Heart Rate Variability in Patients Who Visit Dept. of Oriental Medical Opthalmology & Otolaryngology & Dermatology : Those Who Have Self-Recognized Stress (안이비인후피부 환자의 HRV 검사 : 자가 스트레스 인식자를 대상으로)

  • Objective: Stress is one of the most important etiological factors in our lives. Human body makes effort to keep his homeostasis from stress by using autonomic nerve system. So we need markers which can evaluate autonomic nerve system easily. HRV represents such markers. Sympathetic and Parasympathetic nerve system are part of autonomic nerve system, related with cur body and diseases in ENT, Dermatology, Ophthalmology are also deeply related with stress, Sympathetic and Parasympathetic nerve system. So we can expect that it can help understanding patients to evaluate autonomic nerve system with HRV Research Methods and Procedures: Forty one patients who have seif-recognized stress in Dept. of Oph & Otolaryngo & Dermatology were selected. SDNN. RMSSD, LF, HF, norm-LF, norm-HF, LF/HF ratio, LnLF, LnHF were checked. Results: Men showed lower SDNN, HF, and LnHF than women(p<0.05). Age over-forty group showed significant lower SDNN, LF, HF, LnLF, LnH(p<0.01), and lower RMSSD(p<0.05) than age under-forty group. There's no big differences for age at norm-LF, norm-HF, and LF/HF ratio. Skin disease group showed higher SDNN, RMSSD, LF, HF, LnLF, LNHF than the others, facial palsy group showed higher norm-HF. LF/HF ratio, and mouth-throat group showed higher norm-LF, but there's no statically significant difference. Comparing skin disease group with the others, the others showed statically significant low SDNN(P<0.01), RMSSD, LF, HF, LnLF and LnHF(P<0.05).

A Study on the Correlation of the accompanying symptoms, Heart Rate Variability and Body Component Analysis in 350 Insomnia Patients (불면환자 350명의 동반증상과 심박변이도, 체성분 분석의 연관성에 관한 연구)

  • Objectives : This study is to figure the relations of the heart rate variability, body component analysis and accompanying symptoms of 350 insomnia patients. Methods : For this study we evaluated Heart Rate Variability(HRV) and body component analysis on 350 insomnia patients who visited Dongeui oriental hospital of Dongeui university from January 2008 to March 2012. The accompanying symptoms was collected based on each patient's progress note. Results : 1. There was no difference between PR, LF and VLF of male and female groups. HF was higher in the patients' of female group and LF/HF ratio was higher in the male group. The patients' group of age under 39 had higher HF, LF, VLF and LF/HF ratio than the group over 39. 2. The average of LF was the smallest, and the average of VLF was in the middle, while the average of HF was the largest. 3. Regarding patients' age and gender, as the patients' age increased, their HF, LF, VLF and LF/HF ratio decreased significantly. HF, LF, VLF, and LF/HF ratios were, however, independent on the patients' gender. As the patients' age increased, their BMI increased, while the patients' gender did not affect on their BMI. The amount of visceral fat increased with the patients' age, but wasn't dependent on the patients' gender. 4. As the patients' BMI increased, PR and LF decreased. As the patients' amount of visceral fat increased, PR, HF, LF and VLF decreased. 5. The most frequent accompanying symptoms of the insomnia patients was headache. Neither HF nor LF/HF ratio was dependant on any of the accompanying symptoms. Patients with anxiety showed significantly higher LF than those without anxiety. Patients with fatigue and physical pain showed significantly higher VLF than those without either of them. Conclusions : The study showed that as the insomnia patients age increased, the HF, LF, VLF, LF/HF ratio significantly decreased, but the BMI and visceral fat increased. The HF, LF, VLF, LF/HF ratio BMI, or the visceral fat was independent on the gender. As BMI increased, PR and LF decreased. As visceral fat increased, PR HF, LF and VLF decreased. Patients presenting anxiety had higher LF. Patients either with fatigue or physical pain had higher VLF. Neither HF nor LF/HF ratio had any significant correlation with any of the accompanying symptoms.

