정신치료의 신경생물학적 기전 (Neurobiological Mechanism of Psychotherapy)

  • Polarisation of biological and psychosocial aspects of psychiatry is nowadays main stream. Current knowledges of the interaction between biology and psychology make it possible to consider a truly integrative approach of the two aspects. Research findings suggest that the neuronal plasticity is the key mechanism to answer how the mental function work to an environmental stimuli and how the psychotherapeutic approach work on the brain. Advances in neuroscience research have led to a more sophisticated understanding of how psychotherapy may affect brain function. Even though there have been a tremendous efforts to find out the neurobiological mechanism of mental function, the answer is at best premature. In this article, research findings about of neuronal plasticity, implicit memory, animal studies which were associated with psychotherapy and psychological aspects were reviewed.

오심과 구토를 호소하는 한 여자환자에 대한 단기정신치료 증례보고 (A Case Report on Short-term Psychotherapy of the Woman Patient with Nausea and Vomiting)

  • The author reported a case of short-term dynamic psychotherapy in which relatively good therapeutic results were obtained. And I reviewed several therapeutic factors thought to be important in shot-term dynamic psychotherapy from various literatures. In this case, the frequency of session was limited due to her current reality situations. She had suffered from some adjustmental problems and marital conflict before the onset of the illness. The author could grasp the cental theme of her problems early in treatment period and discussed over it including it's influence on current situation and early childhood with th patient. We also discussed about the relationship between the past and current reality so that she could realize her real problem. At the same time, therapeutic effect was enhanced by family therapy and temporary use of medication.

M&L 심리치료가 불안장애를 가진 취약계층 청소년의 건강한 성장에 미치는 영향 - 단일증례의 양적, 질적 분석 (The Impact of M&L Psychotherapy on the Growth and Development of a Socially Disadvantaged Adolescent with Anxiety Disorders - Quantitative and Qualitative Analysis of a Single Case)

  • Objectives: This study investigates the impact of M&L psychotherapy on an adolescent with anxiety disorders transitioning into adulthood, using a retrospective mixed-method approach. Methods: An adolescent with Diagnostic Statistical Manual of Mental Disorders (DSM)-5-diagnosed anxiety disorders underwent combined treatment with M&L psychotherapy and complex Korean medicine therapy over a period of approximately two years. Patient records tracked the diagnosis, treatments, and progress. In-depth interviews were also conducted. The study used a convergent parallel mixed-method approach, integrating quantitative analysis from psychological assessments, including the State-Trait Anxiety Inventory (STAI-X), the Core Seven Emotions Inventory-short form (CSEI-s), and the Minnesota Multiphasic Personality Inventory-2 (MMPI-2), with qualitative analyses of traditional Korean medicine counseling session notes, interviews, and other qualitative psychological assessments. Results: State anxiety decreased from 40 to 36, and trait anxiety decreased from 42 to 34. MMPI-2 L scale scores decreased slightly to 64T, whereas S scale scores increased to 62T, and FRS scores decreased to 70T from an initial score of 78T. Qualitative analysis revealed an increase in the MMPI-2 Es scale, identified as "subjectivity." Joy (喜) emerged as the highest emotional profile score in the CSEI-S. Scores for anger (怒), thought (思), fear (恐), and fright (驚) decreased, indicating improved negative emotions. Qualitatively, expressions of determination to "move forward" and "strengthen the mind" were noted, suggesting enhanced "lower danjeon (下丹田)." Conclusions: Integrating traditional Korean medicine treatment with M&L psychotherapy for a vulnerable adolescent with anxiety disorders demonstrated efficacy. Further research is warranted to substantiate the effectiveness of M&L psychotherapy and advocate for its wider adoption in mainstream practice.

