Relationship between Osteoporosis and Cardiovascular Risk Factors according to Sasang Constitution in Rural Wonju Cohort (원주농촌코호트에서 사상체질별 심혈관위험인자와 골다공증의 관계)

  • Objectives The purpose of this study was to investigate the relationship between osteoporosis and cardiovascular risk factors according to Sasang constitution. Methods A total of 1773 participants, over 40 years old, were examined as part of a community-based cohort in Wonju city in Gangwon province of South Korea, from June 2006 to August 2009. We measured bone mineral density by Achilles ultrasonometer, serum levels of lipids, fasting blood sugar, insulin and other cardiovascular risk factors like blood pressure and waist circumference. Constitution was verified by a Sasang constitution specialist using the results of PSSC(Phonetic System for Sasang Constitution), facial photos and a simplified Sasang constitutional questionnaire. Results The prevalence rate was 11.2% in total participants, and 10.6% in Soyangin, 10.4% in Taeeumin and 13.9% in Soeumin. In general characteristics, educational and economic property and exercise were concerned with osteoporosis. Systolic blood pressure, pulse rate and total cholesterol were significantly high in osteoporosis group. Age and menopause were the key risk factors for osteoporosis. There was a significantly high prevalence in Soeumin for osteoporosis in men alone. Low physical activity was a major risk factor for osteoporosis. Waist circumference and hip circumference had significantly high odds ratio and weight had significantly low odds ratio. By Sasang constitution, Soyangin has the negative correlation with free fatty acid, Taeeumin has the negative correlation with waist circumference, pulse rate, systolic blood pressure, total cholesterol and low density lipoprotein and Soeumin has the positive correlation with body fat amount. Conclusions Regimens on osteoporosis should be considered according to Sasang constitution. Cardiovascular diseases should be considered according to Sasang constitution. Soeumin should be cautious of osteoporosis and gain weights and reasonable amount of fat food. Soyangin had better do exercise lower body and eat little food containing triglyceride. Taeeumin had better lose weights and eat little cholesterol food.

The study on Mibyeong and Quality of life according to Sasang constitution (사상체질(四象體質)에 따른 미병(未病)과 삶의 질에 관한 연구)

  • Objectives The purpose of this study was to investigate of Quality of life and Mibyeong index of abnormal symptoms by Sasang constitution. Methods A total of 1,100 people were recruited into this study and were collected by Gallup Korea. Sasang constitutions were diagnosed based on KS-15 (Korean sasang constitutional diagnostic questionnaire) Questionnaire which comprised body type, personality and symptom. In addition, SF-12 (Shot form-12) questionnaire was used to evaluate the quality of life status. Mibyeong status was categorized through Mibyeong tool. Data were analyzed through one-way ANOVA to investigate the difference between quality of life and Mibyeong index on Sasang constitution. Results The PCS (physical component scale) which represents the physical health index and MCS (mental component scale) which represents mental health index showed significant differences among Sasang constitution (p<.001). In addition, The Mibyeong Index (p<.001) which represents abnormal symptom type showed significant difference for Sasang constitution. As for the Mibyeong, only pain (p=0.047) and fatigue (p=0.021) had changes for the Sasang constitution. Conclusions Quality of life and Mibyeong index vary significantly according to Sasang constitution. This results suggest the analysis of Mibyeong index by Sasang constitution could provide the setting of direction to promote public health depending on Sasang constitution.

  • Sasang medicine is peculiar medicine that constitution of a human classify four types and differ treatment method by physical constitution. In this way the most important thing is very difficult problem that classification of Sasang constitution and discriminate correctly. Therefore, in this paper targets diagnosis medical appliances development of hybrid form that can behave constitution classification and sees among for this paper to propose about method to grasp characteristic that is morphology about eye, nose, ear and mouth be based on appearance and manner of speaking. In this paper, classified and verified this for Sasang constitution through the QSCC II program at 1 step and present method that more exactly and conveniently analyzing measure each physical constitution feature by survey about eye, nose, ear and mouth at 2 steps. Also, extraction and analyze and verified main area of physical constitution classification based on front face and side face at 3 steps. Such propose method to extraction the principal face region based on face color from front face and side face for correct physical constitution classification diagnosis appliance development through experiment consideration and verification process.