  • There is substantial evidence that anatomical connections and functional interactions exist between vestibular and autonomic systems. Heart rate variability (HRV) including mean, standard deviation, coefficient of variation (CV), power spectrum was analyzed for evaluation of the physiological role of the vestibular system on control of heart rate in rabbits. In anesthetized rabbits, electrical stimulation of the vagus nerve decreased heart rate and decreased LF/HF by increasing HF. On the cervical sympathetic nerve increased heart rate and increased LF/HF by increasing LF. Atropine, cholinergic blocker, increased heart rate and increased LF/HF by reducing HF, and propranolol, ${\beta}$-adrenergic blocker, decreased heart rate and decreased LF/HF by reducing LF> In unanesthetized rabbits, stimulation of the vestibular system induced by rotation or caloric increased heart rate and increased LF/HF by increasing LF> Also electrical stimulation of the vestibular nerve produced the same of effects as rotation or caloric in anesthetized rabbits. These results suggest that Stimulation of the vestibular system increased heart rate not by inhibiting the parasympathetic nerve but by activating the sympathetic nerve.

Heart Rate Variability in Cold Pattern: 3-year Follow-up Study (추적관찰을 통해 살펴본 한증 HRV지표)

  • This study aimed to investigate heart rate variability (HRV) characteristics of cold pattern with repeated measurement data. Participants were taken from a Daejeon University cohort study from 2015 to 2018. Forty-seven of the participants studied displayed cold pattern while 23 showed signs of non-cold pattern. HRV was measured in supine position for 5 minutes at each year, and an 8-item cold pattern questionnaire was used for the diagnosis of cold pattern. SDNN (standard deviation of the NN intervals) and RMSSD (the square root of the mean squared differences of successive NN intervals) were used as time domain analysis, and TP (total power), VLF (power in very low frequency range), LF (power in low frequency range), HF (power in high frequency range), LF norm (LF power in normalized units), HF norm (HF power in normalized units) and LF/HF were used as frequency domain analysis. In the Mann-Whitney U test, LF norm, HF norm, and LF/HF showed differences between the cold pattern group and non-cold pattern group at every measurement, and in the independent t-test, the differences were also observed at three points except for the baseline (2015). In the repeated measures ANOVA, the interaction effects were not observed in all HRV parameters, but the time period effects were observed in SDNN, RMSSD, TP, VLF, LF and HF. There were significant differences between those two groups in LF norm, HF norm and LF/HF. This study suggests that LF norm, HF norm and LF/HF might be a useful indicator of cold pattern properties.

The Effects of Posture and Sleep Deprivation on Heart Rate Variability (자세와 수면 박탈이 심박 변이도에 미치는 영향)

  • Autonomic nervous system (ANS) acts as a control system functioning largely below the level of consciousness, and controls visceral functions. The activity of the ANS has been assessed by means of the heart rate variability (HRV). It has been reported that HRV is dependent on sex, age, body mass index, and smoking, etc. However, the effects of posture and sleep deprivation on HRV have rarely been reported. Objective of our work was to find out which posture is appropriate for stable HRV. We measured the number of sleep deprivation and HRV using power spectrum in six stages for 30 minutes. Increased low frequency (LF) power and high frequency (HF) power indicate enhanced sympathetic and parasympathetic activity, respectively. We determined the LF/HF ratio to minimize individual difference. It was found that sleep deprivation by awakening up subjects was affected by posture, which resulted in changes of LF/HF. Although LF/HF varied with time, it was more stable in sitting than in supine. In conclusion, we recommend sitting posture when measuring HRV because of less sleep deprivation resulting in less variation in LF/HF.