해리성 기억상실과 전환장애를 동반한 환자의 증례 -정신요법을 사용하여- (A Case of Dissociative amnesia with Conversion disorder -By using psychotherapy-)

  • Dissociative amnesia is a psychiatric disorder charicterized by a sudden loss of memory, but which has no organic disease or explanation, It usually occurs after severe psychosocial stress or traumatic experience. As for medical treatment, medicine, psychotherapy and hypnosis are used. This case describes a 35-years-old woman who developed amnesia and depression following severe troublesome events. So she suited without expression and appealed against headache, dizziness, chest discomfort and weakness. Psychoanalytic psychotherapy was enforced for three days with hypnosis. She expressed the felling over the incident which she dose not like, and canceled in the past. She became reconciled with the family and left the husband by the heart. Then she recovered memory completely and body condition also improved. Recovery and growth of self were helped using supportive expression in order to prevent a recurrence. The prognosis is good with complete recovery, and there was no relapse.

  • In oriental medicine, Psychotherapy has been regarded as a part of physical therapy that seemed inseparable from psychological therapy from old times. Oriental Medical Psychotherapies are like following: Ii-Gyeung-Byun-Qi therapy(移精變氣療法), Oh- Ji-Sang-Seung therapy(五志相勝療法), Kyeongja- pyeongji therapy(驚者平之療法), Jieongoron therapy(至言高論療法) and so on. The special feature of these Psychotherapies is the unification of body and mind. For them, various oriental therapies have been under progress. Especially, Many Oriental Medical Psychotherapist insisted that pain of mind evoke pain of body, or vice versa. We observed Oriental Medical Psychotherapies to treat married couple and had a good result. Husband had alcoholism and wife had functional headache. Especially, wife wanted divorce. The problems of divorce and illness in married couple were resolved from Oriental Medical Psychotherapies.

사상의학(四象醫學)에서 성정(性情)의 중절(中節)과 심리치료(心理治療) (Control and Moderation of Natural Human Emotions through Sasang Constitutional Medicine and Psychotherapy)

  • Objectives: Sasang Constitutional Medicine(SCM) is a Traditional Korean Medicine based on the natural human emotions of joy(喜), anger(怒), sorrow(哀), and pleasure(樂). Therefore, in this paper I would like to construct SCM as a Mind-Body Medicine by studying the relation between SCM and psychotherapy focusing on control of emotions. Methods : I have researched the psychological characteristics of the four human physical constitutions, as described by SCM, and psychotherapy with an emphasis on the natural human emotions of joy, anger, sorrow, and pleasure as discussed in the works of Lee Je-ma(李濟馬, 1837-1900). Results and Conclusions : The joy, anger, sorrow, and pleasure in SCM do not simply refer to human emotions. They have an inherent nature(性), emotion(情) and Qi(氣). They are tied to the psychological characteristics of the four human physical constitutions and the relationship between body and mind. The turning point in the transformation from Sasang philosophy to Sasang therapy lies in these emotions. Sasang Constitutional Medicine, which focuses on 'mind matter' and diagnosing illness, emphasizes medicine and food(藥餌), along with health maintenance(調養). In particular, health maintenance requires the following mental practices. First of all, through countinuous awarness(恒戒) and self-reflection(自反), one needs to practice moderation(中) and restraint(節) of the emotions of joy, anger, sorrow, and pleasure. Therefore, 'inward reflection while sitting(坐思)' is necessary. In SCM, this is very similar to meditation in that it enhances psychological self-control through mental training or practice.

  • Oriental Medicine Based Psychotherapy is consisted of 8 sessions which are available for various kinds of patients(mood disorders, somatoform disorders, anxiety disorders, Hwabyung, adjustment disorders etc.). Through 8 sessions, patients were educated on the relationship of mind and body which is the basic concept of oriental medicine and also received several training such as Progressive Muscle Relaxation, Autogenic Training, Breathing Training, Meditation, Qi-training. After these several training they get to think of their problems and inner conflict with people and get some insight about themselves and others, it can help handling their problems. This case report is about the patient with lots of somatic complains through the whole body, who got better after treatment by oriental medicine based psychotherapy added on herbal medicine and acupuncture.

