The Study on History Period and Treatment Outcomes of Patients Admitted to a Korean Medicine Hospital for Low Back Pain (요통으로 한방병원에 입원한 환자의 병력기간별 호전도에 관한 연구)

  • Objectives Low back pain causes disability and incurs large socioeconomic burden. Many cases of acute low back pain progress into chronic conditions. As the treatment period extends, burden of socioeconomic expenses also rises. Thus this study is to investigate whether history period influences improvement rate of pain and disability in the patients who receive conservative Korean medicine treatment. Methods 447 inpatients who received conservative Korean Medicine treatment for the management of low back pain have been grouped into acute, sub-acute and chronic according to the length of history period. NRS, ODI and improvement rate of each group were investigated. Treatment every patient has received as follow; acupuncture, herbal medicine, Chuna manipulation, bee venom, and physical treatment. Results Acute patients showed improvement with NRS of $3.98{\pm}2.50$ and ODI of $28.04{\pm}21.54$. The NRS of sub-acute patients showed improvement with NRS of $2.81{\pm}2.07$, ODI also showed improvement with $11.64{\pm}13.94$. Statistically significant difference was observed in NRS, ODI and improvement for acute patients. Improvement in pain and disability was observed to be larger. Conclusions It demonstrated that receiving valid conservative Korean medicine treatment is beneficial for low back pain patients in improvement of their pain and disability.

A Case Report on a Patient with Acute Herniated Lumbar Disc due to Coughing Treated with Megadose Pharmacopuncture and Combined Korean Medicine (기침으로 급성 악화된 요추 추간판 파열 환자의 대용량 약침 및 복합 한방치료 호전사례 1례)

  • Objective: The purpose of this study was to report the radiological and symptomatic changes in a patient with acute herniated lumbar disc due to coughing, treated with megadose pharmacopuncture and combined Korean medicine. Methods: MRI examination diagnosed an acute lumbar disc herniation by coughing. We performed a combination treatment of megadose pharmacopuncture and integrated Korean medicine. NRS, ODI, EQ-5D, SLR test, and big toe extension test were checked on admission, after two weeks, on discharge, and after four months to confirm symptom improvements. Results: In the acute stage of lumbar intervertebral disc rupture, megadose pharmacopuncture combined with Korean medicine treatment showed improvement in NRS, ODI, EQ-5D, SLRT, and big toe extension tests. In addition, absorption of the herniated intervertebral discs was observed through an MRI scan on discharge. Conclusions: For patients with herniated lumbar disc acutely deteriorated by coughing, a combination of megadose pharmacopuncture with Korean medicine treatment may be a solution.

  • Objectives: The purpose of this study is to evaluate the effect of Korean medicine in a patient with sensory disturbance of the hands and feet diagnosed as hereditary motor and sensory neuropathy (Charcot-Marie-Tooth disease). Methods: A patient diagnosed with hereditary motor and sensory neuropathy (Charcot-Marie-Tooth disease) was treated with herbal medicine (Uchashinki-hwan-gami, Bosinji Granule, Ukgan-san-gami), acupuncture, moxibustion, and bee venom pharmacopuncture. Clinical improvements were evaluated using the numerical rating scale (NRS) and Toronto Clinical Neuropathy Score system (TCNSS). Results: Improvements in the total scores of NRS and TCNS were observed after Korean medicine treatments. NRS score decrease from 8 to 2, and TCNS score decreased from 10 to 7. Conclusion: Korean medicine treatment may be effective for sensory disturbance in hereditary motor and sensory neuropathy (Charcot-Marie-Tooth disease).

