A Study on the Relationship Between Metabolic Syndrome and the Number of Work Hours Based on Data from the 6th Korea National Health and Nutrition Examination Survey (2015) (제 6기 국민건강영양조사 자료(2015)를 활용한 대사증후군 유병과 근로시간에 관한 연구)

  • There are an increasing number of reports that job related factors are associated with hypertension, sleep deprivation, obesity and diabetes. This study analyzed the relationship of working type, working hours and the prevalence of metabolic syndrome. The study's subjects were 2,777 employees (1,446 males and 1,331 females) over 19 years old and their data was from the Korea National Health and Nutrition Survey (2015). The type of work was divided into day work and shift work. The working hours were divided into less than 40 hours, 40 to 49 hours, 50 to 59 hours and 60 hours or more per week. As a result of this study, the prevalence of metabolic syndrome was 26.4% of the males and 19.1% of the females. Generally there was no relationship between metabolic syndrome and type of work (day work and shift work). As a result of analysis of working hours and metabolic syndrome, the workers who worked 40-49 hr were associated with a decreased prevalence of metabolic syndrome as compared to that of workers who worked less than 40 hr (odds ratio: 0.47, 95% confidence interval: 0.24-0.95, p<0.05). Further, positive relationships were found between the number of work hours and the prevalence metabolic syndrome for specific females who worked over 40 hr/wk. It can be concluded from the results that it is important to help workers avoid metabolic syndrome through setting appropriate work hours.

Hazardous Alcohol Consumption and the Risk of Hearing Impairment in Adults Based on the Korean National Health and Nutrition Survey: A Retrospective Study

  • Background and Objectives: To investigate the relationship between hearing impairment and alcohol drinking patterns in South Korean adults. Subjects and Methods: Data collection was performed by Korean National Health and Nutrition Survey from January 1 to December 31, 2012. Data analyses were performed from February 20 to March 3, 2018. Data from 3,860 adults 20 years of age or older without a history of malignancy or chronic otitis media in the Korean National Health and Nutrition Survey 2012 database who participated in the health questionnaires, and who had available results from otologic examinations that included pure tone audiogram, were included. Pure-tone average hearing thresholds were calculated at 500, 1,000, 2,000, and 4,000 Hz. Hearing loss was defined as a pure-tone average >40 dB in one or both ears. The Alcohol Use Disorder Identification Test was used to evaluate drinking statuses of subjects. Data were analyzed using the complex-sample χ2-test of independence and a complex-sample logistic regression analysis. Results: Of the 29,954,319 individuals in the weighted cross-sectional study population, 15,106,040 (50.4%) were men and 14,848,098 (49.6%) were women. A total of 8.1% of men and 7% of women had hearing impairment. The degrees of drinking with appropriate, risky, and hazardous drinking habits were 58.2, 32.1, and 9.7% among men; and 76.4, 12.5, and 11.1% among women, respectively. Among men, the odds ratio of hearing loss increased by 2.506 times when comparing hazardous and appropriate drinking (confidence interval, 1.083 to 5.800, p=0.002). Moderate alcohol consumption (≤2 drinks per day) was not protective for hearing in either group. Conclusions: As hazardous drinking tends to coexist with hearing impairment in men, appropriate prevention and intervention strategies should be emphasized. A longitudinal study to investigate harmful drinking and the mechanism of hearing loss should be performed.

Factors Associated with the Use of Gastric Cancer Screening Services in Korea: The Fourth Korea National Health and Nutrition Examination Survey 2008 (KNHANES IV)

