자가 처방 감초 다량 복용 후 발생한 저칼륨성 하지마비 1례 (A Case of Self-prescribed Licorice-induced Hypokalemic Paralysis)

  • Glycyrrhetinic acid, which is a component of licorice, can cause hypermineralocorticoidism through the inhibition of $11{\beta}$-hydroxysteroid dehydrogenase. So, a high dose intake of licorice can lead to hypermineralocorticoidism with potassium loss and depression of the renin-angiotensin-aldosterone system. We report a 73-year-old man with muscle weakness of proximal lower limbs with hypokalemia (Serum $K^+$: 1.4 mEq/L) due to taking self-prescribed licorice without OMD's diagnosis. He boiled 60~100g licorice / day in water and drank it intermittently for 1 month due to arthralgia and swelling of both his knees. Patient's serum renin activity and aldosterone level were far beyond the normal range. He also had metabolic alkalosis with pH 7.552. After quitting the licorice, hypokalemia and muscle weakness of proximal lower limbs gradually improved within 1week.

  • Objectives : The aim of this study was to investigate the effects of licorice supplementation on muscle injury, plasma cortisol, testosterone and insulin sensitivity after high intensity resistance exercise. Methods : The fourteen health college male students were voluntarily participated in this study and were randomly divided into 2 groups: Control group (CON, n=7), Licorice group (LR, n=7). LR group ingested 2 g/time of licorice extract (mixed with 100 ml of water) two times/day for 10 days while the CON group ingested 100 ml of water. All subjects performed a high intensity resistance exercise (half-squat, 8 RM at 80% one-repetition maximum, 5 sets, 1min rest). Blood samples were collect before (-7) and after (0) licorice supplementation, and then 1 day, 2 day and 3 day post exercise. After 10 day treatment, plasma creatine kinase, cortisol, testosterone, glucose, insulin were measured. To determine the insulin sensitivity, HOMA-IR was calculated. Results : Plasma creatine kinase activities were significantly elevated after exercise, but there was not different between two groups. The plasma cortisol and testosterone levels were not significantly different between two groups. Plasma glucose levels were increased at 1 day and 2 day after exercise in the LR comparing with CON group (P<0.05) but plasma insulin levels were significantly lower in comparison with CON. HOMA-IR were significantly lower in the LR than CON group at 0 day to 3 day (P<0.05). Conclusions : The results of the current study suggest that licorice supplementation for 10 days might not attenuate the high-intensity exercise-induce muscle injury but may enhance the whole-body insulin sensitivity.

감초분말 첨가 파운드 케이크의 품질 및 항산화 활성 (The Quality and Antioxidant Properties of Pound Cakes Containing Licorice Powder)

  • 감초분말 첨가량을 0-8%로 달리하여 파운드 케이크를 제조한 후 물리 화학적 품질, 항산화 특성 및 소비자 기호도를 비교하였다. 감초분말을 첨가한 파운드 케이크의 pH와 수분함량은 각각 6.76-6.84, 13.96-17.80% 범위에서 유의적으로 감소하였다(p<0.05). 케이크의 무게와 높이는 감소한 반면, 굽기 손실률은 증가하였다(p<0.05). 한편 명도($L^*$)는 감초분말의 첨가량이 증가할수록 유의적으로 감소하였고(p<0.05) 적색도($a^*$)는 유의적으로 증가하였다(p<0.05). 대조군 케이크의 경도는 $0.46kg_f$으로 유의적으로 가장 높은 값을 나타내었으며(p<0.05), 감초분말을 첨가량의 증가할수록 단계적으로 감소하여 8% 첨가군의 경도는 $0.29kg_f$로 유의적으로 가장 낮았다(p<0.05). 항산화 특성을 나타내는 DPPH와 ABTS radical 소거능은 유의적 차이를 나타내며 증가하였고(p<0.05), 두 지표간 상관관계가 매우 높은 것으로 나타났다. 소비자 선호도 검사 결과, 4% 첨가군이 모든 평가항목에서 유의적으로 높게 평가되어 케이크의 관능품질과 건강 기능성 효과 등을 고려할 때 감초분말 4%를 첨가한 파운드 케이크가 가장 적절한 것으로 판단된다.

