Cervicomedullary Junction AVM Presenting Recurrent Intraventricular Hemorrhage - A Case Report - (재발성 뇌실내출혈을 일으킨 경연수접합부 동정맥기형 - 증례보고 -)

  • Cervicomedullary junction arteriovenous malformation(AVM) is extremely rare. The authors present a case of a cervicomedullary junction AVM in a 31-year-old woman presenting with recurrent intraventricular hemorrhage (IVH). Magnetic resonance imaging revealed the AVM(of a size of approximately $2{\times}2.5{\times}4cm$) extending from a lower medulla to C2-3 level. Vertebral angiography demonstrated a tightly coiled vascular mass with multiple feeders (radiculomedullary arteries) and irregular-shaped aneurysm at distal part of feeder originating at right C-1 level. The patient underwent superselective embolization of upper nidus and the aneurysm. The pertinent literature is reviewed, and diagnostic and therapeutic implications are discussed.

  • Metabolic syndrome has become quite prevalent within our society. Over the past two decades, the prevalence of metabolic syndrome has sharply increased worldwide and it has become a major public health problem in several countries. It is associated with the global epidemic of obesity and diabetes mellitus and imposes numerous cardiovascular risks. Prostate cancer is the second most common cancer among men, surpassed only by non-melanoma skin cancer. A considerable body of evidence exists suggesting that some components of the metabolic syndrome have been associated with the risk of prostate cancer. These components include obesity, an abdominal fat distribution, and hyperinsulinemia. Androgen deprivation therapy (ADT) is the most widely used therapeutic modality in prostate cancer. It changed the body composition and lipid profile of men with prostate cancer. Androgen deficiency is associated with increased levels of total cholesterol, low-density lipoprotein (LDL)-cholesterol, increased production of proinflammatory factors, and increased thickness of the arterial wall and contributes to endothelial dysfunction. The aim of this review is to evaluate the association between metabolic syndrome and prostate cancer and to discuss the implications of androgen deficiency in men with cardiovascular risk factors. A comprehensive literature search was carried out with the use of PubMed from 1980 through 2011, and relevant articles pertinent to metabolic syndrome and prostate cancer are evaluated and discussed.

  • Aconitum Heterophyllum (A. Heterophyllum) is an indigenous medicinal plant of India and belongs to the family Ranunculaceae. A. Heterophyllum is known to possess a number of therapeutic effects. For very ancient times, this plant has been used in some formulations in the traditional healing system of India, i.e., Ayurveda. It is reported to have use in treating patients with urinary infections, diarrhea, and inflammation. It also has been used as an expectorant and for the promotion of hepatoprotective activity. The chemical studies of the plant have revealed that various parts of the plant contain alkaloids, carbohydrates, proteins and amino acids, saponins, glycosides, quinones, flavonoids, terpenoids, etc. In the present study, a comprehensive phytochemistry and pharmacognosy, as well as the medicinal properties, of A. Heterophyllum are discussed. Scientific information on the plant was collected from various sources, such as electronic sources (Google scholar, Pubmed) and some old classical text books of Ayurveda and Ethnopharmacology. The study also presents a review of the literature on A. Heterophyllum, as well as the primary pharmacological and other important findings on this medicine. This review article should provide useful information to and be a valuable tool for new researchers who are initiating studies on the plant A. Heterophyllum.

  • In the Study of Poem in Health-Preservation, the results were as follows : Though poetry as therapy is a relatively new development in the expressive arts, it is as old as the first chants sung around the tribal fires of primitive peoples. For many centuries the link between poetry and medicine remained obscure. The chant/song/poem is what heals the heart and soul and is used for health-Preservation and the well-being. Poetry Therapy began to flourish in the hands of professional in various disciplines, including rehabilitation, education, library science, recreation, and the creative arts. Mental health professional were exploring the therapeutic value of literary materials, especially of poetry. Their contribution to the emerging discipline was two-fold : 1) emphasis on the evocative value of literature, particularly poetry; and 2) recognition of the beneficial potential of having clients write either their response to poems written by others or original material, drawing on the clients' own experiences and emotions. Especially in Oriental Medicine, the therapy is based on controlling of Mind(心). And it is in harmony with Qi-circulation(氣-循環), so smoothing the circulation of meridians, strengthened Essential-material(精), Qi(氣), Sprit(神).

  • Purpose: This study is to propose direction for healing environment design in hospital architecture considering the characteristics of elderly patients. The primary goal of a public hospital is providing cheap health care and quality service chance to the underprivileged and elderly in the dead zone. Compared to the rapid increase of the aged population and chronic diseases, Ministry of Health & Welfare is currently planning model of health promotion hospital in development plans of local based public hospital. Due to the increased elderly medical expenses, elderly patients' high utilization is considered. Methods: The literature on the design factors of healing environments were investigated. based on this, analyzed drawings of surveyed hospitals ward in architectural characteristics and observe and evaluate directly healing environment design through field surveys. Results: The design of hospital environment affects patient's therapeutic effect. There is no any official formula for hospital design but environment that architect create could be a big part of the healing process. To increase the quality of the environment, apply the characteristic factors and harmonize well as a human-centered healing environment. Implications: Healing environment design for the elderly is first necessary condition due to high proportion of elderly patients.

