Hemifacial Spasm Caused by Fusiform Aneurysm at Vertebral Artery-Posterior Inferior Cerebellar Artery Junction

  • Hemifacial spasm induced by intracranial aneurysm is a rare clinical condition. A 45-year-old male patient presented with a 3-year history of progressive involuntary twitching movement on right face. On radiological study, a dilated vascular lesion compressing the brain stem was found at the junction of vertebral artery and posterior inferior cerebellar artery. On operative field, we found the posterior inferior cerebellar artery and the fusiform aneurysm compressing root exit zone of facial nerve. Microvascular decompression was performed and the facial symptom was relieved without complications.

  • "HanGaekChiHeom" is a book that arranged the medical records left by a Japanese doctor Junso Higuchi who treated a group of diplomats from Chosun and questions and answers exchanged between him and Chosun's doctor ChoSoongSoo. There are 14 kinds of medical records here with treatments and prescriptions as well as the detailed descriptions about the name, age and symptoms of patients. Various diseases and symptoms are included among them such as bums, common cold, skin diseases, edemas, etc. The characteristics of Junso Higuchi seem to be affected by medicine in Ming Dynasty in China.

  • Apoptosis has been hypothesized to be involved in the pathogenesis in schizophrenia. A large number of genes are known to mediate the apoptotic process; Apo-1/Fas (CD95) is a well-known example of such genes. In the present study, MvaI restriction fragment length polymorphism, a polymorphic marker present within the Apo-1/Fas gene, was examined in a population consisting of 226 control subjects and 110 schizophrenia patients, all of them Korean in ethnicity. No statistically significant difference in the genotypic distribution and allelic frequencies was observed between the control and the schizophrenia patient group. To find out the precise effect of Apo-1/Fas gene polymorphisms on the susceptibility to schizophrenia, further studies are warranted to investigate possible involvement of other polymorphisms with a larger sample population.

Isolation of Flavonoids from the Fruits of Cornus kousa Burg

  • Dried, unripe fruits of Cornus kousa Burg. were extracted with 80% aqueous MeOH and the concentrated extracts were partitioned between EtOAc and $H_2O$. From the EtOAc fraction, four flavonoids were isolated through repeated silica gel, ODS and Sephadex LH-20 column chromatographies followed by a preparative HPLC. Based on the spectroscopic data including NMR, MS and IR, the chemical structures of the compounds were determined as kaempferol (1), astragalin (2), hyperin (3) and isoquercitrin (4). These compounds were isolated for the first time from the fruits of this plant.

  • Campsis grandiflora K. Schum. flower was extracted with 80% aqueous MeOH, and concentrated extract was successively partitioned with EtOAc, n-BuOH, and $H_2O$. From n-BuOH fraction, two cyclohexylethanoids were isolated through repeated silica gel and Sephadex LH-20 column chromatographies. Based on physico-chemical data obtained from NMR, MS, and IR, chemical structures of compounds were determined as 1,4-dihydroxy-3,4-(epoxyethano)-5-cyclohexene (1) and cornoside (2). These compounds were isolated for the first time from C. grandiflora K. Schum flower.

Resistance to fluoroquinolone of Staphylococcus pseudintermedius isolated from dogs and cats in Daegu (대구지역 개와 고양이에서 분리된 Staphylococcus pseudintermedius의 fluoroquinolone 내성)

  • The purpose of this study was to investigated the fluoroquinolone (FQ) resistance and presence of gyrA and grlA gene in 87 Staphylococcus pseudintermedius isolates obtained from clinical samples of dogs and cats. Also, the profiles of FQ resistance compared with methicillin resistant S. pseudintermedius (MRSP) isolates. FQ resistance was observed for enrofloxacin (41.4%), ciprofloxacin (39.1%), norfloxacin (36.8%), ofloxacin and levofloxacin (32.2%, respectively), and moxifloxacin (31.0%). Thirty-eight (43.7%) of 87 S. pseudintermedius isolates were resistant to more than one FQ. Twenty-six (64.5%) of 38 FQ resistant isolates were resistant to all the six FQ tested. Of 38 FQ resistant isolates, gyrA gene was detected in all isolates but grlA gene was not found. Moreover, 19 MRSP isolates were resistant to enrofloxacin (63.2%), ciprofloxacin (57.9%), norfloxacin (52.6%), and ofloxacin, levofloxacin and moxifloxacin (47.4%, respectively). FQ resistance were highly prevalence in S. pseudintermedius isolates from dogs and cats. Our results emphasize the prudent use of antimicrobial agents to companion animals is necessary for prevent antimicrobial resistance.

  • Trapa pseudoincisa Nakai, an aquatic plant, was extracted with 80% aqueous MeOH, and the concentrated extract was successively partitioned with EtOAc, n-BuOH, and $H_2O$. The EtOAc fraction gave three compounds, which were isolated through the repeated silica gel and ODS column chromatographies. Based on the spectroscopic data obtained from NMR, MS, and IR, the chemical structures of the compounds were determined as cycloeucalenol (1), ursolic acid (2), and 2${\beta}$,3${\alpha}$,23-trihydroxyurs-12-en-28-oic acid (3). These triterpenoids were isolated for the first time from Hydrocaryaceae plants including T. pseudoincisa NAKAI.

Maxillomandibular advancement surgery after long-term use of a mandibular advancement device in a post-adolescent patient with obstructive sleep apnea

  • Patients with obstructive sleep apnea (OSA) whose phenotype belongs to a craniofacial vulnerability are referred from sleep doctors to orthodontists. In adults, for osseo-pharyngeal reconstruction (OPR) treatment, permanent maxillomandibular advancement (MMA) surgery and use of a temporary mandibular advancement device (MAD) are applied. This case report demonstrates successful treatment of OSA through application of phased MAD and MMA in a 16-year-old male with craniofacial deformity and residual growth potential. This patient showed skeletal and dentoalveolar changes after 7-year MAD use throughout post-adolescence, which affected the design and timing of subsequent MMA surgery, as well as post-surgical orthodontic strategy. This case report suggests that OPR treatment can be useful for treatment of OSA in post-adolescent patients, from an orthodontic point of view, in close collaboration with sleep doctors for interdisciplinary diagnosis and treatment.

  • Many enterprises and public institutions are building their own server stabilization system. Nevertheless, many web servers including some public institutions have frequent obstacles due to the unexpected causes such as traffic concentration. Data loss during each of these situations and business interruption can cause a huge loss. In this study, we propose high-availability Web system that uses load balancing and session clustering. Each deals with the failure due to the over traffic by distributing incoming requests in the multiplexed web server and provides the continuous services by maintaining session information of the users in case of the failure of some servers.
