Spinal Cord Stimulation in the Treatment of Postherpetic Neuralgia in Patients with Chronic Kidney Disease: A Case Series and Review of the Literature

  • Background: Postherpetic neuralgia (PHN) is usually managed pharmacologically. It is not uncommon for patients with chronic kidney disease (CKD) to suffer from PHN. It is difficult to prescribe a sufficient dose of anticonvulsants for intractable pain because of the decreased glomerular filtration rate. If the neural blockade and pulsed radiofrequency ablation provide only short-term amelioration of pain, spinal cord stimulation (SCS) with a low level of evidence may be used only as a last resort. This study was done to evaluate the efficacy of spinal cord stimulation in the treatment of PHN in patients with CKD. Methods: PHN patients with CKD who needed hemo-dialysis who received insufficient relief of pain over a VAS of 8 regardless of the neuropathic medications were eligible for SCS trial. The follow-up period was at least 2 years after permanent implantation. Results: Eleven patients received percutaneous SCS test trial from Jan 2003 to Dec 2007. Four patients had successfully received a permanent SCS implant with their pain being tolerable at a VAS score of less than 3 along with small doses of neuropathic medications. Conclusions: SCS was helpful in managing tolerable pain levels in some PHN patients with CKD along with tolerable neuropathic medications for over 2 years.

  • Cerebral dural sinus thrombosis(CDST) has been described as a rare disease with a variety of patho-etiological factors. The diagnosis of CDST is difficult due to various symptoms and signs, none of which is specific to CDST. But timely diagosis is critical for effective management. The introduction and widespread use of computed tomography(CT), magnetic resonance imaging(MRI) and cerebral angiography made early diagnosis of CDST possible. In particular, MR venography is the most useful tool for establishing a correct diagnosis quickly. In early literature, mortality ranked between 30% and 50% but in more recent series it is between 5.5% and 30%. With the advent of diagnostic and therapeutic tools, early diagnosis and proper management has made the prognosis better. The appropriate therapy for CDST, however, has been the subject of much cortroversy. Individual variations of the venous system and collateral vessels are key factors to decide the proper treatment. In this report, we present two cases with symptomatic CDST treated without open surgical or direct endovascular interventions with good outcome.

Concept Analysis of Preparatory Grief in Terminal Cancer Patients (암환자의 예비슬픔[preparatory grief]에 대한 개념 분석)

  • Purpose: The purpose of this study was to conceptualize and clarify a concept of "preparatory grief" in terminal cancer patients. Method: A hybrid model of concept development was applied to develop a concept of preparatory grief, which included a field study carried out in Busan, Korea. Participants of this study were 8 cancer patients. Results: On the basis of our literature, research and clinical experience, the concept of preparatory grief emerged as a complex phenomenon playing an important role in five areas; physical, emotional, interpersonal, religious, and transcendental dimensions. Two new attributes were defined through a field phase; trust of the post-mortal world and a serene state of mind. Indicators reflected attitudes of sadness, worry, regret, capability to adapt and hope. The results of preparatory grief were loss of energy and interest, emotional chaos, contemplation, taciturnity and restoration. Conclusions : Differentiating among preparatory grief and other symptoms in cancer patients is essential because of therapeutic implications. Understanding preparatory grief is necessary in order to manage cancer patients for promoting quality of life so that its application may have a positive impact on the patient's life.

  • Background: The aim of this study was to investigate whether electrochemotherapy is a clinically and cost-effective treatment option against skin tumors. Materials and Methods: We performed an analysis of the current literature based on database searches in PubMed/MEDLINE and we included articles till July 2012. Terms used for the search were 'electrochemotherapy', 'skin cancer', 'recurrence', and 'cutaneous and subcutaneous tumors'. Only papers published in English were included. In addition, we performed an analysis of the cost effectiveness of the method. Results: The combination of physics and chemistry is the foundation for electrochemotherapy and its efficacy, independent of the tumor histology. Clinical data showed that ECT is well tolerated and can be used in difficult cases without other available treatment options. The analysis also showed that the treatment is feasible and cost-effective. Conclusions: Electrochemotherapy is a clinically efficient safe and cost-effective treatment and clinicians should not hesitate to use it as alternative therapeutic modality or as palliative treatment.