Review on the Pharmacopuncture Patent in Korea (국내 약침 특허 현황에 대한 분석연구)

  • Objectives : The purpose of this study was to analyze the trend of pharmacopuncture in Korean patent in order to establish database for patent technology. Methods : Electronic literature searches for Korean patents related to pharmacopuncture were performed in two electronic databases (Korea Intellectual Property Right Information Service and National Digital Science Library) to June 2017. Patents that were not Korean ones, did not use medicinal herb, only described method of manufacture, or had nothing to do with pharmacopuncture were excluded in this study. The status and application date of patents, Medicinal herb, target diseases, International Patent Classification (IPC), model of experiment and extracting methods were analyzed. Results : A total of 379 patents were retrieved. Based on our inclusion/exclusion criteria, 297 patents were excluded. Of 82 included patents, 27 patents did not include experiments using pharmacopuncture, and 9 patents were invented for treating animals such as pig or calf. In IPC analysis, Bee Venom, Panax (ginseng), Angelica, and Paeoniaceae were used frequently. Musculoskeletal diseases were the most targeted diseases followed by nervous diseases. For extracting, hot water extraction, distillation extraction, and solvent extraction using alcohol, ethanol, or methanol for solvent were commonly used. Conclusions : These data are useful for inventing new patent and extending range of pharmacopuncture in clinical use, however, more systematically analyzed patent studies and pharmacopuncture-related studies for new application on various diseases are needed in further studies.

A Case Report of Cyclophosphamide plus Doxorubicin-induced Side effects in Patient with Both Sides Breast Cancer Treated with Integrative Medicine Therapy Including Pharmacopuncture. (유방암 환자의 cyclophosphamide, doxorubicin 유발 부작용에 대한 약침을 포함한 통합 암 치료 1례)

  • Objectives: To observe the mitigating effects of a Traditional Korean Medicine(TKM) treatment program especially including pharmacopuncture, with Cyclophosphamide and doxorubicin chemotherapy on a both sides breast cancer patient. Methods: AA 74 year-old female patient diagnosed with both sides breast cancer (Right) pT1bpN0M0, (Left) pT1cpN1Mx was admitted to hospital of Dong-eui university in May of 2017. She received Cyclophosphamide and Doxorubicin from May $31^{st}$ to August $2^{nd}$, 2017 followed by TKM treatment consisting of herbal medicine, acupuncture, moxibustion and pharmacopuncture (Trionycis Carapax, Non-toxic Bee Venom, and Cultivated Wild Ginseng Extract) for a period of almost 4 months, from May $13^{th}$ to August $19^{th}$, 2017. Symptoms were evaluated by the grade of chief complaints refer to Common Terminology Criteria for Adverse Events (CTCAE) and Eastern Cooperative Oncology Group (ECOG). Results: TKM including pharmacopuncture alleviated chemotherapy-induced nausea, fatigue, joint pain, diarrhea, insomnia. Conclusions: This case study potentiates TKM with pharmacopuncture's significant efficacy in aiding breast cancer patients suffering from Cyclophosphamide plus Doxorubicin induced adverse effects. Further research should take place for clear understanding of the exact amount of dosage and safety. Moreover it must be accompanied by long-term follow up researches.

Effect of Bong Chuna Manual Treatment Combined with Oriental Medical Treatment on Posterior Inferior Ilium Deviation ; 3 Cases Report (봉추나요법과 한방치료를 병행한 후하방장골 변위 교정 3예)

  • Objectives : The purpose of this study was to observe the effect of Bong (a type of stick which is called bong) Chuna manual therapy (CMT) on the correction of posterior inferior (PI) ilium deviation, and evaluate changes of pain intensity and Oswestry Disability Index (ODI) for the treatment of low back pain. Methods : We measured leg length inequality (LLI), as an indicator of correction of PI ilium, before and after the Bong CMT twice immediately, and also checked the change of LLI in the course of 10 times of treatment. Ten times of Bong CMT with oriental medical treatment, such as acupuncture and bee venom pharmacopuncture, were performed not only to correct the PI ilium deviation but also to alleviate pain. Visual Analog Scale (VAS) and Oswetry Low-back Pain Disability Index (ODI) score were scored to evaluate the pain and function of low back. Results : In this study, LLI was decreased in general while we performed correction of PI ilium deviation immediately, also effectively decreased comparing between before and after 10 times of Bong CMT, Bong CMT with oriental medical treatment improved pain and function of low back. Conclusions : These results suggest that Bong CMT may be an effective method for the correction of PI ilium deviation. However more rigorous study should be followed because of small cases report.

