Upper lip tie wrapping into the hard palate and anterior premaxilla causing alveolar hypoplasia

  • Bony anomaly caused by lip tie is not many reported yet. There was a case of upper lip tie wrapping into the anterior premaxilla. We represent a case of severe upper lip tie of limited lip motion, upper lips curling inside, and alveolar hypoplasia. Male patient was born on June 3, 2016. He had a deep philtral sulcus, low vermilion border and deep cupid's bow of upper lip due to tension of short, stout and very tight frenulum. His upper lip motion was severely restricted in particular lip eversion. There was anterior alveolar hypoplasia with deep sulcus in anterior maxilla. Resection of frenulum cord with Z-plasty was performed at anterior premaxilla and upper lip sulcus. Frenulum was tightly attached to gingiva through gum and into hard palate. Width of frenulum cord was about 1 cm, and length was about 3 cm. He gained upper lip contour including cupid's bow and normal vermilion border after the surgery. This case is severe upper lip tie showing the premaxillary hypoplasia, abnormal lip motion and contour for child. Although there is mild limitation of feeding with upper lip tie child, early detection and treatment are needed to correct bony growth.

Anti-cancer Mechanism of Docosahexaenoic Acid in Pancreatic Carcinogenesis: A Mini-review

  • Pancreatic cancer is a highly aggressive malignant tumor of the digestive system and radical resection, which is available to very few patients, might be the only possibility for cure. Since therapeutic choices are limited at the advanced stage, prevention is more important for reducing incidence in high-risk individuals with family history of pancreatic cancer. Epidemiological studies have shown that a high consumption of fish oil or ${\omega}3-polyunsaturated$ fatty acids reduces the risk of pancreatic cancers. Dietary fish oil supplementation has shown to suppress pancreatic cancer development in animal models. Previous experimental studies revealed that several hallmarks of cancer involved in the pathogenesis of pancreatic cancer, such as the resistance to apoptosis, hyper-proliferation with abnormal $Wnt/{\beta}-catenin$ signaling, expression of pro-angiogenic growth factors, and invasion. Docosahexaenoic acid (DHA) is a ${\omega}3-polyunsaturated$ fatty acid and rich in cold oceanic fish oil. DHA shows anti-cancer activity by inducing oxidative stress and apoptosis, inhibiting $Wnt/{\beta}-catenin$ signaling, and decreasing extracellular matrix degradation and expression of pro-angiogenic factors in pancreatic cancer cells. This review will summarize anti-cancer mechanism of DHA in pancreatic carcinogenesis based on the recent studies.

Implications of 3-Dimensional Printed Spinal Implants on the Outcomes in Spine Surgery

  • Three-dimensional printing (3DP) applications possess substantial versatility within surgical applications, such as complex reconstructive surgeries and for the use of surgical resection guides. The capability of constructing an implant from a series of radiographic images to provide personalized anatomical fit is what makes 3D printed implants most appealing to surgeons. Our objective is to describe the process of integration of 3DP implants into the operating room for spinal surgery, summarize the outcomes of using 3DP implants in spinal surgery, and discuss the limitations and safety concerns during pre-operative consideration. 3DP allows for customized, light weight, and geometrically complex functional implants in spinal surgery in cases of decompression, tumor, and fusion. However, there are limitations such as the cost of the technology which is prohibitive to many hospitals. The novelty of this approach implies that the quantity of longitudinal studies is limited and our understanding of how the human body responds long term to these implants is still unclear. Although it has given surgeons the ability to improve outcomes, surgical strategies, and patient recovery, there is a need for prospective studies to follow the safety and efficacy of the usage of 3D printed implants in spine surgery.

Effects of Skin Mobilization on Pain and Joint Range Improvement in Patients with Axillary Web Syndrome: A Single Case Report

  • Objective: This study was conducted to apply skin mobilization to patients with Axillary web syndrome following breast cancer resection and to see the resulting changes in pain and joint range of motion. Design: Single case study,pre-post comparison. Methods: The subject was a female patient in her 40s who performed a mastectomy after being diagnosed with breast cancer, and then complained of uncomfortable pain from the shoulder joint to the axilla and limited range of motion. To implement a skin mobilization, the palms were adhered to the inner half of the arm, the arms were raised to a pain-free extent, and skin mobilization was performed. The skin was pulled in the direction of axilla and kept for 5 seconds 10 times for a total of 2 sets. Immediate changes in range of motion and pain were identified. Results: Following skin mobilization, there was an immediate increase in range of motion (pre 116°, post 140°) and a decrease in pain (NRS pre 5, post 2). And also uncomfortable pain, which is hard to define in words, also seems to have improved. Conclusions: Skin mobilization, which considers skin mobility for patients with Axillary web syndrome, can be considered for improving range of motion and restoring function in patients with pain due to fibrous bands around veins and lymphatic vessels, and is recommended as a new intervention method not used as a conventional treatment.

