A New Reporter Vector System Based on Flow-Cytometry to Detect Promoter Activity

  • In this study, we report the development of a new dual reporter vector system for the analysis of promoter activity. This system employs green fluorescence emitting protein, EGFP, as a reporter, and uses red fluorescence emitting protein, DsRed, as a transfection control in a single vector. The expression of those two proteins can be readily detected via flow cytometry in a single analysis, with no need for any further manipulation after transfection. As this system allows for the simultaneous detection of both the control and reporter proteins in the same cells, only transfected cells which express the control protein, DsRed, can be subjected to promoter activity analysis, via the gating out of all un-transfected cells. This results in a dramatic increase in the promoter activity detection sensitivity. This novel reporter vector system should prove to be a simple and efficient method for the analysis of promoter activity.

A Survey on the Awareness of Radiation-related Workers and Radiation Workers in the Medical Institutions According to the Dual System (의료기관의 방사선사 중 방사선 관계종사자와 방사선 작업종사자의 이원화 체계에 따른 인식도 조사)

  • Radiologic technologists working at the second and third medical institutions are classified as radiation-related workers and radiation workers according to their working departments, and are subject to double regulation by the Ministry of Health and Welfare and the Nuclear Safety Commission. We will try to understand the system of dualization and to understand the investigation of recognition. The dualized system of radiation-related workers and radiation workers includes the difference in name and terminology, the effective dose limit, the maintenance education and training of radiologic technologists, the period of medical examination, the radiation zone, dose of the woman whose pregnancy is confirmed in radiologic technologists, the qualification criteria of the safety officer, and the period of the regular inspection of the radiological equipment. In the questionnaire survey on the dualization system, there were various items showing significant differences between the radiation-related workers and radiation workers Overall, the radiation workers were more aware of the radiation workers' education and related terms than the radiation-related workers.

  • In order to investigate the efficacy of combined preparations of crude drug, first, 'Kamisamhwang-Tang' were examined for pharmacological effect. Crude drug preparations were extracted successively with $H_2O$ and MeOH, and the extracts were developed on silicagel G plate were examined using the Dual-Wavelength TLC Scanner CS-910(Schimadzu) in ultra violet region. To estimate the clinical usefulness of 'Kamisamhwang-Tang' in heart disease (Symbyung syndrome), the above extracts were made an experiment on effects of the anticonvulsion, antipyretic, analgesic and sedative in mice, rats, and rabbits.

  • We present a patient with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), which was diagnosed and corrected in her 60 s. The patient is the oldest documented survivor of ALCAPA who underwent a surgical repair. ALCAPA should be corrected surgically to restore the dual coronary system at any age and this case shows that the surgical procedure may be performed safely even in an elderly patient.

  • Purpose : Large exon deletions in the DMD gene are found in about 60% of DMD/BMD patients. Multiplex PCR has been employed to detect the deletion mutation, which frequently generates noise PCR products due to the presence of multiple primers in a single reaction as well as the stringency of PCR conditions. This often leads to a false-negative or false-positive result. To address this problematic issue, we introduced the dual primer oligonucleotide (DPO) system. DPO contains two separate priming regions joined by a polydeoxyinosine linker that results in high PCR specificity even under suboptimal PCR conditions. Methods : We tested 50 healthy male controls, 50 patients with deletion mutation as deletion-positive patient controls, and 20 patients with no deletions as deletion-negative patient controls using DPO-multiplex PCR. Both the presence and extent of deletion were verified by simplex PCR spanning the promoter region (PM) and 18 exons including exons 3, 4, 6, 8, 12, 13, 17, 19, 43-48, 50-52, and 60 in all 120 controls. Results : DPO-multiplex PCR showed 100% sensitivity and specificity for the detection a deletion. However, it showed 97.1% sensitivity and 100% specificity for determining the extent of deletions. Conclusion : The DPO-multiplex PCR method is a useful molecular test to detect large deletions of DMD for the diagnosis of patients with DMD/BMD because it is easy to perform, fast, and cost-effective and has excellent sensitivity and specificity.

An Efficient Transmissibility-design Technique for Pneumatic Vibration Isolator (지반진동절연을 위한 공압제진대의 전달률 설계기법)

  • Pneumatic vibration isolator has a wide application for ground-vibration isolation of vibration-sensitive equipments. Recent advances In precision machine tools and instruments such as nano-technology or medical devices require a better isolation performance, which can be efficiently done by precise modeling- and design- of the isolation system. This paper will discuss an efficient transmissibility design method for pneumatic vibration isolator by employing the complex stiffness model of dual-chamber pneumatic spring developed in our previous research. Three design parameters of volume ratio between the two pneumatic chambers, the geometry of capillary tube connecting the two pneumatic chambers and finally the stiffness of diaphragm necessarily employed for prevention of air leakage were found to be important factors in transmissibility design. Based on design technique that maximizes damping of dual-chamber pneumatic spring, trade-off among the resonance frequency of transmissibility, peak transmissibility and transmissibility in high frequency range was found, which was not ever stated in previous researches. Furthermore this paper will discuss about negative role of diaphragm in transmissibility design. Then the design method proposed in this paper will be illustrated through experiment at measurements.

