A COMPARISON OF THE ACCESSORY CANAL FILLING EFFECTS OF THE THREE ROOT CANAL FILLING METHODS WITH GUTTA-PERCHA (Gutta-percha를 이용(利用)한 세가지 근관충전법(根管充塡法)의 부근관충전(副根管充塡) 효과비교(效果比較))

  • In order to compare the accessory canal filling effects of the three root canal filling methods with gutta-percha, the author fabricated artificial root canal mold with the first and second accessory canals of chrome-cobalt alloy. After the artificial root canal was filled with gutta-percha by lateral condensation, vertical condensation and low-temperature thermoplasticized gutta-percha injection-molded method, twenty five times respectively, the gutta-percha forced into the first and second accessory canals were measured with caliper for length. The results were as follows: 1. The filling in both accessory canals was most effective in low-temperature thermoplasticized gutta-percha injection-melded method followed in such order as: vertical condensation method and lateral condensation method (p < 0.01). 2. The filling effect of the second accessory canal was more or less higher than that of the first one (p < 0.05). 3. Low-temperature thermoplasticized gutta-percha injection-molded method was fastest in time needed for root canal filling followed by lateral condensation method and vertical condensation method.

The Effect of a Piezoelectric Ultrasonic Scaler with Curette Tip on Root Substitute Removal in Vitro (큐렛형 팁을 장착한 압전방식 초음파 치석제거기가 치근대체물 삭제에 미치는 영향)

  • Based on current evidence in the literature, it is known that endotoxin is a weakly adherent surface phenomenon and that power-driven instruments can be used to accomplish definitive root detoxification and maximal wound healing without overinstrumentation of root and without extensive cementum removal. And one of the newly developed curette tips used with low power of piezoelectric ultrasonic scaler, is effective to remove calculus and not to remove the excessive cementum. The purpose of this study is therefore, to assess the influence of ultrasonic power and various working parameters on root substitute removal when instrumentation is performed with the curette tip on piezoelectric ultrasonic scaler. This study assessed defect depth, width and area resulting from instrumentation using a piezoelectric ultrasonic scaler with a curette type tip in vitro to acrylic resin block as a root substitute. The working parameters was standardized by the sledge device which controls lateral force(0.5 N, 1 N, 2 N) and instrumentation time(5 sec, 10 sec, 20 sec) and power setting was adjusted 0,2,4,8 in P mode. Power setting had the greatest influence on defect depth compared to lateral force and instrumentation time(standardized regression parameter estimates${\pm}$standard error, $0.37{\pm}0.02$, $0.19{\pm}0.02$, $0.07{\pm}0.02$). The effects on defect area also greatest for power setting($0.57{\pm}0.03$) compared to lateral force and instrumentation time($0.33{\pm}0.03$, $0.12{\pm}0.03$). The effect of the power setting on the defect width($0.15{\pm}0.01$) is not so great as defect depth or defect area compared to lateral force($0.12{\pm}0.01$) and effect of instrumentation time is minimal($0.02{\pm}0.01$). It could be concluded that the power setting has the greatest influence on the defect depth and area in curette type tip with low power of piezoelectric ultrasonic device. Many parameters can be adjusted in various situation in clinical use of piezoelectric ultrasonic scaler but the power setting is the first parameter to be adjusted.

  • Eighty-two maxillary canine were injected with china ink, decalcified, cleared and used in a study, in vitro, to determine the number of root canals, the frequency and location of lateral canals, the location of apical foramens, and the curvature of root canals. The results were as follows; 1.All of the maxillary canines showed the single canals. 2. Of the 82 canals studied, 20.7% of the canals were found to have lateral canals and most ramification of them were located in the apical third of the root. 3. In the case of the curvature, the mesial curvature of the canals (40.3%) usually showed more than the distal curvature of them (13.4%) 4. 41.2% of the apical foramen were located directly on the root apex and 58.5% of them laterally.

  • Sixty six mandibular incisor were injected with China ink, decalcified, cleared and used to study the number of root canals, the frequency and location of lateral canals, the location of apical foramens, the frequency of apical deltas, and the curvature of root canals. The results were as follows: 1. Most of the teeth demonstrated single canal, but 19.7% of the teeth were found to have bifurcated canals. 2. 12% of the teeth were found to have lateral canals and these ramifications were located in the apical third of the root. 3. 25 apical foramens were located directly on the root apex and 41 foramens laterally. 4. 43 canals showed straight, 7 canals distal curvature, 7 labial curvature, 3 mesial curvature.

The results were as follows: 1. Most of the teeth demonstrated single canal, but 5.3% of the teeth were found to have bifurcated or trifurcated canals. 2. Of the 40 canals studied, 52.5% of the canals were found to have lateral canals and these ramifications were usually located in the apical third of the root. 3. 7 apical foramens were located directly on the root apex and 31 foramens laterally. 4. 4 canals showed mesial curvature, 20 canals distal curvature, 11 canals buccal curvature, and 8 canals lingual curvature.

  • Thirty seven human maxillary third mooars were injected with china ink, decalcified, cleared, and used in vitro study to determine the number of root canals the frequency and location of lateral canals, canals per root, and frequency of apical deltas. The results were as follows: 1. Less than half of the examined teeth showed 3 roots and 3 canals. 2. The mesiobuccal root was ound to contain a single primary canal in 65.4% of the teeth examined and two canals in 34.6% of the teeth studed. 3. In mesiobuccal roots with two canals, the separated apical foramen appeared 30.8% of the specimen concerned, but no common apical foramen appeared. 4. Of the 106 canals studied, 46.2% of the canals were found to contain lateral canals and these ramifications were mainly located in the apical third of the root.

The results were as follows; 1. The mesiobuccal root was found to contain a single primary canal in 53% of the teeth studied and two canals in 47 % of the teeth studied. 2. In mesiobuccal roots with two canals, the seperated apical foramen appeared 32% of the specimens and the common apical foramen 15% of the specimens. 3. Of the 347 canals studied, 26.9% of the canals were found to contain lateral canals and these ramifications were mainly located in the apical third of the root.

  • Fifty human maxillary second molars were injected with china ink, decalcified, cleared, and used in vitro study to determine the number of root canals, the frequency and location of lateral canals, canals per root, and frequency of apical deltas. The results were as follows. 1. The mesiobuccal root was found to contain a single primary canal in 62% of the teeth studied and two canals in 38% of the teeth studied. 2. In mesiobuccal roots with two canals, the seperated apical foramen appeared 24% of the specimens and the common apical foramen 14% of the specimens. 3. Of the 169 canals studied, 23.1% of the canals were found to contain lateral canals and these ramifications were mainly located in the apical third of the root.

  • As awareness of root tear of meniscus have been increased, it became necessary to have an intensive repair. Posterior horn tear, especially for a lot of repair methods and the results have been reported recently. The report of the anterior root tear has yet to be found. We recently encountered a patient with an iatrogenic meniscal subluxation caused by past experienced partial menisectomy for discoid meniscus. Thus, we repots the technique and clinical results of arthroscopic repair of subluxated lateral meniscus anterior root tear using suture anchor.
