clinical experience of cardiovascular surgery -An Analysis of 116 cases- (심장혈관질환의 수술치험)

  • From Aug. 1984 to May 1986, 116 cases of cardiovascular surgery including 85 cases of open heart surgery were performed at Korea Veterans Hospital. There were 108 congenital anomalies and 8 acquired valvular heart diseases. Among 108 congenital malformations 92 operations were done for acyanotic group, and 16 operations for cyanotic group. Thirteen cases of open heart surgery for infants or small children under 10kg of body weight were performed, which occupied 15.3 percent of total open heart surgery done in the same period. Common congenital cardiovascular anomalies were ventricular septal defect [40.7%], patent ductus arteriosus [29.6%], tetralogy of Fallot [12.[%], atrial septal defect [11.1%], and pulmonary stenosis [1.9%] in order of frequency. Valve replacement using lonescu-Shiley or Carpentier-Edwards valve was performed for 8 cases of acquired mitral valve disease, and valve replacement using St. Jude valve was done for a case of patent ductus arteriosus with severe mitral insufficiency. There was no mortality in acyanotic congenital malformations and acquired valve lesions, and 5 cases of mortality in cyanotic congenital malformations. Overall mortality was 4.3 percent for total cases and 5.9 percent for 85 cases of open heart surgery.

Status of Oral Health in Relation to the Acknowledgement of Oral Health Trouble and Oral Health Habits in Recipients of Dental Screening in Hospital (병원 내 치과검진 수혜자의 구강건강인식 및 습관에 따른 구강건강상태)

  • Objectives : The purpose of this study was to analyze the status of oral health in relation to the acknowledgement of oral health trouble and oral health habits. Methods : This study was conducted after IRB was received, on 273 patients who had received health screenings between October-1, 2014 and January-31, 2015 at a general hospital in Busan. Results : Regarding the status of oral health in relation to the acknowledgement of oral health trouble, the rates of dental caries and periodontal disease were higher when the patients knew the reasons for trouble about the oral hygiene. With regard to oral health habits, our results showed that the more frequently the patients ate sugary snacks per day, the probability of experiencing periodontal disease was higher than for the patients who did not eat sugary snacks. Our results also, showed that the occurrence of dental caries is very much dependent upon whether a patient had experienced education on toothbrushing. Conclusions : Patients need to become more aware of their oral health through education. A program that emphasizes the importance of preventive oral health behaviors and the maintenance of correct oral health behavior should be developed.

  • Healthy worker effect (HWE) refers to the consistent tendency for actively employed individuals to have a more favorable mortality experience than the population at large. Although HWE has been well known since the 1970s, only a few studies in occupational epidemiology have attempted to fully define and evaluate HWE. HWE can be separated into effects on the initial hiring into the workforce (healthy worker hire effect) and those on continuing employment (healthy worker survival effect). In this review, we summarize the methods for minimizingor adjusting for the healthy worker effect available in occupational epidemiology. It is noteworthy that healthy worker survival effect appears complicated, considering that employment status plays simultaneous roles as a counfounding variable and intermediate variable, whereas healthy worker hire effect may be adjusted by incorporating health status at baseline into the statistical model. In addition, two retrospective cohort studies for workers in the semiconductor industry and Vietnam veterans in Korea, respectively, were introduced, and their results were explained in terms of healthy worker effect.

  • An 18-year-old female was admitted because of dyspnea at rest. A chest computed tomography (CT) scan and fiberoptic bronchoscopy demonstrated a polypoid tumor in the left main bronchus, 0.5cm distal from the carina. Surgical resection of the tumor was performed, along with. A pathological evaluation and the immunohistochemical findings led to the diagnosis of a glomus tumor, which originated from the bronchus, an area where this type of tumor has rarely been reported.

Influence of Iodinated Magnetic Resonance Contrast Media and Isotope 99mTc on Changes of Computed Tomography Number

  • The purpose of the study was to identify how isotope and magnetic resonance imaging (MRI) contrast media impact on noise to computed tomography (CT) examination. For the study, divide the phantoms to two groups: 1) saline, saline + different kinds of contrast agent without $^{99m}Tc$ administration; 2) $^{99m}Tc$ administration: saline, saline + different kinds of contrast agent with $^{99m}Tc$ administration. CT contrast agent was used for Iopamidol$^{(R)}$ and Dotarem. And MRI contrast agent was used for Primovist$^{(R)}$ and Gadovist$^{(R)}$. To obtain an image, we used CT scanner. With an obtained image, we set the $1cm^2$ region of interest in the middle of bottle to measure the noise and CT number. As a result, there was no difference in CT number before and after inserting $^{99m}Tc$ into all contrast media including Normal Saline. However, when it comes to Noise, there was a difference before and after inserting $^{99m}Tc$ into every contrast media except MRI contrast media such as Primovist$^{(R)}$ and Gadovist$^{(R)}$.

