We present an elderly patient with unilateral foraminal stenosis treated by isthmus resection. An 83-year-old female could not walk due to severe leg pain along right L5 sensory dermatome. Despite the laminotomy for spinal stenosis on the right side at the L4-5 level, her leg pain did not improve. Careful review of computed tomography scans and coronal source images of magnetic resonance myelography revealed foraminal stenosis on the right side at the L5 vertebra. Because of medical problem, she underwent isthmus resection on the right side at the L5 level instead of total facetectomy and fusion. After surgery, her leg pain was markedly improved. Isthmus resection showed successful result for this medically compromised elderly patient with unilateral foraminal stenosis.

Purpose: This study compared the effects of Static stretching and Mulligan's Two-leg rotation about the Hamstring flexibility, Hip range of motion, and pain. Methods: The subjects were allocated randomly into two groups: Static stretching group (n=13) and Mulligan's two leg rotation group (n=14). The study was designed with stretching protocols for four minutes and thirty seconds per day five times a week for three weeks in total. Measurements, including the Sit and reach test, Active/Passive Straight leg raise, and Visual analogue scale, were conducted before and after the intervention. The data were analyzed using a paired t-test and independent t-test. Results: The flexibility of the two leg rotation group was higher than the static stretching group after the intervention. The post-sit and reach test value of the two leg rotation group was significantly higher than the pre-sit and reach test value of the static stretching group. In addition, the variance of the sit and reach test of the two leg rotation group was significantly higher than that of the static stretching group. Conclusion: These results showed that two leg rotation techniques have a positive effect on the changes in the sit and reach test and active straight leg raise test. Two leg rotation techniques can be recommended as a self-stretching and easier way to stabilize the lumbopelvic rhythm, reduce the stiffness of the muscle and relieve pain. This is effective in preventing muscle damage, enabling muscle relaxation and reducing the risk of injury to the spine during daily lives and in sports activities.
Purpose: This study aimed to measure static balance of low back pain patients while one-leg standing in abdominal compression belts. Methods: The study included 40 adult males and females at J university, divided into a low back pain patient group and a normal group through the Oswestry disability questionnaire (ODQ). The subjects were instructed to hold a one-leg standing posture for 15 seconds on a balance measurement plate while wearing an abdominal compression belt. Shifting distance (0.1 cm), mean velocity (cm/s), pressure, and contact area were analyzed using BioRescue (BioRescue, RMINGEIEIRIE, Rodez, France). The average value was used to measure the result 3 times for each condition. Results: Both normal and low back pain groups significantly decreased in the speed of sway while wearing the abdominal compression belt. Furthermore, the pressure of the center of motion significantly decreased in the low back pain groups while wearing abdominal compression belt. However, there were no significant differences in the speed of sway or the pressure of center of motion between groups after wearing the abdominal pressure belt. Conclusion: These results suggest that abdominal compression belts are one option for improving balance temporarily. However, balance after wearing abdominal compression vests depends on onset of back pain, age, and symptoms of pain in the groups with low back pain. Further research is needed to investigate muscle activity, dynamic balance, and the effect of the period of wearing abdominal compression belts in the variety of low back pain patients.
Chloroma is a localized myeloblastic tumor which may develop during the course of myelogenous leukemia or as a presenting sign of the disease. A 47-year-old female diagnosed as chronic myelogenous leukemia in her hematologic remission period complained of left lower leg pain. The lumbar-spine series showed multiple osteolytic changes in the left lateral border of the lumbar spine. An inhomogenous soft tissue mass involving left lateral aspects of lumbar vertebrae was identified by CT-scanning. At the first pain attack, lumbar epidural steroid and local anesthetic injection could abolish her pain and the patient could go a few days without pain. The following radiation therapy could also improve the symptom and retain the pain free interval. One month later, a second pain attack occurred and lumbar and caudal epidural steroid and local anesthetic injections could result only in an incidental relief of pain. Radiation and chemotherapy were started but failed to relieve pain. A neurolytic block was considered but the patient's general condition was aggravated and even verbal communication with her became impossible.
The purpose of this study was to identify the above mentioned therapy on the reduction of functional leg length inequality, by the manipulation(Thonpson technique). In 8 patients who have been chronically ill with low back pain and functional leg length inequality, for past 12 weeks, we analyzed and observed the progress of symptom and sign on pelvis(femur head line level, ilium length, ilium rotation), using by X-ray. The results after 12 week treatment, compared with before treatment, were as follows : 1. The improved in femur head line in the manipulation after 12 week treatment was very significant(p<.01) 2. The improved in ilium length in the manipulation after 12 week treatment was very significant(p<.01) 3. The improved in ilium rotation in the manipulation after 12 week treatment was significant(p<.05).
Purpose : The purpose of this study is to investigate the instant effect of Kinesio taping on pain decrease and improvement of functional disorder of the subjects who have lumbar instability. Methods : A total of 20 patients (13 men and 7 women) who have lumbar instability were chosen as the subjects. The experiment was conducted by assigning the subjects into Kinesio taping group and placebo taping group. A visual analog scale (VAS) was used to measure back pain and Biering-Sorensen test was applied to measure the muscle endurance of back extensor muscles. A digital dynamometer was used to test the isomeric contraction strength of lumbar extensor muscle. The subject performed single-leg stance and double-leg stance task and their static balancing ability was measured by a testing device that captures the static balancing ability. Results : In a within-group comparison, Kinesio taping group showed a significant decrease of VAS (p<.05) and a significant increase of endurance and strength of lumbar extensor muscle (p<.05). In Kinesio taping group, the shift distance in anterior-posterior sway and medial-lateral sway during the double-leg stance significantly decrease (p<.05). The shift distance in anterior-posterior sway and medial-lateral sway also significantly decreased during the single-leg stance (p<.05). Placebo taping group showed a significant decrease of visual analog scale (p<.05). In a between-group comparison, Kinesio taping group showed a significantly larger decrease of VAS (p<.05), significant larger increase of muscle endurance and muscle strength (p<.05), and significant larger decrease of anterior-posterior sway in the double-leg stance (p<.05), compared to placebo taping group. Conclusion : Application of Kinesio taping to the subjects with lumbar instability produced positive effect of reducing pain, increasing muscle strength and endurance, and improving static balancing ability.
