COVID-19 후각 이상에 대한 한의학적 고찰 (Olfactory Dysfunction in COVID-19 from a Korean Medical Perspective)

  • Objectives : To analyze symptoms of olfactory dysfunction caused by COVID-19 from a Korean Medical Perspective. Methods : Previous studies dealing with olfactory dysfunction accompanying COVID-19 were studied and analyzed for general characterization. Physiology and pathology of olfactory functions within the classical texts of Korean Medicine were collected and analyzed, through which symptoms of olfactory dysfunction in COVID-19 were examined. Results : Olfactory dysfunction manifested in high ratios in the early stages of confirmed COVID-19 cases, at times independent of other nasal symptoms such as blockage or discharge. There was a high chance of loss of taste being accompanied, while mental problems such as a tendency to have difficulty concentrating were present as well. In most cases, recovery took one to two weeks. From a Korean Medical perspective, physiology of olfactory function is closely linked to the Lungs, Ancetral Qi[宗氣], and the Heart, while its dysfunction could be explained by pathological factors such as Wind-Cold, Fire stagnation, Qi deficiency, Wind stroke, etc. Conclusions : In the context of external contraction disease[外感病], olfactory dysfunction could be caused by problems in the Lungs and Stomach that are responsible for breathing, or the Heart which is involved in recognizing and differentiating scent. General characteristics of COVID-19 imply it to be closely related to the Heart. In clinical application, overall symptoms need to be considered in diagnosis and treatment planning, after which further approaches could made to determine the problem to be of the Lung and Stomach, or of the Heart.

Serratia marcescens의 항균제 내성 및 R plasmid (Antimicrobial Drug Resistance and R Plasmids of Serratia marcescens)

  • Forty clinical isolates of Serratia marcescens were tested for their susceptibility to 19 antimicrobial drugs and studied on the molecular characteristics of R plasmids. Cefotaxime (Ct) was the most effective drug and only 2 (5%) strains were resistant to this drug. Thirteen to 18% of strains were resistant to cefoperazone (Cz), amikacin (Ak). and trimethoprim (Tp), and 28 to 40% were resistant to piperacillin (Pi), nalidixic acid (Na), gentamicin (Gm), and cefoxitin (Cx). A majority of strains were resistant to carbenicillin (Cb), tobramycin (Tp), kanamycin (Km), and cefamandole (Cd), and all to cephalothin. One half of the isolates were resistant to 10 or more drugs. $MIC_{90}$ of Pi to Gm-resistant strains (Gm') were 8 times higher than that to Gm-susceptible strains (Gm'), but $MIC_{90}$ of Ak, Cx, Ct, and Cz were almost the same between both Gm' and Gm' strains. Nine (23.7%) strains among 38 of multiply drug-resistant S. marcescens transferred conjugally their partial patterns of resistance to E. coli or Klebsiella strains, and two S. marcescens strains producing bacteriocin transferred their resistance to Klebsiella only, but not to E. coli. The plasmid profiles of S. marcescens were studied by the methods of SDS lysis and agarose gel electrophoresis. Twenty-four (60%) strains carried one to four plasmids of 1.4. to 144 Mdal, and conjugative R plasmids of 49 to 127 Mdal were noted in transconjugants. MIC levels of drugs in transconjugants were variable by the R plasmids and recipient strains.

Alzheimer's Disease-linked Swedish Amyloid Precursor Protein Mutation Induces Cell Death by Increasing Reactive Oxygen Species Generation

  • The Swedish double mutation (KM670/671NL) of amyloid precursor protein (Swe-APP) is associated with early-onset familial Alzheimer's disease (FAD) and increases amyloid beta peptide production. Although APP/A/3 mediated neurotoxicity is observed both in vitro and in vivo, the relationship between mutant APP expression, A/3 production, and neuronal death observed in the brains of FAD patients remains to be elucidated. In this study, we investigated the mechanisms of Swe-APP-induced cell death in HEK293 and NGF-differentiated PC 12 cells. We found that the expression of Swe-APP induced cytochrome C relase, activation of caspase 3 in HEK 293 and NGF-differentiated PC 12 cells. We also show that the reactive oxygen species (ROS) was detected in Swe-APP expressing HEK 293 cells and NGF-differentiated PC 12 cells and that pretreatment with vitamine E attenuated the cellular death, cytochrome C release induced by Swe-APP expression, indicating the involvement of free radical in these processes. These results suggest one of possible apoptotic mechanisms of Swe-APP which could occur through cytochrome C release from mitochondria and this apoptosis inducing effects could be at least in part, due to ROS generation by Swe-APP expression.