  • Objective: Despite government efforts to increase participation in gastric cancer screening, the rate is still suboptimal in Korea. Therefore, we explored barriers to and predictors of gastric cancer screening participation among a nationally representative sample. Methods: We used the Health Interview Survey sub-dataset derived from the Fourth Korean National Health and Nutrition Examination Survey 2008 (KNHANES IV) to evaluate participation in gastric cancer screening and factors associated with attendance in individuals age ${\geq}40$ years. We enrolled 4,464 subjects who completed the questionnaire and were not previously diagnosed with gastric cancer. Four groups of factors were considered potential predictors of gastric cancer screening in a multivariate analysis: sociodemographic, health behavior, psychological and cognitive, and dietary factors. Results: Overall, 41.3% complied with the gastric cancer screening recommendations. Younger age, lower education level, living without a spouse, frequent binge drinker, and current smoker were significantly associated with less participation in gastric cancer screening. Conclusions: To improve participation in gastric cancer screening, more focused interventions should be directed to vulnerable populations, such as groups with low socioeconomic status or unhealthy behavior. In addition, there should be new promotional campaigns and health education to provide information targeting these vulnerable populations.

Evaluation of Dietary Quality and Nutritional Status according to the Use of Nutrition Labeling and Nutrition Claims among University Students in Chungbuk Area - Based on Nutrition Quotient (충북 지역 일부 대학생에서 영양표시와 영양강조표시 이용에 따른 식사의 질과 영양상태 평가 연구- 영양지수를 이용하여)

  • Objectives: The study examined the status on the use of nutrition labeling and nutrition claims among university students as well as the diet quality and nutritional status using the nutrition quotient (NQ) according to the use of nutrition labeling and claims. Methods: A cross-sectional survey was conducted on university students (86 males and 131 females) from March to April in 2019 in Chungbuk area. Results: The percentage of subjects who reported that they recognized nutrition labeling was 80.2% of the total, and 63.8% of them indicated that they used nutrition labeling for their food choices. In addition, 22.6% of the total subjects said that they used nutrition claims. The group of subjects using nutrition labeling showed significantly higher mean scores in balance (P < 0.01), diversity (P < 0.05), moderation (P < 0.01), and dietary behavior (P < 0.001) along with higher mean NQ score (P < 0.001) compared to the group not using nutrition labeling. The group using nutrition claims had significantly higher mean scores in balance (P < 0.001), diversity (P < 0.01), moderation (P < 0.001), and dietary behavior (P < 0.001) as well as total NQ scores (P < 0.001) compared to the group not using nutrition claims. The group of subjects using both nutrition labeling and nutrition claims showed significantly higher mean NQ scores than the group of subjects using either nutrition labeling or nutrition claims (P < 0.05). Conclusions: In this study, university students' use of nutrition labeling and nutrition claims appeared to be related to their diet quality and nutritional status.

Associations between dietary patterns and screen time among Korean adolescents

  • Data are limited on the association between dietary patterns and screen time among Korean adolescents. The present study identified dietary patterns of 691 adolescents, aged 13-18 years, who had participated in the Third Korean National Health and Nutrition Examination Survey (KNHANES III) and analyzed their associations with screen time. Screen time was defined as the time spent watching TV, using a computer, or playing video games was calculated as a sum of all these times. Dietary patterns and their factor scores were derived from a food frequency questionnaire using the factor analysis method. To analyze the association between dietary patterns and screen time, we conducted multiple linear regression analysis. We also performed multiple logistic regression analysis to estimate odds ratios (OR) of excessive screen time (2 hours or longer per day) and 95% confidence intervals (CI). We identified 2 dietary patterns labeled "the Korean healthy dietary pattern" and "the Western diet and fast foods pattern". The former included mixed grains, legumes, potatoes, red meat, eggs, fish, dairy products, fruits, vegetables, seaweeds, and mushrooms, whereas the latter included noodles, bread, red meat, poultry, fast foods, snack, and soft drinks. After controlling for potential confounding factors, factor scores for the Korean healthy dietary pattern were inversely associated (P-value for trend < 0.01) and those for the Western diet and fast foods pattern were positively associated with the screen time (P-value for trend < 0.01). Adolescents in the top tertile of the scores for the Korean healthy dietary pattern had a multivariable-adjusted OR [95% CI] of 0.44 (0.25-0.75) for excessive screen time compared with those in the lowest tertile. On the basis of these findings, adolescents who have excessive screen time may need to be encouraged to consume a more healthy diet.

