A Case of Epidermal Cyst Using Surgical Method After Bufonis Venenum Pharmacopuncture Anesthesia (섬수약침 마취후 수술적으로 제거된 표피낭종 증례)

  • Objectives : The aim of this study is to report the effect of surgical method after Bufonis Venenum Pharmacopuncture Anesthesia on epidermal cyst. Methods : A 49-year-old male was presented with a $1.5{\times}1.5(cm)$ sized epidermal cyst on the upper back. Local anesthesia was achieved with Bufonis Venenum pharmacopuncture. After 5 minutes, 2.5cm incision was made using 11th blade. To minimize bleeding, the laser($CO_2$ Hani-maehwa laser-surgical mode) was excised to the cyst wall. After incision, cyst were separated and removed using adson forceps and iris scissors. Then buried intradermal suture and simple interrupted suture were performed. Yeonkyopaedok-san was administered for 5 days for anti-inflammatory effects. After 10 days, the suture was removed after confirmation of skin adhesion. Results : Cyst size measured $1.0{\times}1.0{\times}1.0(cm)$. Until the suture is removed, Adverse effects were not reported. Conclusions : Bufonis Venenum pharmacopuncture applies to local anesthesia. Korean medicine cautery method applies to laser($CO_2$ Hani-maehwa laser). It can be considered that To expand the application of surgical treatment in korean medicine.

TREATMENT OF MUCOCELE USING THE $CO_2$ LASER : CASE REPORT ($CO_2$ Laser를 이용한 점액종의 치료)

  • A mucocele is a thin-walled, bluish, fluctuant swelling occurring just beneath the oral mucosa and filled with mucoid material. It usually results from damage to a salivary gland following which there is extravasation of mucus into the tissues, or occasionally it forms from dilatation of the duct of a salivary gland. The accepted treatment of a mucocele is excision of the sac and the associated minor salivary gland. Use of the $CO_2$ laser in oral surgery has several advantages. It is a very precise means of cutting tissue and causes little adjacent nontarget tissue damage. There is excellent coagulation of small blood vessels and consequently operative hemorrhage is greatly decreased. There is instant sterilization of the operative site, which decreases bacteremia. The operation sites were completely healed without any infection or complication and discomfort from swelling or pain was not noted in all cases throughout the healing process. In addition, following this therapy, there is little contraction or scarring.

Lasing Effects on the Dorsal Tongue Mucosa of Adult Rats by Pulsed Nd:YAG Laser (펄스식 Nd:YAG 레이저 조사에 따른 성체 흰주 혀배면 점막의 변화)

  • The author examined the clinical and histological changes on the dorsal tongue mucosa of adult Sprague-Dawley rats after lasing by pulsed Nd:YAG laser. The dorsal tongue was lased for 3 seconds by 1.5, 2.0, 2.5, 3.0, 3.5, 4.0W and 10, 15, 20, 20pps. After tissue changes were observed clinically, the excised samples were observed histologically and the width of tissue destruction was analyzed quantitatively under the Confocal laser microscope respectively. The following results were obtained : 1. Whitening of peripheral tissue was observed more as increasing pulsed per second (Hz) below power 2.5W and observed at all parameters above power 3.0W. 2. Charring of mucosal surface was observed at all parameters but mild at parameters below power 2.0W. 3. The destruction of epithelium was observed at all parameters and tissue destruction was extended to lamina propria at higher pulses per second. 4. The width of tissue destruction was more widening as increasing energies per pulse (p<0.001) and the narrowest at 20Hz than 10Hz, 15Hz and 30Hz(p<0.05). As the above results, author suggests power below-3.5W and 20Hz as the lasing parameters for oral soft tissue therapy.

  • The nurses manually carry out the intravenous therapy for the patients. Using an Arduino, the fine flow controlling device was invented to provide an ongoing patient care. The medication is injected through a peristaltic pump, and the amount of the solution is controlled with a RGB color sensor. The power of the device is supplied through the batteries. An amount of the injection is measured with LIG strain sensor fabricated by 355nm UV pulsed laser. This system will provide a better medical service.

