Strangulating Large Colon Volvulus: A Diagnostic and Surgical Challenge in a Post-Partum Thoroughbred Mare

  • An 11-year-old thoroughbred post-partum broodmare presented with the symptoms of colic. The physical and clinical examinations indicated a large colon displacement or volvulus. Immediate surgical intervention was performed, and a strangulating large colon volvulus (LCV) was defined as a volvulus of >540° in the counterclockwise direction. After correcting and assessing the gut viability, approximately 80% of the entire large colon was resected and anastomosed using an end-to-end technique. With supportive care after surgery, the horse regained its appetite and vitality without significant clinical complications and was discharged on postoperative day 9. This report presents the first surgical correction using a large colon resection and anastomosis (LCRA) and the critical care for a strangulating LCV in a horse in Korea. This case enhances the current knowledge of clinical LCV and the related considerations for treatment.

  • The disease causing colic is the greatest cause of death in the domestic horse population today and accidental deaths as a result of colic leads to decreased productivity and economic losses in the horse breeding industry. Large colon impaction is the most common cause of colic in horses. Horses may ingest sand either by eating from the ground in sandy locations, or because of its inclusion in hay. In certain areas sand impaction is relatively common. The horse (4 year, female) presented with abdominal pain, which was charactered by willing to lie down, pawing, and looking at flank. A month later, the horse died suddenly at sand paddock. Post-mortem examination revealed the sand impaction as a lot of feces, sand and gravels in the right dorsal colon. In addition, there was necrosis and debilitation in mucosal lining of the large colon and rupture site on the right dorsal colon. The purpose of this article is to review sand impaction and emergencies in Thoroughbred horses.

성숙한 당나귀의 큰잘록창자 폐색 증례 (A Case of the Large Colon Impaction in an Adult Donkey)

  • 본 연구의 목적은 성숙한 당나귀의 큰잘록 창자 폐색 증례를 진단하여 보고하고자 한다. 당나귀(Equus africanus asinus; 암, 6세) 1마리가 제주경마공원 동물병원에 전날부터 시작된 식욕부진 증상으로 내원하였다. 장음소실, 탈수, 심한 복강 확장 등의 증상을 확인하였으나 호흡수 및 심박수는 정상이었고, 복통 증상은 없었다. 직장검사에서 폐색된 부분이 잘록 창자임을 확인하였다. 위세척, 수액요법 및 비스테로이드 계열의 진통제를 투여하였으나 다음날 새벽 갑자기 몸을 떨며 기립곤란과 같이 신체상태가 악화되기 시작한지 2시간 만에 폐사하였다. 부검에서 특히 왼배쪽 잘록 창자와 같은 큰잘록 창자에서 다량의 식괴를 발견하였고, 막창자와 곧창자는 정상이었다. 당나귀는 내원하기 11일 전에 새로 입사해서 사양 관리체계가 바뀌었다. 사료변경, 수분섭취 감소, 운동부족 등이 질병을 유발하였을 것으로 생각된다. 그러나 당나귀의 정확한 급사원인은 더 연구해야 할 과제로 남는다.

Nonoperative management of colon and mesocolon injuries caused by blunt trauma: three case reports

  • The therapeutic approach for colon injury has changed continuously with the evolution of management strategies for trauma patients. In general, immediate laparotomy can be considered in hemodynamically unstable patients with positive findings on extended focused assessment with sonography for trauma. However, in the case of hemodynamically stable patients, an additional evaluation like computed tomography (CT) is required. Surgical treatment is often required if prominent mesenteric extravasation, free fluid, bowel infarction, and/or colon wall perforation are observed. However, immediate intervention in hemodynamically stable patients without indications for surgical treatment remains questionable. Three patients with colon and mesocolon injuries caused by blunt trauma were treated by nonoperative management. At the time of admission, they were alert and their vital signs were stable. Colon and mesocolon injuries, large hematoma, colon wall edema, and/or ischemia were revealed on CT. However, no prominent mesenteric extravasation, free fluid, bowel infarction, and/or colon wall perforation were observed. In two cases, conservative treatment was performed without worsening abdominal pain or laboratory tests. Follow-up CT showed improvement without additional treatment. In the third case, follow-up CT and percutaneous drainage were performed in considering the persistent left abdominal discomfort, fever, and elevated inflammatory markers of the patient. After that, outpatient CT showed improvement of the hematoma. In conclusion, nonoperative management can be considered as a therapeutic option for mesocolon and colon injuries caused by blunt trauma of selected cases, despite the presence of large hematoma and ischemia, if there are no clear indications for immediate intervention.

Dietary Fiber and Large Bowel Cancer

  • Large bowel cancer correlates tightly to dietary factors such as dietary fiber and fat. Dietary fiber prevents the large bowel cancer in different modes of action which depend upon physicochemical and fermentable properties. Water-soluble fiber is fermented easily by intestinal microbes producing short chain fatty acids ; in contrast, water-insoluble fiber occurs effectively more rapid transit time due to greater bulk of gut content, though it is unfermentable. Not only short chain fatty acid is utilized in the proximal and distal colon as primary energy source, but also it lowers pH in the colon to normalize cellular differentiation and helps to stimulate peri staltic movement by acting as an osmotic laxative. In particular, butyric acid may also regulate gene expression and cell growth, though it is an important respiratory fuel for the colonocyte. Since dietary fiber and non-digestible oligosaccharides are the major source of butyric acid, this provides a possible link between dietary fiber and oligosaccharide and prevention of large bowel cancer. But, as with many links between dietary fiber and large bowel cancer, a direct casual association has not been established. In addition, RDA of dietary fiber which is 20~25g/day for adult Japanese, appears to be reasonable for the defecation once daily and the prevention of large bowel cancer.