The effect of chewing difficulty on depression in Korean adults over aged 40 years : The 7th Korean National Health and Nutrition Examination survey (2016~2018) (제7기(2016~2018) 국민건강영양조사를 이용한 40세 이상 한국 성인의 저작불편감이 우울에 미치는 영향)

  • Objectives: The purpose of this study was to analyze the effect of chewing difficulty on depression in Korean adults to provide basic data on improving the quality of life related to oral health of Korean adults. Methods: The raw data of the analysis were obtained from the dataset of the seven Korea National Health and Nutrition Examination Survey conducted between 2016 and 2018. The subjects included 9,535 people who underwent oral examination and answered questions. The collected data were analyzed using the SPSS (ver 21.0) program using composite samples, and the mean crossover analysis and logistic return analysis were performed. Results: An analysis of the effects of chewing difficulty on depression in Korean adults showed that those with chewing difficulty had a 2.24 fold higher risk of depression (95% CI: 1.73-2.90) and a statistically significant difference (p<0.05). Conclusions: The association between the chewing difficulty on depression could be confirmed, and measures improve of the oral health and preventing depression should be prepared.

Korean Food Exchange Lists for Diabetes Meal Planning: Revised 2023

  • A food exchange list is a tool developed to help diabetic patients control their energy intake and plan balanced meals. Korean food exchange lists were first developed in 1988, revised in 1995, and updated again in 2010. With rapidly changing dietary habits and increasing demand for diverse food cultures, the Korean Diabetes Association in cooperation with 4 related organizations established a Task Force Team (TFT) to revise food exchange lists in March 2022. Starting with a workshop, TFT held 11 official revision meetings, culminating in a public hearing in May 2023. The final revised version of Korean food exchange lists was published in December 2023. Key outcomes of the revision are summarized as follows: 1. Based on the National Standard Food Composition Table 10.0 database, the existing classification system and nutrient standards for each food group remain unchanged this time. 2. Based on a survey conducted among diabetes educators, the number of items on the food exchange lists has increased from 339 in 2010 to 435 this time. 3. Considering patients' usual eating habits, meal planning examples were developed distributing food group exchange units by energy level based on 3 types of proportions of carbohydrate energy (40%-45%, 50%-55%, 60%-65%). 4. Due to limitations in real-time updates for rapidly changing information, detailed guidance on how to access and interpret the data is provided. These revisions will help people with diabetes manage their blood sugar levels and facilitate the implementation of healthy meal planning in various other conditions, including obesity.

Dietary Molar Ratios of Phytate : Zn and Phytate $\times$ Ca : Zn for Zn Nutrition Assessment in Koreans

  • Zinc nutriture in South Koreans was evaluated by estimating Zn, Ca, and phytate intake, and the molar ratios of phytate : Zn and the millimolar ratios of phytate $\times$ Ca Zn. food consumption data from the 95 National Nutrition Survey was used. For the present study, data from the nationwide, large city and urban area level were used. No standard deviation measures were provided in the 95 National Nutrition Survey : only mean values were reported. Nationwide daily intake of Zn and Ca were 10.1 mg/day and 426.5 mg/day, respectively. The estimated daily phytate intake was 1676.6 mg/day nationwide. The molar ratio of phvtate : Zn, the millimolar ratio of phytate $\times$ Ca : Zn and the phytate $\times$ Ca Zn mmol per 4.2 MJ (1000 kcal) were 15.9, 168.9 and 91.8 in nationwide, respectively. The major food groups for zinc intake were meat, poultry products (43%), and cereals and grain products (18%). Sixty two percent of Zinc was from animal food sources. Cereal and grain products supplied most of the phytate intake (46%), followed by seasonings, fruits, and legume products. The major food source of phytate was rice (39%) . The results of the present study suggest that Zn status of Koreans maybe influenced by high dietary intake of phytate and high molar ratios of phytate : Zn and phytate $\times$ Ca : Zn. These results raise concerns about Zn status of Koreans, who consume a diet higher in phytate than Western diets. further research is necessary to confirm whether such poorly available dietary Zn has any impact on the health of Koreans.