The Cross Sectional Study of Autonomic Nerve Balance And Stress In Patient Who Visted Dept. of Oriental Medical Dermatology (HRV 검사를 통한 피부과 내원환자의 자율신경 균형 및 스트레스 단면 연구)

  • Objective : Etiologically, stress is a important factor to immune system and skin disease. Human body is controled to keep his homeostasis from stress by autonomic nerve system. So we investigated autonomic nerve balance and stress level of patients with skin disease by heart rate variability(HRV) test. Research Methods and Procedures : Fifty two patient(from 15 years old to 35 years old) who visited Dept. of Oriental medical Dermatology, Kyunghee university at first and had an examination by HRV test, were selected as the subject of our study. We checked items of HRV test(such as SDNN, RMSSD, VLF, LF, HF, TP, norm-LF, norm-HF, LF/HF ratio, LnHF and LnHF), and analyzed time and frequency domain by sex, age, onset, color on end of patient's tongue, sweating and sleep condition. Results and Conclusion : Men showed significant higher SDNN, RMSSD, LF, norm-LF, norm-HF, LF/HF ratio, LnLF and LnHF than women(p <0.05). Patients who were attacked by skin disease within 6 months, showed significant higher TP and VLF than over 6 months(p <0.05). Patients with normal color on the end of their tongue showed significant higher TP and VLF than with flare color. We analyzed correlation to color on the end of tongue and RMSSD. Prevalence risk(RMSSD <20ms) in the case of normal color on the end of tongue was 5 times as high as in the case of flare color(p <0.1). And we analyzed correlation to age and LF/HF ratio. Prevalence risk(LF/HF ratio <1) of patients from 15 years old to 25 years old was 1.467 times as high as from 26 years old to 35 years old(p <0.1).

Differential Effect of n6 and n3 Polyunsaturated Fatty Acids on Plasma Lipids in Rats Fed Low and High Fat Diets (식이지방 수준에 따라 n6 와 n3 계 불포화지방산이 혈장 지질수준에 미치는 영향에 관한 비교연구)

  • To compare the hypolipidemic effects of n6 and n3 PUFA at different fat levels, male Sprague Dawley rats were fed either low fat (LF, 10% Cal) or high fat (HF, 40% Cal) diet which was different only in fatty acid composition for 6 weeks. Dietary fats were beef tallow, corn oil, perilla oil, and fish oil concentrate as a source of saturated fatty acid, n6 linoleic acid(LA). n3 ${\alpha}-linolenic$ acid(LL) and n3 eicosapentaenoic acid(EPA)+docosahexaenoic acid(DHA), respectively. VLDL fraction was separated by ultracentrifugation and chemical composition was determined by thin layer chromatography. Plasma cholesterol level was increased by n6 LA but decreased by n3 LL and n3 EPA in LF and HF diets, and the hypocholesterolemic effect of n3 EPA was most significant in HF diet. HDL-Chol level was raised by n6 LA in LF and HF diets, but significantly reduced by n3 EPA in HF. Plasma TG level was reduced by n6 LA n3 LL and EPA in LF and HF with the reduction of lipogenic enzyme activity only by n3 PUFAs. The proportion of TG in VLDL fraction was significantly lowered by n3 EPA in LF and HF. The proportion of apo-B in VLDL fraction was not changed in LF, but was significantly decreased in HF by n3 EPA. Therefore, the hypotriglyceridemic effect of n3 PUFA could be from the reduced lipogenesis in liver and resulted in the depressed secretion of TG as VLDL in LF and HF with significant lower production of apoB in HF diet.

Effect of Auricular Acupuncture for Mental Stress on Heart Rate Variability(HRV) (이침 요법이 정신적 스트레스를 가한 성인의 심박변이도에 미치는 영향)

  • Objectives : The purpose of this study was to assess the effect auricular acupuncture for acute mental stress using power spectrum analysis of the heart rate variability(HRV). Methods : 10 healthy volunteers participated in this study. After 5 minute rest, the first mental stress was provided for 5 minute. And then subjects rested for 15 minute. The second mental stress was provided for 5 minute. The acupoint, Shin-Mun point of the ear was stimulated for 15 minute. HRV was recorded before and after the first and second mental stress, and after auricular acupuncture stimulation. Results : After mental stress, normalized LF and LF/HF ratio is significantly increased. Before and after simple rest, normalized LF and normalized HF is significantly changed, but LF and LF/HF ratio is not significantly changed. On the other hand, before and after auricular acupuncture treatment, normalized LF, normalized HF is significantly changed, and also LF and LF/HF ratio is significantly decreased. Conclusion : The result suggest that auricular acupuncture can decrease more significantly in cardiac sympathetic activity due to mental stress than simple rest.

