Average Dietary Energy Intake does not Increase as BMI Increased in the National Health and Nutrition Examination Survey Data of Korea

  • Although the idea that obese people consume higher calorie diets is widely accepted, many dietary surveys have shown that obese people do NOT consume larger amounts of energy. We had an opportunity to study the relationship between calorie intake and obesity in Korea from the data contained in the '98 National Health and Nutrition Examination Survey of Korea. The survey was executed nationwide for two months - from Nov. 1 to Dec.30 in 1998. The survey included 10,876 (aged >10 years) subjects of whom 9,771 underwent health examinations. Surveyors visited each household and checked health status, measured anthropometry and blood pressures, collected blood and urine samples, and interviewed from the health questionnaires. Well-trained dietitians evaluated the food consumption of 11,525 subjects over the age of 1 year with the 24-hour recall method. The number of subjects from whom a complete health examination and food consumption information was obtained was 8,004. Subjects were classified by BMI (< 20, 20-22, 22-24, 24-26, 26-28, 28 $\leq$) and into newly diagnosed patients with DM (FBS $\geq$ 126 mg/㎗), hypertension (SBP $\geq$ 140 mmHg or DBP $\geq$ 90 mmHg) and hyperlipidemia (Total cholesterol $\geq$ 220 mg/㎗ or TG $\geq$ 200 mg/㎗). Our main results were as following:1) their average energy intake was 2,029.6 $\pm$ 908.5 ㎉ and BMI is 22.6 $\pm$ 3.4 kg/$m^2$;2) a comparison of nutrient intakes by BMI level did not show a significant difference of energy intake even though BMI increased (BMI, < 20: 1,999 ㎉ ∼ 28 $\leq$: 2,028 ㎉);and 3) Even in newly diagnosed patients with diabetes, hypertension or hyperlipidemia, their energy consumption was not significantly increased as BMI increased (from BMI 20). There are several possible explanations for these results:1) Reduced physical activity caused the weight of obese people to increase even with the same energy intake;2) people underreported their energy consumption;or, people intentionally reduced their energy consumption due to self-image regarding their obesity. We might also hypothesize that there is a metabolic problem conceiving obese people, because calorie intake was not higher in obese people than in non-obese people in Korea. Further research is necessary for re-evaluating these current conclusions.

  • Purpose: This descriptive survey-based study aimed to identify the factors associated with obese adults' quality of life (QOL) using a secondary analysis of data collected through the 8th Korea National Health and Nutrition Examination Survey (2019). Methods: In this study, factors affecting the health-related quality of life of obese adults were analyzed by categorizing them into three independent variables: general characteristics, health behaviors, and mental health characteristics. Using t-test and analysis of variance (ANOVA), an analysis was performed to determine the differences in obese adults' QOL depending on their general characteristics, health behaviors, and mental health conditions. A hierarchical regression analysis was performed to identify the factors that affect obese adults' QOL. Results: In obese adults' general characteristics, the risk factors for QOL were older age, male, lower-income level, presence of spouse, and occupation. With regard to health behaviors, the risk factors were lack of exercise. In terms of mental health conditions, the risk factors were stress and suicidal ideation. Conclusion: Obese adults' QOL was affected by age, sex, income level, absence/presence of a spouse, occupation, health behaviors (e.g., exercise), and mental health conditions (e.g., stress and suicidal ideation).

국민건강영양조사 자료에 근거한 고령 복합만성질환자의 건강 및 영양상태 특성 (Health and Nutrition Status of Elderly People with Multimorbidity: A Korea National Health and Nutrition Examination Survey (2013~2015))

  • Objectives: This study investigated the health and nutritional status of the elderly according to the number of chronic diseases, using data obtained from the Korea National Health and Nutrition Examination Survey 2013~2015. Methods: Data from a total of 2,310 individuals, aged 65 years and over, were used for the analysis. The elders were divided into 0 (n=375), 1 (n=673), 2 (n=637) and 3 or more (n=625) groups, by considering the number of chronic diseases. Results: Compared to other groups, the elderly subjects who were living with their spouse had the highest ratio in group 0 (P < 0.05), whereas subjects without economic activities had highest ratio in 3 or more group (P < 0.05). The EQ-5D index of subjects in the 0 group (0.90 ± 0.01) was higher than that in the 3 or more group (0.86 ± 0.01) (P < 0.05). After adjusting for confounding factors, the energy intake of subjects was determined to be lowest in the 3 or more group (P < 0.05). Protein (P < 0.05) and riboflavin (P < 0.05) intakes of the 3 or more group were also lower than other groups. Conclusions: This study indicates that multimorbidity of the elderly is associated with their health and nutritional status. The nutrients intake of the elderly, especially energy, protein and riboflavin, tended to be lowest in the 3 or more group. Further research is required to elucidate the risk factors related to presence of multimorbidity in the elderly.

