한방치료로 호전된 섬망을 동반한 좌측 중대뇌동맥경색 환자의 치험 1례 (A Case Report of a Patient with a Left Middle Cerebral Artery Infarction and Delirium Treated with Korean Medicine)

  • Objectives: The purpose of this case study is to describe the effect of Korean medicine on a patient with delirium caused by a left middle cerebral artery infarct. Methods: The patient was treated with Sopungbosimdodam-tang, acupuncture, and moxibustion. The Korean version of the Delirium Rating Scale (K-DRS), Neelon and Champagne (NEECHAM) Confusion Scale, and Korean Version of the Mini-Mental State Examination (MMSE-K) were used to evaluate the effect of the treatment. Results: After 50 days of treatment with Sopungbosimdodam-tang, the patient's K-DRS score decreased from 16 to 8. The NEECHAM Confusion Scale also improved from 17 to 23, and the MMSE-K score improved from 1 to 7. Conclusion: This clinical study suggested that Korean medicine could contribute greatly to the treatment of delirium caused by a left middle cerebral artery infarct.

수공예활동을 이용한 그룹 작업치료 중재프로그램이 뇌졸중 환자의 일상생활 수행 능력과 삶의 질 향상에 미치는 효과 (Effects of Stroke Arts and Craft Group Occupational Therapy Intervention Program on Improving the Quality of Life and Activity of Daily Living)

  • 목적 : 본 연구는 수공예활동을 이용한 그룹 작업치료 중재프로그램이 뇌졸중 환자의 일상생활 수행능력과 삶의 질 향상에 미치는 효과를 알아보고자 한다. 연구방법 : 뇌졸중 환자 10명을 대상으로 진행하였으며, 두 집단 사전-사후 설계(Pretest-Posttest control group design)를 사용하였다. 실험은 사전검사 1회, 중재 10회, 사후 검사 1회로 총 12회기로 주 2회 6주간 실시하였다. 인지기능은 한국판 간이 정신상태검사 (Mini-Mental State Examinatin for Korean: MMSE-K)와 일상생활평가로 한글판 수정 바델 지수 (Korean Version of Modified Bathel Index: K-MBI)와 캐나다 작업수행 측정 (Canadian occupational performance measure: COPM)을 사용하였고, 삶의 질 평가는 한국판 세계 보건기구 삶의 질 간편형 척도(The World Health Organization Quality of life- BREF: WHOQOL-BREF)를 중재 전 후에 시행하였다. 기술통계와 비 모수 통계를 이용하여 결과분석을 하였다. 결과 : 실험군과 대조군의 사전 - 사후 결과를 비교한 결과 일상생활 활동 수행능력 부분에서 K-MBI의 점수 증진은 있었으나, 유의미한 차이는 없었다. COPM은 실험군에서만 수행도 전 후에 유의미한 차이가 있었으며, 실험군, 대조군 모두 만족도에 유의미한 차이가 있었다. WHOQOL-BREF에서 실험군에서 실험 전 후 및 집단 내에 유의미한 차이가 있었으며, 집단 간에도 유의미한 차이가 있었다. 결론 : 본 연구 결과를 통하여 뇌졸중 환자에게 수공예활동을 적용한 그룹 작업치료 중재프로그램이 작업수행능력과 삶의 질 증진에 효과가 있음을 확인 할 수 있었다.

두침요법을 포함한 한의 복합치료로 호전된 감각성 실어증 치험 1례 (A Case Report of Wernicke Aphasia by Korean Medicine Treatment, Including Scalp Acupuncture)

  • The purpose of this case study is to report the effectiveness of scalp acupuncture and Cheungsindodam-tang on Wernicke's aphasia in patients with left middle cerebral territory infarction. Scalp acupuncture, herbal medicine (Cheungsindodam-tang), and moxibustion were administered for 66 days to improve the patient's symptoms. The degree of improvement in symptoms was evaluated using the Korean version of the Western Aphasia Battery (K-WAB) and Mini-Mental State Examination-Korea (MMSE-K). Quality of life was evaluated with the Korean version of the Stroke and Aphasia Quality of Life Scale-39 (K-SAQOL-39). After 66 days of treatment, the aphasia quotient score of the K-WAB test increased from 19 to 34.6 after treatment. The K-SAQOL-3 score increased from 2.77 to 3.18, and the MMSE-K score increased from 4 to 21. This case report suggests that Korean medical therapy can be effective in improving the language functions of patients with Wernicke's aphasia.

