Comparison of Sit and Reach Test, Straight Leg Raise Test and Visual Analogue Scale When Applying Static Stretching and Mulligan's Two Leg Rotation in Young Adults with Hamstring Shortness

  • Purpose: This study compared the effects of Static stretching and Mulligan's Two-leg rotation about the Hamstring flexibility, Hip range of motion, and pain. Methods: The subjects were allocated randomly into two groups: Static stretching group (n=13) and Mulligan's two leg rotation group (n=14). The study was designed with stretching protocols for four minutes and thirty seconds per day five times a week for three weeks in total. Measurements, including the Sit and reach test, Active/Passive Straight leg raise, and Visual analogue scale, were conducted before and after the intervention. The data were analyzed using a paired t-test and independent t-test. Results: The flexibility of the two leg rotation group was higher than the static stretching group after the intervention. The post-sit and reach test value of the two leg rotation group was significantly higher than the pre-sit and reach test value of the static stretching group. In addition, the variance of the sit and reach test of the two leg rotation group was significantly higher than that of the static stretching group. Conclusion: These results showed that two leg rotation techniques have a positive effect on the changes in the sit and reach test and active straight leg raise test. Two leg rotation techniques can be recommended as a self-stretching and easier way to stabilize the lumbopelvic rhythm, reduce the stiffness of the muscle and relieve pain. This is effective in preventing muscle damage, enabling muscle relaxation and reducing the risk of injury to the spine during daily lives and in sports activities.

군부대에서 신체활동시 병사의 상해 유형과 원인 비교 분석 (Comparison of Pattern and Cause of Activity Injury in Military Service)

  • This research focused on studying the phenomena of activity injuries occurred in military service, the object of which is soldiering soldiers in army. I made a inquiry paper including 6 items such as injury frequency, athletic event when you are injured, injury pattern, injury part in body and injury time according to military classes, and distributed it to elected 421 soldiers, the number of which is the total number of injured soldiers having time for physical activities in a year(last 2002 year). The results were as follows ; First, Injury frequency was according to private middle classes, private second classes, private last classes, private first classes. Second, Athletic event when you are injured was according to soccer, basketball, running and Martial Art(Taekwondo). 1) An abrasion was the most injury pattern when playing soccer. 2) A sprain was the most injury pattern when playing basketball. Third, Most causes of injuries dued to his / her carelessness, which is his / her excessive greed to win. Fourth, The most injury pattern was a skin abrasion. Fifth, The most injury part in body was a leg. Sixth, There was the most injury in summer. Wednesday on week, especially afternoon when you have time for physical to improve your battle-power.

Huge Psoas Muscle Hematoma due to Lumbar Segmental Vessel Injury Following Percutaneous Endoscopic Lumbar Discectomy

  • We present a case of an acute psoas muscle hematoma following percutaneous endoscopic lumbar discectomy. A 60-year-old female who presented with far lateral lumbar disc herniation underwent endoscopic discectomy on the right side at the L4-5 level. On the second postoperative day, the patient complained of severe right flank and leg pain and her blood pressure decreased. A computed tomography scan showed a large acute psoas muscle hematoma at right L4-5 level. The patient was transfused with packed red blood cells and placed at absolute bed rest. After observing the patient in intensive care, the severe flank and leg pain subsided, but the mild back pain persisted. Although percutaneous endoscopic lumbar discectomy is an effective minimally invasive surgical technique for the treatment of lumbar disc herniation, this case highlights the inherent risks of acute lumbar segmental vessel injury.

  • We experienced a case of patient of incomplete spinal cord injury following slipping off his clothes. He had complications : both leg paresisㆍboth arm weaknessㆍurine disturbanceㆍboth leg & arm sensory disorder . We could treat these complications with the methods of medical treatments ; acupunctureㆍmoxa therapyㆍherb-medicationㆍelectro-acupuncture & muscle exercises. We obtained results that motor grade and muscle atrophy were improved.

  • Background: This study aims to examine correlations between quadriceps angle, lower extremity muscle strength and leg length discrepancy. Methods: This study selected 96 healthy women university students as the subjects of research. Quadriceps angle, lower extremity muscle strength and leg length discrepancy were measured. The statistical analysis of the data SPSS/window (version 12.0) were analyzed using the pearson correlation analysis. Results: There were negative correlations between the muscle strength of the right hamstring muscles and the right quadriceps angle in supine and standing positions. Functional leg length discrepancy of left and right quadriceps angle in supine and standing position showed positive correlations. Conclusions: The quadriceps angle affect the knee. An abnormal angle caused weakening of balance. Muscle strength, leg length discrepancy, and affected lower extremity alignment and knee function. These conclusions may prevent exercise limitation or disorders in the subjects and treating the patients with knee injury or patellofemoral pain syndrome with basic therapy intervention.

하지에서의 역행성 지방근막 피판술: 유리피판술을 대치할 수 있는가? (Reversed Adipofascial Flap in Lower Leg: Can It Replace the Free Flap?)

