radiation therapy (SBRT) with two different dose regimens for small hepatocellular carcinomas (HCC) ${\leq}3cm$ in size. Materials and Methods: We retrospectively reviewed 44 patients with liver-confined HCC treated between 2009 and 2014 with SBRT. Total doses of 45 Gy (n = 10) or 60 Gy (n = 34) in 3 fractions were prescribed to the 95% isodose line covering 95% of the planning target volume. Rates of local control (LC), intrahepatic failure-free survival (IHFFS), distant metastasis-free survival (DMFS), and overall survival (OS) were calculated using the Kaplan-Meier method. Results: Median follow-up was 29 months (range, 8 to 64 months). Rates at 1 and 3 years were 97.7% and 95.0% for LC, 97.7% and 80.7% for OS, 76% and 40.5% for IHFFS, and 87.3% and 79.5% for DMFS. Five patients (11.4%) experienced degradation of albumin-bilirubin grade, 2 (4.5%) degradation of Child-Pugh score, and 4 (9.1%) grade 3 or greater laboratory abnormalities within 3 months after SBRT. No significant difference was seen in any oncological outcomes or treatment-related toxicities between the two dose regimens. Conclusions: SBRT was highly effective for local control without severe toxicities in patients with HCC smaller than 3 cm. The regimen of a total dose of 45 Gy in 3 fractions was comparable to 60 Gy in efficacy and safety of SBRT for small HCC.

Effect of Phenobarbital on the Hepatic Clearance of Diltiazem in Isolated Rat Hepatocytes (흰쥐 분리 간세포에 있어서 딜티아젬의 간클리어런스에 미치는 페노바르비탈의 영향)

  • In order to study the effect of phenobarbital(PB) on the hepatic transport of diltiazem(DTZ), $Ca^{2+}$ channel blocker, we used isolated hepatocytes of rat which was intraperitoneally pretreated with phenobarbital sodium(75 mg/kg) for four days once a day. For the isolation of rat liver cells, a modification of the two step procedure of Seglen was used. DTZ was dissolved in incubation buffer to the final DTZ concentrations of 200, 400, 600, 800 and 1000 ng/ml in order to elucidate the uptake characteristics of DTZ by hepatocytes. Reactions were stopped at 10, 20, 30, 45, 60, 90, 120 and 300 sec. The initial velocity was determined by disappearance of diltiazem in the hepatocyte suspension. On the other hand, to determine the effect of PB on the in vitro hepatic intrinsic clearance of DTZ we obtained the metabolism rates of DTZ in the control and the PB-pretreated rat hepatocyte at various time intervals. According to pretreatment with PB, the size of hepatocyte and the amount of protein per $10^6$ cells were significantly (p<0.01) increased from $26.92{\pm}0.1364\;m$ to $35.31{\pm}1.00\;m$ and from $468{\pm}6.5\;{\mu}g/10^6$ cells to $628.8{\pm}12.1{\mu}g/10^6$ cells, respectively. In the case or hepatic uptake of diltiazem, $K_m$ was not different in the normalization by cell numbers and increased from $2.90\;{\mu}M\;to\;13.89\;{\mu}M$ in the normalization by protein amount. $V_max$ was increased regardless of normalization by protein amount and cell numbers, from $1.21\;{\mu}mole/min \;{\cdot}\;mg\;protein\;to\;3.96\;{\mu}mole/min\;{\cdot}\;mg\;protein\;and\;from\;2.38\;{\mu}mole/min\;{\cdot}\;10^6\;cells\;to\;2.83\;{\mu}mole/min\;{\cdot}\;10^6\;cells$, respectively. The in vitro hepatic intrinsic clearance of DTZ was significantly (p<0.01) increased from $0.640{\pm}0.038\;ml/mim\;{\cdot}\;10^6\;cells\;to\;2.385{\pm}0.212\;ml/min\;{\cdot}\;10^6\;cells$ due to PB-pretreatment. These results suggest that the uptake of DTZ by hepatocyte is extremely fast and PB enhances the hepatic intrinsic metabolic clearance of DTZ.