A Study on Wearing and Manufacturing of Surgical Gowns (수술가운의 생산 및 착용현황에 관한 연구)

  • Surgical operating gowns are worn by doctors and nurses in operating theater to address dual function of preventing transfer microorganisms like bacteria and body fluids from the operating staffs to patients. and also from patients to staff. They must allow necessary mobility without rubbing and must resist tearing. These gown must be designed to fit a diversity of body shapes and sizes with a limited range of sizes as hospitals will stock limited quantities. Also they must proper in various operating situations. In order to suggest improving design features of surgical gowns, it has been done to analyze current situation of medical uniform manufacturing and wearing. Surgical gown were made with plain fabric of cotton 100% and had a little size system without sexual division. Many doctors were unsatisfied with surgical gown size and wearing comfort. Surgical operating gowns need continuos research for new material and design as surgical situation, and new sizing system.

Design and Evaluation of Blending Algorithm for Rate Adaptive Pace: Simulation Study (심박수 적응형 심박 조율 알고리즘 설계 및 평가: 시뮬레이션 연구)

  • In this study, we designed a blending algorithm for rate adaptive pacing for cardiac pacemaker. Generally, rate adaptive pacing (RAP) is applied to patients whose heart rate does not rise during exercise for chronotropic incompetence (CI) patient. It is very important to develop an algorithm for RAP that can be properly applied to CI patients. In order to design an RAP algorithm we used dual sensors. Firstly, we designed a bio-signal measurement system based on the dual sensors, which are accelerometer and respiratory system. Secondly, we conducted treadmill test for the simulation experiment while using 3-lead ECG as reference. Finally, we designed a blending algorithm based on activation state of the dual sensors. The proposed blending algorithm was subdivided into three sections based on the accelerometer signal, which are rapidly increased section (W1), hardly changed section (W2), and decreased section (W3). Each weight is set aside for each section. To evaluate this algorithm, ten healthy adult males were participated. The correlation and Root Mean Square Error between the proposed algorithm and the reference were compared, and shown to be r=0.88 and 2.82 bpm, respectively. These results show that the proposed blending algorithm of dual sensors enables proper tracking of the heart rate during exercise. Also, it shows the possibility that the proposed blending algorithm can be applied to improve quality of life of the chronotropic incompetence patient.

A Study on the Punishment of Unlicensed Medical Practice -Focusing on Collaboration between Medical and Non-medical Personnel- (무면허 의료행위 처벌에 관한 고찰 -의료인과 비의료인의 협업관계를 중심으로-)

  • Today, the medical system is changing into a comprehensive health care system in which collaborative relationships between medical professionals and non-medical personnels in neighboring occupational areas. The current medical act brands such "collaboration" as unlicensed medical practice, and punishes non-medical personnel who acted in the risk management of doctors as well as doctors collaborated with non-medical personnel as unlicensed medical practice. In order to narrow the gap between the legal system that regulates unlicensed medical practices and the medical reality, it is necessary to overcome the structural limitations of dualistic, nationalistic, and identity-oriented regulation of unlicensed medical practices. The legal interests of unlicensed medical practice have a dual nature as a personal legal interest of "human life and body" as well as a national legal interest of "maintenance and protection of the nation's medical license system", and it should be noted that the criteria for judging the legal interests protected by the regulations of criminal punishment should be found in "personal legal interest theory." In addition, when determining which behavior is a medical practice and evaluating its risk, the dimension of behavior and measures should be considered in a fair manner without being biased against the subject (identity) of the action. In other words, judging unlicensed medical practice should depend on whether the risk of side effects that may result from the act is reasonably managed. Considering the prospect of therapeutic dialogue between medical professionals and patients, it would be desirable for medical law policies to move in a way that does not fundamentally block the possibility of collaboration among pluralistic medical personalities.

Flow diversion of a middle cerebral artery pseudoaneurysm secondary to a gunshot wound: A case report

  • Pseudoaneurysms are rare but devastating complications of penetrating head traumas. They require rapid surgical or endovascular intervention due to their high risk of rupture; however, complex presentations may limit treatment options. Our objective is to report a case of severe vasospasm, flow diversion, and in-stent stenosis complicating the treatment of a middle cerebral artery pseudoaneurysm following a gunshot wound. A 33-year-old woman presented with multiple calvarial and bullet fragments within the right frontotemporal lobes and a large right frontotemporal intraparenchymal hemorrhage with significant cerebral edema. She underwent an emergent right hemicraniectomy for decompression, removal of bullet fragments, and evacuation of hemorrhage. Once stable enough for diagnostic cerebral angiography, she was found to have an M1 pseudoaneurysm with severe vasospasm that precluded endovascular treatment until the vasospasm resolved. The pseudoaneurysm was treated with flow diversion and in-stent stenosis was found at 4-month follow-up angiography that resolved by 8 months post-embolization. We report the successful flow diversion of an middle cerebral artery (MCA) pseudoaneurysm complicated by severe vasospasm and later in-stent stenosis. The presence of asymptomatic stenosis is believed to be reversible intimal hyperplasia and a normal aspect of endothelial healing. We suggest careful observation and dual-antiplatelet therapy as a justified approach.