  • Cleidocranial dysplasia is rare inherited skeletal dysplasia. It was first reported by Martin in 1765. Subsequently, Marie and Sainton independently documented the criteria of the disease. Cleidocranial dysplasia is a bone disorder caused by a defect in the CBFA1 gene of chromosome 6p21. This gene guides osteoblastic differentiation and appropriate bone formation. Patient with cleidocranial dysplasia has maxillary deficiency, high and narrow palate, prolonged retention of primary teeth, unerupted permanent teeth and supernumerary teeth. Therapeutic options in these patients include of autotransplantation of selected impacted teeth, forced eruption of permanent teeth, full denture, overdenture and implant-supported prosthesis. We report a patient with cleidocranial dysplasia. This patient was treated with implant supported bar overdenture. Despite of gene defect that affects osteoblastic activity, bone remodeling and osseointegration occurred in our patient. So, we report this case with review of literature.

Literature on Applications of Prescriptions Including Pueraria Thunbergiana Bentham in Dongueibogam ("동의보감(東醫寶鑑)" 중 갈근(葛根)이 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察))

  • This report carried out to study 59 prescriptions related to the use of Pueraria thunbergiana bentham in Dongueibogam This study is based on Dongueibogam which has been widely used for herbal clinic in Korea. And this study is investigated to make sure of the range of treatment, nature of disease, the pathology and the dosage of Pueraria thunbergiana bentham in Dongueibogam. The following conclusions were reached through investigations on the prescriptions that used in Dongueibogam as a various role. 1. The prescriptions that Pueraria thunbergiana bentham was taken as a 15 for monarchy, 15 for attendant, 11 for assistance and valet herb are utilized for therapeutic treatments such as cough, fever etc, mainly Sanghan syndromes in the chapter of Wind. 2. The Pueraria thunbergiana bentham was also utilized as a key ingradients in the chapter of Eye and Onyeok epidemic. 3. The most frequent mass of the Pueraria thunbergiana bentham when used as a monarchy herb is 2jeon, and the standard limited range of adding Pueraria thunbergiana bentham to medical prescription is 7pun$\sim$5jeon per dose. But, when used as a attendant herb, an typical and regular adding dose is the range of 3pun5ri$\sim$1yang2.5jeon,and 3pun$\sim$1.5jun in the case of assistant. 4. The phathogenic factors and phathology of Pueraria thunbergiana bentham in the prescription are based on the chapter of Cold when used as a monarchy herb and on the chapter of Wind in the case of attendant herb. 5. The Seungmagalgeontang is the most useful base prescription which use the Pueraria thunbergiana bentham as the main ingredient.

Baduanjin Exercise for Shoulder Pain: A Systematic Review (견관절 통증에 대한 팔단금 운동의 효과: 체계적 문헌 고찰)

  • Objectives : This study aimed to verify the clinical effectiveness of Baduanjin for shoulder pain. Methods : We searched 10 electronic databases (PubMed, EMBASE, Cochrane Library, CAJ, KISS, KISTI, KMBASE, RISS, NDSL, and OASIS) up to May 2020. We included randomized controlled trials (RCTs) using Baduanjin for shoulder pain. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : As a result of the search, a total of 6 RCTs were eligible for analysis. Of these 6 studies, one applied Baduanjin alone and 5 cases used Baduanjin in combination with other treatments, such as acupuncture or electroacupuncture. Except for 2 studies, descriptive analysis was performed without meta-analysis due to the high level of heterogeneity between studies. A high risk of bias was observed in all studies. Conclusions : When considering the results of the included papers, there was limited information derived on the therapeutic effect of using Baduanjin as a single intervention, but the results showed significant effectiveness when Baduanjin was combined with other oriental interventions, such as acupuncture and electroacupuncture, in the clinical field. Although all RCTs included in the analysis were biased to CAJ and the number was limited, this study was conducted in accordance with the methodological process of systematic literature reviews and revealed the clinical effects of Baduanjin on shoulder pain.