Anti-tumor Activity of Protein-bound Polysaccharides Extracted from Mycelia of Lentinus edodes (표고버섯 균사체로부터 추출한 단백다당체의 항암효과)

  • Protein-bound polysaccharides (PBP) were extracted from the mycellia of Lentinus edodes SR-1, and their anti-tumor activities and immunopotentiating properties were observed. The amounts of PBP needed to extend the doubling time twofold (1 unit) were found to be 1 mg for mouse leukemic cells $P_{388}\;and\;L_{1210}$; 4.4, 3.6 and 6.6 for bowel cancer cells, HCT-48, HRT-18, HT-29 respectively; and 2.6 mg for liver cancer cell, Hep G2. When $P_{388}\;and\;L_{1210}$ were treated with 4 mg of PBP, more than 90% of the cell number were reduced in 48 hours. However, 9 mg of PBP and 72 hrs of incubation time were needed to obtain the same effect for HRT-18, HT-29, and Hep G2. The significant reduction of cell size was observed as the amount of PBP and the incubation time increased. Mice spleen weight and plaque forming cell number increased when the cancer cells were treated with PBP.

Human and Animal Study on the Natural Food for Obesity and Metabolic Syndrome Risk Factors (비만 및 대사성증후군 위험인자에 대한 천연물 식품의 인체 및 동물 효능연구)

  • In this study a natural composition containing oriental herbs, KSH28, for reducing obesity and metabolic syndrome was constructed and its efficacy was evaluated in animal and human. To investigate the anti-obesity effect of KSH28, animal study was conducted using high fat diet-induced obese mice. KSH28 significantly decreased body weight and adipose tissue in high fat diet-fed obese mice. The mean size of fat cells in adipose tissue was significantly reduced. Glucose and triglyceride levels were also significantly decreased. To elucidate its efficacy in human, a natural food containing KSH28 with grains, vegetables, vitamins, minerals and dietary fibers was constructed and 40 subjects (8 male and 32 female) were tested for the change of body composition, blood pressure and blood lipid profile. All subjects had 2 pack (309 each) of natural food per day for 4 weeks. Compared to the baseline value, body fat was significantly reduced, however, water, protein and mineral contents in the body were not changed, suggesting selective reduction of fat tissue. Blood pressure and serum lipid profile were significantly decreased to reduce risk for metabolic syndrome. Serum GPT, a liver function indicator, was not changed and no significant side effects were detected. Therefore, it was shown that the KSH28 is a safe and effective composition for reducing obesity and metabolic syndrome.

  • Objectives: This study was investigated the improvement effects of Pakistani Ephedra herba-containing Gangji-hwan-1, 2, 3 (Di-fatty; DF-1, 2, 3), Chinese Ephedra herba-containing Gangjihwan-4 (DF-4) and combination of DF-1 and Gamisoche-hwan (GSH) on obesity in a high fat diet-fed obese mouse model. Methods: Eight-week-old C57BL/6N mice were divided into seven groups: a normal lean group given a standard diet, an obese control group given a high fat diet, and DF-1, 2, 3, 4, and DF-1+GSH groups given a high fat diet with DF-1, 2, 3, 4 (40, 80, 160, 80 mg/kg), and DF-1+ GSH (80 mg/kg), respectively. After 8 weeks of treatment, body weight gain, feeding efficiency ratio (FER), blood lipid markers, liver histology, and fat weight and histology were examined. Results: Body weight gain was significantly decreased in DF and DF-1+GSH groups compared with control. The extent of decreases was eminent in DF-1+GSH group. FER and circulating concentration of leptin were decreased in DF and DF-1+GSH groups compared with control. Circulating concentrations of triglyceride, glucose and insulin were decreased in DF and DF-1+GSH groups compared with control. The size of adipocytes were decreased by DF and DF-1+GSH groups compared with control, whereas the adipocyte number per unit area was increased by them, suggesting that DF and DF-1+GSH groups decreased the number of large adipocytes. Conclusions: In conclusion, these results suggest that DF and DF-1+GSH groups decrease FER, plasma leptin concentration, blood anti-obesity biomarkers and fat mass, improves body weight gain. In addition, these effects were more effective in DF-1+GSH combination group than in DF-1, 2, 3, 4 groups.