  • In the field of the theory about constitution(體質), the theory of 'Yun Qi(運氣) has close relations hip with the formation of theory of constitution. It is seen in the chapter 'Osangjungdae-ron("五常政大論")' that the theory of influenced the theory of constitution and talent by the 'Concept of Evaporation(氣化)'. In the chapter 'Yin-Yang twenty five men types' of the book the 'Ling Shu("靈樞")' the twenty five constitution types by the theory of 'Yan Qi is stated. And in the book 'Nei Ching("內經")' the constitution types of five elements' motion is described. In the theory of the 'five elements' motion and six kinds of natural factors' had such a problem that the individuals having the same born year and Qi had the same prescriptions irrespective of each other's constitution. So for the resolution of this problem the constitution theory of 'Yun Qi' had been formed by the mediation of the 'MyengRiHak(命理學)' and the theory of 'Yun Qi' in the book the 'OUnYukKiHanYeHakBoGam("五運六氣韓醫學寶鑑")'. But following problems should be resolved in chrono-inedieine. Those are the difference of men who has born in some year-day-time, each 'Yun Qi's brea king point, local time, summer time, living circumstances, twins, the concept of 'Zhung Won(中元)', etc.

Analytic Study for Syndrome-differentiation and Sasang-constitution in 72 Adults with Chronic Fatigue (만성 피로를 주증으로 하는 성인 72명의 변증과 체질별 분석 연구)

  • Objective : This study was conducted to find the relationship between syndrome-differentiation and Sasang-constitution in chronic fatigue patients. Methods : The subjects were 72 adults with chronic fatigue who visited Dunsan Oriental Hospital of Daejeon University between March 2007 and April 2007. Their main complaint was fatigue for longer than 6 months and they did not have any physical or mental problems. We measured their fatigue degree by using Chalder fatigue scale and health habits. To evaluate relationship between syndrome-differentiation with sasang-constitution, The patients were divided into four syndrome-differentiations, such as liver and kidney asthenia of Yin, spleen and kidney deficiency of Yang, deficiency of lung and spleen qi, and deficiency of heart and spleen blood. We also diagnosed Sasang-constitution by using Questionnaire for Sasang Constitution Classification II (QSCCII). Results : Among the 72 patients, 41.4%, 34.5% and 24.1% of belonged in Soyangin, Soeumin and Taeumin respectively. Interestingly, we found a meaningful correlation between syndrome-differentiation and physiological function by Sasang-constitution. Conclusion : The result may help Oriental medicine understanding and treatment of chronic fatigue-related diseases and patients.

Factors Affecting Mental Health among College Students - Sassang Constitution and Ego State centered Approach- (대학생의 정신건강에 영향을 주는 요인 - 사상체질과 자아상태를 중심으로 -)

  • Purpose: The purpose of this study was to address differences between mental health according to sasang constitution and ego state among college students. Methods: Data for this cross-sectional study were collected by administration of questionnaires eliciting Woo's ego state scale, QSCC II for the sasang constitution classification, and SCL-90-R for mental health to 393 college students. Analysis was performed using IBM SPSS (version 19.0). Results: The Free Child ego and Adapted Child ego differed significantly among sasang types. The ego-gram pattern of So-eum type exhibited the N pattern (Nurturing Parent (NP)>Adapted Child (AC)>Adult (A)>Free Child (FC)>Critical Parent (CP), AC-high type), however, the ego-gram pattern of other constitution types showed the M pattern (NP>FC>A>AC>CP). No statistically significant differences in mental health were observed among sasang types, however, among ego states, AC and CP showed negative correlation with mental health status. Between So-eum type and So-yang type, AC was the factor predicting mental health. Between Tae-eum type and undefined type, AC and CP were factors predicting mental health. Conclusion: These findings suggest that sasang constitution could be an important factor in understanding the ego state and mental health status. We conclude that sasang constitution should be considered in interpretation of mental health status presentation in clients.

  • Objectives : The purpose of this study is to report the relationship between iridological constitution and interleukin 1 beta (IL-1 $\beta$) gene polymorphism. Methods : Iris constitution were diagnosed by automatic Iris analysis system, Bexel Irina(Korea). The blood was stored at - $20^{\circ}$... until it was ready to be extracted. The genomic DNA was extracted by inorganic procedure. The concentration of DNA was estimated by absorbance at 260 nm. The interleukin-1 beta (IL-1 $\beta$) gene polymorphism was detected by PCR amplification. Results & Conclusions : The author classified 166 individuals according to Iris constitution, and determined IL-1 $\beta$ genotype. The frequencies of Iris constitutions as follows : neurogenic type, 41 (24.7%); abdominal connective tissue weakness type, 53(31.9%); cardio-renal connective tissue weakness type, 50 (30.1%); the others type, 22 (13.3%). Especially, the frequency of abdominal connective tissue weakness type was significantly higher in err genotype than in the remaining constitutions. As a result, The author demonstrated the association among IL-1 $\beta$ genotype, IBD and Iris constitution.

