Exonic SNP (rs7144, 3’-UTR) in CD46 Molecule and Complement Regulatory Protein (CD46) Gene Associated with Excess Syndrome to Categorize Korean Bronchial Asthma Patients (한국인 기관지 천식 허증(虛證), 실증(實證) 환자와 CD46 유전자 다형성과의 관계)

  • Objectives In this study, we divided Korean asthma patients into excess syndrome or deficiency syndrome groups according to clinical phenotype. Genetic analysis was conducted to investigate the association of exonic SNPs in the CD46 gene polymorphism with the clinical phenotype based on the differentiation syndrome of the bronchial asthma patients.Methods There were 95 healthy patients (control group) and 53 asthma patients. (The deficiency syndrome group included 24 and the excess syndrome group 29). We searched the exonic areas of the CD46 gene in the NCBI website SNPs with <0.01 minor allele frequency (MAF) and <0.01 heterozygosity. We finally selected two SNPs: rs138843816, Ser13Phe and rs7144, 3’-UTR. Hardy-Weinberg equilibrium was calculated using SNPStats.Results There were significant differences in the codominant 1 model and the dominant model between the healthy group and the asthma group. There were significant differences between deficiency syndrome group and the excess syndrome group in the genotype frequencies and in the codominant 1 model, the dominant model, and the log-additive model. The allele frequency of rs7144C showed a significant difference between the deficiency syndrome group and the excess syndrome group. Two-SNP haplotype analysis showed a significant difference in frequency in the deficiency syndrome group and in the excess syndrome group. There were significant differences between the healthy group and the excess syndrome group in the codominant 1 model, the dominant model, and the log-additive model. The frequency of the rs7144 C allele exhibited a significant difference in the demonstration. SNP haplotype analysis between the healthy group and the excess syndrome group showed a significant difference in the frequency of the CT haplotype and the CC haplotype.Conclusions The results indicate that two CD46 SNPs (rs138843816, Ser13Phe and rs7144, 3′–UTR) might be associated with the symptomatic excess syndrome in Korean asthma patients.

Clinical Characteristics of Asthmatic Patients Who Visited Emergency Room (응급실로 내원한 천식환자들의 임상적 특성)

  • Background : Despite remarkable progress of understanding the pathophysiology and therapy of bronchial asthma, asthma morbidity and mortality are on the rise. Also hospitalization and attending rates of emergency department for asthma have been increasing gradually. We analyzed clinical characteristics and prognosis of patients who visited emergency room due to asthma attack in order to define clinical characteristics of these group of patients. Method : We reviewed 105 adult asthmatic patients who attended emergency department of Korea University Hospital between August 1995 and July 1996, retrospectively. Results : 103 patients(56 female, 47 males, mean age : 48.6 years) attended-68 self referral, 18 practitioner referral and 17 OPD transfer- and 86 patients(83.5%) were admitted. Attending emergncy department was clearly more frequent in December(13.6%) and May(12.6%). Time lag between onset of asthmatic attack and arrival at the hospital was $14.2{\pm}15.5$ hour and initial peak expiratory flow rate was $166.7{\pm}68.3L/min$.(43.3% predicted) The commonest cause for visiting emergency room was aggravation of asthma due to upper respiratory tract infection in mild asthmatics. About half of them had history of previous ER visits. Their prognosis was not bad, but after discharge, about half of patients escaped from OPD follow-up. Conclusion : As a group they merit detailed attention and follow up arrangement. Clinician need to monitor and review the treatment plans, the medications, the patient's management technique, and the level of asthma control. For this group, plans for longer term treatment, including asthma education program and adjustment of overall treatment plan should be made.

Clinical Study of Quality-of-Life in Asthmatic Patients managed by Oriental Medical Treatments (한방치료에 따른 천식환자의 삶의 질에 대한 임상적 평가)

  • Objective : Asthma is a lung disorder characterized by periodic attacks of wheezing alternating with periods of relatively normal breathing. And quality of life in asthmatic patients shows a tendency to decrease. So we had a clinical study whether if oriental medical treatment could improve their quality of life or not. Methods : The 15 cases were new patients that visited at division of respiratory system, department of internal medicine in Kyunghee medical center from the 1 st of July, 2000 to the 1 st of August. All of them were diagnosed as asthma and treated by western medical therapy. We used a Quality of Life Questionnaire for adult Korean Asthmatics(QLQAKA) and evaluated the results after two times answers, at an interval of two weeks. Results : The mean of the first answer was 2.84 marks and 3.36 ones after two weeks. If there is a difference of over 0.5 points between the two, we can admit significant effect. After the asthmatic patients treated by oriental medical therapy, they gained a difference of 0.52 points, 10.4%. Conclusions : We suggest that oriental medical therapy can improve the quality of life of asthmatic patients arid we can alternate side effects of western medical therapy with oriental one. if they carry out two different treatments at the same time.

