초음파검사에 의한 간 크기 측정방법 및 정상 성인의 체격지수별 참조범위 (Liver Size Measurement Method by Ultrasonography and Reference Range based on Normal Adult Physique Index)

  • Liver size is an important component in the diagnosis and follow-up of diffuse liver disease when testing for liver disease using ultrasonography. However, difficulties lies in determining the presence of hepatomegaly and liver atrophy because the method used for measuring liver size differs from one examiner to another and there is no relevant standard based on body build. The present study aims to propose a more objective method for liver size measurement and a reference range based on body build. A total of 260 normal adults (130 males, 130 females) participated in the study. Ultrasonography was performed in all participants to measure the size of the right lobe, left lobe, quadrate lobe, and caudate lobe of liver. Based on Physique Index(PI), a value derived from multiplying weight(kg) by height($m^2$), size of physique was divided into three groups including Group I with PI<150, Group II with $150{\leq}PI{\leq}250$, and Group III with PI>250. Thus, mean liver size by PI and a reference range with 95% reliability were suggested. The superoinferior diameter of right lobe was $12.34{\pm}1.18cm$ in males and $11.07{\pm}0.93cm$ in females, and its reference range was 10.64~11.0cm for Group I, 11.78~12.12cm for Group II, and 13.02~13.84cm for Group III. The anteroposterior diameter(T) of left lobe was $5.93{\pm}1.09cm$ in males and $5.18{\pm}0.99cm$ in females, and its reference range was 4.77~5.17cm for Group I, 5.49~5.79cm for Group II, and 6.68~7.44cm for Group III. The transverse diameter was $3.51{\pm}0.60cm$ in male participants and $3.42{\pm}0.49cm$ in female participants and its reference range was 3.29~3.51cm for Group I, 3.36~3.55cm for Group II, and 3.52~4.0cm for Group III. The caudate lobe index was $11.65{\pm}2.88cm^2$ in males and $9.62{\pm}2.18cm^2$ in females and its reference range was $8.83{\sim}9.75cm^2$ for Group I, $10.62{\sim}11.47cm^2$ for Group II, and $11.89{\sim}14.26cm^2$ for Group III. As a basic measurement method of liver size, the present study suggests measuring the superoinferior diameter for right liver lobe, the anteroposterior diameter for left liver lobe, the transverse diameter for quadrate lobe, and the caudate lobe index for caudate lobe. It is expected that this method along with its relevant reference range can be used as useful indicators in determining hepatomegaly and liver atrophy upon the diagnosis and follow-up testing of diffuse liver disease.

  • Liver size is one of the important standards used to establish differential diagnoses for dogs with suspected hepatic diseases. No previous reports have been reported about comparison for radiographic measurements of canine liver size by left and right recumbency. The retrospective study was performed in dogs that underwent abdominal radiography with left, right and dorsal recumbency at the Veterinary Teaching Hospital of Gyeongsang National University from March 6, 2014 to December 31, 2014. We investigated differences of radiographic liver size according to direction of lateral recumbency. We found differences of radiographic measurements in canine liver size between left and right recumbency. Thus, positional effect should be considered in case of monitoring the canine liver size regularly.

간신티그램상 간크기의 새로운 평가방법 -간.복부횡경비- (A New Method of Liver Size Estimation on Hepatic Scintigram -Hepato-abdominal Ratio)

  • Estimation of liver size is essential in the diagnosis of liver disease, Many approaches have been attempted in evaluation of liver size such as the measurement of length, area and volume. Among these, area and volume measurements are accurate but complicated, so we commonly use formerly introduced various linear measurements, but in scintigraphy one must calculate the actual liver size using rate of reduction, which is time consuming. Because of these reasons, we carried out present study to represent liver size by means of a simple liver measurement like we express the cardiac size by cardiothoracic ratio. Our cases consisted of 100 clinically normal subjects as the normal group and 50 patients suffering from liver disease and diagnosed to have hepatomegaly on abdominal palpation and scintigram at Dept, of Radiology of St. Mary's Hospital, Catholic Medical College during the period of 8 months from Jan. 1980. We measured the liver size using 4 linear diameters(Fig. 1). And as the reference measurement, the distance from the right margin of the liver to the left margin of the spleen was measured. We called this "abdominal transverse diameter(ATD)". The results were as follows; 1) The smallest value was recorded in the midline vertical diameter (MVD). It was $4.2{\pm}0.4cm$ in normal group and $5.0{\pm}0.6cm$ in the hepatomegaly group. 2) The diameter using other methods ranged from 5.6 to 7.2 cm in the normal group and from 6.3 to 7.5cm in the hepatomegaly group. 3) There was significant difference in the ratio of each diameter to ATD between the normal and hepatomegaly group (<0.01). We called this "hepato-abdominal ratio". 4) The "hepato-abdominal ratio" using MVD is $0.43{\pm}0.06$ in the normal group and $0.53{\pm}0.07$ in the hepatomegaly group. The "hepato-abdominal ratio" of MVD was most significantly different between normal and hepatomegaly group. 5) The tolerance limits(99%) of "hepato-abdominal ratio" using MVD is from 0.41 to 0.45 in the normal group and from 0.51 to 0.55 in the hepatomegaly group. Therefore, by reasons of error during measurement and convenience of memory, it was warranted to suggest hepatomegaly when "hepato-abdominal ratio" using MVD is more than 0.5 in the interpretation of hepatic scintigram.