Purpose : The purpose of this study was to analyze and compare the surface electromyography(EMG) activity of trunk region muscle between normal subjects and chronic low back pain(CLBP) patients during one leg stance. Methods : The subjects were 27 people, were consisted of 12 subjects who don't have low back pain and 15 subjects who have low back pain from 19 to 28 year of age(mean age 22.22). We used surface EMG to evaluate the activity of the Rectus abdominis, External abdominal oblique, Quadratus lumborum, Gluteus medius muscle. We used independent two samples t-test for statistical data. Results : The result of this study showed that the maximal voluntary isometric contraction(%MVIC) ratio of the trunk muscles was higher in patients with CLBP than in normal subject. But there were no statistically significant differences. There were statistically significant differences of the activity of the Quadratus lumborum, Gluteus medius muscle(p<0.05).
Objective : To evaluate the pain improvement of the lumbago patients taken the electro-acupuncture therapy using Visual Analogue Scale. Methods : The 25 patients of 46 lumbago patients visited department of Acupuncture & Moxibustion in National Medical Center from 1st March to 20th August had taken the electro-acupucture therapy more than 5 times. We checked on their age, sex, onset, radiating pain, back pain past history and Straight Leg Raising test. And we evaluated their improvement of back pain using Visual Analog Scale(VAS) at each visit. Then we analyzed the pain improvement by their age, sex, onset, radiating pain, back pain past history, Straight Leg Raising test, visiting frequency and Visual Analog Scale on 1st visit(VAS1) Results : VAS of 24 patients has improved and VAS of one patient has not been changed. There's no patient getting worse. The women and the positive group at SLR Test had better result of pain improvement. Conclusion : The lumbago patients taken electro-acupuncture more than 5 times showed significantly different pattern of pain improvement according to the sex and the SLR test.
PURPOSE: This study aimed to compare the degree of lumbopelvic rotation during the active straight leg raise (ASLR) test in chronic low back pain with and without lumbar segmental instability. METHODS: A total of 71 patients with chronic low back pain were recruited for this study. The subjects who tested positive for more than three of the five lumbar segmental instability tests (prone lumbar instability, lumbar passive extension test, anterior posterior mobility test, passive straight leg raise, age) were categorized into the lumbar segmental instability positive group. Patients who tested positive for less than three of the five tests were categorized into the lumbar segmental instability negative group. The lumbopelvic rotation was measured three times during ASLR and a mean was determined. Subjective heaviness during the ASLR was measured on 6 point scale. RESULTS: There was a statistically significant difference in the lumbopelvic rotation angle between the groups with and without lumbar segmental instability (p<.01). There was no significant difference in the subjective heaviness during ASLR. The mean lumbopelvic rotation angle during ASLR was $13.54{\pm}2.86^{\circ}$, and $8.81{\pm}2.47^{\circ}$ in the positive and negative groups, respectively (p<.01). The cut-off value of the lumbopelvic rotation during was $10.5^{\circ}$, the sensitivity was 82.9%, and the specificity was 80.6%. CONCLUSION: These results suggest that lumbopelvic rotation is more prevalent in patients without lumbar segmental instability. Clinically, this important when diagnosing chronic low back pain with lumbar segmental instability, as the lumbopelvic rotation angle during the ASLR test can be used to aid in diagnosis.

Objective : To determine whether the outcomes of percutaneous epidural neuroplasty (PEN) are influenced by the type of lumbar disc herniation (LDH) and evaluate the effectiveness of PEN in patients with single-level LDH. Methods : This study included 430 consecutive patients with single-level LDH who underwent PEN. Before treatment, the LDH type was categorized as bulging, protrusion, extrusion, and sequestration, while Pfirrmann grades were assigned according to imaging findings. Visual analog scale (VAS) scores for back and leg pain and success rates (Odom's criteria) were assessed at 1, 3, 6, and 12 months after treatment. Results : The mean preoperative VAS scores for back and leg pain were 6.90 and 4.23, respectively; these decreased after PEN as follows : 2.25 and 1.45, respectively, at 1 month; 2.61 and 1.68, respectively, at 3 months; 2.28 and 1.48, respectively, at 6 months; and 2.88 and 1.48, respectively, at 12 months (p<0.001). The decrease in VAS scores for leg pain was significantly greater in the extrusion and sequestration groups than in the other two groups (p<0.05); there were no other significant differences among groups. More than 70% patients exhibited good or excellent 12-month outcomes according to Odom's criteria. Subsequent surgery was required for 59 patients (13.7%), with a significantly higher rate in the extrusion (25.0%) and sequestration (30.0%) groups than in the bulging (7.3%) and protrusion (13.8%) groups (p<0.05). Nevertheless, subsequent surgery was not required for >70% patients with extrusion or sequestration. Patients with Pfirrmann grades 1-3 (14.0-21.5%) showed a significantly higher rate of subsequent surgery than those with Pfirrmann grade 0 (4.9%; p<0.05). Conclusion : Our findings suggest that PEN is an effective treatment for back and leg pain caused by single-level LDH, with the outcomes remaining unaffected by the LDH type.