A Study on Changes in Blood Eosinophil, Serum IgE and T Lympocyte Subpopulation after Sochongryongtang to Asthmatic Patients (소청룡탕치료 기관지천식환자의 혈액내 호산구수와 혈청IgE 및 T림프구아형의 변화)

  • Background : Nowadays asthma is considered to be an inflamatory disease characterized by airborne hyper-responsiveness and pulmonary eosinophilia. Objective : We aimed to identify the effects of Sochongryongtang on blood eosinophil, serum IgE and T lympocyte subpopulation in asthmatic patients. Material and Methods : The subjects consisted of fifteen patients with asthma who had been treated with Sochongryongtang for two weeks from February 2001 through June 2001. Sochongryongtang is herbal decoction which has been used for the traditional therapeutic agent of asthma. Results : The blood eosinophil and serum IgE in a normal controlled group. However, the T lympocyte subpopulation in asthmatic patients was not significantly different from the T lympocyte subpopulation in a normal group. The patients were treated with Sochongryongtang for two weeks. No significant difference in the blood eosinophil, serum IgE and T Iympocyte in the sub population. After treatment with Sochongryongtang for two weeks, FEV 1 increased significantly over 0.5 points out of total scores. Conclusion : This study shows that Sochongryongtang has effects on improvement of pulmonary function and quality of life in asthmatic patients. However, the patients who were treated with Sochongryongtang for two weeks showed no significant difference in the blood eosinophil, serum IgE and T lympocyte subpopulation. Further long-term studies must be made on a large number of asthmatic patients.

Objectives The main ingredient of ephedra is ephedrine which affects on autonomic nervous system induce some adverse effects just like vasoconstriction, hypertension, tachycardia, miosis, insomnia, dizziness, headache, and etc. and heart disease. If we use Mahuang according to the Sasang constitution classification in clinic, we could not only may minimize the anxiety but maximize the potential curative value in Asthma Treatment. 2. Methods On a day three times in Mahangjungchentang taken patients who with Ventricular septal defect in the aftermath of the RBBB. We are observed that the main symptoms of change, vital sign, sleep, stool, urine, heart disease. 3. Results Teaumin with asthma, the effect of Mahuang, and the side effects are fewer. The individual effects of herbs are important. but Sasang constitutional effects are important too. 4 Conclusions Mahuang can increase heart disease. But there was a difference among Sasang constitution classification. This has no side effects from Teaumin than other constitutions. If we use Mahuang according to the Sasang constitution classification in clinic. We have an excellent effect on the treatment of asthma.

  • Objectives: Patients with excessive tooth wear should first be diagnosed for the etiology of the tooth wear. Causes of tooth wear include bruxism, clenching, and taking medications for systemic diseases. After identifying the cause of tooth attrition, the final prosthesis should be restored with an appropriate vertical dimension. Methods: A 79-year-old man with worn out teeth desired a whole dental treatment. He was on medications for high blood pressure and asthma. The treatment proceeded with a consultation with a medical doctor. The medications for asthma evoked multiple teeth wear and a loss of the vertical dimension. After recovery of 3 mm of vertical dimension, 2 months of evaluation was followed by an interim prosthesis. Results: The increased vertical dimension caused no problem in function and esthetics, and the final restoration was performed with a full monolithic zirconia crown. Group function, adequate anterior guidance, and the occlusal plane were determined. Conclusions: After the final restoration, the patient was both esthetically and functionally satisfied, and a night guard splint was delivered to prevent prosthesis fracture. The patient was informed about the potential tooth wear associated with asthma drugs and educated to visit the clinic regularly.

Association Study of NDFIP2 Genetic Polymorphism with Asthma in the Korean Population (한국인에서 NDFIP2 유전적 다형성과 천식의 상관 연구)

