Spinal Arachnoiditis after Continuous Epidural Block (지속적 경막외 차단술 후 발생한 척수거미막염)

  • A 35-year-old female patient was referred to our hospital with neurologic symptoms after continuous epidural block performed 2 days earlier. She die not have any prior no previous lumbar surgery or experience trauma, intraspinal hemorrhage, infections or other known causative factors to associate with neurologic symptoms. Continuous epidural block is widely used for postoperative pain control. Complications can occur with this block including postduralpuncture headache, epidural abscess and rare cases of arachnoiditis etc. We experienced such a case of spinal arachnoiditis after continuous epidural block. Neurologic examination revealed painful bilateral hypoesthesia below $S_2$ level dermatomes, urinary and fecal incontinence and various degrees of leg weakness. The following day, the patient was noted to have bilateral sacral radiculopathies and lesion on proximal portion of both tibial nerve. CSF study reported: protein 264 mg/dl, sugar 64 mg/dl, WBC $7/mm^3$. L-spine MyeloCTscan results were unremarkable. She was discharged after a month of hospitalization and has regular checkups but her neurologic symptoms show no signs of improvement.

Effect of Water Drinking on the Postprandial Fall of Blood Pressure in the Elderly (식후저혈압 위험 노인의 식전 물 섭취가 식후 혈압변화에 미치는 효과)

  • Purpose: The purpose of this study was to examine preventive effects of water drinking on postprandial fall of blood pressure in the elderly. Method: Participants were 25 elders who had experienced postprandial fall of systolic blood pressure of more than 15mmHg in a previous study. Within subject repeated measures design using random order allocation was used to control extraneous variables among participants. During the experiment, each participant had to drink 400mL water before lunch, whereas when in the control, they only had their lunch. Blood pressure and heart rate were measured for both times before lunch and at 15 minutes intervals up to 90 minutes after lunch. Data were analyzed using descriptive statistics, repeated measures of ANOVA, paired t-test and Bonferroni adjustment as Post-hoc analysis. Results: Water drinking before lunch elicited significant pressor effects by increasing systolic and diastolic pressure $13.9{\pm}4.7/5.2{\pm}2.2mmHg$ compared blood pressure during the control p<.005, p<.022. Heart rate, however, did not change significantly at either time. Conclusion: Drinking water before meal is recommended for elders who experience postprandial hypotension. However, the appropriate volume of water to prevent postprandial fall in blood pressure should be further studied.

  • In this study, we intended to describe a human case of lumbosacral canal sparganosis in People's Republic of China (China). A 56-year-old man was admitted to Xiangya Hospital Central South University in Changsha, Hunan province, China after having an experience of perianal pain for a week. An enhancing mass, a tumor clinically suggested, was showed at the S1-S2 level of the lumbosacral spine by the examination of magnetic resonance imaging (MRI) with gadolinium contrast. The patient was received the laminectomy from S1 to S2, and an ivory-white living worm was detected in inferior margin of L5. In ELISA-test with cerebrospinal fluid (CSF) and serum samples, anti-sparganum antibodies were detected. He had a ingesting history of undercooked frog meat in his youth. By the present study, a human case of spinal sparganosis invaded in lumbosacral canal at the S1-S2 level was diagnosed in China. Although the surgical removal of larvae is known to be the best way of treatment for sparganosis, we administered the high-dosage of praziquantel, albendazole and dexamethasone to prevent the occurrence of another remain worms in this study.

Prevention of thromboembolism with ticlopidine and aspirin after cardiac valve replacement (심장변막치환후 Ticlopidine과 Aspirin의 혈전방지 효과)

