고용보험제도의 실질적 사각지대 규모와 원인에 대한 성별 분석 (A Gender-Sensitive Analysis on the scale and causes of the blind spots in the employment insurance system)

  • 이 연구는 우리나라 고용보험제도의 사각지대를 젠더 관점에서 분석하여 고용보장 측면에서 여성에게 실질적인 제도적 보장이 제공되고 있는지의 여부를 실증적으로 검토하고 이를 토대로 고용보험제도의 향후 발전방안에 대해 제언하는 것을 목적으로 한다. 분석 자료로 한국노동패널 12차년도(2009년)와 20차년도(2017년) 자료를 이용하였으며, 사각지대의 원인 분석을 위해 로지스틱 회귀분석(logistic regression)을 실시하였다. 연구결과, 고용보험의 실질적 사각지대는 2009년에 성별 격차가 10%p 이상이었으나 2017년에 5%p 정도로 감소하여 실질적 사각지대의 성별 격차는 다소 완화된 것으로 파악되었다. 고용보험 미가입 원인의 경우 다른 인구사회학적 요인들을 통제할 경우 여성의 고용보험 미가입 가능성이 남성보다 낮아, 노동시장 내 여건이 성별로 동일하다면 여성의 고용보험 가입률이 오히려 남성보다 높아질 가능성이 있는 것으로 나타났다. 분석결과에 따른 정책적 제언은 다음과 같다. 현재의 고용보험제도는 성별 자체에 의한 가입률 차이보다는 노동시장 특성이 미치는 영향이 더 큰 것으로 나타나, 성별 노동시장 격차를 완화시키는 것이 고용보험 가입의 성별 격차를 완화시키는 중요한 방법이라는 점을 알 수 있다. 이를 위해 단기적으로는 현재 시행되고 있는 사회보험료 지원 사업이 효과적일 수 있으나, 근본적으로는 여성의 고용환경을 개선시키기 위한 정책적 노력과 더불어 정규직·고임금 노동자를 중심으로 한 현재의 고용보험제도가 불안정 일자리 및 저임금 노동자들을 포괄할 수 있도록 개편될 필요가 있다.

주요 간호활동의 건강보험 수가 적절성에 관한 연구 (A Study on the Appropriateness of Health Insurance Fee in Main Nursing Practices)

  • Purpose: This study was to present improvement strategy and the problems of the nursing fee in national health insurance system. Methods: A total of 23 nursing activities performed by nurses were selected. Data were collected the relative value score and criteria of the Health Insurance Review & Assessment Agency. Sixty clinical nursing experts panels were composed and nursing time surveyed self-reported method. The actual fee was calculated through the nursing time, relative value score and actual labor costs. Nextly, the labor costs analyzed was compared with that in the national health insurance. Results: Although the practices were mainly performed by the nurse, other occupations have been recorded as main practitioners and the time of the nursing activity is partially improperly reflected. Additionally, although the nurse practiced mainly in glucose (semi-quantitative) test, it was confirmed that the principal practitioner was described as a clinical pathologist. The the labor cost gap was estimated that is 9.3 times (median) and 11.9 times (average) in this analysis. Conclusion: This study suggests that it is necessary to legislate a policy that can improve the quality of clinical nursing by reinforcing the appropriateness and improving nursing fee through reflection of the actual time spent for nursing care.

전문간호사의 수급 현황과 건강보험 급여화 방안 (Demand-supply of Advanced Practice Nurse (APN) and Alternative Benefit Strategies in the National Health Insurance)

  • Purpose: The purpose of this paper is to analyze the demand and supply of advanced practice nurses and suggest alternative benefit strategies in the Korean national health insurance. Methods: A revised demand & supply model was used to estimate the excess supply of APNs, and policy making process and key actors in the Korean health insurance were considered to develop a political approach to the APN issue. Results: The social demand for APNs is currently estimated to be less than 50% of its supply and the APN education program fell into difficulties in recruits. No reimbursement mechanism for APN's services in the national health insurance has given no economic incentive to hospital managers who have monopsony power in nursing labor market, which has caused the demand shortage of APNs in hospital industry. Payment for APN's services recognized as one of the most significant strategies to booster the social demand for APN's services should be carefully designed and implemented in the national health insurance. In line with this, key actors in health insurance policy decision-making include government, national assembly, labor unions, NGOs, civic groups, medical associations, and academia. Conclusion: The basic researches for APN's activities and cost-effectiveness analysis in clinical settings are required to support the strategies aforementioned. Constructing a policy network among key actors is able to make the payment strategy feasible, which will increase the socal demand for APNs.

  • The purpose of this study is to analyze the employer's perception and corresponding types of the Industrial Accident Compensation Insurance System. To analyze the employer's perception and corresponding types of the Industrial Accident Compensation Insurance System, interviews and surveys were conducted. Based on the analysis of interview and survey results, we proposed policy alternatives to raise awareness of the Industrial Accident Compensation Insurance System and increase the utilization of the Industrial Accident Compensation Insurance System.

