Delayed bleeding after implant surgery in patients taking novel oral anticoagulants: a case report

  • The use of novel oral anticoagulants (NOACs) has increased in recent times in an effort to overcome the shortcomings of warfarin. They are being used primarily for the prevention of thrombosis caused by atrial fibrillation and offer the advantages of having fewer drug interactions than warfarin, no dietary restrictions, and no requirement for regular blood tests. Although there is reportedly less postoperative bleeding even if the drug is not discontinued during procedures that can cause local bleeding, such as dental procedures, no well-designed clinical studies have assessed postoperative bleeding associated with the use of these drugs. This article reports a case of a 74-year-old male patient who was taking rivaroxaban. The patient underwent a dental implant procedure after discontinuing rivaroxaban for one day and subsequently suffered delayed bleeding on postoperative day 6. Accordingly, this article also reports that the use of NOACs may also lead to delayed bleeding.

  • 지난 5년간 Th1/Th2 기전을 보다 자세히 이해할 수 있는 면역연구분야로서 조절 T세포와 수지상세포에 관련된 연구가 많아졌다. 이중 내재면역과 적응면역의 고리 역할을 하는 수지상세포는 다양한 방법을 통해서 Th1/Th2 면역반응을 유도한다. 이에는 수지상 세포 자체의 성질(DC1;Th1/DC2;Th2)과 TLR 수용체 종류(TLR9;Th1/TLR2;Th2) 등에 따라 결정된다. 앞으로 항원제시방법, 세포내 신호전달방법과 더불어 태내부터 감작되는 알레르겐이 내재면역계에 어떻게 영향을 미치는가에 대한 폭넓은 연구가 필요한 상태이다.

  • The aorta is the most common major thoracic artery injured by blunt chest trauma. Injuries to major aortic arch branch arteries can also occur but are much less common than aortic injuries in the setting of blunt trauma. Although internal mammary artery (IMA) injury is uncommon and rarely diagnosed in cases of blunt chest trauma, it is one of the important sources of bleeding in chest trauma. IMA bleeding can cause ongoing blood loss and may lead to serious conditions such as extensive hemothorax, anterior mediastinal hematoma or its catastrophic complication, cardiac tamponade. However such arotic and branch artery injuries are not easily detected by plain radiograph, and are detected indirectly because of associated mediastinal hematoma. Herein, we report a case of IMA injury caused by blunt chest trauma secondary to pedestrian traffic accident. The injured patient was successfully treated by transcatheter arterial embolization (TAE).

Successful Treatment of a Large Pulmonary Arteriovenous Malformation by Repeated Coil Embolization

  • Pulmonary arteriovenous malformations (AVMs) are caused by abnormal vascular communications between the pulmonary arteries and pulmonary veins, which lead to the blood bypassing the normal pulmonary capillary beds. Pulmonary AVMs result in right-to-left shunts, resulting in hypoxemia, cyanosis, and dyspnea. Clinical signs and symptoms vary depending on the size, number, and flow of the AVMs. Transcatheter embolization is the treatment of choice for pulmonary AVMs. However, this method can fail if the AVM is large or has multiple complex feeding arteries. Surgical resection is necessary in those kind of cases. Here, we report the case of a patient with a 6-cm pulmonary AVM with multiple feeding arteries that was successfully treated by repeated coil embolization without surgery.

  • Pulsus paradoxus has been defined as a decrease in systolic blood pressure (SBP) of 10 mmHg or more during inspiration. This report describes pulsus paradoxus detected by pulse oximetry during dental procedure. Case: A 10 years old boy who had impacted mandibular premolar with malformation scheduled for extraction under intravenous sedation with Fentanyl and Propofol. The patient showed upper airway obstruction with stridor and pulsus paradoxus. Though pulsus paradoxus is generally critical condition, in this case, respiration and other vital sign was maintained comparatively well with care in administering oxygen and considerate monitoring of pulse oximetry and capnography. Discussion: Noninvasive continuous monitoring of pulse oximetry allows recognition of pulsus paradoxus which can lead to serious problems. Clinicians should know very well about it and be able to manage of this kind of situation.

Native 1st Metatarso-Phalangeal Joint Infection: A Rare Case Report

  • Septic arthritis is a serious medical condition that can lead to significant complications if misdiagnosed or mismanaged. A rare case of a 1st metatarso-phalangeal joint septic arthritis in a native joint is presented in a patient with no significant risk factors. A 41-year-old patient was referred by his general practitioner owing to ongoing pain and swelling over his native 1st metatarso-phalangeal joint with difficulty on weightbearing for three months. After a series of investigations, including blood tests and a foot magnetic resonance imaging, which were inconclusive, the patient was led to the operating theatre for sampling and washout of his joint. The samples taken in the theatres revealed septic arthritis with Streptococcus mitis as the causative microorganism. The patient was treated with six weeks of oral antibiotics with a good functional outcome. This case report illuminates this rare condition and makes foot and ankle surgeons aware of its existence. A high suspicion for this condition can prevent misdiagnosis and mismanagement.

