The Effects of Cognitive Therapy in Major Depressive Disorder (주요우울장애에 대한 인지치료의 효과)

  • Objectives : Cognitive therapy is the most extensively researched psychological treatment for nonpsychotic unipolar outpatient depressive disorders. This review focused on the utility of this approach in severe or chronic depressive disorders, in relapse prevention and also on the potential benefits of combining cognitive therapy with medication. Methods : The author reviewed original studies and quantitative analyses on the effects of cognitive therapy, predictors of response, and neuroimaging studies of cognitive therapy in major depressive disorder. The sources used for the literature search were data bases : PubMed, EMBASE, CDSR on the internet, references in papers or books. Results : This review suggests that cognitive therapy is as effective as antidepressant medication in severe depressive disorders. And cognitive therapy can be an effective alternative to antidepressant medication. Patients benefited significantly more from combined cognitive therapy and antidepressant treatment than from either treatment alone. Most importantly, the addition of cognitive therapy to usual treatment appears to protect against future relapse in individuals known to be at high risk of repeated episodes of depression. In addition, subjects who received cognitive therapy showed significantly greater improvements in chronic depression than receiving antidepressant medication. Pooled data suggests that there is a significant relationship between the therapist's level of training or experience, the type of therapy used and patient outcome. Recent functional imaging studies examining brain changes following cognitive therapy report a variety of regional effects, but there is no consistent pattern across the few published studies. Conclusion : Cognitive therapy has proved beneficial in treating depressive patients. Despite empirical data supporting its efficacy, there are still problems in gaining access to cognitive therapy in clinical practice.

  • Qigong is one energy-healing intervention used to prevent and cure ailments and to improve health through regular practice. Although qigong-neither itself nor its postulated mechanism of action - are within the paradigm of modern Western medical science, effects on the human body could be possible. This study aims to review the bibliography, biological responses and therapeutic effects of Qigong. In the process, this review will grasp trends in this field of studies and will direct further researches into the right direction. The computerized Korean databases were searched from their respective inceptions up to January 2008. The search terms used were 'qi', 'qigong', 'doin', 'training', 'bioenergy', 'life nurturing' and random or Korean language terms related to qigong. Several specialized journals were also manually searched for relevant articles. Since the 1990s, Qigong papers in the Korean Literature is increased. Published 152 papers on Korean medicine were more than Physical education and Nursing. 78 papers published in The Journal of Korean Academy of medical Gi-gong are the best in many areas related to Korean medicine. Most people have submitted papers related to Qigong was Lee Gi-nam. The depth study for the each Qigong is needed. Specifically, I think it should be a clinical studies and qualitative research methods for evaluation are needed.

Literature Review on Syndrome Differentiation and Herbal Medicine of Sjogren's Syndrome - Focusing on Chinese Traditional Medicine's Journals - (쇼그렌 증후군의 변증과 처방에 관한 문헌적 고찰 - 중국 논문 중심으로 -)

  • The objective of this study was to investigate the diagnosis and treatment of Sjogren's syndrome(SS) such as syndrome differentiation and herbal medicine by reviewing Chinese traditional medicine's journals. The journal search was carried out using China National Knowledge Infrastructure(CNKI) and PubMed from January 2007 to July 2012. Searching key words were the various combination of "Sjogren's syndrome", "Traditional Chinese Medicine", "herbal medicine", and "syndrome differentiation". The final selection of 57 studies were extracted and summarized by two researchers independently. The syndrome differentiation was classified as yin deficiency with fluid depletion, yin deficiency with dryness heat, dryness toxin with yin damage, internal obstruction of static blood, dual deficiency of qi and yang, dampness-heat obstructing, wind with external contraction, liver qi depression, blood deficiency and wind-dryness, dual deficiency of yin and yang, and internal obstruction of phlegm-blood stasis. Liriope platyphylla(麥門冬), Rehmania glutinosa(生地黃), and Scrophularia buergeriana(玄蔘) were primarily prescribed to tonify yin, engender fluid and moisten dryness.

  • Keloid scars are often considered aesthetically unattractive and frustrating problems that occur following injuries. They cause functional and cosmetic deformities, displeasure, itching, pain, and psychological stress and possibly affect joint movement. The combination of these factors ultimately results in a compromised quality of life and diminished functional performance. Various methods have been implemented to improve keloid scars using both surgical and non-surgical approaches. However, it has proven to be a challenge to identify a universal treatment that can deliver optimal results for all types of scars. Through a PubMed search, we explored most of the literature that is available about the intralesional injection treatment of hypertrophic scars and keloids and highlights both current (corticosteroid, 5-fluorouracil, bleomycin, interferon, cryotherapy and verapamil) and future treatments (interleukin-10 and botulinum toxin type A). The reference lists of retrieved articles were also analysed. Information was gathered about the mechanism of each injection treatment, its benefits and associated adverse reactions, and possible strategies to address adverse reactions to provide reliable guidelines for determining the optimal treatment for particular types of keloid scars. This article will benefit practitioners by outlining evidence-based treatment strategies using intralesional injections for patients with hypertrophic scars and keloids.