  • This study was perfromed to develop the assessment guideline and endpoints for clinical trial with anticancer herbal medicine. The botanical products used to humans for long time may be applied to phase 3 clinical trial after submitting the evidences for safety and efficacy of them or completion of basic requirement of phase 1 and phase 2 for safety confirmation and dose determination. Syndrome improvement was chiefly evaluated by Zubrod and karnofsky(%) methods. We suggest the general clinical trial assessment with botanical products, by following assessment points, that is, tumor size for 50 points, survival fate for 10 points, major syndromes for 40 points. It is recommendable that the each symptom of Qi deficiency syndrome, blood deficiency syndrome and Qi stagnation syndrome was allocated by assessment points, Similarly, the each symptom was given the assessment points according to the severity of symptom, for example, slight for 3 points, moderate for 2 points and severe for 1 point in hepatocelluar carcinoma and lung cancer. Then, the efficacy of botanical products was evaluated by the difference between pre-treatment and post-treatment. Asking the neoplastic patients of questionnaire on physical, emotional, cognitive, social and role subjects availability, three more syndromes (Fatigue, Pain and Nausea/Vomit), quality of life(QOL) will be evaluated by GLM statistics. In addition, in case of lung cancer, 13 questions will be asked by the EORTC QLQ-C13 forms. As the assessment of endpoints for efficacy to reduce side effects induced by chemotherapy and radiotherapy, the data of image scanning and hemato-urinalysis can be usefully applied on immune response, weight loss, indigestion, hemopoietic damage and injury of liver and kidney, while the changes of syndromes of side effect can be evaluated by differentiation methods of Qi and blood and five viscera. However, it is still necessary to determine the ratio between scientific analytical method and Oriental differentiation method as well as confirm the Oriental assessment endpoints by clinical trial. In addition, we suggest the continuous development of assessment endpoints on other carcinomas except of hepatocelluar carcinoma and lung cancer in future.

Mass Mortalities of Cultured Striped Beakperch, Oplegnathus fasciatus by Iridoviral Infection (이리도 바이러스 감염에 의한 양식 돌돔, Oplegnathus fasciatus의 대량폐사)

  • From August to October 1998, over 60% mortality of cultured striped beakperch Oplegnathus fasciatus was occurred in net cages along the southern coast of Korea. Moribund fish showed some clinical signs of lethargic behavior, dark coloration or decoloration, severe gill anemia and enlargement of spleen. Also enlarged basophilic cells showing Feulgen -positive reaction were observed in the tissue section of spleen, kidney, liver and heart of the diseased fish. GF cells inoculated with spleen homogenate of diseased fish produced cytopathic effect of enlarged and rounded cells, therefore the causative virus was isolated from diseased fish. Striped beakperch fingerlings intraperitoneally inoculated with the causative virus ($10^4TCID_{50}$/0.1 ml) revealed symptoms similar to those of naturally infected fish and died from 7 to 14 days post injection. Transmission electron microscopy revealed that the causative virus was enveloped icosahedral particle with 120~130 nm in diameter. PCR products of the expected size (500 bp) were amplified with a primer set based on the ATPase gene of RSIV(red sea bream iridovirus) using template DNAs which were extracted from the spleen of diseased fish and GF cells inoculated with the causative virus. According to the analysis of nucleotide sequence of these PCR products, the sequence from ATPase cDNA gene of the causative virus showed 95% homology with that of RSIV. These results indicate that the mass mortality in the cultured striped beakperch was caused by the infection of iridovirus similar to RSIV.

Meningeal Hemangiopericytomas : Clinical Features, Treatment and Long-term Outcome (수막 혈관주위세포종의 임상상, 치료 및 장기 추적 결과)

