Lack of Association of BIRC5 Polymorphisms with Clearance of HBV Infection and HCC Occurrence in a Korean Population

  • BIRC5 (Survivin) belongs to the inhibitor of apoptosis gene family. The BIRC5 protein inhibits caspases and consequently blocks apoptosis. Thus, BIRC5 contributes to the progression of cancer allowing for continued cell proliferation and survival. In this study, we identified eight sequence variants of BIRC5 through direct DNA sequencing. Among the eight single nucleotide polymorphisms (SNPs), six common variants with frequencies higher than 0.05 were selected for larger-scale genotyping (n=1,066). Results of the study did not show any association between the promoter region polymorphisms and the clearance of hepatitis B virus (HBV) infection and hepatocellular carcinoma (HCC) occurrence. This is in line with a previous study in which polymorphisms in the promoter region does not influence the function of BIRC5. Initially, we were able to detect a signal with the +9194A>G, which disappeared after multiple corrections but led to a change in amino acid. Similarly, we were also able to detect an association signal between two haplotypes (haplotype-2 and haplotype-5) on the onset age of HCC and/or HCC occurrence, but the signals also disappeared after multiple corrections. As a result, we concluded that there was no association between BIRC5 polymorphisms and the clearance HBV infection and/or HCC occurrence. However, our results might useful to future studies.

  • We aimed to investigate the differences in health screening, including medical checkups and cancer screening, between HBV carriers and non-carriers in the Republic of Korea. In the fifth Korean National Health and Nutrition Examination Survey (KNHANES V), conducted between 2010 and 2012, 17,865 persons who answered regarding their HBV-infection status, medical checkup history, liver cancer screening and general cancer screening within the past years were included in the final analysis. In total, 295 persons were HBV carriers. Logistic regression models were used to compare the health check-up rate between the HBV carriers and non-HBV carriers. The HBV carriers were more likely to have been screened for liver cancer [adjusted odds ratio (OR): 2.83, 95% confidence interval (95%CI): 1.90-4.21] or cancer [OR: 1.44, 95%CI: 1.04-1.99]. The HBV carriers showed a probability of receiving medical checkups that was identical to that of the non-carriers [OR: 0.99, 95%CI: 0.72-1.35]. The HBV carriers, who were at higher risk of developing chronic liver disease, were more likely to be screened for cancer, including liver cancer, than the non-HBV carriers; no difference in the rate of medical checkups was observed between the HBV carriers and non-HBV carriers.

  • Hepatitis, liver cirrhosis and liver cancer caused by viral infection are among the most prevalent causes of death in Korea. Several medicines have been in use despite their nonsatisfactory effects on these disease. Some herbal medicines put to use recently have not shown beneficial effects, either. This paper evaluates the effects of extracts from 10 traditional Korean herbal medicines on rats with hepatic damage induced by galactosamine. Rubus coreanus showed an anti-inflammatory effect as shown on the data of activities of serum transaminases.

  • 1. Hepatic ascariasis represented by milk spots on the liver surface was found in 475(31.7%) out of 1,500 pigs. 2. For histopathological studies, 170 cases were collected at random from the milk spotted liver From these cases ascaris larva was found in 14 cases(8.2%). 3. Grossly, the occurrence of milk spots was not confined to any particular lobes of the liver. The lesions, however, were more frequently found in the diaphragmatic surface than in the visceral surface. The extent of the spots was limited to the subcapsular region and the deeper tissue was not affected. 4. Histologically, hemorrhage and eosinophilic infiltration were observed in the area invaded by the larva. The increase of connective tissue followed by a form of chronic lymphocytic hepatitis was observed at the late stage of infection. There was numerous eosinophilic infiltration around the larva trapped in the liver tissue. Consequently, encapsulation of the larva was followed by organization and calcification.

The Effects of Intrapatient Variability in Tacrolimus Concentration on Clinical Outcomes Immediately After Liver Transplantation (Tacrolimus의 혈중농도 변동성이 간이식 예후에 미치는 영향)

  • Background: Tacrolimus, a calcineurin inhibitor, is an immunosuppressant used in post-transplantation maintenance therapy. The drug has a narrow therapeutic range and requires periodic therapeutic drug monitoring. Although many studies have reported the effects of intrapatient variability of tacrolimus on survival, rejection, and complications in renal transplant recipients, very few studies have reported these effects in liver transplant recipients. The purpose of this study was to evaluate the effect of intrapatient variability of tacrolimus on clinical outcomes after liver transplantation. Methods: Intrapatient variability was calculated using individual, averaged tacrolimus concentrations. Patients were divided into two groups according to their median variability value: high-variability and low-variability groups. The rate of deviation from the therapeutic range, incidence of acute rejection, post-transplant diabetes, incidence of infection, and estimated glomerular filtration rate (eGFR) after transplantation were compared between the groups. Results: Of the total patients (n=82), the high-variability group (n=41) exhibited significantly greater deviation from the therapeutic range (65.92% vs. 56.84%; p<0.001). There was no significant difference in acute rejection or post-transplantation diabetes incidence or eGFR; however, the number of infection in the first 6 months was significantly lower in the low-variability group (0.4 vs. 0.9 times; p=0.039). Multiple linear regression analysis showed that the number of infection significantly increased as intrapatient variability increased (p=0.015). Conclusion: High intrapatient variability in tacrolimus concentrations was strongly associated with an increased frequency of deviation from the suggested therapeutic range and an increased number of infection.