  • A soft tissue defect of the lower leg or foot presents a challenging problem. Reconstructive surgeon should be armed at all points of wound site, tendon and bone exposure, injury of major vessel and so on in the lower limb. We reconstructed the defects of lower legs and feet of 25 patients between February, 1997 and December, 2003. Applying reversed adipofascial flap with skin graft on a soft tissue defect of the lower leg or foot is challenging. We did a comparative study of 25 reversed adipofascial flaps with 51 free flaps. All 25 cases of reverse adpofascial flap reconstruction were successful except for a partial loss of skin graft in 3 occasions. The reversed adipofascial flap had a merit of a short operation time and hositalization, a high success rate and minimum complications. Besides major vessels in the lower leg are better preserved and donor morbidity is minimal. However, the flap is unmerited in reconstructing a hug hallowed defect and in the leg with poor blood circulation and once previous surgery. The operators may consider the feasible substitution of reversed adipofascial flap for free flap before applying in the lower leg.

Use of the cross-leg distally based sural artery flap for the reconstruction of complex lower extremity defects

  • Cross-leg flaps are a useful reconstructive option for complex lower limb defects when free flaps cannot be performed owing to vessel damage. We describe the use of the extended distally based sural artery flap in a cross-leg fashion for lower extremity coverage in three patients. To maximise the viability of these extended flaps, a delay was performed by raising them in a bipedicled fashion before gradual division of the tip over 5 to 7 days for cross-leg transfer. Rigid coupling of the lower limbs with external fixators was critical in preventing flap avulsion and to promote neovascular takeover. The pedicle was gradually divided over the ensuing 7 to 14 days before full flap inset and removal of the external fixators. In all three patients, the flaps survived with no complications and successful coverage of the critical defect was achieved. One patient developed a grade 2 pressure injury on his heel that resolved with conservative dressings. The donor sites and external fixator pin wounds healed well, with no functional morbidity. The cross-leg extended distally based sural artery flap is a reliable reconstructive option in challenging scenarios. Adequate flap delay, manoeuvres to reduce congestion, and postoperative rigid immobilization are key to a successful outcome.

약침과 한약의 복합 치료를 이용한 탕화상 환자 치험 1례 (A Case Report of Scald Burns Treated with Pharmacoacupuncture)

  • We report the case of a 65-year-old female with burn injury from scalds. She was observed with a burn injury of the right arm and leg that occurred 6 days before. She was treated with pharmacoacupuncture Soyeom, and herb medicine daily for 3 weeks. After Twenty days, burn wound size was reduced and tissue regeneration was accelerated. In conclusion, it is our belief that pharmacoacupuncture and herb medicine is effective to treat burn injury, and future studies will be required to ascertain this method on burn injury.

Comparison of the Muscle Activities in the Lower Extremities during Weight-bearing Exercises

  • Purpose: Weight-bearing exercise is a type of physical exercise that is widely performed for rehabilitation after acquiring nervous-system diseases or sports-related injuries. It is one of the most commonly prescribed rehabilitation programs for strengthing of the lower extremities. Weight-bearing exercise is important for the conduct of such activity of daily living (ADLs) as walking, and up and down the stairs. The purpose of this study was to investigate the muscle activities during one-leg standing and one-leg squatting, the two most representative weight-bearing exercises. Methods: A total of 43 elderly (60~70 years old) males who could perform weight-bearing exercises were included in the study. During the one-leg standing and one-leg squatting, the electromyographic (EMG) signals were quantified as maximum voluntary isometric contraction (%MVIC) using surface EMG, and then the muscle activities of the lower extremities during the two exercises were compared. For statistical analysis, an independent sample t-test and one-way ANOVA were performed. Results: The results of the study are as follows: (1) in the one-leg standing, the activity of the gluteus medius was the greatest among the vastus medialis, vastus lateralis, bicep femoris, (2) in the one-leg squatting, the activity of the vastus medialis was the greatest; and (3) the activity was greater in the one-leg squatting than in the single-leg standing exercise. Conclusion: The one-leg standing and squatting exercises are suitable for strengthening the muscles for the prevention of and recovery from lower-extremity injury, and for functional ADL in elderly people. In addition, dynamic exercise was shown to be more effective than static exercise for strengthening the muscles.

도서형 복재 신경피부 피판술을 이용한 하지 연부 조직 결손의 재건 (Reconstruction of the Soft Tissue Defect of the Lower Leg with Saphenous Neurocutaneous Island Flap)

  • Purpose: We present clinical usefulness of saphenous neurocutaneous island flap for reconstruction of soft tissue defect of the lower leg, especially anteromedial aspect, including foot and ankle. Materials and Methods: Thirteen cases of soft tissue defects in the lower leg including foot and ankle which were 6 cases of pretibial area, 2 cases of anteromedial aspect of distal two third, 2 cases of ankle, and 3 cases of foot were treated saphenous neurocutaneous island flap. They were proximally based flap 3 cases and distally based flap 10 cases. Clinically the flaps ranged in size from $4{\times}5\;cm$ to $6{\times}12\;cm$. Results: All of the flaps except 1 case survived completely. Three cases, however, had marginal necrosis. One case of flap failure was proximal tibia fracture accompanied with injury of the flap pedicle which was difficult in flap elevation, subsequently. Conclusion: The saphenous neurocutaneous island flap is a simple, reliable procedure with a versatility for soft tissue coverage of the lower leg, especially anteomedial aspect, including foot and ankle. In case of another injuries accompanied near the saphenous nerve, careful attention should be made.