  • Objectives : Meningeal hemangiopericytoma is a rare tumor. Clinical and radiological features are similar to those of an ordinary meningioma. But its biological behavior is quite different from those of a meningioma as it brings profuse bleeding in the operation field, frequent recurrence and metastasis to other systemic organs even in the case of gross total resection. In order to find out the prognostic factors and to compare longterm outcome after various treatment modalities, the authors reviewed consecutive 20 operated cases of meningeal hemangiopericytoma to characterize their clinical features, surgical outcomes and effectiveness of the radiation therapy. Methods : Twenty patients with a hemangiopericytoma were treated between 1982 and 1999 in our department. They are composed of 13 patients of hemangiopericytoma and 7 patients who were initially diagnosed as angioblastic type meningioma and then confirmed as a hemangiopericytoma after review of their previous histopathology slides. The mean follow-up period was 99 months(1-256 months). The long-term outcomes after various treatment modalities were evaluated according to recurrence-free survival and image follow-up. Recurrence-free survival curves are compared between the patient groups according to the extent of removal and radiation therapy. Results : There were 10 cases of recurrence and 4 cases of distant metastases ; lung, liver and femur(2 cases) respectively. The 5-year recurrence rate was 37%, while the 10-year recurrence rose to 77%. There was also statistically significant difference of median recurrence-free survival between the completely-resected group(Simpson grade 1 or 2) and partially-resected group(Simpson grade 3 or 4 or 5) ; 137 months compared to 47 months, respectively(p=0.009). The median recurrence-free period after subtotal resection of tumor and postoperative radiotherapy was 47 months compared to 117 months of the patients who underwent gross total resection of tumor and did not receive radiotherapy. But radiation therapy alone did not show significant difference in recurrence-free survival. Marked tumor volume reduction and easy removal of tumors without bleeding were found in 2 recurred cases. Conclusion : The extent of tumor resection and presence of metastasis are the most important factors related to long-term outcome of the patients with hemangiopericytoma. Radiation therapy after the first operation did not have a role in extending the recurrence-free survival, but it affected favorably to decrease the size of residual mass and intraoperative bleeding during the second operation.

Characteristics of Viral Endothelial Cell Necrosis of Eel (VECNE) from Culturing Eel (Anguilla japonica, Anguilla bicolar) in Korea (국내 양식산 뱀장어(Anguilla japonica, Anguilla bicolar)에서 발생하는 바이러스성내피세포괴사증(VECNE)의 감염 특성)

  • From 2017 to 2018, the disease has been monitored at four culturing eel farms in Incheon and Gyeonggi region in Korea. As a result, diseases with gill congestion frequently occurred. This disease occurred regardless of size of eel, but the frequency and cumulative mortality were high in eels within 3 months after stocking. The infected fish showed pathological histopathological features such as intense congestion and dilation in the central venous sinus (CVS) of gill filaments and hemorrhages in liver and kidney. Hexagonal viral particles measuring about 70 nm in diameter was observed in nuclei and cytoplasm of gill vascular endothelial cells. Molecular biologic diagnosis by both PCR and genetic analysis has been revealed that the causative agent of this disease is Japanese eel endothelial cells-infecting virus (JEECV), the cause of viral endothelial cell necrosis of eel (VECNE), which is mainly reported in Japan. This study is the first report on the characteristics of JEECV and VECNE infection in domestic eel farms.

Effects of Lonicera caerulea extract on adipocyte differentiation and adipogenesis in 3T3-L1 cells and mouse adipose-derived stem cells (MADSCs) (댕댕이나무 열매 추출물이 지방전구세포와 마우스 지방유래줄기세포의 분화 및 지방 생성 억제에 미치는 영향)

  • Purpose: Obesity is a major health problem of global significance because it is clearly associated with an increased risk of health problems, such as nonalcoholic fatty liver disease (NAFLD), diabetes, cardiovascular diseases, and cancer. Lonicera caerulea (LC) originates from high mountains or wet areas and has been used as a traditional medicine in northern Russia, China, and Japan. LC contains a range of bioactive constituents, such as vitamins, minerals, and polyphenols. This study examined the anti-obesity effects of LC during differentiation in preadipocytes. Methods: The cell viability assay was performed after the differentiation of 3T3-L1 cells for 7 days. Oil Red O staining was used to visualize the changes in lipid droplets in 3T3-L1 cells and mouse adipose-derived stem cells (MADSCs). The mRNA expression of obesity-related genes was determined by quantitative real-time PCR. Results: According to the results of Oil Red O staining, the lipid levels and size of lipid droplets in the adipocytes were reduced and the LC extract (LCE, 0.25-1 mg/mL) markedly inhibited adipogenesis in a dose-dependent manner. The treatment of LCE also decreased the mRNA expression of peroxisome proliferator-activated receptor ${\gamma}$ ($PPAR{\gamma}$), CCAAT/enhancer binding protein-${\alpha}$ ($C/EBP{\alpha}$), and sterol regulatory element binding protein 1 (SREBP1) in 3T3-L1 cells. Western blot analysis showed that the $PPAR{\gamma}$, $C/EBP{\alpha}$, and SREBP1 protein levels in both 3T3-L1 and MADSC were reduced in a dose-dependent manner. Conclusion: These results suggest that LCE can inhibit adipogenic differentiation through the regulation of adipogenesis-related markers.