우리나라 고등학생의 식품 및 영양소 섭취 변화 추이 - 2007~2015 국민건강영양조사를 이용하여 - (Trends in Food and Nutrient Intake of High School Students based on the Korea National Health and Nutrition Examination Survey 2007~2015)

  • This study assessed yearly trends of food and nutrient intake among high school students aged 16 to 18 years (n=2,377) using the 2007~2015 Korea National Health and Nutrition Examination Survey (KNHANES). Yearly trends of food or nutrient intake were analyzed via logistic regression analysis. The results showed that consumption of sugars & sweets, and beverages & alcohols was increased rapidly during this period (p<0.0001). Intake of meat & meat products, and fish & shellfish also was increased (p=0.0008). Intake of grains and grain products was increased until year 2011 but declined after 2012 (p=0.0025). Consumption of vegetables, and milk & milk products was decreased (p=0.0395). Intake of protein, fat, thiamin, riboflavin, niacin, and iron was increased (p=0.0445). Carbohydrate energy ratio was decreased, whereas fat energy ratio was increased (p=0.0235). Most nutrient intakes satisfied the dietary reference intakes for Koreans except dietary fiber (19.6~26.2%), calcium (46.9~55.2%) and sodium (more than 221.4%) during this period. There was a significant positive correlation between most food group intakes and most nutrient intakes (p=0.0468). Therefore, it is crucial to increase dietary fiber and calcium intake and decrease consumption of sugars, fats and sodium through diverse eating of food groups to ensure balanced nutrition of subjects.

Dietary intake and nutritional status of Korean children and adolescents: a review of national survey data

  • In Korea, several national cross-sectional surveys monitor the diet, nutritional status, and health status of children. This continual dedicated national surveillance system contributes to the identification of nutritional and health issues, establishment of public health policies, and development of nutrition recommendations. This paper provides recent information about the Korea National Health and Nutrition Examination Survey and the Korean Youth Risk Behavior Web-based Survey and describes key nationwide survey findings published in the last 5 years on infant feeding practices and the dietary intake and nutritional status of Korean infants, children, and adolescents. There have been increasing trends in children, and teenagers who skip breakfast, eat fast food, consume sugary drinks, have vitamin D deficiency, and are obese. This review will inform pediatricians, nutritionists, and other health care practitioners who track children's growth and development. It may also help researchers and policymakers identify diet-related policies and strategies for chronic disease prevention in Korean infants, children, and adolescents.

노인의 구강건강행위가 치주상태에 미치는 영향 - 2010년 국민건강영양조사를 이용하여 - (The Effects of oral health behaviors of elderly on periodontal status (From the 2010 Korean National Examination Health and Nutrition survey))

  • Purpose: The purpose of this study was to survey periodontal status using oral health index in the elderly population and determine methods for improving oral health status. Methods: The date collected from 'The fifth korean national health and nutrition examination survey 2010' was analyzed in this study. Statistical analysis was done using the Statistical Package for Social Sciences version 19.0 for windows. We determined frequencies and percentage, calculating means, and standard deviations, and determining statistical significance using logistic regression analysis, multiple regression analysis. Results: General characteristics showed differences in oral health behaviors associated with gender, age, residence, education. General characteristics showed differences in periodontal status associated with gender, family type, gender, residence, education. Oral health behaviors showed differences in periodontal status associated with using oral health care products, dental treatment. Conclusion: In conclusion, in order to improve oral health of the elderly, the importance and needs of periodontal status should be emphasized. periodontal status related education for the elderly should be operated.

치아홈메우기와 치아우식과의 연관성 분석: 제7기 국민건강영양자료조사 이용 (Association between sealants and caries assessed using the Seventh Korea National Health and Nutrition Examination Survey (KNHANES))

  • Objectives: This study aimed to evaluate the relationship between pit and fissure sealants of the molars, and caries prevalence and experience to assess the effect of sealants on caries prevention. Methods: Data from 16,119 people were extracted from the seventh Korean national health and nutrition examination survey, which was conducted from 2016 to 2018. Demographic, socioeconomic, oral health-related behavioral, and oral examination survey data were collected as independent variables. SAS statistics for complex samples and multiple logistic regression analysis (Windows ver. 9.4) was used for the statistical analyses. Results: Caries prevalence and experience were significantly associated with age, sex, daily toothbrushing frequency, interdental cleaning, and having molar sealant. Multiple logistic regression analysis revealed a significant association between having molar sealants and the caries prevalence and experience among participants under 30 years of age after adjusting socio-economic variables and toothbrushing-related factors. Conclusions: This study confirmed the effect of molar sealants on caries prevention in Korea. Our results support that the application of molar sealants in children, adolescents, and young individuals is effective in preventing caries.