  • Prevention of thrombombolism after rosthetic cardiac valve replacement is essential for the patients. About 90% of patients are free of major and minor thromboembolic complications 5 year after replacement of cardiac valves with prosthetic devices when they are under control of anticoagulant therapy. Ticlopidine is a drug that alter platelet function to have an antithrombotic effect. It is an antiaggregating agent which inhibits primary platelet function to have an antithrombotic effect. It is an antiaggregating agent which inhibits primary platelet aggregation induced by ADP and increases the production of prostaglandin $D_{2}$. Aspirin in small doses inhibits platelet synthesis of prostaglandins by irreversibly blocking the enzyme cyclo-oxygenase. Platelet secretion and aggregation are impaired with Ticlopidine and Aspirin. the thromboembolic event sof 54 patient s who were treated with Ticlopidine and Aspirin after cardiac valve replacement were evaluated and compared with that of 79 patients who were treated with Wafarin and Aspirin after the same type of operation. The follow-up period ranged from 4 to 110 months (mean of 48 months). there were 11 major thromboembolic episodes including three deaths in the warfarin goup during mean follow-up period of 56 months. two cases of CVA and one hemoarthrosis were noted due to overdose of Warfarin. Inticlopidine group, there was only one fatal thromboembolic epdisode three month after mitral valve replacement during mean follow-up period of 18 months. Two episodes of hypermenorrhea resulting anemia ere noted in the ticlopidine group. We measured the parameters of platelet function in aggreagation curve of platelet with platelet aggregometer (chrono-log Aggregometer, Model No. 430) Aggregation test was performed with three final concentrations of epinephrine in 10 uM/L, ADP in 5uM/L. 28 patients with prosthetic cardiac valves and 35 healthy volunteers were subgrouped as follows to analyze the effect of antithrombotic drugs used. Group I ; 11 patients treated with 250-500 mg of ticlopidine and 0.5gm of Aspirin as a daily single dose after cardiac valve replacement (14 St. Jude Medical and 1 Carpentier-Edwards, 9 patients with atrial fibrillation among them) Group II ; 10 patients treated with 3-5 mg of Warfarin and 0.75 gm of Aspirin daily to prolong prothrombin time around 20 seconds for more than 6 months and single Aspirin dose was maintained afterward as a life-long regimes(3 St. Jude Medical, 1 Hall-Kaster and 7 Carpentier-Edwards valve, 9 patients in atrial fibrilation). Group III ; 7 patients who quit anticoagulant treatment (Warfarin + Aspirin) 6-12 months after the regime as group II (3 St. Jude Medical. 1 bjork-Shiley, 1 Hall-Kaster, 3 Carpentier-Edwards valve, 2 of them are with atrial fibrillation). Group IV ; 35 healthy vounteers (28 males and 7 females). The following results were obtained. 1. The mean maximal platelet aggregability in Group I induced by 10uM/L epinephrine was 15.6%, and 17.5 and 18.7% in BM in proportion to the induction by 5 and 10 uM/L ADP. 2. The mean maximal platelet aggregability in Group II induced by 10uM/L epinephrine was 16.5%, and 27.4 and 44.7% in BM in proportion to the induction by 5 and 10uM/L ADP. 3. The mean maximal platelet aggregability in group III induced by 10uM/L epinephrine was 65%, and 56.5 and 51.8% in BM in proportion to the induction by 5 and 10 uM/L ADP. 4. The mean maximal platelet aggregability in the normal subjects induced by 10 uM/L epinephrine was 64%, and 65 and 69% in Bm inproportion to the induction by 5 and 10 uM/L ADP. 5. Reversible change of platelet aggregation curve induced by 5 and 10uM/L was noted all of the patients in Group I. conclusion : Ticlopidine is an antiaggregating agent which inhibits primary platelet aggregation induced by ADP, and increases the production of prostaglandin $D_{2}$. Ticlopidine and Aspirin produced a significant inhibition of platelet in the presence of ADP and epinephrine in our study. Acccording to our brief experience, 250 mg of ticlopidine and low dose of Aspirin resulted synergistic superior effect to each drug alone in prevention of thromboembolism after prosthetic cardiac valve replacement.

Clinical Practice of Blood Transfusion in Orthotopic Organ Transplantation: A Single Institution Experience

  • Background: Orthotopic organ transplantation, a treatment option for irreversible organ dysfunction according to organ failure, severe damaged organ or malignancy in situ, was usually accompanied with massive blood loss thus transfusion was required. We aimed to evaluate the adverse impact of blood transfusion on solid organ transplantation. Materials and Methods: From January, 2009 to December, 2014, patients who received orthotopic organ transplantation at Far Eastern Memorial Hospital medical center were enrolled. Clinical data regarding anemia status and red blood cell (RBC) transfusion before, during and after operation, as well as patient outcomes were collected for further univariate analysis. Results: A total of 105 patients who underwent orthotopic transplantation, including liver, kidney and small intestine were registered. The mean hemoglobin (Hb) level upon admission and before operation were $11.6{\pm}1.8g/dL$ and $11.7{\pm}1.7g/dL$, respectively; and the nadir Hb level post operation and the final Hb level before discharge were $8.3{\pm}1.6g/dL$ and $10.2{\pm}1.6g/dL$, respectively. The median units (interquartile range) of RBC transfusion in pre-operative, peri-operative and post-operative periods were 0 (0-0), 2 (0-12), and 2 (0-6) units, respectively. Furthermore, the median (interquartile range) length of hospital stay (LHS) from admission to discharge and from operation to discharge were 28 (17-44) and 24 (16-37) days, respectively. Both peri-operative and post-operative RBC transfusion were associated with longer LHS from admission to discharge and from operation to discharge. Furthermore, it increased the risk of post-operative septicemia. While peri-operative RBC transfusion elevated the risk of acute graft rejection in patients who received orthotopic transplantation. Conclusions: Worse outcome could be anticipated in those who had received massive RBC transfusion in transplantation operation. Hence, peri-operative RBC transfusion should be avoided as much as possible.

