상악골 수평골절단술 후 비외형 변화에 관한 임상적 연구 (A CLINICAL STUDY OF THE NASAL MORPHOLOGIC CHANGES FOLLOWING LEFORT I OSTEOTOMY)

  • The facial esthetics are much affected by nasal changes due to especially its central position in relation to facial outline and so appropriately evaluated should be the functional and esthetic aspects of the nose associated with the facial appearance. Generally, a maxillary surgical movement is known to induce the changes of nasolabial morphology secondary to the skeletal repositioning accompanied by muscular retraction. These changes can be desirable or undesirable to individuals according to the direction and amount of maxillary repositioning. We investigated the surgical changes of bony maxilla and its effects to nasal morphology through the analysis of the lateral cephalogram in the Le Fort I osteotomy. Subjects were 10 patients(male 2, female 8, mean age 22.3 years) and cephalograms were obtained 2 weeks before surgery(T1) and 6 months after surgery(T2). The surgical maxillary movement was identified through the horizontal and vertical repositioning of point A. Soft-tissue analysis of the nasal profile was performed employing two angles: nasal tip projection(NTP), columellar angle(CA). Also, alar base width(ABW) was assessed directly on the patients with a slide gauge. The results were as follows; 1. Both anterior and superior movement above 2mm of maxilla rotated up nasal tip above 1mm. Either anterior or superior movement above 2mm of maxilla made prediction of the amount & direction of NTP changes difficult. Especially, a correlation between horizontal movement of maxilla and NTP rotated-up was P<0.01. 2. Both much highly anterior and superior movement of maxilla is accompanied by more CA increase than either highly. Especially, the correlation between horizontal movement of maxilla and CA change was P<0.05. 3. Anterior and/or superior movement of maxilla was accompanied by the unpredictable ABW widening. 4. The amount of changes of NTP, CA, and ABW is not in direct proportion to amout of anterior and/or superior movement of maxilla. 5. Nasal morphologic changes following Le Fort I osteotomy are affacted by not merely bony repositioning but other multiple factors.

안면골 골절의 발생 양상에 관한 7년간의 추적조사연구 (SEVEN-YEAR RETROSPECTIVE STUDY OF FACIAL FRACTURE)

  • This report constitutes a 7-year study of facial fractures based on a series of 616 patients who were treated for facial fractures as in-patients at Chosun University Dental Hospital between 1998 and April 2005. The following results were obtained : 1. The ratio of men to women was 5 : 1. 2. The major etiological factors were falls (36.2%), traffic accidents (23.4%), punches (18.1%), sports (8.3%), and accidents related to work (2.8%). 3. They were most frequent in the second (28.8%), first (26.6%), and third (12.3) decades, in that order. 4. The sites of frequent mandible fracture are the symphysis (32%), left mandibular angle (25.3%), and right mandibular angle (10.9%). 5. The most frequent maxillary fractures were zygomatic fractures (46.4%), Le Fort I fractures (28.8%), and Le Fort II (12.0%) fractures.

하악지 시상절단술시 견고 고정 나사의 골편간/골내 길이 및 비율에 대한 연구 (A STUDY ON BONE-CONTACT TO INTER-SEGMENTAL LENGTH RATIO OF RIGID FIXATION SCREWS USED IN BSSRO FOR MANDIBULAR SETBACK)

  • Objective: To evaluate the ratio between bone-contact length and inter-segmental length of the rigid fixation screw used in bilateral sagittal split ramus osteotomy (BSSRO) for mandibular setback. Material and Methods: Records of 40 patients with Class III malocclusion were selected. 20 of them had BSSRO, while the other 20 had BSSRO with maxillary LeFort I osteotomy. All of the patients had three noncompressive bicortical screws inserted at the gonial angle through transcutaneous approach. Two screws were inserted antero-posteriorly above inferior alveolar nerve and one screw was inserted below. The lengths of bone-contact and that of inter-segmental part were measured using cone-beam computed tomography. Ratio between these two measured lengths was calculated. Results: Both bone-contact and inter-segmental lengths were longer in BSSRO group than in BSSRO with maxillary LeFort I osteotomy group. Ratio of bone-contact to inter-segmental length was lower in BSSRO group than in BSSRO with Lefort I group. Both bone-contact and inter-segmental lengths were longer at the antero-superior position than at the inferior position. However, their ratio showed little difference. Conclusion: This study suggest that stability of screws in BSSRO group was greater than in BSSRO with Lefort I group. Stability of screws at the antero-superior position was greater than at the inferior position. Ratio of bone-contact to inter-segmental lengths was 0.2 in average.