  • Objectives: This study provides evidence for clinical practices by analyzing present clinical Korean medicine articles about benign paroxysmal positional vertigo (BPPV). Methods: Articles published up to July, 2017 were found by searching "BPPV", "Benign positions", and "Vertigo" as keywords in three domestic electric databases (i.e., OASIS, NDSL, RISS). Results: The initial search resulted in 228 articles; however, only 20 articles satisfied the selection criteria and were analyzed. Of these, 40% were published in the Journal of Korean Oriental Internal Medicine, and 35% were published in 2003. When diagnosing BPPV patients, VAS scores are primarily used for clinical assessments rather than objective diagnostic tools. Banhabaekchulcheonmatang was the most frequent herbal medicine used to treat BPPV, and the most commonly used therapeutic acupuncture point was Jok-samni (ST36). The most common diagnosis of BPPV was dam-eum according to nine studies. Conclusions: Systematic and large-scale research for the standardization of BPPV treatment is needed.

  • Objectives : The Hwabyung Research Center of The Korean Society Of Oriental Neuropsychiatry has attempted to develop the 'Clinical Guidelines for Hwabyung'. Methods : The Hwabyung Research Center constructed a committee of experts and advisory group. Relevant literature was collected and evaluated in order to find out effective Pharmacotherapy (Herbal medicine), acupuncture and moxibustion, and cupping of Hwabyung. Results : We found some evidences that proved the effectiveness of Pharmacotherapy (Herbal medicine), acupuncture and moxibustion, and cupping in the treatment of Hwabyung. 1) We recommend Pharmacotherapy (Herbal medicine), acupuncture and moxibustion, cupping, psychotherapy and management, other therapeutic approaches and education for Hwabyung. 2) Pharmacotherapy (Herbal medicine) should be determined according to the pattern identification of Korean medicine. Further, the prescription should be cost-effective, safe and have no side effects. Also, the interaction between Herbal and Western drugs should be considered carefully. 3) There are general acupuncture, scalp acupuncture, Pharmacopuncture, Sa-am acupuncture as well as acupuncture treatments for Hwabyung. 4) Moxibustion and cupping are useful, relieving various somatic symptoms of Hwabyung. Conclusions : Pharmacotherapy (Herbal medicine), Acupuncture and Moxibustion, Cupping for Hwabyung were studied. We hope that the 'Clinical Guidelines for the Treatment of Hwabyung' are helpful for oriental medical doctors and patients.

Research Trends on the Clinical Studies of Qigong in the Korean Journal (한국 저널에 게재된 기공관련 대조군 임상연구 현황 분석)

  • Qigong is one energy-healing intervention used to prevent and cure ailments and to improve health through regular practice. Although qigong -neither itself nor its postulated mechanism of action- are within the paradigm of modern Western medical science, effects on the human body could be possible. This study aims to review the bibliography, biological responses and therapeutic effects of Qigong. In the process, this review will grasp trends in this field of studies and will direct further researches into the right direction. The computerized Korean databases were searched from their respective inceptions up to January 2008. The search terms used were 'qi', 'qigong', 'doin', 'training', 'bioenergy', 'life nurturing' and random or Korean language terms related to qigong. Several specialized journals were also manually searched for relevant articles. Since the 1990s, Qigong papers in the Korean Literature is increased. Clinical research studies are among the most control design study. Research subjects are less patient than the general public. The most common treatment disease was hypertension. The depth study for the each Qigong is needed. Specifically, I think it should be a clinical studies and qualitative research methods for evaluation are needed.