The Usefulness Evaluation of Radiation Shielding Devices in PET Scan Procedures (PET 검사 프러시저별 방사선 차폐기구의 유용성 평가)

  • Purpose: he use of PET scanners and the number of patient in Korea have been increased for recent several years dramatically. For this reason, technologists have more possibilities to be exposed to the radiation. The hospitals using PET scanners should make an effort to reduce the radiation exposure dose. The purpose of this study was to evaluate the radiation exposure does when using radiation shielding devices. The evaluation was performed through questionnaire survey and experiment. Materials and Methods: First, the technologists who had experience working in PET center in 2008-2009 were surveyed with questionnaire and TLD Figures, personal opinion of utilization of radiation shielding devices are analyzed. Second, we measured the shielding rate of shielding devices which have been using in PET study procedures. We divided the procedures into four steps; distribution, moving, injection of $^{18}F$-FDG and patient setup. Results: First, the results of this survey, using of L-block+Syringe shield, L-block, Syringe shield, No shield during the injection, were each 58.5%, 20%, 9%, 12.3%. The TLD values according to utilization of radiation shield, using both L-block+Syringe Shield and L-block showed the lower TLD values, and Syringe shield only or No shield showed the higher TLD values. Second, the results of experiments according to PET study procedures measured the shielding rates as follows. The shielding rates during the distribution using L-block, L-block+Apron shield were measured 97.4%, 97.7%. The shielding rates during the $^{18}F$-FDG delivery to the injection room using mobile Syringe shield, Syringe holder, Syringe shield carrier were each 81.7%, 98.9%, 99.7%. The shielding rates during the injection using Syringe shield, L-block, L-block+Syringe shield were measured each 51.9%, 98.3%, 98.7%. The shielding rates of Apron were measured in each 30, 60, 90, 120, 150 cm distance. The measurement were each 16.9%, 14.2%, 16.6%, 17.1%, 18.1%, 18.6%. Conclusion: The most effective method for radiation shielding is to using L-block during the $^{18}F$-FDG distribution and Syringe shield carrier during in moving $^{18}F$-FDG. For the $^{18}F$-FDG injection, L-block+Syringe shield have to be used. The shielding effect of Apron has shown average 16.4%. According to the survey of questionnaire, the operators recognized well risk of the radiation exposure but, tended ignore in working. The radiation dose according to recognition of radiation exposure risk was not relevant. but radiation dose according to utilization of radiation shield lower the more use it. The main reason of no use of shielding devices is cumbersome, 55% of the respondents answered. I'm sure, by use of radiation shield in all PET procedure, radiation exposure will be reduced considerably.

The Effect of the Perception of Organizational Culture of Employees on Organizational Effectiveness in a Newly Established General Hospital (일개 신설종합병원 구성원의 조직문화인식이 조직유효성에 미치는 영향)

  • Purposes: This study was to analyze the effects of organizational culture attributes on organizational effectiveness in newly established general hospital. Method: For this purpose, this study sampled 981 hospital employees working for E hospital opened on Apr. 1, 2019 in Seoul. A total of 981 questionnaires were distributed to them, and 888 ones responded to the survey, which had been conducted from Oct. 17, through Oct. 25, 2019. 793 responses were used for the final analysis. The data collected were processed using the SPSS 19.0K for descriptive statistics, T-test, ANOVA, Pearson's correlation coefficient and regression analysis. Findings: First, The type of organizational culture perceived most by hospital employees was 'hierarchy-oriented' (3.53) followed by 'relation-oriented'(3.33), 'task-oriented'(3.23), 'innovation-oriented'(3.19) and job satisfaction scored 3.13, organizational commitment scored 3.28 on their order. Second, 'Relation-oriented', 'task-oriented' and 'innovation-oriented' was positively correlated with job satisfaction and organizational commitment. hierarchy-oriented' was negatively correlated with job satisfaction and organizational commitment. Third, Factors that significantly influencing on organizational effectiveness in a newly established general hospital were as follows. Factors influencing job satisfaction included 'innovation-oriented', 'relation-oriented' and factors influencing organizational commitment included 'relation-oriented', 'innovation-oriented' and age. Practical Implications: In order to increase the organizational effectiveness of the E-General Hospital, it is necessary to gradually move from a hierarchica l-oriented culture to an innovation-oriented and relationship-oriented culture. Since age also affects organizational commitment, it is necessary to promote stability and growth by promoting the mentor-menti system for new members with a low level of experience and experience.