  • Extrapleural hematoma results from blood accumulating between the parietal pleura and the endothoracic fascia, whereas hemothorax shows pooling in the pleural space. Extrapleural hematoma results from an intact parietal pleura that blocks blood from escaping the pleural cavity. Extrapleural fat, a fat layer outside the pleura in the chest wall between the parietal pleura and the endothoracic fascia, is pathognomonic on computed tomography. We diagnosed traumatic extrapleural hematoma and treated it with video-assisted thoracic surgery. We report here on this case along with a review of the literature.

Analysis of Time-Distance and Kinematic Gait Parameters Between Unilateral Trans-Tibial Amputees and Healthy Subjects (하퇴의지 착용자와 정상성인 보행간의 시간-거리 및 운동형상학 변수 분석)

  • 본 연구는 하퇴의지 착용자와 정상 성인간의 시간-거리, 운동형상학 변수를 조사하고 비교하기 위하여 실시하였다. 연구 대상자는 외상으로 인한 하퇴 절단자로서 내골격식 하퇴의지를 착용하고 독립적으로 보행이 가능한 20명과 연령, 신장으로 짝짓기한 대조군(matched control group) 20명이 참여하였다. 보행분석은 Vicon Clinical Manager Software (VCM)를 내장한 PC에 5개의 카메라가 연결되어 있는 Vicon 512 Motion Analysis System (MAS, Oxford Metrics Inc.)을 이용하였다. 하퇴의지 착용군의 단하지 지지시간이 정상 성인군에 비해 유의하게 짧았으며(p<.05), 하퇴의지 착용군에서 슬관절의 선전이 증가되었으며 족관절의 저측굴곡이 감소되었다(p<.05). 하퇴절단자들의 보행개선을 위해서는 하퇴절단자 개인의 보행능력에 알맞은 부품의 선택, 체계적인 보행훈련 및 평가, 보행능력 향상을 위한 근력강화 프로그램 등 체계적인 재활훈련 프로그램이 필요한 것으로 사료된다. 본 연구는 연구대상자의 수가 제한되어 있으므로 연구의 결과를 일반화하기에는 제한점이 있으나, 향후 편측 하퇴절단자의 보행연구에 대한 기초 자료로 사용될 수 있을 것이라고 생각된다.

Temperature in Nerve Conduction and Electromyography (신경전도와 근전도검사에서의 체온)

  • Among the various physiological factors that affect nerve conduction velocity (NCV), temperature is the most important. Because the influence of temperature is the most important source of error. It is known from animal experiments that conduction is eventually completely blocked at low temperatures, the myelinated A fibers being the first affected and the thin fibers of group C the last. Many studies showed that the NCV decreases linearly with lowering temperature within the physiological range. The distal motor latency increased by $0.2msec/^{\circ}C$ drop in temperature between $25^{\circ}C$and $35^{\circ}C$ in the median, ulnar and peroneal nerves. The temperature affect the neuromuscular transmission; The miniature endplate potential (MEPP) and endplate potential (EPP) are increase with increasing temperature. In myasthenia gravis, the reduction in the decremental response is observed following cooling. The lowering temperature make increase the amplitude of sensory compound action potential; make enlarge the surface area of compound muscle action potential with very little increase in amplitude; make diminish the fibrillation potential and increase the myotonia in needle electromyography (EMG). Because of these findings mentioned above, the skin temperature should be routinely monitored and controlled during nerve conduction tests and needle EMG and should be taken into account when interpreting the findings.

A Study on the Personnel Records Management System of the U.S. Army in terms of Veterans' Welfare (보훈복지 관점에서 살펴본 미 육군의 인사기록 관리 체계 연구)

  • This study draws out the implications based on the analysis of the U.S. Army's practices(Army Military Human Resource Record(AMHRR) and interactive Personnel Electronic Records Management System(iPERMS), Archival Information Service) and applies it to our military by paying attention to the importance of military personnel records and the thorough records management that are the foundation for promoting the welfare service projects of those who have committed to the nation. Through these, it is expected to contribute to the smooth promotion of veteran welfare projects and to create a climate that honors the military at the bottom of society in the future.

Relationship between Nurses' Positive Psychological Capital and Adaptation of Job Rotation (간호사의 긍정심리자본과 직무순환 적응의 관계)

  • Purpose: This study was conducted to identify the nurses' positive psychological capital and adaptation of job rotation and to examine their relationship. Methods: This cross-sectional study was conducted from March 23, 2021 to March 31, 2021, the study participants were 160 nurses who worked at a general hospital in Seoul. The collected data were analyzed using the IBM SPSS/WIN 24.0 program with descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficient. Results: The positive psychological capital of nurses scored an average of 3.30±0.43 out of 5. Nurses' adaptation of job rotation scored an average of 3.00±0.35 out of 5. Positive psychological capital and adaptation of job rotation showed a significant positive correlation (r=.70, p<.001). Conclusion: This study shows that the greater level of positive psychological capital was associated with the increased adaptation of job rotation among nurses. It is necessary to develop an practical program to help nurses, who have recently experienced job rotation, adapt to the changed job by improving the positive psychological capital.