A Case Report of Oriental Medical Treatment for the Left Lower Limb Monoplegia after Herniated Intervertebral Disc Operation at L-spine (요추 추간판 탈출증 수술 후 발생한 좌하지 단마비의 한의학적인 치료 증례보고)

  • Objectives: This study was performed to report the case of oriental medical treatment for the left lower limb monoplegia after herniated intervertebral disc(HIVD) operation at lumbar(L)-spine. Methods: A 38-year-old man who underwent lumbar HIVD operation at a hospital admitted with motor weakness of left lower limb, a little decreased left leg radiation pain and low back pain. We treated him by acupuncture, herbal medicine, bee venom injection moxibustion, cupping treatment physical therapy and measured with visual analogue scale(VAS), Oswestry disability index(ODI), Roland-Morris disability questionnaire(RMDO) and manual muscle test(MMT) from 8th February to 4th May 2011. Results: After treatment most symptoms decreased, VAS score changed from 10 to 3, ODI changed 71% to 37% RMDQ changed 13 to 8 and MMT changed from 3 to 5. Conclusions: Our study suggested that oriental medical treatments are significantly applicable to the monoplegia and pain after lumbar HIVD operation. And further studies ire required to identify underlying mechanism of the treatment.

A Clinical Case of Oriental Medical Treatment for the Paraplegia after Lumbar Epidural Nerve Block (요추 경막외 차단술 후 발생한 하지마비의 한의학적인 치료 효과 - 증례 보고 -)

  • Objectives : This study was performed to report the effect of oriental medical treatment in the paraplegia and pain after epidural nerve block. Methods : A 39-year-old woman who underwent epidural nerve block at a local clinic was admitted with motor weakness of lower limbs, severe lower radiating pain and decreased sensation when voiding and defecating. We treated her by acupuncture, a herbal medicine, a bee venom injection, moxibustion and cupping treatment and physical theraphy from 11th July 2008 to 14th October 2008. Results : After treatment, most symptoms decreased, VAS(Visual Analog Scale) score changed from 9 to 2. and examinated muscle power changed from 2-3 to 4-5. Conclusions : Our study suggested that oriental medical treatments are significantly effective in the paralplegia and pain after epidural nerve block. And further studies will be aid to identify underlying mechanism of treatment.

Therapeutic Effect of Bee-Venom and Dexamethasone in Dogs with Facial Nerve Paralysis (개 안면신경마비에 대한 봉독과 덱사메타손의 치료효과)

  • Although canine facial nerve paralysis(FNP) occurs similarly in humans, there is no properly recognized therapy using Western medicine for idiopathic causes. To elucidate therapeutic measures by acupuncture(AP) on canine FNP, we examined the therapeutic effect of injection-AP on the artificially induced canine FNP. Twelve dogs on artificially induced canine FNP were divided into a control group(4 dogs), an experimental dexamethasone-treated group(dexamethasone group, 4 dogs) and an experimental bee venom-treated group(apitoxin group, 4 dogs). Saline (1 ml) was intramuscularly injected into the head muscle after the induction of FNP in the control group. On the other hand, injection-AP with dexamethasone was performed on such acupoints as LI04, LI20, ST02, ST07, TH17, SI18, GB03 and GB34, twice per week after induction of FNP in the dexamethasone group. In addition, injection-AP with $100{\mu}g$ of apitoxin was performed on the same acupoints as the dexamethasone group twice per week after the induction of FNP in the apitoxin group, respectively. The changes of the clinical symptoms of FNP with each treatment during the experimental period were recorded by using clinical scores, respectively. The changes of serum creatine kinase(CK) activities along with each treatment were determined using an autoanalyzer. The significant differences of clinical scores were detected on day 14(p<0.05) in the apitoxin and dexamethasone groups, compared with those in the control group, respectively. However, significant difference was not detected between the apitoxin and dexamethasone groups. Significant differences of serum CK activities were detected on day 7(p<0.05) and day 14(p<0.05) in the dexamethasone and apitoxin groups, compared with those in the control group, respectively. However, significant difference was not detected between the dexamethasone and apitoxin groups. In condition, injection-APs with apitoxin and dexamethasone were all effective for treatment of canine FNP and the therapeutic effect by injection-AP with apitoxin was similar to that of injection-AP with dexamethasone.