  • Anti-ulcer activity of licorice alkaloid fraction, separated from Glycyrrhiza glabra L. by fractionation according to general alkaloid purification procedure, were studied with various experimental gastric ulcers. In Shay ulcer, licorice alkaloid fraction inhibit significantly response rate and severity of ulcer. But there was no significant changes in volume and pH of gastric contents. A male rat was fixed on a board and immersed up to breast in water of $25^{\circ}C$ for 20 hrs. Hemorrhage and erosion develop in all the animals on the mucosa of the glandular portion of stomach. Licorice alkaloid fraction (5 mg/kg, 10 mg/kg) inhibit significantly in the severity of stress ulcer but there was no changes in the response rate. Chlorpromazine produce marked inhibition in the severity and response rate of ulcer It was suggested that at least the anti-ulcer activity of licorice alkaloid fraction was not mediated by central depression.

Effect of Glycyrrhizae Radix on the Expression of UDP-Glucuronosyltransferase-1A1 (UGT1A1) in Rat Liver

  • Licorice has been widely used in combination with other herbs or synthetic drugs for various disorders. In an effort to study the effect of licorice roots (Glycyrrhizae Radix, GR) and glycyrrhizin on the hepatic glucuronidation, we have previously found that the pretreatment of GR or glycyrrhizin for 6 days resulted in a marked increase in the enzymatic activity of 3-methylcholanthrene (3-MC)-inducible hepatic UDP-glucuronosyltransferase (UGT) isozyme that has high affinity toward phenolic substrates (p-nitrophenol form, UGTIA) in Sprague-Dawley rats. As an approach to elucidate the mechanism for the enzyme activation by licorice in rat liver, we examined the levels of hepatocellular mRNAs for UGTIA upon the treatment of GR or glycyrrhizin. The hepatic mRNAs were extracted from Sprague-Dawley rats and Wistar rats after the treatment of the methanol extract of GR (1 g/kg, p.o.), glycyrrhizin (23 mg/kg, p.o.) for 6 days, or 3-MC (40 mg/kg, i.p.) for 3 days. Using the UGT1A1 CDNA as a probe, we found that the mRNAs for the enzyme were induced by 3-MC treatment while those were influenced neither by GR nor by glycyrrhizin in both strains of rats. These results indicate that the activation of rat liver UGTI A by licorice and glycyrrhizin was not due to the induction of mRNAs for the enzyme.

만성 감초 중독으로 유발된 미네랄코르티코이드 과잉증후군 1예 (A case of chronic licorice intoxication-induced apparent mineralocorticoid excess syndrome)

  • Licorice is a perennial herb belonging to the legume family that mainly grows in northeastern China, Mongolia, Siberia, and other regions. It is used in traditional medicine in the form of dried roots in the East and the West. The main active component of licorice, glycyrrhizin, is known to produce mineralocorticoid effects when consumed chronically, which can lead to apparent mineralocorticoid excess syndrome. Herein, we present the case of a 72-year-old woman who was admitted to the emergency room with severe generalized weakness and difficulty keeping her neck upright, which had developed after daily consumption of licorice-infused water for the past 2 months. Blood tests revealed metabolic alkalosis and severe hypokalemia, and an electrocardiogram showed ventricular bigeminy. The patient was treated with daily potassium and spironolactone supplements, leading to a significant improvement in muscle strength after a week. One week later, the patient was discharged, showing rare ventricular premature contractions on electrocardiography, but with no specific complaints. Chronic licorice ingestion leading to hypokalemia and muscle weakness can be life-threatening, necessitating the discontinuation of the causative agent, close monitoring, and cautious supplementation of potassium and spironolactone as treatment.

화장품 중 유용성감초추출물의 유통기한 예측을 위한 가속수명 시험연구 (A Study on the Accelerated Life Test for the Estimation of Licorice Durability in Cosmetics)