Accidental case of large colon impaction in a horse

  • A 5 years old gelding (Thoroughbred, Equus caballus) had shown severe abdominal pain, colic, after overeating of hay in the feed storage. following through treatment, it subsequently died. Grossly, the large colon was impacted with firm mass of food and congestion of blood vessel in the intestinal wall. There were mild peritonitis and pleuropneumonia. This case demonstrates typical large colon impaction with hard consistence ingesta due to improper management of horse.

  • The study was designed to observe the effect of different dietary fats on the incidence of colorectal tumor and in vivo cell proliferation in colon carcinogenesis. Male Sprague Dawley rats were intrarectally infused with chemical carcinogen(methylnitrosourea, MNU) and fed 16%(w/w) fat diet containing one of dietary fats(beef tallow, corn oil, perilla oil) for 30 weeks. To measure in vivo cell proliferation, the incorporation of 5-bromo-2-deoxyuridine(BrdU) into DNA was localized using the monoclonal anti-BrdU antibody. Large number of tumors were found in the distal colon and tumor incidence was increased in the order of perilla oil(57.7%)$\alpha$-linolenic acid rich in perilla oil could have a protective effect against colon cancer compared to saturated fatty acid or n-6 linoleic acid.

A Rare Case of Undifferentiated Carcinoma of the Colon Directly Invading the Duodenum

  • Colorectal cancers often invade adjacent organs; however, direct duodenal invasion is rare. Adenocarcinoma is the most common type of colorectal cancer, but an undifferentiated carcinoma type is unusual. Herein, we present a case of undifferentiated carcinoma of the colon that directly invaded the duodenum and metastasized to distant lymph nodes. An 85-year-old female patient was admitted with a 7-cm-sized colonic mass invading the duodenum, detected by computed tomography. Positron emission tomography revealed fluorodeoxyglucose uptake in the colon, duodenum, and aortocaval lymph nodes. A large encircling mass in the ascending colon and an ulcerated mass in the duodenum were revealed by colonoscopy and esophagogastroduodenoscopy, respectively. Pathologic examinations of the colon and the duodenum revealed nonglandular, diffusely infiltrating atypical round cells, confirming undifferentiated carcinoma of the colon. The histologic type of this tumor was distinguished using immunohistochemical (IHC) markers. Finally, microscopic characteristics and IHC markers aided in identifying the histologic type of colorectal cancer.

식이유형과 대장암 위험도와의 관련성 분석 (Analyses on the Associations of Dietary Patterns with Colon Cancer Risk)

  • Dietary pattern analysis is important complementary approach for identifying associations between diet and chronic disease. A case-control study was conducted in order to examine dietary patterns and the risk of colon cancer in Korea. Data were collected from both 137 cases with either colorectal cancer or large bowl adenomatous polyps and 134 controls regarding social-demographic characteristics and food intake using a semi-quantitative food frequency questionnaire. We conducted factor analysis and identified 6 major dietary patterns: 'Well-being diet' characterized by higher intakes of potatoes, yogurt, soybean paste and vegetables, 'Meat & fish', 'Milk & juice', 'Pork & alcohol', 'Rice & kimchi', and 'Coffee & cake'. We calculated factor scores for each participant and examined the associations between dietary patterns and colon cancer risk. After adjusting for potential confounders, there was a relative risk for colon cancer of 0.16 (95% confidence interval, 0.07 - 0.34) when comparing the highest with the lowest tertile of the 'Well-being' pattern. Significant trends of decreasing risk of colon cancer also emerged with the 'Milk & juice' (OR = 0.40, 95% CI = 0.20 - 0.79). In contrast, inverse associations of the risk were found for 'Pork & alcohol' (OR = 1.92, 95% CI = 0.93 - 3.97), 'Coffee & cake' (OR = 2.18, 95% CI = 1.07 - 4.46). For the 'Meat & fish' pattern, the decreased risk of colon cancer was observed in the second tertile, but not in the highest tertile when comparing to the lowest. The 'Rice & kimchi' pattern had a nonsignificant association with the risk. These data suggest that major dietary patterns derived from the FFQ associated with the risk of colon cancer in Korea. Since foods are not consumed in isolation, dietary pattern research in natural eating behavior may be useful for understanding dietary causes of colon cancer.

개의 결장 간질종양에 대한 영상의학 및 면역조직화학 진단 1예 (Medical imaging and immunohistochemical diagnosis of gastrointestinal stromal tumor originated from colon in a dog)

  • Gastrointestinal stromal tumor (GIST) is one of the mesenchymal tumors originated from gastrointestinal submucosa. A 10 year-old, male, mixed breed dog with persistent diarrhea, anorexia and lethargy was referred to Haemaru Animal Referral Hospital. Large mass originated from the transverse colon was observed and large amount of ascites and free gas were found on abdominal radiography and ultrasonography. The ascites was septic exudate mixed with bacteria that consisted with intestinal perforation. There was no metastatic lesion. This mass was tentatively diagnosed as adenocarcinoma, leiomyosarcoma (LMS) and lymphosarcoma and surgical resection and histilogical examination were planned. However, according to owner's request, the patient was euthanized and then the necropsy was performed. About 10 cm sized mass originated from the cecum, ascending colon and transverse colon was adhered to surrounding mesentery and the perforation and large amount of ascites were observed. GIST was suspected on histopathologic examination and confirmed according to CD 117 expression in immunohistochemistry. GIST, derived from interstitial cells of Cajal, can be distinguished from LMS and leiomyoma (LM) on the basis of expression of CD117 (KIT) immunohistochemically. GIST has a different biological behavior and clinical course compared with LMS and LM, therefore definite diagnosis for GIST using immunohistochemistry is clinically important to predict the precise prognosis of the patient.