An Analysis of the Study Tendency on Rheumatoid Arthritis -Focusing on Domestic Theses for a Degree and Journal Since 2004- (류마토이드 관절염의 연구동향에 대한 소고(小考) -2004년 이후의 국내 학위논문 및 학회지 논문을 중심으로-)

  • Objectives : To research the trend of studies related to rheumatoid arthritis and to indicate the hereafter direction for its study in oriental medicine and its treatment. Methods : I reviewed 75 domestic oriental medical journals, and 50 medical journals about rheumatoid arthritis published after 2004, and comparative analysis was made. And these theses were classified by college, year, field of study, subject. Results : The following are the results of this study. 1. Classified by oriental medical college, Dae-jeon college published the most theses, followed by Dong-guk, Kyung-hee, Se-myung and Dong-shin college. Han-yang college published the most theses among college of medicine. 2. Classified by type of thesis, experimental theses(70 pieces, 94%) showed higher rate than that of clinical theses(4 pieces, 5%) in oriental medical studies. However, in medical studies, clinical theses(34 pieces, 68%) showed higher rate than that of experimental theses(15 pieces, 30%). 3. Analyzed by subject, the most dealt subject in oriental medicine was herb medication, followed by herbal acupuncture, single herb, electroacupuncture, sasang & gene, acupuncture & moxibustion, complex accordingly. The most dealt subject in medical clinical journals was standards of diagnosis & prognosis, followed by medication, gene analysis, pathogenesis, clinical pattern, operative treatment and complication accordingly. 4. In theses related to herb medication, most of the subject was to evaluate anti-inflammatory and immunoregulatory effects of herb medication with geopungseup, jibitong, hwalhyeolgeoer function. The tendency of experimental methods was focusing on understanding anti-inflammatory and immunoregulatory mechanisms through molecular biologic methods by analyzing cytokine and gene. 5. Most of theses related to herbal acupuncture were experimental studies verifying ant-inflammatory and immnoregulatory effects through methods observing change of cytokine and immunoregulatory factors. Regarding remedies for herbal acupuncture, Ulmus davidiana Planch was most preferred, followed by bee venom. 6. In theses related to single herb, Boik-yak was most prefered, followed by Geopungseup-yak and Hwalhyeolgeoer-yak. Regarding methods of research, there were tendency of shifting from methods verifying travail, anti-inflammatory and anti-pyretic effects through a test of behavior, morphometry, serology and temperature measurement of the rectum and the skin into verifying anti-inflammatory and immunoregulatory effects through observing inflammatory cytokine in the joint and cells of spleen. 7. In theses related to electroacupuncture, ST36 and adjuvant were most preferred as acupoints and induced factor. The tendency of experimental methods was turning from verifying mechanism of travail effect to analyzing inflammation and pain inducing factors. 8. Diverse medical clinical studies were published. Subjects such as diagnosis and prognosis, medication, gene analysis, clinical pattern, operational treatment, complication and pathogenesis were published. Especially, studies about standards of early diagnosis, and research on possible parallel medications with methotrexate were actively inquired. 9. Most of theses related to medical experimental studies was about mediators and receptors related to inflammatory induction and osteoclasia mechanism. Also, it was presented blockage of them can be effective on rheumatoid arthritis. Conclusions : The oriental medicine studies have merit in its diversity of treatment, but it clinical studies is lacking compared to experimental studies. Also, more diversity of subjects is necessary. Therefore, complementary measures are needed. Hereafter, oriental medicine research about rheumatoid arthritis needs more clinical research verifying the effectiveness and safety in clinical field. Also, we require oriental medical standard of diagnosis and researches on pathological generation which would make early checkup and prognosis possible.