골격성 III급 부정교합 환자에서 하악골의 이동량과 교합평면의 변화에 따른 술 후 안정성 (Skeletal Stability after Orthognathic Surgery in Severe Skeletal Class III Malocclusion Patients according to Changes in Anteroposterior Discrepancy and Occlusal Planes)

  • Purpose: The purpose of this study was to evaluate postsurgical facial hard tissue stability after orthognathic surgery with/without posterior impaction in skeletal class III malocclusion patients, and to evaluate the horizontal relapse tendency, according to changes in anteroposterior discrepancy and occlusal planes. Methods: Ninety patients, who had undergone orthognathic surgery in Pusan National University Dental Hospital, were enrolled in this study. Three main groups were classified as follows: Thirty patients underwent mandibular setback bilateral sagittal split ramus osteotomy (BSSRO) only (BSSRO group, BG); another thirty patients underwent mandibular setback BSSRO and Le Fort I osteotomy with posterior impaction (posterior impaction group, PG); and another thirty patients underwent mandibular setback BSSRO and Le Fort I osteotomy without posterior impaction (non-posterior impaction group, NPG). Preoperative (T0), immediate postoperative (T1) and six-month follow-up period (T2) lateral cephalograms were taken, and various parameters were measured. The analyses were done by linear and angular measurements between T0-T1 and T1-T2, to evaluate postsurgical facial hard tissue stability. Results: Mean horizontal relapse rates were distributed from 11.81% to 19.08%, and there were significant postsurgical changes (0.52 mm~2.44 mm) at the B point in all 3 groups. But, there were no statistical differences on relapse rate among BG, PG and NPG patients. Conclusion: In this study, the postsurgical stabilities of BSSRO and Le Fort I osteotomy with/without posterior impaction in skeletal class III malocclusion patients were acceptable. There were no significant statistical differences in mandibular stability according to changes in anteroposterior discrepancy and occlusal planes.

하악전돌증 환자의 양악 수술 시 상악골 후상방 회전이동 여부에 따른 안면부 경조직 변화량에 대한 비교 연구 (A comparative study on the change of postoperative facial hard tissue profile after maxillary rotational surgery)

  • Purpose: This study evaluated retrospectively the postsurgical facial hard tissue profile of a Le Fort I osteotomy with/without posterior impaction and rigid internal fixation to correct mandibular prognathism. After observing a difference between the two groups, this measurement was used to prepare a treatment plan for 2-jaw surgery. Patients and Methods: Thirty patients who had undergone orthognathic surgery in Pusan National University Dental Hospital were enrolled in this study. Fifteen patients were treated using a Le Fort I osteotomy with posterior impaction and mandibular setback bilateral sagittal split ramus osteotomy, and the other fifteen patients were treated without posterior impaction. The preoperative (T0), immediate postoperative (T1) and six-month follow-up period (T2) cephalograms were taken and difference between T1-T0 and T2-T2 was analyzed. Results: Both groups was FH-ABp, SNB and ANB showed significant changes in the measurement, whereas only the posterior impaction group showed a change in the SN-U1, occlusal plane, posterior facial height, surgical movement difference from the L1 and B-point. There was no significant statistical change between the immediate postoperative (T1) and six-month follow-up (T2) hard tissue analysis in the two groups. Conclusion: A Le Fort I osteotomy with posterior impaction is considerable for patients with a flat occlusal plane angle, large posterior facial height, prominent B-point, pogonion and labioversed incisal inclination if the indications are well chosen.

치료법을 달리한 상악골절후 부정유합 2예 (THE CARE OF DELAYED MALUNION AFTER MAXILLARY FRACTURE BY DIFFERENT METHODS:REPORT OF THREE CASES)

  • 현대 사회에서 외상의 특성은 다발성 손상이 빈발하는 것이며 상악골절시 생명을 위협하는 다발성 전신손상이 동반되면 수술이 지연되어 골절부의 지연된 부정유합을 보일 수 있다. 또한 골절된 양상에 따라서는 관혈적 정복술의 적용여부가 고려되는데 특히 골절편의 분쇄가 심하면 수술시 골절부위의 이개 후 정상위치로의 이동이 어렵고 설사 골절편의 이동이 이루어졌다고 하여도 정상위치로의 고정이 용이하지 않을 뿐만 아니라 분쇄골절편의 상실량이 많아지면 골이식까지 시행해야 하는 등 외과적이술식의 적용에 많은 난관이 예상된다. 따라서 상악골절 후 통상 6주일 간의 시간경과가 없다면 오히려 상악골절편의 이동방향을 예측하여, 전방견인용 headgear 또는 headcap 을 사용한 교정적인 치료를 시도함이 바람직할 수도 있다. 더우기 reverse headgear를 이용한 치료법은 골절된 경우가 아니더라도 상악골의 전방이동에 유용함이 실험적으로나 임상적으로 입증된 만큼 이를 상악골절후 후퇴된 상악골의 전방견인에 적용할 경우 교정치료의 최대장애인 cortical bone의 연속성이 골절로 인해 끊어지고 섬유성 유착상태로 남게되어 교정력의 효과가 치아이동 뿐만 아니라 골편의 이동에 매우 유익하리라 사료된다. 이에 저자는 상악골적이 분쇄양상인 한 환자에서 headgear와 headcap을 이용한 교정치료법을 5개월간 적용해 양호한 결과를 얻었고, 골절양상이 분쇄형이 아닌 증례에서는 외상후 5주간 경과 되었지만 관혈적 정복술을 시행하여 정상교합을 회복했으며, 심한 두부 손상으로 6개월만에 의식 회복을 한 LeFort I, II, III, 환자에서는 두부방사선 계측학적 분석, 교합기 상에서의 모형분석, 전산화 단층촬영 검사 등을 시행한 후 새로운 골면에다 LeFort I -osteotomy with iliac bone graft를 시행해 정상교합 및 안모추형을 개선시켰기에 이를 보고한다.

