Surgical correction of cleft lip lower-lip deformity: a report of three cases

  • Cleft lip lower-lip deformity is a secondary deformity in patients who underwent primary cheiloplasty of the upper lip, characterized by an enlarged and anteriorly rotated lower lip. In these cases, soft-tissue imbalances remain even after skeletal correction with orthognathic surgery, and additional soft tissue treatment is required for lip harmony and esthetic facial balance in CLP (cleft lip palate) patients. This study describes three cases of transverse myomucosal excision of the lower lip for correction of cleft lip lower-lip deformity to restore facial esthetic balance. Each patient underwent orthognathic surgery, rhinoplasty, or upper lip revision cheiloplasty according to condition. Postoperatively, volume of the lower lip decreased and lip harmony was improved in all three patients. The surgeon should fully understand the anatomical structure around the lips and be able to evaluate overall harmony of the soft tissue. When a lower lip deformity is present, careful surgical planning and execution are important for each patient.

개선된 Noordhoff 방법을 이용한 양측성 구순열의 교정 (Correction of Bilateral Cleft Lip Using Modified Noordhoff Technique)

  • Purpose: The authors accessed the anthropometric measurements of fourty non-cleft normal a three-month-old infant and using this obtained data as a basic guideline, authors applied the modified Noordhoff technique for the treatment of bilateral cleft lip. Methods: Over a period of 10 years, a total of 21 bilateral cleft lips were operated. 13 cases of complete and 8 cases of incomplete bilateral cleft lip and palate. In the complete type of bilateral cleft palate, elastic head cap and passive intraoral appliance were applied at 1 to 2 week of age for 2 months duration. The definitive cheiloplasty was performed at 3 months of age using the modified Noordhoff technique. Results: After a follow-up period ranging one to nine years, most patients presented with cosmetically and functionally satisfying results, with an exception of two cases where an undesired peaking effect of the vermilion and dimpling of the vermilion mucosa was encountered. Conclusion: Accessing the anthropometric measurements of fourty non-cleft normal three-month-old infant and using this obtained dara as a guideline, the modified Noordhoff technique can be applied to either complete or incomplete bilaterally cleft lip providing more naturally pleasing and cosmetically satisfying scars that lie in harmony with the philtral ridges, lip tubercle positioned just below the vermilion and a distinct white line and Cupid's bow.

악정형 장치를 이용한 구순구개열 환자의 상악골 전방견인 (Maxillary Protraction in the Cleft Patients Using the Orthopedic Appliances)

  • Cleft lip and palate patients often develop maxillary retrusion due to the combined effects of the congenital deformity and the scar tissue after surgical repairs. Maxillary protraction in the cleft patients using orthopedic appliances (i.e. face mask) or distraction osteogenesis during early childhood helps to achieve more balanced skeletal harmony and favorable occlusion for future growth to occur. Kinds, indication, protocol for use of the traditional orthopedic appliances will be discussed. Also the facemask with miniplate system recently developed will be introduced.

악안면 연조직에 관한 X선학적 연구 (ORENTGENOGRAPHIC STUDIES ON THE SOFT TISSUE PROFILE)

  • Modern orthodontics implies not only occlusal excellence, but also the positioning of teeth to produce optimal facial harmony for the individual patients. Several methods have been used in the study of facial height, width and depth were made from living subjects. These methods, however, complicate to control the subjects, therefore many investigators have used profile cephalometric technics. Practically, cephalometric technics were used m orthodontic treatment, maxillo-facial surgery and anthropometric studies. Author was studied to investigate the normal standards of soft tissue profile in Korean adolescences. The subjects consisted of 53 males and 54 females from 17 to 22 years of age and with normal occlusion and acceptable profile. Aluminum filter was designed to obtain both hard and soft tissue structures on a single film. Eight profile landmarks were plotted and drawn on the tracings of all cephalograms and eighteen depth, height and angles were measured from each landmarks of the cephalograms. The following conclusIOns were obtained from this studies; 1. Total facial convexity was 170. 75 in males and females samples and lower facial and: labiomandibular convexity were each of 141.44, 171.05. 2. Maxillary and mandibular sulcus angulations were 137.61, 129.52 and upper and lower lip inclinations were each of 123.26 and 49.56 in male and females. 3. Soft tissue depth of several points were as follows; Subnasale 18.74㎜ in males and 16.65㎜ in females Pogonion 13.40㎜ in males and 13.07㎜ in females upper lip 14.06㎜ in males and 11.91㎜ in females . lower lip 15.46㎜, 13.63㎜ in males and females 4. The protrusion of nose were 16.28㎜ in males and 15.56㎜ in females 5. The vertical length of upper and lower lips were 25.67㎜, 52.96㎜ and the lip posture was indicated 93.43 per cent (closed state) in centric occlusions.