일개 보건소에서 시행한 독거노인 한방 가정방문 결과보고: 이침치료를 활용한 인지기능 개선을 중심으로 (Results of the Korean Medicine Visiting Care Service for Solitary Elderly in a Public Health Center)

  • Objectives: To conduct and report the results of a public health promotion program in Korean medicine (KM), namely the KM Visiting Care Service for Solitary Elderly, from November 2018 to April 2019. Methods: Six elderly people living in a rural area received the KM visiting care service, twice a week, for four months. This service consisted of acupuncture, auriculotherapy, and supportive counseling to manage their musculoskeletal pain, cognitive impairment, and/or depression. The changes of symptoms were assessed using Numeric Rating Scale (NRS), Korean version of Mini-Mental State Examination for Dementia Screening (MMSE-DS), Korean version of Montreal Cognitive Assessment (MoCA-K), and Geriatric Depression Scale-Short form Korean (GDS-SF-K). Results: Through the 4-months KM visiting care service, the overall subjects' NRS-rated pain decreased slightly. Most showed improvement in MMSE-DS and/or MoCA-K, except one subject who was diagnosed with Alzheimer's disease. Depression assessed by GDS-SF-K showed improvement in a few subjects who were unable to walk independently. Satisfaction assessed through survey was generally high in all subjects. Conclusions: This KM Visiting Care Service for Solitary Elderly may help improve the pain and cognitive function of frail solitary elderly in rural areas. However, the protocol need to be improved to optimize the effect.

수두증을 동반한 뇌경색 환자의 실어증 치료 증례보고 1례 (A Case Report of Treatment for Aphasia in Patients with Cerebral Infarction with Hydrocephalus)

  • The purpose of this case study was to report the effectiveness of aphasia in patients with cerebral infarction with hydrocephalus. A patient diagnosed with a middle cerebral artery infarction and hydrocephalus was treated with Gami-cheongeumjowi-tang, acupuncture, and moxibustion. The degree of improvement of symptoms was evaluated using the Korean version of the Frenchay Aphasia Screening Test (K-FAST) and the Mini-Mental State Examination-Korea (MMSE-K). At the time of admission, the K-FAST score was 7 points and the MMSE-K score was 4 points because no words other than 'Yes' could be spoken. At discharge, the K-FAST score was 22 points and the MMSE-K score was 19 points. A particularly noticeable improvement was found in spoken and written comprehension, reading, and repetition. The study findings indicate that Korean medicine treatment can be used to treat the symptoms of patients with aphasia. The periventricular region may be distorted by hydrocephalus, resulting in Broca's aphasia in a cerebral infarction of the middle cerebral artery inferior division.

  • Background : Vascular dementia occurs mainly due to cerebral vascular disease. So we performed this clinical study to investigate the incidence and characteristics of vascular dementia in stroke patients. Methods : This study was performed on the patients hospitalized from April 1, 1998 to August 31, 1998 at the department of circulatory internal medicine, hospital of Oriental medicine, Kyung-Hee University, and diagnosed cerebral infarction or hemorrhage by Brain CT or MRI. we devided the patients into two groups; vascular dementia group and non dementia group according to MMSE-K(Mini Mental State Examination Korean version), Hasegawa dementia scale. Patients were diagnosed dementia using DSM-IV. We compared general characteristics, stoke types and laboratory findings between the two groups and investigated the correlationship between MMSE-K and Hasegawa dementia scale. Results : Results showed that the incidence of vascular dementia was about 27.8% in stroke patients. The greater number of subjects with dementia were women in the lower educational classes and had lower MBI(Moderfied Bathel Index) scores. Vascular dementia were more common in patients with large brain lesion size($>20cm^3$). There was a positive correlationship between Hasegawa scores and MMSE-K.

  • The purpose of this study was to evaluate the effect of Korean medicine in a patient with cognitive impairment, emotional disturbance, and sleep disturbance due to a diffuse axonal injury associated with a traumatic brain injury. The patient was treated with herbal medicine and acupuncture. The treatment effects were evaluated using the Korean version of the Mini-Mental State Examination (MMSE-K) and the Global Detraction Scale (GDS), and by observing clinical symptoms. Improvements in the total scores of MMSE-K and GDS were observed after the Korean medicine treatments; the MMSE-K score increased from 13 to 23 and the GDS score decreased from 5 to 4. The emotional and sleep disturbances were also reduced. These case report findings suggest that Korean medicine may be effective for treating symptoms of diffuse axonal injury in patients with traumatic brain injury.