Predictors of the Compliance of Breast Self-Examination among Christian Women in Pusan and Ulsan Areas (부산, 울산지역 기독교여성의 유방 자가검진 이행 예측요인)

  • The purpose of this study was to identify the influencing factors on the compliance of breast self-examination (BSE) among 205 Christian women living in Pusan and U1san areas. Structured self-administered questionnaires were used. Knowledge levels were measured by the instrument developed by Choi(l996). Self- efficacy and health belief were measured by the instruments developed by Champion (1993) and modified by Choi(1996). Women who did BSE more than once within 6 months were categorised as compliant. Data was analyzed by descriptive statistics, t- test, χ² test and logistic regression analysis using SPSS WIN program. The results of this study were summarized as follows; 1. 15.1% of women performed the BSE at least once during the last 6 months. 2. Non-complier and compliant women showed statistically significant differences in the level of self efficacy and barriers. 3. Predictors of BSE compliance were ‘history of having breast disease’, ‘experience of receiving recommendation for BSE’, ‘barriers’, and ‘experience of meeting with breast cancer patients’ and explained 14.8% of variance in compliance of BSE. For future clinical practice, health professionals, including nurses, need to actively suggest to women the importance of BSE, and to distribute the information through posters and pamphlets at clinics and public health centers. Including the opportunity to meet patients in health education centers can be another strategy to increase women's motivation for BSE. For future research, it is necessary to recruit women from broader geographic areas utilizing various sources such as community centers to increase generalizability of the findings.

Perception and production of Korean and English stops by bilinguals with extensive experience residing in the U.S.: Individual patterns

  • This study aimed to examine how Korean-English bilinguals make use of VOT and F0 cues in perception and production of Korean (lenis vs. aspirated) and English (voiced vs. voiceless) stops. It was explored whether bilinguals with extensive experience living in the U.S. exhibit native-like or interactive patterns in the cue use for both languages. Participants produced monosyllabic word-initial stops within a carrier sentence in each language, and performed forced-choice identification tasks with synthesized stimuli varying in 7 VOT steps and 7 F0 steps with base tokens of /$t^han$/ for Korean and /$t{\ae}n$/ for English. Listeners were required to select either /tan/ or /$t^han$/ for Korean and either /$d{\ae}n$/ or /$t{\ae}n$/ for English. The results from binary logistic regression analyses for each listener indicated that all bilinguals placed greater weight on F0 than VOT when distinguishing between the Korean lenis and aspirated stops, and greater weight on VOT than F0 in distinguishing between the English voiced and voiceless stops. In terms of production, all participants showed remarkably overlapping ranges in the VOT dimension and separating ranges in the F0 dimension for the stop contrast of Korean, while forming overlapping ranges in the F0 dimension and separating ranges in the VOT dimension for the stop contrast of English. These results indicate that the bilinguals with extensive exposure to L2 manage the stop systems of the two languages independently, both in perception and production, employing the opposite cue use for stops in the two languages. It was also found that the absolute beta-coefficient values of the perceptual cues for Korean stops were generally smaller than those for English and those reported in a previous study as for later bilinguals, which may have resulted from Korean not being their dominant language.

An Analysis of Nursing Students Attiudes Toward Life (간호학생의 생명에 대한 태도 유형 분석 : Q-방법론적 접근)

  • This study was designed to identify nursing students' attitudes toward life through a Q-methodology. A Q-sample was formed through a review of the literature and interviews(n=160)l The final Q-sample consisted of 37 statements out of an initial 100 statements after consultation with an expert panel and pilot testing. The P-sample consisted of 14 university nursing students and 27 junior college nursing students, which was selected by convenience sampling method. Data were analyzed by the Q-analysis method. The correlation between type 2 and type 3 was relatively high (r=0.539) ; that between type 1 and type 3 was lowest (r=0.014). The first type of attitude was the “rational utilitarian” type. Students in this type valued life relative to the quality of life. They agreed with euthanasia and artificial abortion if the quality of life was threatened. The criteria for their judgement were scientific knowledge and rationality. The second type of attitude was the “Christian deontologic” type. These students appreciated the sanctity of life according to Christian dogma. They disagreed with euthanasia and artficial abortion. And they disagreed strongly that life should be created by scientific development, because only God creates life. The third type of attitude was the “unconditional deontologic” type. These students agreed with the sanctity of life, not from Christian belief but from belief in the sanctity of life. The final type of attitude was the “prima facie(conditional) deonologic” type. These students appreciated the value of life and humanity. They expressed concern for others' life and suffering. They do not want to afflict others with their own miseries. This group showed a dual value system toward themselves and others. So they experience conflict between their concern for their on and others' conditions. These nursing students' values may have been influenced by their clinical experience in hospitals and other nursing fields. Through this study, we may realize the importance of education in nursing ethics for discussion of ethical conflicts and to support ethical nursing practice.