Radiographic Liver Size Evaluation after Portosystemic Shunts Ligation in 13 Cases

  • Thirteen dogs were diagnosed as congenital extrahepatic single PSS by intraoperative mesenteric portovenography or computed tomographic examination, repair surgery was performed by using an ameroid constrictor. Hepatic size was measured from the right lateral view using liver length/T11 length ratio. This measurement was performed on follow-up check of PSS ligation patients. Hepatic size parameter of pre-operative PSS patients is $4.13{\pm}0.13$ (range, 3.11-4.83). After surgery, hepatic size parameter of post-operative PSS patients is $4.79{\pm}0.19$ (range, 3.78- 5.93). Although follow-up periods varied 2 and 26 weeks, all patients showed increased liver size compared to that on pre-operative radiographs (P < 0.01). The increase rate was 1.01-1.46 times than those of preoperative radiographs. But in 5 patients, post hepatic liver size was small compared to the others, which showed low increase rate of body weight, total protein, albumin, and glucose level. It was thought that small value of hepatic size parameter was due to delay of hepatic regeneration. In conclusion, radiographic hepatic size parameter of pre- and post-operative patients is considered an effective evaluation for restoring after PSS surgery.

Liver Color Scan에 대(對)한 고찰(考察) (Observation of Liver Color Scan)

  • In the past few years, scintigraphy has become increasingly important in clinical practice, and the use of a color-printing technique has permited a more accurate interpretation of the scan image. Our liver color scintigrams consist of 51 hepatomas, 35 liver cirrhosis, 22 liver abscessis, 10 hepatitis and other 13 cases of the liver diseases which were clinically arid pathologically diagnosed at Sevarance Hospital, Yonsei Univ., since Feb. 1969 through Sept. 1969. These scintigrams have been analized in terms of various pathologic morphology, such as size, shape, margin of the liver, distribution of radioactivity, and shape of the space occupying lesions. The results are as follows: 1. Enlargement of the liver was the most common finding in the diseased livers. The Rt. lobe enlargement was particularly prominent in the liver abscess. 2. Irregular distribution of radioactivity in the liver (so called mottling) was present in 78% of hepatoma, while it was seen only in 31% of liver abscesses. 3. Liver cirrhosis tends to show perihilar accumulation of the isotope (57%). 4. The deformity of the lower most angle of the Rt. lobe, and the Lt. lateral margin of the Lt. lobe was also impressive throughout the cases ($74{\sim}95%$ of all diseased livers). 5. The frequency of visualization of the spleen was influenced by the size of space occupying lesions and the amount of functioning liver. 6. Differentiation between the liver abscess and hepatoma seems to be possible on scintigram, when shape and margin of defect and patterns of distribution of radioactivity in the remaining liver are clearly demonstrated.

  • The purpose of this study was to evaluate the pathologic results of hepatic parenchyma parameters such as liver parenchyma, liver surface, liver margin and liver, portal vein, spleen size, And to evaluate the usefulness of fibrosis progression and hepatic ultrasonography. The sensitivity, specificity, positive predictive value, and prognostic value according to the stage of fibrosis and grade of inflammation were divided into two groups according to the morphologic variable "A" through ultrasound and "B" We evaluated the predictive value and predicted the variables to evaluate fibrosis in clinical diagnosis and treatment of patients with chronic liver disease. The sensitivity and specificity of hepatic fibrosis in hepatic morphologic variables and other size variables were highest in liver surface and edge. The morphologic parameters used in the evaluation of fibrosis were clinically relevant in distinguishing the fibrosis stage from the results of liver biopsy.