Efficient Inoculation Method of Beauveria bassiana for Production of Bombycis corpus and Evaluation of Its Liver Protection Activity (백강잠(白彊蠶) 생산을 위한 Beauveria bassiana의 효율적인 접종법 및 백장잠의 간보호 활성 검정)

  • When inoculating with B. bassiana 101A for the mass production of B. corpus, the infection ratio was high with regardless of the treating time with highly-humidity if the concentration of spore was 1.0${\times}$$10^8$ spores/m/, but that was low if the concentration was 1.0${\times}$$10^7$ spores/m/. In the study of the activities according to the coserving temperature or days of the B. bassiana spawn, the infection ratio of 90% was maintained for 12 days in the temperature of $4^{\circ}C$. However, the infection ratio was rapidly dropped to the below of 5% after conserved for 48 hours in the temperature of $25^{\circ}C$. Besides, the activities of the original isolate had no difference after conserved for 12 months in the temperature of $4^{\circ}C$, so that the infection ratio 90% could be mintatined. In the measure of liver-protecting activities of B. bassiana 101A, the recovering effect was 43.5% and 65.7% respectively in the poisonous treatment induced with galactosamine, compared with liver-protecting activities of Silymarin and DDB in the $H_2O$ fraction. In the poisonous treatment induced with $CCl_4$ the recovering effect was 100% and 69.3% respectively, compared with liver-protecting activities of Silymarin and DDB in the EtOAc fraction.

  • Hepatitis E virus (HEV) infections cause epidemic or sporadic acute hepatitis, which are mostly self-limiting. However, viral infection in immunocompromised patients and pregnant women may result in serious consequences, such as chronic hepatitis and liver damage, mortality of the latter of which reaches up to 20-30%. Type I interferon (IFN)-induced antiviral immunity is known to be the first-line defense against virus infection. Upon HEV infection in the cell, the virus genome is recognized by pathogen recognition receptors, leading to rapid activation of intracellular signaling cascades. Expression of type I IFN triggers induction of a barrage of IFN-stimulated genes, helping the cells cope with viral infection. Interestingly, some of the HEV-encoded genes seem to be involved in disrupting signaling cascades for antiviral immune responses, and thus crippling cytokine/chemokine production. Antagonistic mechanisms of type I IFN responses by HEV have only recently begun to emerge, and in this review, we summarize known HEV evasion strategies and compare them with those of other hepatitis viruses.

Protection Against Salmonella Typhimurium, Salmonella Gallinarum, and Salmonella Enteritidis Infection in Layer Chickens Conferred by a Live Attenuated Salmonella Typhimurium Strain

  • In the present study, we investigated the protection conferred by a live attenuated Salmonella enterica serovar Typhimurium (ST) strain against Salmonella Typhimurium, Salmonella Gallinarum (SG), and Salmonella Enteritidis (SE) infection in layer chickens. Birds were orally primed with the attenuated ST strain at 7 days of age and then boosted at 4 weeks post prime immunization (PPI). Sequential monitoring of plasma IgG and mucosal secretory IgA (sIgA) levels revealed that inoculation with ST induced a significant antibody response to antigens against ST, SE, and SG. Moreover, significant lymphoproliferative responses to the 3 Salmonella serovars were observed in the immunized group. We also investigated protection against virulent ST, SE, and SG strain challenge. Upon virulent SG challenge, the immunized group showed significantly reduced mortality compared to the non-immunized group. The reduced persistence of the virulent ST and SE challenge strains in the liver, spleen, and cecal tissues of the immunized group suggests that immunization with the attenuated ST strain may not only protect against ST infection but can also confer cross protection against SE and SG infection.

Vibrio vulnificus infection: a persistent threat to public health

  • Vibrio vulnificus is a gram-negative bacterium that can cause serious, potentially fatal infections. V. vulnificus causes three distinct syndromes: an overwhelming primary septicemia caused by consuming contaminated seafood, wound infections acquired when an open wound is exposed to contaminated warm seawater, and gastrointestinal tract-limited infections. Case-fatality rates are higher than 50% for primary septicemia, and death typically occurs within 72 hours of hospitalization. Risk factors for V. vulnificus infection include chronic liver disease, alcoholism, and hematological disorders. When V. vulnificus infection is suspected, appropriate antibiotic treatment and surgical interventions should be performed immediately. Third-generation cephalosporin with doxycycline, or quinolone with or without third-generation cephalosporin, may be potential treatment options for patients with V. vulnificus infection.