Porokeratosis ptychotropica: a case report

  • Porokeratosis ptychotropica is an uncommon form of porokeratosis, which was initially described in 1995. It is clinically characterized by symmetrical reddish to brown-colored hyperkeratotic, verrucous, or psoriasiform plaques on the perianal and gluteal regions. The lesions tend to integrate and expand centrally, with small peripheral satellite lesions. Early skin biopsy and appropriate diagnosis are essential because malignant change occurs in 7.5% of porokeratotic lesions. Conventional treatment options include topical steroid, retinoid, imiquimod, 5-fluorouracil, isotretinoin, excimer laser, photodynamic therapy, intralesional steroid or bleomycin injection, cryotherapy, carbon dioxide (CO2) laser, and dermatome and excision, but none seem to achieve complete clearance. A 68-year-old woman presented with diffuse hyperkeratotic scaly lichenoid plaques on the buttocks that had persisted for several years. A skin biopsy of the buttocks revealed multiple cornoid lamellae and intense hyperkeratosis. There were some dyskeratotic cells beneath the cornoid lamellae and the granular layer was absent. Porokeratosis ptychotropica was diagnosed based on the characteristic clinical appearance and typical histopathological manifestations. She was treated with a CO2 laser in one session and topical application of urea and imiquimod cream for 1 month. The lesions slightly improved at the 1-month follow-up. We herein present a rare case of porokeratosis ptychotropica.

  • Photodynamic therapy has been suggested as an alternative treatment to current cancer therapy which resulting in a variety of side effects because photodynamic therapy targets specific cancer cells and does not have a significant effect on normal cells. Typically, laser was used as a photodynamic therapy, but this was limited due to high cost and heat reaction. However, compact light emitting diodes that can emit light of various wavelengths have been developed at a low cost, which has a great influence on the low cost development of photodynamic therapy equipment. On the other hand, in the study of photodynamic therapy, the data on the direct effect of visible light are relatively small. Therefore, in this paper, we propose a novel cancer therapeutic module by developing a cancer cell proliferation inhibition module based on an Arduino that is relatively inexpensive, and able to use light of various wavelengths.

The Effect of Botulinum Toxin Type A for the Vocal Fold Granuloma (Botulinum Toxin Type A (BOTOX$^{\circledR}$)를 이용한 성대 육아종의 치료효과)

  • Background and Objectives : The vocal fold granuloma has been associated with vocal abuse, gastroesophageal reflux, endotracheal intubation and habitual throat clearing etc.. Granuloma is benign growth of hypertrophic granulation tissue. It is usually located on the posterior third of vocal fold, in one or both vocal process of the arytenoid cartilage In spite of the voice therapy, steroid therapy, anti-reflex therapy and surgical procedure. The distinct advantage and uniform success rate of each methods have not been generally shown. Authors report that localized injection of botulinum toxin type A (BOTOX$^{\circledR}$) is the promising method both as an initial treatment and as an alternative treatment in patients who do not respond to standard therapy or who are poor surgical candidates. Materials and Method : We carried out a retrospective study of 9 patients with the diagnosis of vocal fold granuloma on the videostroboscopic examination from Jan 2000 to Mar 2003. The botulinum toxin type A was injected into one or both thyroarytenoid muscle or lateral cricoarytenoid muscle under the electromyography. The average dosage ranges from 6U to 8U per injection. Results : Unilateral vocal fold granuloma in 7 patients had been resolved completely within 2-3 months after first injection : 5 patients received th\ulcorner GER medical therapy in addition to the Botulinum toxin injections, 2 patients was resolved completely who had shown recurrence after $CO_2$ laser vaporization. 2 patients who had shown recurrence after $1^st$ injection were also completely resolved within 6 months after further injections. Conclusion : We expect that localized injection of Botulinum toxin type A can provide an alternative treatment for the refractory cases to the traditional forms of therapy and avoid the recurrence in conjunction with proper medical and voice therapy against GER and vocal abuse.

Temperature Rise During Laser Photodynamic Therapy in a Mouse Tumor Model

  • Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided intro two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weights). The other group received no Photofrin E Mice from both groups were irradiated for approximately 15 minutes at 100,300, or $500{\;}mW/\textrm{cm}^2$ with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photofrin II increased surface temperature by at least 40% and the temperature rise due to $300{\;}mW/\textrm{cm}^2$ irradiation exceeded values for hyperthermia. Lights and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise.

  • Peri-implantitis is the inflammatory process, such as edema, bleeding, pus, of the mucosa surrounding dental implants. As the symptoms become severe, the surrounding bone is absorbed causing the implant surface to be exposed. Clinicians treat periimplantitis in various ways since a gold standard for the treatment of peri-implantitis has not been established. Various treatment methods include mechanical, chemical surface treatment and surgical excision, and recently decontamination of the implant surface using various types of lasers has been proposed. Thus, this study reviews the types of lasers and its effects that can be used for the treatment of peri-implantitis.

  • This study was performed to investigate the biostimulation effects of low level laser therapy (LLLT) on the fungus, Candida albicans, during the short term of cell cycle. Samples were divided into 6 groups which were P7, P9, P11, P15< CW and CO. All samples were irradiated for 1 minute with 2 hours of elapsed time during about 27 hours of the cell cycle of Candida albicans, and the optical density was assessed by spectrophotometry every 2 hours. It was found that there was no difference between the control and any other groups irradiated with 2 hours of short interval.