  • Asthma is a chronic inflammatory airway disease. There are many factors including genetic and environmental factors that influence asthma. The mitogen-activated protein kinase (MAPK) pathway is involved in maintaining the T helper cells 1 and 2 (Th1/Th2) balance and plays an important role in the development of asthma. In this study, the correlation between the NDFIP2 gene that regulates the MAPK pathway and asthma was analyzed. The genetic polymorphism of the NDFIP2 gene was analyzed between 193 asthma patients and 3,228 healthy controls in Korea. As a result, 4 single nucleotide polymorphisms (SNPs) showed a significant correlation (P<0.05) and high relative risk with asthma. Among them, rs2783122 of NDFIP2 showed a statistically significant association with asthma (P-value=9.76×10-6, odds ratio (OR)=1.67, 95% confidence interval (CI)=1.33~2.10). In the SNP imputation on the NDFIP2, 16 SNPs were discovered, and all of them showed significant correlation with asthma and high odds ratio. The genotype-based mRNA expression analysis revealed that the group of minor alleles of rs1408049 showed increased mRNA expression. Increased NDFIP2 expression causes the activation of the MAPK pathway, and this may influence the development of asthma. In conclusion, the polymorphisms of NDFIP2 are associated with asthma development and this can provide the basis for new guidelines for the management of asthma in the Korean population.

Association of Single Nucleotide Polymorphisms in Interleukin-12 Receptor (IL-12Rβ1 and IL-12Rβ2) with Asthma in a Korean Population

  • Asthma is a chronic disease and occurs in airway in the lung. The cause of the disease has not been identified, it is assumed that both genetic and environmental risk factors play an important role in the development of asthma. Interleukin (IL)-12 is a cytokine regulating T-cell and NK cell. In this study, we analyzed the genetic polymorphisms of IL-12 receptor genes (IL-12Rβ1 and IL-12Rβ2) in asthma patients and normal individuals in a Korean population. We analyzed single nucleotide polymorphisms (SNPs) in IL-12Rβ1 and IL-12Rβ2 using the genotype data of 193 asthma cases and 3,228 healthy controls from the Korea Association REsource for their correlation with asthma case. IL-12Rβ1 and IL-12Rβ2 genes showed statistically significant polymorphism association with asthma case. As a results, 16 SNPs from IL-12Rβ1 and IL-12Rβ2 genes showed statistically significant association with asthma. Among them, rs375947 SNP in IL-12Rβ1 showed the greatest statistical correlation with asthma (P-value = 0.028, Odds Ratio = 1.27, 95% Confidence Interval = 1.03~1.57). The groups with minor allele of IL-12Rβ1 and IL-12Rβ2 showed increased risk of asthma. The genotype-based mRNA expression analysis showed that the group of minor allele of IL-12Rβ1 showed decreased mRNA expression. Decreased IL-12Rβ1 expression causes decreased IL-12 signaling, and this affects developing asthma. In conclusion, the SNPs in IL-12Rβ1 and IL-12Rβ2 may contribute to development of asthma in a Korean population.

  • Background: Measurement of peak expiratory flow rate (PEFR) in a follow-up examination for a chronic airway disease is useful because it has the advantages of being a simple measurement and can be repeated during examination. The aim of this study was to examine the annual decrease of PEFR in asthma and chronic obstructive pulmonary disease (COPD) patients and to confirm the factors which influence this decrease. Methods: From May, 2003 to September, 2010, the annual decrease of PEFR was obtained from asthma and COPD patients attending an outpatient pulmonary clinic. PEFR was measured using a Mini-Wright peak flow meter (Clement Clarke International Ltd. UK), and we conducted an analysis of factors that influence the change of PEFR and its average values. Results: The results showed an annual decrease of $1.70{\pm}12.86$ L/min the asthmatic patients and an annual decrease of $10.3{\pm}7.32$ L/min in the COPD patients. Age and $FEV_1$ were the predictive factors influencing change in asthma, and $FEV_1$ and smoking were the predictive factors influencing change in COPD. Conclusion: We confirmed the annual decreasing PEFR in patients with chronic airway disease and identified factors that work in conjunction with $FEV_1$ to influence the change.

Correct Use of Inhalers in Patients with Obstructive Pulmonary Disease (폐쇄성 폐질환자의 흡입기 사용 정확성 정도)

  • Purpose: The purpose of this study was to evaluate the correct use of inhalers and to examine the patients characteristics affecting correct use of inhalers in patients with obstructive pulmonary disease. Method: Subjects were 280 patients with obstructive pulmonary disease who visited the internal pulmonary outpatient departments in 2 university hospitals in Korea. Inhaler use of each patient was assessed using a checklist recommended by the National Asthma Education and Prevention Program Expert Panel. Result: Most subjects used a DPI or MDI. The overall mean score of correct use was 72.09. Most subjects responded they knew well how to use their inhalers. Most frequent error was 'not fully exhaling before inhalation'. A younger age, higher educational level, living with family, and awareness of inhaler use were significantly associated with correct use of inhalers. Conclusion: The findings of this study showed that most of patients were unable to use inhalers correctly even though they had prior educational experiences. Regular assessing of inhaler use and adequate inhaler education considering patients characteristics by nurses are needed.