Association between drinking behaviors and components of metabolic syndrome in subjects in their 20s and 30s: data obtained from the Korea National Health and Nutrition Examination Survey (2016-2018)

  • BACKGROUND/OBJECTIVES: Numerous studies have examined the relationship between drinking behaviors and metabolic syndrome (MetS) for adults, but these include very few studies for young adults. This study therefore undertook to investigate the association between drinking behaviors and components of MetS among adult drinkers aged 20-30 years. SUBJECTS/METHODS: Using the 2016-2018 Korea National Health and Nutrition Examination Survey data, drinking behaviors of adults in the age group 20-30 years were divided into 4 groups: 1) group A, good drinking habits; 2) group B, frequent binge drinking but not frequent drinking; 3) group C, frequent drinking but not frequent binge drinking; 4) group D, frequent drinking and binge drinking. The association between MetS components and drinking behaviors was analyzed by applying multiple logistic regression analysis. RESULTS: We determined the prevalence risk compared to group A. In men, the prevalence risk of high triglyceride (TG) increased 2.051-fold in group C and 1.965-fold in group D. Moreover, in group D, the prevalence risk of low high density lipoprotein cholesterol (HDL-C) increased 0.668-fold, high blood pressure (BP) increased 2.147-fold, and MetS increased 1.567-fold. In women, there was an increased prevalence risk of low HDL-C (0.353-fold) and MetS (3.438-fold) in group C, whereas group D showed increased prevalence risk of abdominal obesity (2.959-fold), high TG (1.824-fold, and low HDL-C (0.424-fold). CONCLUSIONS: Our study indicates that frequent drinking increases the risk of high TG, whereas frequent and binge drinking increases the risk of high TG, low HDL-C, high BP, and prevalence of MetS in men. In women, frequent drinking without binge drinking increases the risk of low HDL-C and MetS, whereas frequent and binge drinking increases the risk of abdominal obesity, high TG, and low HDL-C. We propose that improvements in the drinking behaviors can reduce the prevalence of MetS.

Nutrients Intake and Health Status by Fruits and Vegetables Intake in Adolescents Based on the 2013~2015 Korean National Health and Nutrition Examination Survey (2013~2015년(6기) 국민건강영양조사 자료를 이용한 청소년의 과일·채소 섭취에 따른 영양소 섭취와 건강상태의 비교)

  • This study was conducted to compare the nutrient intake, physical characteristics, and health status according to daily fruits and vegetables intake of adolescents using the Korean national health and nutrition examination survey for the sixth period (2013~2015). Subjects were 1,096 adolescents aged 13~18 years. The average daily intake of fruits and vegetables was $518.49{\pm}4.94g$. The two groups (insufficient, sufficient) were divided by fruits and vegetables intake (<500 g/day, ${\geq}500g/day$) and the ratio of the insufficient group was 75.9% while that of the adequate group was 24.1%. The mean of intake of energy, calcium, phosphorus, sodium, potassium, iron, thiamin, niacin, vitamin C and dietary fiber differed significantly between the insufficient and sufficient groups (P<0.01, P<0.001). However, there was no significant difference in physical characteristics between groups. The self-perceived health status was better in the sufficient group (P<0.05), while the insufficient group was aware of more stress in their daily life (P<0.05). The serum triglyceride was significantly higher in the sufficient group (P<0.05), while the total cholesterol was lower (P<0.05). As the characteristics of fruits and vegetables are different, it will be necessary to determine standards for consumption of each type. It is also necessary to consider whether a fruits and vegetables intake of 500 g would be a reasonable standard for adolescents. Nevertheless, the results of this study indicate efforts to increase fruits and vegetables intake by young people are necessary.
