주관절의 외측방 회전 불안정성에 대한 외측 척측부인대 재건술-1례보고- (Lateral Ulnar Collateral Ligament Reconstruction for Posterolateral Rotatory Instability of the Elbow Joint - A Case Report -)

  • 본 교실에서는 주관절에 발생한 후외측방 회전 불안정성에 의한 재발성 탈구 환자에 대해 Nestor 술식을 이용한 외측 척측부인대 재건을 1례 치 험하였다. 주관절의 재발성 탈구의 치료에 있어서는 병인중 가장 많은 후외측방 회전 불안정성에 대한 이해가 필요하며 그에 따른 외측 척측부인대 재건등의 수술적 수기와 술후 적절한 재활 치료가 치료결과에 큰 영향을 미치게 된다.

  • Physical examination and surgical findings and symptoms are often inconsistent with magnetic resonance imaging (MRI) findings when diagnosing chronic ankle lateral ligament instability, and confirmed surgical findings are used as the gold standard in most clinical studies. Anterior drawer testing is considered unreliable because normal findings are highly variable, and its accuracy ranges from 50% to 100%. Furthermore, radiographic stress imaging, such as in anterior drawer stress view, is performed under manual stress or using a stress device, and its findings also vary widely and confuse when interpreting stress views. The average accuracy of MRI findings is around 85% (range, 66%~91.7%), and thus, cannot be used as a primary indicator for surgery. For patients with suspected lateral ankle ligament instability, based on symptoms and physical examination findings, MRI may be useful for identifying lesions in ankle joints and for differentiating them from other conditions.

  • We performed lateral ankle ligament reconstructions using Achilles allograft on patients who had failed previous Brostrom repair. The bone plug is fixed with an interference screw into the calcaneus, the tendon graft is passed through a fibular tunnel, and then anchored into the talus with the biotenodesis screw. The graft is strong enough to maintain joint stability until graft incorporation and remodeling occurs. In patients with chronic failed lateral ankle instability requiring graft for ligament reconstruction, this technique allows anatomic reconstruction without the need to sacrifice autogenous peroneal tendons.

열손실에 의한 확산-열 불안정성의 가속화 (Acceleration in Diffusive-thermal Instability by Heat Losses)

  • The dynamic behaviors of counterflow non-premixed flame have been investigated experimentally to study effects of heat losses and Lewis number on edge flame oscillation, which result from the advancing and retreating edge flame motion of outer flame edge at low strain rate flame. For low strain rate flame, lateral conduction heat loss in addition to radiation heat loss could be more remarkable than the others. Oscillatory instabilities appear at fuel Lewis number greater than unity. But excessive lateral conduction heat loss causes edge flame instability even at fuel Lewis number less than unity. The excessive heat loss caused by the smaller burner diameter in which the flame length is an indicator of lateral conduction heat loss extends the region of flame oscillation and accelerates oscillatory instability in comparison to the previous study with the burner diameter of 26mm. Extinction behaviors quite different from the previous study are also addressed.

만성 족근 관절 외측 불안정성에서 변형된 Brostrom 술식이 결과에 영향을 미치는 관련 요소 분석 (Analysis of Affecting Factors of Modified Brostrom Procedure in Chronic Ankle Lateral Instability)

  • Purpose: To evaluate the overall results and its major affecting factors(athlete/associated injury) of modified Brostrom procedure for chronic ankle lateral instability. Materials and Methods: Fifty-one patients (51 ankles) with chronic ankle lateral instability were sub-grouped athletes and non-athletes and again sub-grouped with associated injury and without associated injury. Associated injuries were osteochondral defect(6), peroneal tendon abnormality(10), os subfibularae(4), os subtibialae(4), anterior impingement(5) and loose body(2). The overall surgical results were evaluated and also see whether or not athletes and associated injury affect the result. Result: Of the 51 operations performed, there were 39 excellent, 8 good and 4 fair as a whole. Of the 24 athletes operated, there were 19 excellent, 3 good, and 2 fair. Among the 27 non-athletes, there were 20 excellent, 5 good and 2 fair results. The average time to ordinary life in the non-athlete group was 2.5 months and average time to sports activity in the athlete was 4.5 months postoperatively. Those who didn't have any associated injury had all excellent result, on the contrary, those who had associated injuries, 14 excellent, 8 good and 4 fair results. All the fair results were associated with osteochondral lesion of talus. Whether or not the patient is athlete does not affect the surgical results, but whether or not there were associated injuries does affect the surgical results. Conclusion: Modified Brostrom procedure for chronic ankle lateral instability itself is an excellent to good operation. This procedure can be used in athletic populations who need high demand of sports activity. But chronic ankle lateral instability with associated injury group showed variable, less satisfactory results than those without associated injury. Careful preoperative and intraoperative examination should be done to find out the associated injury with chronic ankle lateral instability.