국민건강영양조사를 활용한 나트륨 섭취량과 열량 대비 나트륨 섭취량에 대한 코호트 분석 (A Cohort Analysis on Sodium and Sodium-calorie Intake with the Korean National Health and Nutrition Examination Survey)

  • The purpose of this study was to analyze a cohort, age, and time effects on sodium and sodium-calorie intake using the 2007~2017 Korea National Health and Nutrition Examination Survey (KNHANES). For analysis, we have constructed cohort panel data that combine KNHANES data in a time series. The age effect gradually increased from age 45~49 and then decreased afterwards. The time effect showed an increase in average sodium intake until 2010 followed by a subsequent decrease. The cohort effect showed that the sodium intake was the highest for the War of Liberation 1946~1953, and that the younger the latter, the lower the sodium intake. According to a cohort analysis, the younger the generation, the lower the sodium intake compared to the calorie intake, according to the baby boomers. Based on the results of this study, efforts should be made to educate and promote the dieting and providing low-salt meals to reduce sodium intake by generation. An analysis of the health hazards including sodium by generation and age is thought to enable the establishment relevant policies.

아동 및 청소년의 라면 섭취에 따른 식품 및 영양소 섭취 실태 비교 - 2005년 국민건강영양조사 심층분석 결과 - (A Comparison of Food and Nutrient Intakes between Instant Noodle Consumers and Non-Consumers among Korean Children and Adolescents)

  • Instant noodle is one of the most popular foods in Korea. The objective of this study was to examine the association of instant noodle consumption and food and nutrient intake among children and teenagers in Korea. We used dietary data from 24-h recall of 1,748 subjects aged 7-19 years who participated in the 2005 Korea National Health and Nutrition Examination Survey. Those who consumed instant noodle once or more during the survey period were categorized as "instant noodle consumer (INC)" and were compared for food and nutrient intakes with the others (non-INC). The average age of the INC was 13.4 and that of the non-INC was 12.4; girls consumed more instant noodles than boys (p < 0.05). There was no significant difference in BMI between the two groups. With the exception of grains, as compared to the non-INC, the INC consumed significantly less amounts of potatoes, vegetables, mush-rooms, fruits, spices, seafood, and milk and dairy products. With respect to nutrient intakes, the INC-group showed significantly higher nutrient intakes of energy, fat, carbohydrate, sodium, thiamine, and riboflavin; however, as compared to the non-INC-group, the INC-group showed significantly less intakes of calcium, niacin, and vitamin C. The overall result suggests that consuming instant noodles may lead to excessive intake of fat and sodium, but it may cause an increased intake of thiamine and riboflavin. Therefore, nutrition education that helps children and teenagers choose a balanced meal while consuming instant noodle should be carried out. In addition, manufactures of instant noodles should consider nutritional aspects in product development processes.

생애주기별 당뇨질환자의 구강건강영향요인 (Factors influencing oral health according to life cycle characteristics of patients with diabetes)

  • Objectives: This study aimed to identify the factors influencing the relationship between diabetes and oral health in Korean adults. Methods: We analyzed 5,319 adults who were included in the 9th Korea National Health and Nutrition Examination Survey. The data were analyzed for demographic characteristics, daily health care, and oral health care according to diabetic conditions using a complex sample analysis. Multiple logistic regression analysis was performed to analyze the factors influencing the oral health of patients with diabetes. Results: The factors influencing the oral health of patients with diabetes varied according to life cycle. In late middle-aged adults, statistically significant differences were observed in sex (p<0.001), educational level (p=0.030), economic activity (p=0.018), aerobic exercise (p=0.034), smoking (p=0.004), periodontal therapy (p=0.011), and prosthesis production/repair (p=0.025). In younger elderly individuals, statistically significant differences were found in terms of whether they lived together (p=0.027) and educational level (p=0.032). Conversely, no statistically significant differences were observed in the older elderly group. Conclusions: The results of this study showed that the level of oral health of patients with diabetes is already determined in middle and old age; therefore, a system should be prepared to ensure that health care can be systematically performed in late middle-aged adults.