  • After we looked back on the process western psychotherapy, and consider current meaning and necessity of buddhism. we concluded as follows 1. We can apply buddhism to psychotherapy in terms of oriental medicine. Especially buddhism with long history in the orient has similarity with oriental medicine and common cultural background. therefore it will be useful especially to koreans 2. Whereas buddhism's object is Nirvana, that of medicine is to return to the above view. to apply various methodologies of buddhism to psychology of oriental medicine you must select useful parts and drop others 3. Whereas buddhism centers on human mind, oriental medicine wholly describes body and mind without separation, buddhism put stress on inside of consciousness and oriental medicine put stress on revealing process 4. It is difficult to combine buddhism to oriental medicine theoretically. In combining it is easier to accept buddhism practically rather than theoretically 5. To apply buddhism clinically we have to modify buddhism to modern societies and to research and discussion to combine it to the system of oriental medicine

가정폭력 피해여성들의 사건충격과 정서 그리고 삶의 질에 대한 M&L 트라우마 심리치료 프로그램의 효과에 대한 임상연구 (The Effects of M&L Trauma Psychotherapy on Impact of Events, Affection, and Quality of Life among Female Vicims of Family Violence)

  • Objectives: The purpose of this study is to investigate the effects of the M&L Trauma Psychotherapy Program on: Impact of Event Scale - Revised (IES-R-K); Euro QOL-5 Dimension (EQ5D); Five-facet Mindfulness Questionnaire (FFMQ); and psychological tests. Methods: Eight middle-aged women subjected to domestic violence participated in the two-day M&L Trauma Psychotherapy Program. The Program was executed 5 times 2 days for about 3 hours. We evaluated IES-R-K, EQ5D, FFMQ, Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), State-Trait Anger Expression inventory (STAXI) and SUDS before the Program and for four weeks afterwards, to analyze the effects of the M&L Trauma Psychotherapy Program. Results: The scores of IES-R-K, BDI, STAI-S, STAI-T, Hwa ST and Hwa CT decreased significantly (p<0.05). EQ5D scores increased significantly (p<0.05) and overall physical condition scale (VAS) increased. In the FFMQ scores, all five facet scores increased slightly. Scores of SUDS decreased significantly right after the program, and remained decreased four weeks later, rather than before the Program (p<0.05). Conclusions: The results suggested that the M&L Trauma Psychotherapy Program improved post-traumatic stress, quality of life and mindfulness skills, and had positive responses to psychological problems - depression, anxiety, anger and distress.

한의정신치료적 접근을 통한 만성통증 환자의 M&L 심리치료 활용 1례 보고 (A Case Report on the Use of M&L Psychotherapy for Chronic Pain Patients through a Traditional Korean Psychiatric Therapeutic Approach)

  • Objectives: To report effects of M&L psychotherapy combined with Korean medical treatments on a patient with chronic low back pain. Methods: We treated a patient with chronic low back pain using M&L psychotherapy alongside a combination of Korean medical treatments, which included acupuncture, pharmacopuncture, cupping, and herbal medicine. The patient was diagnosed based on the ICD-11 diagnostic criteria for chronic primary pain. To evaluate the patient's progress, we used the Numerical Rating Scale (NRS), Oswestry Disability Index (ODI), Visual Analogue Scale (VAS), Beck's Depression Inventory (BDI), and Beck's Anxiety Inventory (BAI). Results: After three weeks of inpatient treatment and two months of outpatient treatment, the patient showed significant improvement in both pain and psychological symptoms. The NRS score decreased from 7 to 2. The BDI score decreased from 54 to 11 and the BAI score decreased from 33 to 9. Conclusions: This study demonstrates that M&L psychotherapy combined with Korean medical treatments can significantly reduce chronic low back pain and improve psychological symptoms such as depression and anxiety. The patient's notable improvements in NRS, BDI, and BAI scores suggest that an integrated treatment approach addressing both physical and mental health aspects is effective for managing chronic pain. These findings support potential benefits of holistic and multidisciplinary strategies for treating patients with chronic pain.