구순구개열환자에서 골신장술을 통한 상악골 열성장의 치험례 (Distraction Osteogenesis for Maxillary Hypoplasia in a Cleft Patient)

  • 본 교실에서는 상악골의 열성장을 보이는 구순구개열환자에서 RED 장치를 이용한 골신장술을 통하여 상악골의 점진적인 전방이동을 실시하고 약 3년정도의 추시기간을 포함하는 현재까지 특별한 기능 장애없이 양호한상, 하악관계 및 안모를 보이는 증례를 문헌고찰과 함께 보고하는 바이다.

르포트씨 1급 골절단술을 시행 받은 환자들에서 Cone-beam Computed Tomography를 이용한 수술 전, 후의 상악동의 평가 (Evaluation of Maxillary Sinus Using Cone-beam Computed Tomography in Patients Who Underwent Le Fort I Osteotomy)

  • Purpose: The aim of this sturdy was to assess the prevalence and change in pathologic findings in the maxillary sinus by using preoperative and postoperative cone-beam computed tomography (CBCT). Methods: The subjects included 83 patients with maxillary sinus abnormalities who underwent orthognathic surgery between January 2010 to December 2010. The CBCT analyses were classified according to the thickness of maxillary sinus membrane; Normal (membrane thickness<2 mm), mucosal thickening (membrane thickness ${\geq}2mm$ and <6 mm), partial opacification (membrane thickness>6 mm but not complete), total opacification, and polypoidal mucosal thickening. The diameters of the maxillary sinus ostium on the coronal cross-sectional view were also calculated. Results: Out of 166 maxillary sinuses in 83 patients, 42 (25.3%) maxillary sinuses before surgery and 37 (22.3%) maxillary sinuses after surgery showed abnormalities. A decrease in the diameters of maxillary ostium was observed after surgery (P<0.05). However, there was no significant difference in mucosal thickness both, preoperatively and postoperatively. Conclusion: The orthognathic surgery didn't deteriorate the maxillary sinus abnormaility. Despite the low prevalence of sinus complications in orthognathic surgery, all the patients should be informed of the possibility of sinusitis that could require the surgical intervention before surgery.

골신연술에 의한 성인 구순구개열자의 중안면함몰의 개선: 증례보고 (TREATMENT OF MIDFACE DEFICIENCY ON ADULT CLEFT LIP AND PALATE INDIVIDUALS BY DISTRACTION OSTEOGENESIS : CASE REPORT)

  • Maxillary deficiency, anterior cross bite, constriction of maxillary arch, malaligned teeth are frequently observed in patients with cleft lip and palate. Surgery and orthodontics, combined intervention are needed to correct maxillary deficiency. Distraction osteogenesis that currently used has many advantages like less relapse tendency, more advancement of maxilla, capable in growing patients. In case 1, 18 years old girl with BCLP had severe midfacial deficiency and multiple missing of teeth. LeFort I osteotomy, followed by maxillary distraction osteogenesis utilizing rigid external distraction device(RED) system, was performed. After a 6-day latency period, distraction proceeded at a rate of 1mm per day (at 1st week, 1.5mm/day). Total advancement was 19mm. The RED device left in place for the additional 4 weeks for consolidation. After the RED device was removed, face mask was applied with elastic traction for 5 weeks. After achieving acceptable facial appearance and occlusion, orthodontic appliance was removed. The results after 4 years follow-up was sustained pretty well without aggravation of velopharyngeal function. In case 2, 22 years old man with UCLP had severe midfacial deficiency and palatally erupted upper 2nd premolars due to arch length discrepancy, but the anterior segment of maxillary did not show constriction and crowding. patient had no arch width discrepancy, crowding was concentrated on premolar region. Segmental LeFort I osteotomy was performed. After a 6 - day latency period, using internal distraction device, distraction proceeded at a 0.5mm per day(at 1st week, 0.75 - 1 mm/day). Total advancement was 15mm. After internal distraction device was removed, face mask was applied with elastic traction for 4 weeks. After surgical-orthodontic treatment, facial appearance and occlusion was improved pretty good, and after 46 months follow-up the result was retained well.