뇌과관류증후군에서 보일 수 있는 정맥울혈 1례 (Venous Congestion in Cerebral Hyperperfusion Syndrome: A Case Report)

  • 뇌과관류증후군은 경동맥 스텐트 삽입술 또는 내막 절제술 시행 시 발생할 수 있는 드문 합병증으로 대사 요구량보다 관류가 더 많은 상태를 유발하는 다양한 기전에 의해 발생한다. 주 임상 증상은 편측성 두통, 고혈압, 발작 및 국소 신경계 결손이 있으며, 심한 경우 지주막하 출혈 및 뇌실질 출혈로 영구적 장애 또는 사망까지 유발할 수 있다. 일반적으로 뇌과관류증후군은 두개경유도플러, 관류 뇌자기공명영상 및 단일광자방출컴퓨터단층촬영으로 진단할 수 있다. 저자들의 연구에서는 내경동맥 스텐트 이후 확인한 혈관조영술에서 의미있는 정맥울혈 증상을 보여 뇌과관류증후군을 진단할 수 있었다. 환자는 증상성 양쪽 내경동맥 협착을 보이고 있었고, 협착으로 인해 곁순환 동맥들이 잘 발달하게 되었다. 이렇게 곁순환 동맥이 잘 발달된 상태에서 환자에게 내경동맥 스텐트를 삽입한 이후 대뇌 혈류량이 증가되며 혈류의 방향이 바뀌어 정맥 울혈이 생길 수 있으며, 경동맥 스텐트 삽입술 또는 내막 절제술 시행 이후 정맥울혈이 보일 시 뇌과관류증후군을 예측할 수 있다. 이 연구는 내경동맥 스텐트 삽입 후 바로 시행하는 혈관 조영술을 통해서 뇌과관류증후군을 확진할 수 있음을 보여준 1례의 보고이다.

Genome-wide Survey of Copy Number Variants Associated with Blood Pressure and Body Mass Index in a Korean Population

  • Hypertension is the major factor of most death and high blood pressure (BP) can lead to stroke, myocardial infarction and cardiac failure. Moreover, hypertension is strongly correlated with body mass index (BMI). Although the exact causes of hypertension are still unclear, some of genetic loci were discovered from genome-wide association study (GWAS). Therefore, it is essential to study genetic variation for finding more genetic factor affecting hypertension. The purpose of our study is to conduct a CNV association study for hypertension-related traits, BP and BMI, in Korean individuals. We identified 2,206 CNV regions from 3,274 community-based Korean participants using the Affymetrix Genome-Wide Human SNP Array 6.0 platform and performed a logistic regression analysis of CNVs with two hypertension-related traits, BP and BMI. Moreover, the 4,692 participants in an independent cohort were selected for respective replication analyses. GWAS of CNV identified two loci encompassing previously known hypertension-related genes: LPA (lipoprotein) on 6q26, and JAK2 (Janus kinase 2) on 9p24, with suggestive p-values (0.0334 for LPA and 0.0305 for JAK2 ). These two positive findings, however, were not evaluated in the replication stage. Our result confirmed the conclusion of CNV study from the WTCCC suggesting weak association with common diseases. This is the first study of CNV association study with BP and BMI in Korean population and it provides a state of CNV association study with common human diseases using SNP array.

Test turnaround Time for Complete Blood Cell Count using Delta and Panic Value Checks and the Q-flag Limit

  • Test turnaround time (TAT) is the lead time from reception to reporting. In the complete blood cell count (CBC), 4 units of the XE-2100 (Sysmex Corp., Japan) processed around 80% of quantity, 1 unit of the LH-780 (Beckman-Coulter Incorp., USA) processed around 10% and 1 unit of ADVIA-2120 (Siemens AG, Munich, Germany) processed around 10%. We analyzed the change in the TAT for the CBC for over 7 years, from January of 2005 to December of 2011. The delta check made alterations of delta to WBC, hemoglobin, hematocrit, platelet and metamyelocyte, however, did not made them to band neutrophil, eosinophil, basophil and monocyte. The panic value check made alterations of panic value to hemoglobin, hematocrit, platelet and monocyte. In the criteria of currently slide review, LH-780 and ADVI-2120 analyzers prepared suspect flags of "Blast, Imm NE2, Immature granulocyte, Imm NE1, Left shift, Variant lymphocyte, Atypical lymphocyte, Platelet clumps and NRBC". The New slide review in the XE-2100 analyzer altered the preparations of a smear slide more than a "Platelet clumps flag(${\geq}200unit$), a single flag excluding the "Platelet clumps flag (${\geq}250unit$) and a multiple flag (${\geq}200unit$)". Also, below the 240 unit, medical technologists prepared manual slides selectively according to their evaluations. The automatic reporting rate was 33.4% without alterations, whereas it was 41.0% without alterations, and was thus improved by 7.6%. The slide review rate was 15.2% before using the Q-flag limit, whereas it was 12.1% for a reduce 3.1%. TAT was 45 minutes without the creation alterations of the delta and panic value checks, whereas it was 35 minutes after making alterations of the delta and panic value checks and thus was shortened by 10 minutes. We came to the conclusion that the establishment and operation of delta and panic value checks and slide review criteria suitable for laboratory environment can reduce unnecessary smear slides, re-checking, re-sampling, re-testing, telephone inquiries and concentrated workloads during specific times of the day.

근경련의 신경생리학적 발생기전, 평가 및 치료기법에 관한 연구 (The Investigation of Neurophysiological Mechanisms, Assessment and Treatments on The Muscle Spasm)

  • The purpose of this study was to investigate the neurophysiological mechanisms, assessment of muscle spasm and treatments that could apply to clinical field. Spasm is a painful pattern of contraction of muscle caused by chronic or acute trauma, excessive tension, or organic disorders. Aside from pain, muscle spasm creates shortenning of muscles and limits motion. Untreated spasm and protective immobility due to pain lead to decreased local blood flow in the muscles and result in a vicious cycle of muscle spasm and paul. The assessment of muscle spasm involve muscle tone assessment, tissue compliance, and joint ROM. Each of these aessessments utilize as a part of the patient's condition Clinical managements involve drug management and physical therapy. Expecially, physical therapy is one of the most important techniques to reduce muscle spasm. Physical therapy includes applying heat and cold, electrical stimulation, massage, and traction. This investigation should entourage phisical therapists to experiment further with various techniques to reduce muscle spasm.