Sasang Constitution Detection Based on Facial Feature Analysis Using Explainable Artificial Intelligence (설명가능한 인공지능을 활용한 안면 특징 분석 기반 사상체질 검출)

  • Objectives The aim was to develop a method for detecting Sasang constitution based on the ratio of facial landmarks and provide an objective and reliable tool for Sasang constitution classification. Methods Facial images, KS-15 scores, and certainty scores were collected from subjects identified by Korean Medicine Data Center. Facial ratio landmarks were detected, yielding 2279 facial ratio features. Tree-based models were trained to classify Sasang constitution, and Shapley Additive Explanations (SHAP) analysis was employed to identify important facial features. Additionally, Body Mass Index (BMI) and personality questionnaire were incorporated as supplementary information to enhance model performance. Results Using the Tree-based models, the accuracy for classifying Taeeum, Soeum, and Soyang constitutions was 81.90%, 90.49%, and 81.90% respectively. SHAP analysis revealed important facial features, while the inclusion of BMI and personality questionnaire improved model performance. This demonstrates that facial ratio-based Sasang constitution analysis yields effective and accurate classification results. Conclusions Facial ratio-based Sasang constitution analysis provides rapid and objective results compared to traditional methods. This approach holds promise for enhancing personalized medicine in Korean traditional medicine.

A Comparative Study on Responses to the Sasang Constitution Questionnaire for Patients between Korean and Japanese (한국인과 일본인의 환자용 사상체질 설문지 문항 응답 비교 연구)

  • Objectives This study was performed to compare responses of Korean to the Sasang Constitution questionnaire with those of Japanese and to learn difference in characteristic according to the Sasang Constitution between two countries. Methods 301 Korean visiting the department of the Sasang Constitution, Dong-Eui Medical Center in Busan, Korea from November 2006 to September 2010 responded to the SSCQ-P(Sasang Constitution Questionnaire for Patients). Sasang Constitution specialist interviewed subjects and diagnosed their Sasang Constitution. 361 Japanese visiting the center for Kampo Medicine, Keio University in Tokyo, Japan from January 2010 to February 2011 responded the SSCQ-J(Sasang Constitution Questionnaire for Japanese). The Sasang Constitution was diagnosed in the same way as Korean. We compare responses to the SSCQ-P in Korean with those to the SSCQ-J in Japanese. Results 1. Among Soyangin related 58 items of Sasang Constitution questionnaire, 26, 46.36% items had statistically significant response results in both Korean and Japanese and response disposition of all these items was same. Among Taeeumin related 68items, 36, 52.94% items had statistically significant response results in both Korean and Japanese. Of these, response disposition of 35 items was same and that of 1 item was different. Among Soeumin related 71 items, 31, 43.66% items had statistically significant response results in both Korean and Japanese. Of these, response disposition of 28 items was same and that of 3 items was different. 2. The proportion of items having statistical significance and same disposition in both Korean and Japanese by Sasang Constitutional characteristic category[Features and Way of Speaking, Physical Appearance, Temperament and Talent, Pathological Syndromes] was as follows; In Soyangin, the proportion in Pathological Syndromes was 27.8% and that in the others was more than 41.7%. In Taeeumin, the proportion in Pathological Syndromes was 33.3% and that in the others was more than 57.9%. In Soeumin, the proportion in Features and Way of Speaking was 70.6%, that in Physical Appearance was 8.3% and that in the others was 30~40%. Conclusions The response disposition of many of items having statistical significance between Korean and Japanese was same and that of a few was different. From this, there are many common Sasang Constitutional characteristics between two countries, and possibility of applying the Sasang Constitutional Medicine of Korea to Japan.

Study on the Correlation between Sasang Constitution and the Data of Pulse Diagnosis Device - A Systematic Review in Korean Articles - (사상체질과 맥진기 검사 결과와의 상관성에 대한 연구 - 국내 논문에 대한 체계적 고찰 -)

  • This study aimed to examine the possibility of data from pulse diagnosis device to be used for diagnosis of Sasang constitution. Systematic searches of 5 major Korean medical database were conducted for articles published up to May 2012. Searching key word was "Sasang" or "Constitution". Studies dealt with correlation between Sasang constitution and the pulse wave data from pulse diagnosis machine were included. Totally 2886 studies are searched and 3 studies are added from references of evaluated articles. Among them, 12 studies were met our inclusion criteria (2 Xishu Mac, 2 Self-made pulse diagnosis machine, 8 3D-Mac). Three of the eight 3D-Mac studies intended to develop a formula of constitutional differential diagnosis, 2 studies compared variables by Sasang constitution in healthy group and 3 studies compared variables by Sasang constitution in healthy group and special disease group. They all reported some significant variables, however results are not consistent between studies. The accuracy of the formula of constitutional differential diagnosis using 3D-Mac pulse diagnosis devices is 46.0% by now. Improvements in measuring pulse are required to achieve more accurate result and be used for diagnosis of Sasang constitution. Compensating B.M.I. among Sasang constitutional groups and separating constitutional pulse factors from acquired characteristics are also needed in further study.