Patellofemoral Instability in Children: Imaging Findings and Therapeutic Approaches

  • Patellofemoral instability (PFI) is common in pediatric knee injuries. PFI results from loss of balance in the dynamic relationship of the patella in the femoral trochlear groove. Patellar lateral dislocation, which is at the extreme of the PFI, results from medial stabilizer injury and leads to the patella hitting the lateral femoral condyle. Multiple contributing factors to PFI have been described, including anatomical variants and altered biomechanics. Femoral condyle dysplasia is a major risk factor for PFI. Medial stabilizer injury contributes to PFI by creating an imbalance in dynamic vectors of the patella. Increased Q angle, femoral anteversion, and lateral insertion of the patellar tendon are additional contributing factors that affect dynamic vectors on the patella. An imbalance in the dynamics results in patellofemoral malalignment, which can be recognized by the presence of patella alta, patellar lateral tilt, and lateral subluxation. Dynamic cross-sectional images are useful for in vivo tracking of the patella in patients with PFI. Therapeutic approaches aim to restore normal patellofemoral dynamics and prevent persistent PFI. In this article, the imaging findings of PFI, including risk factors and characteristic findings of acute lateral patellar dislocation, are reviewed. Non-surgical and surgical approaches to PFI in pediatric patients are discussed.

유리건을 이용한 족관절 외측 인대 재건술의 적응증과 근거 (Indications of Lateral Ankle Ligament Reconstruction with a Free Tendon and Associated Evidence)

  • Ankle sprain is one of the most common musculoskeletal injuries. Although most ankle sprains respond well to conservative measures, chronic instability following an acute sprain has been reported to occur in 20% to 40% of patients. Some individuals are eventually indicated for a lateral ankle ligament reconstruction due to persistent ankle instability. More than 80 surgical procedures have been described to address lateral ankle stability. These range from direct repair of the anterior talofibular ligament (ATFL) and of the calcaneofibular ligament (CFL) to reconstructions based on the use of autograft or allograft tissues. However, the best surgical option remains debatable. The modified $Brostr{\ddot{o}}m$ procedure is most widely used for direct ligament repair, but not always possible because of the poor ATFL or CFL quality or deficiency of these ligaments, which prevents effective shortening imbrication. Furthermore, the importance of a CFL reconstruction has been emphasized recently. On the other hand, it is difficult to achieve an efficient CFL reconstruction during the $Brostr{\ddot{o}}m$ procedure. Others have reported that an anatomic reconstruction of injured ligaments restores the normal resistance to anterior translation and inversion without restricting subtalar or ankle motion, and as a result, anatomic reconstructions for lateral ankle instability utilizing an autograft or allograft tendon have gained popularity.

만성 족관절 외측 불안정성의 수술적 치료: 봉합술과 재건술의 비교 (Surgical Treatment of Chronic Lateral Ankle Instability: Repair versus Reconstruction)

  • Surgical treatment to restore stability in the ankle and hindfoot and prevent further degenerative changes may be necessary in cases in which conservative treatment has failed. Anatomical direct repair using native ligament remnants with or without reinforcement of the inferior retinaculum is the so-called gold standard operative strategy for the treatment of lateral ankle instability. Non-anatomical lateral ligament reconstruction typically involves the use of the adjacent peroneus brevis tendon and applies only those with poor-quality ligaments. On the other hand, anatomic reconstruction and anatomic repair provide better functional outcomes after the surgical treatment of chronic ankle instability patients compared to a non-anatomic reconstruction. Anatomical reconstruction using an autograft or allograft applies to patients with insufficient ligament remnants to fashion direct repair, failed previous lateral ankle repair, high body mass index, or generalized ligamentous laxity. These procedures can provide good-to-excellent short-term outcomes. Arthroscopic ligament repair is becoming increasingly popular because it is minimally invasive. Good-to-excellent clinical outcomes have been reported after short and long-term follow-up, despite the relatively large number of complications, including nerve damage, reported following the procedure. Therefore, further investigation will be needed before widespread adoption is advocated.

  • Many high speed mechanical systems incorporate gearing for speed reduction. This study investigates the stability of mulit-rotor geared systems supported on oil film bearings taking into consideration the coupling between torsional and lateral dynamics. The emphasis of the study is on the analysis of the interaction between the combined torsional and whirl insta- bilities. The feasility of inducing a lateral and the torsional instability to neutralize an anticipated unstable condition is investigated. The possibility of suppressing the instabilities by controlling the parameters of the oil film bearings is also considered.