한국 젊은여성의 심미적 안모형태에 관한 두부방사선 계측학적 연구 (A CEPHALOMETRIC ANALYSIS ON FACIAL ESTHETICS OF KOREAN YOUNG ADULT FEMALE)

  • Esthetic is one of the prime goal of the orthodontic treatment. It is represented by the word of harmony or balance. There is no absolute esthetic norm but it is important to study about it in orthodontic and orthognatic surgical treatment planning. The purpose of this study is to obtain the norm of Korean female adolescents who were candidates of 1986, 1987 Miss Korea Beauty Contest. The 77 individual samples were selected and studied by cephalometric analysis. Obtained results were as follows: 1 Korean esthetic norm was obtained. 2. Simple correlation coefficient was obtained between each measurements. 3. Holdaway ratio of Korean beauty was 3.3 : 1. 4. To compare with caucasian, profile of Korean beauty had less prominant nose, more prominant lip and deeper sulcus depth.

  • 상악 전치부의 보철 수복은 치과 분야에서 심미성이 크게 요구되는 치료이다. 상악 전치부의 보철 수복을 통하여 치아의 외형, 배열, 색조 등을 개선하면 심미성을 향상시키는 것이 가능하다. 특히 전치부 심미 수복을 위해서는 진단과정에서 환자의 안모 및 치아와 입술과의 관계, 치아와 치은 관계 등을 평가하고, 환자가 요구하는 바를 정확히 파악할 필요가 있다. 환자와 술자 및 기공사 간의 원활한 의사소통을 통하여 환자의 요구사항을 충실히 반영하기 위한 도구로서 DSD (digital smile design) 개념을 적용할 수 있다. 이 증례에서는 상악 중절치의 선천적 결손으로 인하여 대칭성을 구현하기 불가능한 상황에서 DSD를 활용하여, 환자 안모와 상악 전치부의 전체적인 조화를 추구하는 것을 목표로 하였으며, 이를 통해 환자와 술자 모두 만족할 만한 심미적인 결과를 얻었기에 이를 보고하고자 한다.

Angle씨 제 2 급 1류 부정교합환자 치료 전후의 안모 연조직 변화에 관한 두부방사선 계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON SOFT TISSUE PROFILE CHANCES IN PRE-POST TREATMENT OF ANGLE'S CLASS II DIVISION 1 MALOCCLUSION)

  • The purpose of orthodontic treatment is to produce functional occlusion and to create or maintain facial esthetic harmony. The soft-tissue covering of the face also plays an important role in facial esthetics, speech and other physiologic functions. The study of the soft-tissue profile is important for the planning of orthodontic treatment. The author studied cephalometric X-ray films on 49 patients (23boys, 26 girls) with Angle's class II division 1 malocclusion, ranged from 9 to 13 years of age. Roentgenocephalmetric X-ray films were taken pre and post orthodontic care. Tracings were made in usual manner. The obtained results were as follow. 1. There was no significant sexual difference on mean changes. 2. In the comparison of the soft-tissue thickness changes, Ls-Ls' and Si-Si' in male subjects were remarkable. 9. There were significant correlations between osseous (Ss') change and soft-tissue (Ss) chang, of maxilla in male and female subjects subsequent to orthodontic treatment. 4. The ratios between the protraction of the Ss' and that of the Ss were 1:1.5 in all sexes, the ratios between the Si' and that of the Si were 1:1.4 in male and 1:1.2 in female. 5. There were significant correlations between maxillary central incise. angulation change $({\angle}A)$ and upper lip inclination change $({\angle}B)$ in all sexes. 6. There were little correlations between change in distance difference of Is and Ii and change in distance difference of Ls and Li in all sexes in all sexes.