  • We have experienced a case of comparatively rare anteromedial meniscofemoral ligament (AMMFL) in which the anterior horn of the medial meniscus was attached to the posterolateral wall of the femoral intercondylar fossa. AMMFL was diagnosed and confirmed during therapeutic arthroscopy. The report suggests the need for increased awareness of the possible presence of this. Additionally, we illustrate the anomaly, discuss its controversial clinical significance and review the literature.

  • Pulmonary lymphangioleiomyomatosis is a chronic destruct8ive disease of the lung affecting women of childbearing ages which eventually leads to respiratory failure. Lung transplantation is the only conclusive therapeutic measure because this disease responds poorly to other therapies, To date only a few reports in the literature describes the clinical experience of the bilateral sequential lung transplantation of this rare condition. We performed a bilateral sequential lung transplantation on a 32-year-old woman suffering from lymphangioleiomyo-matosisw. The heart-lung block was harvested from a 51-year-old donor. We transplanted the left lung first through the clam-shell incision. As the hemodynamics deteriorated suddenly during the dissection of the right lung the right lung was transplanted under the cardio-pulmonary bypass. Although the patient's lung function was initially satisfactory the patient died of sepsis and subsequent cardiogenic shock at the postoperative 18th day. Autopsy findings showed infection of Candida albicans on the pericardium and the left lung which had been initiated possibly from the left bronchial anastomosis site,. Through detailed review of the clinical course we concluded that lung transplantation could have been performed safely on this disease provided that early diagnosis and proper management or the oppor-tunistic infection have been carried out.

  • The poor survival rates of patients with carcinoma of a tongue, despite of modern therapy, is well recognized. One of the most important prognostic factors is status of the cervical lymph nodes. There have been a long-standing debate about the treatment of cervical lymph nodes in early-stage tongue cancer. There are two major treatment opinion. The one is surgical excision of primary tumor with prophylactic neck dissection, simultaneously, and the other is to delay the cervical therapy until cervical lymph node is palpable. Recently we have experienced the early cervical metastasis in three patients who had been diagnosed as a carcinoma of the tongue. They were T1, T2 lesion and no palpable node was found. But histopathologic examination showed the occult metastasis or delayed cervical metastasis was occured. By the review of literature and clinical experience, we could conclude the prophylactic neck dissection offers a better chance for success than therapeutic neck dissection of palpable lymph nodes, in case of oral tongue cancer.

Development of Nursing Intervention Protocol for Childhood Cancer at Early Diagnosis Stage (소아암 환자의 초기 진단단계의 간호중재 프로토콜 개발)

  • The main purpose of this methodological study was to develop an assessment tool and intervention protocol for child and family with childhood cancer at early diagnosis stage. The assessment tool and intervention protocol was developed by extensive literature review and consultation with experts. Review of nine domestic and sixty-six international journal articles were done to identify stress, interventions, coping strategies and adjustment of children with cancer and their family. Results were as follows; First, assessment at the early diagnosis stage need to include information on patient, family, and patient/family attitude toward diagnosis and treatment. Second, intervention protocol for children with cancer includes control physical symptoms, manage the side effects of chemotherapy and diagnostic or therapeutic procedures, control emotional responses, provide support and information, assist decision-making and adjust to environment. Third, intervention protocol for family includes controlling emotional responses, provision of informations, inducing family support to patient, improving family cohesion, supporting siblings and supporting spiritual growth. In conclusion, the early diagnosis stage in cancer treatment is important for child and family since this stage greatly affects the overall adjustment of child and family to live with cancer. Therefore, pediatric nurses need to be sensitive to the need of patient/family and systematically manage their needs at this stage.

Efficacy and Safety of Gabapentin in the Treatment of Chronic Cough: A Systematic Review

  • Despite recent clinical guidelines, the optimal therapeutic strategy for the management of refractory chronic cough is still a challenge. The present systematic review was designed to assess the evidence for efficacy and safety of gabapentin in the treatment of chronic cough. A systematic search of PubMed, Embase, Cochrane Library databases, and publications cited in bibliographies was performed. Articles were searched by two reviewers with a priori criteria for study selection. Seven relevant articles were identified, including two randomized controlled trials, one prospective case-series designed with consecutive patients, one retrospective case series of consecutive patients, one retrospective case series with unknown consecutive status, and two case reports comprising six and two patients, respectively. Improvements were detected in cough-specific quality of life (Leicester Cough Questionnaire score) and cough severity (visual analogue scale score) following gabapentin treatment in randomized controlled trials. The results of prospective case-series showed that the rate of overall improvement of cough and sensory neuropathy with gabapentin was 68%. Gabapentin treatment of patients with chronic cough showed superior efficacy and a good safety record compared with placebo or standard medications. Additional randomized and controlled trials are needed.