  • Korea's social health insurance system was introduced in 1977, which has made a universal coverage possibly by July 1989. Korean government had pursued a single objective for the last decade to put the whole population under the coverage of medical security, and the objective was achieved within 12 years. The rapid accomplishment is primarily due to such factors as limited benefits, high copayment rate, low contributions as well as rapid economic growth. There are several sources of pressure for the implementation of social health insurance such as health professional group, labor unions, politicians, international organizations etc.. However it is important to look at the feasibility of social health insurance. Among other things, it is necessary to identify the administrative infrastructure of insurance system and to assess income for source of fund. As many developed countries, Korea began to apply health insurance to the employees of the large firms, and the expansion based on employment status. Thus the several funds system was inevitable according to the gradual expansion strategy. However many persons had criticized several funds system in respect with equity and efficiency aspects. In the short history of the Korean health insurance, whether one fund or sever or funds had been the most controversial issue. In Febrary 1999, the National Assembly passed the act of one fund system. From July 2000 separate funds will be unifed under new health insurance scheme. In this study we will analyze the policy making process on implementation, expansion and integration of health insurance system of Korea. And also analyse problems related to policy making.

종합전문요양기관과 종합병원의 선택진료 결정요인 (Determinants of selecting a doctor in specialized medical institutions and general hospitals)

  • This research was performed to investigate the determination factors of medical service to cover the fee for selecting a doctor which is one of the most important causes of debilitating national health insurance in Korea. Data was from Korea Health Panel and analyzed by Dutton(1986)'s medical service model which was an extended Anderson Model and was widely used in the researches on determination factors of medical service. The results were as follows; In the determinants of selecting a doctor in specialized medical institutions and general hospitals, patients with serious diseases selected doctors more often than other patients. By industrial accident compensation insurance law and enforcement ordinances, insurance covers the fee of selecting a doctor in the hospitals appointed by Labor Welfare Corporation for the patients in critical conditions under industrial accident compensation insurance, while health insurance patients pay the fee themselves for selecting a doctor in all cases. It is suggested that patients with serious diseases proved by medical opinion be provided with health care insurance in selecting a doctor and that the health insurance benefit coverage be enhanced by staged lowering of patient's cost-sharing.

노인장기요양보험제도의 노동공급효과 분석 - 부양가구원과 여성가구원을 중심으로- (The Effect of Long-Term Care Insurance on Labor Supply)

  • 본 연구의 목적은 노인장기요양서비스를 이용하는 부양가구원과 여성가구원의 노동공급효과를 확인하는데 있다. 노인장기요양보험제도의 공식적 돌봄 서비스가 부양가구원들의 비공식적 돌봄을 대체한다면, 이는 부양가구원들의 시간제약을 변화시켜 노동공급을 증가시킬 것이다. 따라서 이러한 노동공급효과는 각국의 공적 돌봄 서비스의 제도적 수준에 따라 달라질 수 있다. 지금까지 서구국가들에서는 이에 대한 연구가 활발히 전개되어 왔으나, 이러한 실증분석들은 혼재된 결과를 보인다. 선행연구들에서는 무엇보다 방법론상 내생성의 문제가 지속적으로 제기되어 왔다. 본 연구는 이러한 문제를 해결하고자 유사실험설계인 PSM(Propensity Score Matching)과 DD(Double Difference) 결합모형을 활용하여 선택편의를 최소화하였다. 한국복지패널 3차, 9차 데이터를 활용한 분석 결과, 한국의 노인장기요양보험제도의 노동공급효과는 나타나지 않았다. 단순이중차이분석에서는 노동시간과 근로소득이 유의미하게 증가한 것으로 나타났지만, 더 엄격하다고 볼 수 있는 이중차이 고정효과모형분석에서는 그 효과가 나타나지 않았다. 이를 서구의 경험적 연구결과에 비춰보면, 한국의 공적 돌봄서비스인 노인장기요양보험제도가 비공식적 돌봄을 대체하지 못하고 일부만을 보완하는 수준에 머물러 있는 것이라고 해석할 수 있다. 본 연구의 분석결과는 그동안 제기되어온 노인장기요양서비스의 급여불충분성에 대한 증거가 될 수 있으며, 노인장기요양서비스의 제도적 배열이 대상자 및 부양가족의 욕구에 맞도록 선택되어져야 한다는 함의를 가진다.

An Empirical Simulation for the Relevance of Alternative Systems to Unemployment Insurance in Korea

  • Using the simulated data set which is based upon the data set merging Economically Active Population Survey(EAPS) and the Supplementary Survey (SS) in 1998-2001, this paper examines the relevance of alternative programs Unemployment Insurance Savings Account (UISA) and Pension-funded Unemployment Benefit (PUB) - to unemployment insurance system in Korea. Estimating the relative size of unemployment benefit and IA balance under a specific type of UISA or PUB by simulation, this paper yields the two main results. First, replacing UI by UISA with the same benefit being maintained would be beneficial in terms of search efficiency in general, although its effectiveness is a little doubtful as for the non-regular workers. Second, the PUB is better than UISA as an alternative to UI, and also works relatively well even for the non-regular workers.

  • This study examined the possibility of applying the industrial accident insurance balance ratio to the calculation of the converted accident ratio in the credibility assessment of the PQ. Some correlations between the industrial accident insurance balance ratio and the converted accident ratio were found in the general tendency, however, there were no significant correlations between the two in the individual companies. Therefore, this study came to the conclusion that it is not appropriate to directly apply the industrial accident insurance balance ratio to the calculation of the converted accident ratio.