Medication-Related Osteonecrosis of the Jaws: A Literature Review

  • Medication-related osteonecrosis of the jaw (MRONJ) is a serious side effect of antiresorptive agents and bone-modifying agents. It is of the utmost importance to know the management of the MRONJ to improve the patient's quality of life. This study comprehensively reviews the current definitions of MRONJs, and antiresorptive medications, clinical manifestation and staging, risk factors, treatment strategies, and prevention methods of MRONJ. The disease is defined as an exposure of bone and osteonecrosis of the jaw in the oral cavity for at least 8 weeks in patients taking antiresorptive drugs or antiangiogenic agents and with no history of radiotherapy treatment of the jaws. Many articles have reported risk factors associated with MRONJ such as systemic diseases, antiresorptive medication, oral infection, and poor oral hygiene. Osteonecrosis and antiresorptive medications including bisphosphonate and denosumab have been strongly associated, but the pathology of MRONJ is only limited. Hence, an effective and appropriate management and treatment for MRONJ is still to be defined. The objectives of MRONJ treatment are to minimize osteonecrosis and relieve symptoms, and many treatments are suggested from conservative treatment to marginal resection, but this remains controversial. Appropriate treatment of MRONJ remained difficult, although many studies are being covered.

Endoscopic Management of Anastomotic Leakage after Esophageal Surgery: Ten Year Analysis in a Tertiary University Center

  • Background/Aims: Anastomotic leakage after esophageal surgery remains a feared complication. During the last decade, management of this complication changed from surgical revision to a more conservative and endoscopic approach. However, the treatment remains controversial as the indications for conservative, endoscopic, and surgical approaches remain non-standardized. Methods: Between 2010 and 2020, all patients who underwent Ivor Lewis esophagectomy for underlying malignancy were included in this study. The data of 28 patients diagnosed with anastomotic leak were further analyzed. Results: Among 141 patients who underwent resection, 28 (19.9%) developed an anastomotic leak, eight (28.6%) of whom died. Thirteen patients were treated with endoluminal vacuum therapy (EVT), seven patients with self-expanding metal stents (SEMS) four patients with primary surgery, one patient with a hemoclip, and three patients were treated conservatively. EVT achieved closure in 92.3% of the patients with a large defect and no EVT-related complications. SEMS therapy was successful in clinically stable patients with small defect sizes. Conclusions: EVT can be successfully applied in the treatment of anastomotic leakage in critically ill patients, while SEMS should be limited to clinically stable patients with a small defect size. Surgery is only warranted in patients with sepsis with graft necrosis.

위종양성 병변에 대한 내시경 절개 점막하 박리법의 임상적 유용성 (Clinical Availability of Endoscopic Incision and Submucosal Dissection for the Treatment of Gastric Neoplasms)

  • 목적: 최근 위선종 및 조기위암에 대해 새로운 내시경적 치료법인 내시경 절개 점막하 박리법은 기존의 내시경적 점막절제술 보다 더 넓은 적응증을 가지고 있지만 시술의 난이도가 높고 합병증의 위험이 있어 보편적으로 시술되고 있지 않다. 이에 내시경 절개 점막하 박리법으로 치료한 위의 종양성 병변의 치료 성적을 바탕으로 그 임상적 유용성을 알아보고자 하였다. 대상 및 방법: 내시경 절개 점막하 박리법을 시행한 164명의 환자들의 179개의 위종양성 병변에 대하여 완전절제 및 일괄절제 여부, 합병증, 재발 여부 등을 분석하였다. 결과: 179개의 병변은 조기위암 126예(70.3%), 선종 42예 (23.4%)였다. 병변의 침윤 깊이를 확인할 수 있었던 조기위암 420예는 각각 점막1층(sm1) 0.8% (1/120), 점막2층(m2) 38.3% (46/120), 점막3층(m3) 25% (57/120), 점막하1층(sm1) 11.7% (14/120), 점막하2층(sm2) 1.6% (2/120)이었다. 종양의 일괄절제율과 완전절제율은 각각 96.0%, 85.2%였다. 합병증은 천공 8예(4.4%), 출혈 38예(21.2%)가 발생하였으나, 천공 1예를 제외한 나머지는 비수술적으로 치료가 가능하였다. 결론: 본 연구에서 내시경 절개 점막하 박리법은 위종양성 병변의 근치적 치료에 비교적 안전하고 효과적인 술기이고, 향후 보다 장기간의 추적관찰을 통해 그 유용성이 검증되어야 할 것으로 생각한다.

표층부(T1) 식도암에 있어서 암종의 침윤정도에 따른 림프절 전이의 양상 (Lymph Nodes Metastasis Pattern and Prognosis of Resected T1 Esophageal Cancer)