  • Purpose: The purpose of the study was to review functioning components of studies for measuring functional information using International Classification of Functioning, disability and health (ICF) in Korea. Methods: A literature review was conducted and eligible studies were identified via search of RISS, KISS, DBpia, KoreaMed, PubMed, and ISI databases published from 2001 to 2015. For inclusion in this review, a study should be published in a peer-review journal, must have participation of Korean researchers and/or developed in Korea, and should provide functioning data related ICF. Descriptive studies containing only ICF concepts and review studies were also excluded. Collected functioning data in each study were analyzed using frequency based on ICF domain. Data assessment was performed by two independent reviewers. Results: Eighty publications were included in the analysis. The majority of studies collected data from clinical patients (n=38) and from the disabled (n=28). Fourteen studies assessed functioning data from the elderly and students under special education. The studies reported functioning data using various tools, including ICF itself, ICF checklist, coresets, and conventional measurement tools. Body function domain was most commonly used and the least used domain was the body structure across areas. Interestingly, increasing data related to environmental factors was observed in all studied populations. Conclusion: Functioning data was collected from four domains of ICF across professional areas. The most common collected data was body function and activity and participation for which conventional measurement tools are already available. To understand clients, components of environmental factors that might influence a person's functioning should be considered.

Meta-Analysis on the Effect of Therapeutic Horseback Riding on Children with Developmental Disabilities and Neural Patients

  • Purpose: This study aimed to investigate the evidence that therapeutic horseback riding can improve balance, muscle, ADL, equivalenc, GMFM, gait, emotion with developmental disabilities and neural patients. Methods: To conduct meta-analysis, the search focused on studies that employed therapeutic horseback riding for developmental disabilities and neural patients for which eight databases (KIS, RISS, DBpia, National Assembly Library, Pubmed, Embase, Google scholar and Cochrane Library) were used to extract literature published from 2002 to September 2019. The data were analyzed the RevMan 3.5.3 program. Results: As a result of meta-analysis, therapeutic horseback riding total effect size is 0.552 for children with developmental disabilities and neural patients. And effect size result of according to assessment type variable first, balance effect size is 0.594. Second, muscle activities effect size is 0.425. Third, ADL effect size is 0.430. Fourth, equivalance effect size is 0.640. Fifth, GMFM effect size is 0.482. Sixth, gait effect size is 0.400 and seventh emotion effect size is 0.876. Conclusion: These findings is horseback riding is effective The effect size by outcome was observed to be the effective for children with developmental disabilities and neural patients. and also the horseback riding provided the positive effects of balance, muscle activities, ADL, equivalance, GMFM, gait, emotion for children with developmental disabilities and neural patients. It is hoped that this study will contribute to the development of effective treatments for children with developmental disabilities and neural patients therapeutic horseback riding and the development of study.

Meta-analysis on the Blood Lipids as Risk Factors of Coronary Heart Diseases in Koreans (한국인 관상동맥성 심질환의 위험요인으로서 혈청지질에 관한 메타분석)

  • Objectives : To determine the relations between seven blood lipids such as total cholesterol(TC), triglyceride(TG), HDL-cholesterol(HDL), LDL-cholesterol(LDL), apolipoprotein A-1(Apo A1), apolipoprotein B(Apo B) and lipoprotein(a)(Lp(A)) and the coronary heart diseases(CHD), the quantitative techniques of meta-analysis were applied to studios of blood lipids and CHD in Koreans. Methods : We searched the Korean and the English literature published from 1980 to August, 1997 by manual search and bibliography review. Information on sample size, study design, participant characteristics(gender, age) and blood lipid levels were abstracted by reviewers using inclusion criteria. Estimates of the effect sizes of blood lipid levels on CHD in Koreans and corresponding 95% confidence intervals were calculated using random-effect models. Results : We identified 16 case-control studies to apply meta-analysis. The overall effect sizes for CHD were 20.3(95% CI: 14.23-20.22) in TC, 24.8(95% CI: 12.6-36.86) in TG, 15.16(95% CI: 3.99-26.33) in LDL, -3.48(95% CI: -5.79 - -1.17) in HDL, -9.78(95% CI: -16.98 - -2.58) in Apo-a1, 17.88(95% CI: 9.72-26.05) in Apo B and 18.95(95% CI: 17.88-20.02) in Lp(a). Conclusions : Our results suggested that seven blood lipids were significantly associated with CHD in Koreans. Well-designed and prospective studies between blood lipids and CMD in Koreans should be peformed.

