Management for Gait Disturbance and Foot Pain in a Patient with Klippel-Trenaunay-Weber Syndrome : A case report

  • Background : Klippel-Trenaunay-Weber syndrome (KTS) is a rare congenital medical condition characterized by complex vascular malformation. KTS consists of a classic triad of capillary malformation (hemangioma), venous malformations and bone or soft tissue hypertrophy causing limb asymmetry. The aim of this report is to describe management for gait disturbance and foot pain in a Patient with KTS using custom-made total contact insole. Case presentation : A 32-year-old man with KTS presented with a 3-year history of gait disturbance on hard surface due to right first toe pain and Achilles tendon tightness. The patient had soft tissue hypertrophy, varicose veins and port-wine stains over the right lower limb associated with KTS. True leg length discrepancy was 2 cm. We prescribed custom-made total contact insole to protect his deformed foot and correct leg length discrepancy. The insole of right side included wedge shaped heel lift and the insole of left side included full length lift to add extra support on unaffected side. Also, we provided compression stocking and physiotherapy including manual lymphatic drainage for lymphedema and stretching exercise for tightness in right lower extremity. At 3 years follow-up, postural alignment including pelvic obliquity was improved using a custom-made total contact insole. The degree of scoliosis and foot pain were also reduced. Conclusion : An individualized and multidisciplinary approach is essential regarding the complexity of comorbidities in patients with KTS. For patients with KTS, orthotic management should be considered to prevent and correct deformities related to KTS. Active orthotic management, compression stocking and physiotherapy can enhance the quality of life and function in patients.

  • There are many problems in management of high-rise building construction performed in downtown area due to limited spaces and equipments for increasing kinds and quantities of material. By these restrictions, problems such as increase of construction cost. delay of construction period, decrease of productivity, occurrence of accident are caused. Recently, studies on JIT concept have focused on construction materials as a solution for the problems. But, studies about finishing material are insufficient yet. Accordingly, this study suggests flowing of current material management, JIT management effect, and development direction of lifting-up and procurement system with Low-emissivity double Glazing, among of main finishing materials.

  • A building structures was used the facility management phase through $planing\cdotdesign\cdotconstruction$. phase. building's length of life was shorter than its physical length of life, because of the lack of law and institution, the lack of related agencies' preparation and owner's preparation. Also, considering the construction life-cycle, the facility phase accounted for $70\%$ of whole official lift, therefore, it needed to be opposed to systemic facility management. Current facility management system practically managed the defect of facility and almost never communicated between the other phase. In the side of policy, domestic law, institution, and policy related to building structure focused built-up market. and then the management of existing facilities was not consolidated efficiently. The purpose of this study was to suggest the direction of facility management construction for managing, utilizing, and operating public facilities, moreover, to study the direction of current facility management system improvement corresponding to total computer integrated construction system under government agencies.

Treatment of dental implant-related maxillary sinusitis with functional endoscopic sinus surgery in combination with an intra-oral approach

  • The present report describes the case of a patient who underwent maxillary sinusitis right after dental implant installation with sinus lifting. Computed tomography scan revealed a dental implant (#16) was protruded inside the right maxillary sinus and confirmed the obstruction of ostium. A symptom remission was gained with the dual approaches combined by functional endoscopic sinus surgery and an intra-oral approach. Fully recovered function and healing of sinus were identified after 10 months follow-up. We report the case of sinusitis caused by protrusion of implants with sinus floor lift procedures and propose that practitioners should be aware of the possible its complications and management.

  • The effective management of major construction resources is the key to the success in high-rise residential building construction projects. It is even more important and challenging as well especially in CBD(central business district) area under tight project duration since little leeway in interface between construction trades, severe constraints in staging n and heavy demands for lifting among construction resources. In order to meet high demands among major construction resources, highly effective construction resource management information system is required This research develops and provides a system which selects and disseminates these information from each phase of procurement transportation, lift-up and storage for major construction resources This research applies the Kanban system, well recognized material information system technique in automobile manufacturing industry, in concrete work processes. It develops concrete procurement information system utilizing VSM(Value Stream Mapping) and also applies the model in actual super-high rise residential building construction project in order to analyze the benefit of the research model.

  • The customer relation marketing in which companies can utilize to control and to get the filtered information efficiently has appeared in ubiquitous commerce. It is applying data mining technique to build the management that can even predict and recommend products to customers. In this paper, we proposed the case of customer relation management application using the associative mining. The proposed method uses the associative mining composes frequent customers with occurrence of candidate customer-set creates the association rules. We analyzed the efficient the feature of purchase customers using the hypergraph partition according to the lift of creative association rules. Therefore, we discovered strategies of the cross-selling and the up-selling. To estimate the performance, the suggested method is compared with the existing methods in the questionnaire dataset. The results have shown that the proposed method significantly outperforms the accuracy than the previous methods.

  • Lifting plan in the large spacial structure is an important factor influencing the efficiency and economy of the construction process. The purpose of this study was deriving the requirements for lifting techniques as the basic research in the double spoke wheel roof structure construction. In the lift up erection method, management plan of the interference error in the column and outer-ring was needed that occur during lifting roof structure. In the bent erection method, material usage reduction plan was required by the structural design of the temporary bent. In the hybrid erection method, lifting plan was needed that minimizes weather condition and crane usage. All lifting techniques were required Value Engineering model for reduction of cost and construction period.

  • To mitigate space restriction and to raise productivity, some shipbuilding companies use floating-docks on the sea instead of dry-docks on the land. In that case, a floating-crane that can lift very heavy objects (up to 3,600 tons) is used to handle the blocks which are the basic units in shipbuilding processes, and so, very large blocks (these are called the mega-blocks) can be used to build a ship. But, because these mega-blocks can be made only in the area near the floating-dock and beside the sea, the space is very important resource for the process. Therefore, our problem is to make an efficient spatial schedule for the mega-block assembly yard. First of all, we formulate this situation into a mathematical model and find optimal solution for a small problem using a commercial optimization software. But, the software could not give optimal solutions for practical sized problems in a reasonable time, and so we propose a GA-based heuristic algorithm. Through a numerical experiment, finally, we show that the spatial scheduling algorithm can provide a very good performance.

  • Urban Regeneration has an object to restore a function of urban through physical/cicumstance, social lift/culture, industrial/economic regeneration. It has characteristics like existence of a lot of projects, complextity of stakeholders' structure, long period of project life cycle to achieve variety of project's promotion strategy. To accomplish a Urban regeneration that has above characteristics successfully, information about other projects should be managed organically. And it should provie information linked up with each projects by integrated management. We need a decision making method that helps users cope with requirements of change during project, and offers information when each users need to make a decision timely. So I propose a strategy to build the IETM considering urban regeneration characteristics.