  • 배경: 흉부 식도암에서의 림프절 전이는 식도암이 점막하층에만 국한된 경우라도 흔하게 발견된다고 보고되고 있다. 림프절 전이가 수술 후의 예후에 큰 영향을 주고 있음에도 불구하고 조기식도암의 림프절 전이 양상은 완전히 조사되어 있지 않으며 림프절 절제술의 역할에 대해서도 여전히 의견이 분분하다. 대상 및 방법: 저자는 1995년 12월부터 2001년 8월까지 수술한 표층부(T1)식도암 환자 44예를 대상으로 림프절 전이의 양상을 후향적으로 연구하였다. 결과: 총 44예의 환자 중에서 림프절 전이는 총 10예(22.7%)에서 있었다. 식도암의 침윤 정도에 따라 볼 때 상피성 점막층에 국한된 경우 3예 중 0예, 점막고유 점막층까지 침윤된 경우 4예 중 0예, 근육성 점막층까지 침윤된 경우 4예 중 2예, 그리고 점막하층까지 침윤된 경우 33예 중 8예에서 림프절 전이가 발견되었다. 반회후두신경 림프절 전이는 5예, 복강내 림프절 전이가 8예에서 있어 흉부내 림프절 전이 3예보다 흔히 있었다. 수술사망은 없었고 병원사망이 점막고유층 환자에서 1예, 만기사망이 점막하식도암 환자에서 1예 있었다. 병원사망을 제외한 3년 생존율은 점막식도암이 100%, 점막하식도암은 97.0% (32/33예)였다(p>0.05). 림프절 전이가 없는 환자들의 3년 생존율은 100%, 림프절 전이가 있는 환자들은 90.0% (9/10예)였다(p>0.05). 결론: 표층부(T1) 식도암은 수술적 절제 후 생존율이 우수하다. 그러나 표층부(T1) 식도암에서 근육성 점막층 이상의 침윤이 있는 암에서는 림프절전이가 있을 수 있으며 특히 반회후두신경 림프절과 복강내 림프절 등 원격 림프절에도 빈번한 전이가 관찰되므로 광범위한 림프절 절제술이 필요하다고 생각된다.

삼차신경초종의 외과적 치료 (Surgical Management of Trigeminal Neurinoma)

  • Objective : Trigeminal neurinomas are rare tumors that may locate in the middle fossa or posterior fossa and straddled both the middle and posterior fossa, according to their origin in the nerve complex. The aim of this study was to analyze the clinical presentation, operative approaches employed and outcome in 15 patients who were treated surgically, with special emphasis on surgical approach. Method : Between 1994 and 1998, a total of fifteen patients were histopathologically identified as neurinomas originating from the trigeminal nerve complex at the tumor clinic in the neuroscience center of the our university. Results : The surgical approach to these tumors depends on their anatomical location and tumor size. Six patients had tumors confined to the middle fossa, five patients had tumors limited to the posterior fossa, and four patients both in middle and posterior fossa components of their tumors. Nine neurinomas were removed via the conventional approach(pterional, subtemporal, suboccipital) and six were excised using skull base approach(transzygomatic subtemporal, orbitozygomatic, transpetrosal). Total resection of the tumor was possible in 10 cases. Total resection of tumor was accomplished in 83% of patients following skull base approach compared with 56% of patients following conventional approach. The surgical outcome was excellent or good in 13 cases, fair in one and, poor in one. There was no operative death. In the immediate postoperative period, aggravation of preoperative facial hypesthesia and 6th cranial nerve palsy were common. Although, these deficits were generally transient, eight patients remained with some degree of trigeminal hypesthesia, two had facial weakness, one neurotrophic keratitis, one diplopia, and one mastication difficulty. Conclusion : Surgical approach to the trigeminal neurinoma depends on the tumor location and tumor size. Skull base approach provides more complete tumor excision without increased morbidity compared to conventional approach. Surgeons have to be meticulous in order to reduce postoperative complication.

재발한 위암의 진단에 사용된 FDG-PET의 유용성 (Usefulness of a FDG-PET Scan in Assessing Recurrent Gastric Cancer)

  • Purpose: For curative resection of recurrent gastric cancer, it is imperative that there be no unrecognized foci of tumoral disease outside the operation field. PET (positron emission tomography) with FDG (18 fluoro-2 deoxy-D-glucose) is a whole-body imaging technique that exploits the increased rate of glycolysis in tumor cells to detect disease. The authors evaluated the usefulness of FDG-PET in assessing resectability of recurrent gastric cancer. Materials and Methods: Seven patients with recurrent gastric cancer were studied with FDG-PET from December 1998 to October 2000. All FDG-PET images were interpreted in conjunction with conventional diagnostic methods. All imaging results were correlated with the pathological diagnosis and clinical outcome. Results: A final diagnosis of recurrence was obtained at 14 sites in all 7 patients by histology or clinical follow up. Locoregional recurrence, including distant metastasis, developed in 6 of 7 patients and distant recurrence in only one. FDG-PET detected all recurrent sites (5 locoregional and 5 distant) in 5 patients without peritoneal recurrence, but did not detect peritoneal seeding in 2 patients with peritoneal recurrence. The accuracy of FDG-PET in estimating resectability was $71.4\%$ (5/7), and that of CT and PET together was $85.7\%$ (6/7). A curative resection could be performed in three of the recurrent patients (2 locoregional and 1 distant recurrence). Conclusion: Our results suggest that FDG-PET may be useful for detecting locoregional and distant recurrence of gastric cancer and for selecting appropriate treatment. However, considering that FDG-PET was limited in detecting peritoneal seeding and determining the exact anatomical extension of tumor, it should be used in conjunction with other anatomical images.