  • 유용성감초추출물(oil soluble licorice extract)은 미백에 효능이 있는 원료로서, 미백 효과가 최적으로 유지되는 유통기한을 알아야 할 필요가 있다. 실온 상태에서의 화장품 유통기한을 측정하는 것이 제품의 개발 기간에 비해 상대적으로 길기 때문에 가혹 조건하에서의 성분 변화 실험을 통해 실제 유통기한을 예측하고자 하였다. 이를 위해 유용성감초추출물의 수명과 가속온도 스트레스와의 관계를 아레니우스식에 의해 분석하였다. 가속 온도 스트레스 설정 시 화장품 제형이 변하지 않는 온도 범위내인 50, 55 및 $60^{\circ}C$로 설정하였고, 이 구간 내에서 일정 스트레스 부과 방식을 설정하여 실험을 수행하였다. 본 연구에서 화장품 중 유용성감초추출물의 수명은 초기함량의 10%가 감소되는 시간으로 정의하였으며, 각 온도에서의 유용성감초추출물의 수명은 $50^{\circ}C$에서 580 h, $55^{\circ}C$에서 319 h, $60^{\circ}C$에서 166 h로 측정되었다. 실험 결과값을 아레니우스 모델에 적용하여 유용성감초 추출물의 수명과 온도 사이의 수명 관계식[log(수명)=-35.0243+1.15322$\times$(11604.83/온도)]을 도출하였고, 이 식에 의해 $25^{\circ}C$에서 유용성감초추출물의 수명은 26 개월로 예측할 수 있었다. 예측 결과는 유용성감초추출물을 실온에서 측정한 결과로 검증하였으며 95% 신뢰수준에서 수명 예측값과 실제값은 유의하지 않았다. 이 연구 방법은 주름 및 미백개선용 기능성 화장품들의 유통기한을 빠르고 정확하게 예측하는데 기여할 것으로 기대된다.

  • The investigation aimed to study the effects of methanol fraction of licorice (FM 100) and glycyrrhizin on prostaglandin synthetase activity, in relation to their analgesic effects. Effects of FM 100 and glycyrrhizin on the activity of prostaglandin synthetase extracted from bull seminal vesicles were examined by the modified method of Takeguchi et al. The analgesic effect of FM 100 was tested in mice by the acetic acid writhing method. FM 100 was administered orally to mice. BSV prostaglandin synthetase activity was inhibited significantly by FM 100 in a dose-dependent manner, whereas the activity was slightly inhibited by glycyrrhizin. Statistically significant analgesic effects were also observed with FM 100. The results suggest that analgesic effect of licorice may be due to the inhibition of prostaglandin synthesis.

Extraction and separation of glabridin from licorice by reversed phase high performance liquid chromatography

  • The extraction and separation of glabridin from licorice root by HPLC was performed in this work. First, by investigating the different extraction solvents, extraction methods and extraction times, a one-hour ultrasonic extraction procedure with ethyl acetate as the extraction solvent was optimized. Then the ethyl acetate extraction was applied to RP-HPLC for separation of glabridin. The column efficiencies and resolutions were experimentally investigated with different mobile phase compositions. Baseline separation of glabridin was obtained under the mobile phase composition of 50/50 vol.% (ACN/water). The retention time of glabridin was 20.3 min. The peak of glabridin was collected from the HPLC elution for several times and identified by LC/MS. Under the optimum extraction and HPLC separation methods, 1.26 g of glabridin per kg licorice root could be extracted.

감초, 겨자 및 키토산을 첨가한 저염 된장의 발효 특성 (Fermentation Properties of Low-Salted Doenjang Supplemented with Licorice, Mustard, and Chitosan)

  • 감초, 겨자, 키토산을 첨가하고 전통된장의 염도를 30% 낮춘 된장을 제조하여 된장의 발효특성을 조사하였다. 염도를 12.7%에서 10.2, 8.9%로 낮춘 된장에 감초, 겨자, 키토산 등 첨가물의 첨가는 된장의 내부의 균수, 색도, 산도에 거의 영향을 미치지 않았다. 저염된장은 된장표면에 white pellicle-forming strain이 발생하였으나 첨가물을 함유하고 있는 된장에서는 발생되지 않았다. 발효 20일째의 첨가물을 첨가한 염도 8.9% 된장의 아미노태 질소함량은 332 mg%로서 발효 40일의 염도 12.7%의 된장과 유사하였다. 관능평가에서는 첨가물을 첨가한 저염된장에서 구수한 맛 등이 높게 평가되어 종합적 기호도가 가장 높게 나타났다. 이러한 결과로부터 감초, 겨자, 키토산의 첨가는 된장의 발효시간을 단축시키고 기호성을 향상시킬 수 있으며 된장의 염도를 8.9%까지 낮출 수 있을 것으로 판단된다.