Noninvasive markers for esophageal varices in children with cirrhosis

  • Background: The diagnosis of esophageal varices (EV) is based on the findings of esophagogastroduodenoscopy (EGD), biopsy, and serum markers. Thus, noninvasive cost-effective tests through which high-risk EV children can be diagnosed are needed. Purpose: This cross-sectional study aimed to identify the noninvasive markers for EV in children with liver cirrhosis. Methods: A total of 98 children with liver cirrhosis were evaluated in this study. The spleen size, platelet count, serum albumin, liver function test results, and risk scores were evaluated prior to endoscopy. The endoscopic investigations aimed to identify the presence of EV and red signs, and determine varices sizes. Results: Endoscopy revealed varices in 43 subjects (43.9%). The spleen size, platelet count, international normalized ratio, aspartate aminotransferase to platelet ratio index (APRI), platelet count to spleen size ratio, and risk score differed significantly between patients with and without EV on univariate analysis; however, the logistic regression analysis showed no differences, indicating that none of these parameters were independently associated with the presence of EV. Conclusion: Platelet count, risk score, platelet count to spleen size, and APRI can be useful tools for the identification of high-risk patients with EV and might reduce the need for invasive methods like EGD.

국소성(局所性) 간병변(肝病變)에서 간(肝)스캔의 진단적(診斷的) 가치(價値)에 대한 분석(分析) (Evaluation of Liver Scintigraphy in Patients with Focal Liver Disease (S.O.L): An Analysis of Histologically Confirmed 401 Cases)

  • The accuracy of liver scanning was evaluated in 124 cases with primary or metastatic liver tumors, including a few benign localized lesions and in 277 cases without such lesions. All findings were histologically verified by operation or autopsy. 1) Of the 124 cases with space occupying lesions(SOLs) in the liver, 92 cases were detected by liver scan(sensitivity 74.2%). And of the 277 cases without such lesions, 266 cases were evaluated as no SOLs(specificity 96.0%). The overall accuracy was 89.3%. 2) The authors evaluated the liver scan sensitivity in each of different diseases with SOLs. The sensitivity was 88.9% in primary liver carcinoma, 82.4% in primary liver carcinoma with cirrhosis, 88.7% in liver abscess, and 100% in hemangioma. The sensitivity was low in metastatic tumor(65.8%). 3) The sensitivity of the SOLs in the right lobe was 53.3% and left lobe 27.7%. In the interlobar area, detectability was 41.7%. 4) The authors compared the sensitivity of the liver scan with the size of the SOLs. The smaller the size of the SOLs, the lower the detectability. In the pachy lesions, the sensitivity was 46.6%.

Primary Culture of Human Hepatocytes from Small Size Sample

  • Human and rat hepatocytes were isolated by nonperfusion method and cultured for longer than 5 days. Human liver biopsy sample and rat liver were used as hepatocyte source. Several physical and chemical factors which were influencing on hepatocyte isolation procedure were examined and a batch isolation procedure was established for small size sample of rat liver. Isolated hepatocytes showed normal morphlologica characteristics in microscopy and electron microscopical examinations and a morphologica response to phalloidin. Isolated cells were cultured as a monolayer and proven to have intact morphological characteristics for longer than 15 days. Because human liver sample is harder and tighter compared with rat liver, a standard procedure for rat hepatocytes was slightly modified to reduce mechanical damage. Similarly with rat hepatocytes, isolated human hepatocytes showed a normal morphological characteristics and could be cultured for longer than 15days. Human and rat hepatocytes were examined on their functional integrities including cytochrome-P450 related enzyme activity and it's inducibility, hormonal inducibility of AIB uptake and TAT activity, albumin synthesis, DNA synthesis, cellular protein maintenance. In all parameters used in the present study, human and rat hepatocytes showed normal functional characteristics.

CT영상에서의 AlexNet과 VggNet을 이용한 간암 병변 분류 연구 (Malignant and Benign Classification of Liver Tumor in CT according to Data pre-processing and Deep running model)

  • Liver cancer is one of the highest incidents in the world, and the mortality rate is the second most common disease after lung cancer. The purpose of this study is to evaluate the diagnostic ability of deep learning in the classification of malignant and benign tumors in CT images of patients with liver tumors. We also tried to identify the best data processing methods and deep learning models for classifying malignant and benign tumors in the liver. In this study, CT data were collected from 92 patients (benign liver tumors: 44, malignant liver tumors: 48) at the Gil Medical Center. The CT data of each patient were used for cross-sectional images of 3,024 liver tumors. In AlexNet and VggNet, the average of the overall accuracy at each image size was calculated: the average of the overall accuracy of the $200{\times}200$ image size is 69.58% (AlexNet), 69.4% (VggNet), $150{\times}150$ image size is 71.54%, 67%, $100{\times}100$ image size is 68.79%, 66.2%. In conclusion, the overall accuracy of each does not exceed 80%, so it does not have a high level of accuracy. In addition, the average accuracy in benign was 90.3% and the accuracy in malignant was 46.2%, which is a significant difference between benign and malignant. Also, the time it takes for AlexNet to learn is about 1.6 times faster than VggNet but statistically no different (p > 0.05). Since both models are less than 90% of the overall accuracy, more research and development are needed, such as learning the liver tumor data using a new model, or the process of pre-processing the data images in other methods. In the future, it will be useful to use specialists for image reading using deep learning.