  • The purpose of this study was to compare the concept of community and community health, community health assessment tool, and community health nursing diagnosis based on the concept of 'Community as Client'. The method for this purpose was to search the articles and textbooks related to community assessment and review the contents by the researchers who were 5 community health nursing faculties and 1 doctoral candidate. The sources of articles were limited in Public Health Nursing and the Journal of Community Health Nursing. As the result, three types of conceptual model were classified: epideiological model. fuctional model. system model. System model by Newman and Helvie included more comprehensive concept of community health than others. Helvie model suggested the most specific indicators among them. The components of nursing diagnosis in the system model had the subjectives. problems and the related factors. It makes the nursing care plan related to the nursing diagnosis. But there was no nursing diagnosis system among the three model. It is needed to compare the nursing intervention based on the concept of 'Community as Client'. It will be helpful to the community health nursing practice to develop the nursing diagnosis system based on the system model. For the community health nursing education, it is suggested to try the case study by the using three types of model. Finally, it is needed to validate the community assessment tool in Korean setting.

Literature Review on Syndrome Differentiation and Herbal Medicine of Reflex Esophagitis - Focusing on Chinese Journals - (역류성 식도염의 변증과 처방에 관한 문헌적 고찰 - 중국 논문 중심으로 -)

  • Objectives : This study investigated oriental diagnosis and treatment of reflex esophagitis (RE) such as syndrome differentiation and herbal medicine by reviewing Chinese journals. Methods : A journal search was performed using China National Knowledge Infrastructure (CNKI) from January 2006 to December 2010. The keywords were the various combinations of 'reflex esophagitis', 'Chinese medicine', 'syndrome differentiation', and 'herbal medicine'. All types of journal entries that explained or referred to herbal medicines and definite syndrome differentiations were included. The Types of the journals were randomized clinical trials (RCT), reviews, essays, case series and reports. Vague syndrome differentiations and experimental studies were excluded. Selected journals were extracted and summarized by two researchers independently. Results : 47 Chinese entries were finally selected. 61% of them were case-control trials that had non-randomized, active controlled clinical trials. 36 kinds of syndrome differentiations about RE were investigated. We summarized and divided them into 10 syndrome differentiation groups. 肝胃不和(syndrome of liver qi invading the stomach) in syndrome differentiation, 和胃降逆 (harmonize the stomach and direct qi downward) in method of treatment, and Jwa kum-whan (左金丸) were cited with high frequency in RE. Conclusions : We find that many kinds of syndrome differentiation groups and herbal medicines for reflex esophagitis are used in China, compared with Korea. Further studies will be required about oriental syndrome differentiation and treatment of this disease.

  • Objectives : The purpose of this study was to investigate two subjects: the diagnostic value of bilateral lowering of electrical activity at point H4,5,6 of Ryodoraku and the mechanism for Ryodoraku phenomena. Methods : Electrical activities of Ryodoraku test and electrogastrography recorded simultaneously and monitored continuously from 16 cases of functional dyspeptic patients were collected and their variations were grouped by the topics of discussion which were peculiarity, stability, lagging, alterability, and anomaly. Ryodoraku recordings obtained from 6 patients with different gastrointestinal diseases and 1 normal healthy person were used as control. The results are discussed with Nakatani's suggestion, theory of sympathetic nerve and Meridian Principle, respectively. Finely, coincidence of stomach arrangement between anatomy and meridian system in Ryodoraku was also evaluated. Results : Time-course variation showed a regular relationship between the typical pattern of Ryodoraku at point H4,5,6 and gastric myoelectrical activity. However, an irregular relationship and atypical pattern of Ryodoraku occasionally appeared. A literature search suggested that electrical response at the Ryodoraku point H4,5,6 may be dependent on an afferent sympathetic spinal reflex transmitted from the stomach. However, there was no evidence for making clear whether bilateral lowering of electrical activity at this point was induced by hypofunction of local sympathetic nerve in the skin itself or of signals transmitted from the gastric sympathetic nerve or not. The coincidence of 19% could not provide a visceral arrangement of the stomach between anatomy and meridian systems. Conclusions : Bilateral lowering of electrical activity at Ryodoraku point H4,5,6 has value as a diagnostic index for gastric dysmotility of functional dyspepsia. This phenomenon is associated with spinal reflex transmitted from the afferent sympathetic nerve in the stomach but not that of meridian function.