음악 기반 슬링운동 프로그램이 치매환자의 인지, 보행 및 기능적 운동성에 미치는 효과 (Effects of Music-based Sling Exercise Program on Cognition, Walking, and Functional Mobility in Elderly with Dementia: Single-blinded, Randomized Controlled Trial)

  • PURPOSE: This examined the effects of a sling exercise based on music on the cognition, physical performance of patients with dementia. METHODS: Thirty subjects with dementia volunteered to participate in this study. All subjects were allocated randomly to either the experimental group or control group, with 15 subjects in each group. All subjects underwent the exercise program for an average of 60 minutes per day for 16 weeks. The experimental group performed sling exercise based on music, and the control group performed the general exercise program. Assessments were made using the Korean version of mini-mental state examination (MMSE-K), 10 m walk test (10MWT), Tinetti mobility test (TMT), and Katz's Index of Independence in activity daily living (KIIADL) to detect changes in the cognitive level and physical performance before and after the 16-week training period. A paired t-test was conducted to compare the within-group change before and after the intervention. An independent t-test was performed to compare the between-group difference. The statistical significance level was set to α=.05 for all variables. RESULTS: The experimental group showed significant within-group changes in the MMSE-K, 10MWT, TMT, and KIIADL (p<.05). The control group showed a significant change in only the KIIADL (p<.05). A significant difference was observed between the experimental group and the control group regarding the change in MMSE-K and KIIADL after the interventions (p<.05). CONCLUSION: A music-based sling exercise program effectively improves cognition, physical performance, and ADL in patients with dementia. Further studies with a wider range of subjects and scientific equipment will be needed to strengthen the results of this study.

여성 노인의 식이요인과 인지기능 감퇴의 관련성 (Association of Dietary Factors with Cognitive Impairment in Older Women)

  • Objective : To examine the association between dietary factors and cognitive impairment in older Korean women living in the community. Methods : Wave 2 data, from the Suwon Longitudinal Aging Study (SLAS), of 365 women aged 65 years or over, were used. The Korean version of the 30-point Mini-Mental State Examination (MMSE-K) was used to assess cognitive impairment (score$\geq$19). Dietary habits and frequencies of food group consumption were also examined. Results : A total of 67 women (18.4%) were found to be cognitively impaired. In bivariate analyses, nondietary factors, such as age, marital status, education, income, self-rated health, depression, emotional support, social activity, exercise, and dietary factors, such as self-rated nutritional status, frequency of beans and bean products and milk and dairy products consumption were associated with cognitive impairment. In the multivariate analysis, a higher frequency ($\geq$1 vs. <1 time/day) of beans and bean products (OR=0.48, 95% CI: 0.23-0.99) and milk and dairy products (OR=0.25, 95% CI: 0.10-0.61) consumption was inversely associated with cognitive impairment, after adjustment for non-dietary factors. Conclusion : These results suggest that dietary factors may play a significant role in cognitive impairment of older Korean women.

노인 고혈압 환자의 인지기능, 우울, 사회적 지지 및 자가간호에 관한 연구 (Cognitive Function, Depression, Social Support, and Self-Care in Elderly with Hypertension)

  • Purpose: The purpose of this study was to examine the relationship among cognitive function, depression, social support, and self-care in elderly with hypertension. Methods: The subjects were 132 elderly with hypertension living in Seoul, Korea. Data were collected through face-to-face interviews using the Korean version of Mini-Mental State Examination(MMSE-K), Short form geriatric depression scale, social support questionnaire 6, and hypertension self-care scale. Results: Thirty-four percent of the subjects had questionable dementia and forty-two percent of the subjects were depressed. Means for social support were 2.40 for network size and 4.07 for satisfaction. The mean score of hypertension self-care was 60.34, indicating that the subjects took care of themselves moderately well. Cognitive function was negatively related to depression. Social support network and satisfaction were negatively related to depression. Self-care was negatively related to social support network. Conclusion: Programs are needed for elderly with hypertension to improve their cognitive function, depression, and social support. Also further studies are needed to confirm the factors related to self-care in the elderly with hypertension.