A Study on the Relationship with Acupuncture Stimulation and Stress Using Heart Rate Variability (심박변이도를 통한 침자극과 스트레스의 상관관계 연구)

  • Objectives : The purpose of this case-control research was to investigate the effects of acupuncture stimulation on autonomic nervous system for patients with HRV and to find out relationship with anti-stress effects. Methods : The study group consists of 24 patients with self-recognition of stress as the case group, and 20 normal person as the control group by similar age. We measured HRV of case and control groups before acupuncture stimulation, prick acupuncture in Hegu(LI4), Taichong(Liv3), Shenme(H7), Neiguan(P6), Zusanli(S36). After treating for 20 minutes, measurement values of HRV and PSV were compared for pre-acupuncture and post-acupuncture. Results : 1. LF norm, HF norm, LF/HF between the case and control groups were significant different in HRV before acupuncture stimulation in the 1st experiment. 2. HRT, SDNN, SDSD, LF norm, HF norm, and LF/HF of the case group were significant different in HRV after acupuncture stimulation in the 1st experiment. HRT of the case group was significantly different in HRV after acupuncture stimulation in the 1st experiment. 3. LF norm, HF norm, LF/HF of the case group were significant different between the 1st and 2nd experiment in HRV before acupuncture stimulation. 4. LF norm, HF norm, and LF/HF were significant different between the 1st and 2nd experiment in HRV of patients whose symptoms improved. But HRV of patients whose symptoms unimproved didn't show significant difference. Conclusion : The results suggest that acupuncture stimulation is associated with changed activity in the sympathetic and parasympathetic nervous system. Measurement values of HRV is suitable to estimate the activity of automatic nervous system.

The Effect of the Physical Therapy Treatment Room Environment Using Microwave Diathermy on the Autonomic Nervous System of Human Body (극초단파치료기를 사용하는 물리치료실의 환경이 물리치료사의 인체자율신경계에 미치는 영향)

  • PURPOSE: The purpose of this study is to investigate the effect of the physical therapy treatment room environment using microwave diathermy on the autonomic nervous system of human body. METHODS: Participants were 24 healthy adults. Standard deviation of all normal R-R intervals(SDNN), root mean square of successive differences(RMSSD), low frequency(LF), high frequency(HF), LF/HF ratio were compared in microwave irradiation and non-irradiation group. Data were analyzed in Wilcoxon's signed-ranks test and Mann-Whitney U test. RESULTS: Standard deviation of all normal R-R intervals (SDNN), root mean square of successive differences (RMSSD), low frequency(LF), high frequency(HF), LF/HF ratio were not significantly different in microwave irradiation group. Standard deviation of all normal R-R intervals(SDNN), root mean square of successive differences (RMSSD), low frequency(LF), high frequency(HF), LF/HF ratio were not significantly different in microwave non-irradiation group. Standard deviation of all normal R-R intervals(SDNN), root mean square of successive differences (RMSSD), low frequency(LF), high frequency(HF), LF/HF ratio were not significantly different between two groups. CONCLUSION: There was no significant change in the sympathetic nervous system and parasympathetic nervous system regardless of the presence of microwave irradiation. There was no significant change in the autonomic nervous system adaptability regardless of the presence of microwave irradiation.According to this study, microwave diathermy does not have significant effect on the autonomic nervous system.Future study is necessary to investigate the long term effect of the physical therapy treatment room environment using microwave diathermy on the autonomic nervous system of the human body.