농촌지역(農村地域)의 한방의료이용실태(韓方醫療利用實態) 일부(一部) 농촌지역(農村地域)의 군보건소이용자(群保建所利用者)를 중심(中心)으로- (Study on Utilization of Oriental Medicine by Residents in Rural Areas)

  • Socioeconomic status in this county progressed rapidly, this has brought about many changes in health care fields, namely, pattern of disease prevalence and morbidity, increase of the aged people, and also availability of health care in rural areas. According to the utilization study of medical care, it showed that the oriental medicine is used for the treatment of lasted chronic disease not the minor and common diseases which is quick in its effect. Particularly, in rural areas. prevalence of chronic disease is higher than that in urban areas. Although the health cafe need of the oriental medicine is high in rural areas, the distribution of manpower and facilities is lower than that in urban areas. Therefore the government has planned to implement the demonstration project for the oriental medicine at the designated 3 health centers in rural areas. The purpose of this study was to collect the utilization level of oriental medical care of the people in rural areas. To meet the purpose of this study, patient interview were applied. 790 patients visited to health center in project areas were selected and analyzed by experienced interviewers from 2 April to 21 April 1990. The major findings of this study were as follows ; 1) Of the 790 patients, 32.6 percent of the respondents had experience of using the oriental medicine. As for the utilization by age and sex. 54.8% of those was female and 70.7% was 40 years of age and more. 2) Reaction to the question of educational achievement showed that on schooling and primary school graduates accounted for 63.1%. 3) The most user of oriental medicine resides in country level, where the health center is located, and 80 percent of those users resides within 10Km. 4) More than 50% of the total was the chronic diseases which lingered for more than 3months. 5) 32.6 percent of the total cases used the oriental medicine. 61.2% among those was treated by oriental medical care hospital and 38.8% by oriental drug dispensaries etc. 6) The contont of oriental medical care varied ; 50.1% for prescription of herb drugs for treatment, 25.1% for health maintenance and 23.9% for acupuncture, moxibustion etc. 7) As for the motivation for using the oriental medicine. 56.6% of the respondents was for treatment of diseases and 27.9% wes for strengthening the physical weakness. 8) As for the effectiveness of the oriental medicine. 70.3% of the total cases satisfied with that treatment and 84.2% of the total cases will use the oriental medicine when is provided by health center.

화력발전소 영향권 주민 거주지의 이격 거리별 중금속 및 휘발성유기화합물 대사체 노출 농도 비교 분석 (Comparative Analysis of Heavy Metal Exposure Concentrations and Volatile Organic Compound Metabolites among Residents in the Affected Area According to Residential Distance from a Coal-fired Power Plant)

  • Background: There are concerns about the health effects of various environmental pollution exposures among residents living near coal-fired power plants (CFPP). Objectives: This study attempted to compare the concentrations of heavy metals in blood and urine and those of urinary volatile organic compound (VOC) metabolites according to the residential separation distance. Methods: Participants in the study totaled 334 people who have lived for more than 10 years in areas within 10 km of a CFPP. The separation distance was analyzed in quartiles by dividing it into Q1 (88 people), Q2 (89 people), Q3 (89 people), and Q4 (68 people). We explained the purpose of this study to the participants and collected blood and urine after obtaining signatures on a participation agreement. Results: The study participants were 102 males (30.5%) and 232 females (69.5%), with an average age of 71. The average length of residence and distance were 43.8 years and 4,800 meters. The geometric mean concentrations of Pb, Cd, and Hg in blood and As and Cd in urine were respective 1.35 ㎍/dL, 1.43 ㎍/L, 3.16 ㎍/L. They were 167.88 ㎍/g for creatinine and 1.58 ㎍/g creatinine. The metabolite concentrations of VOCs were 50.67 ㎍/g creatinine in t, t-muconic acid (t, t-MA), 10.73 ㎍/g creatinine in benzyl mercapturic acid, 317.05 ㎍/g creatinine in phenylglyoxylic acid, 123.55 ㎍/g creatinine in methylhippuric acid, and 190.82 ㎍/g creatinine in mandelic acid. The concentration of Pb in the blood and Cd and t, t-MA in the urine of residents within affected area of the CFPP showed statistically significant differences among distance groups. Conclusions: The concentration of urinary VOCs metabolites, especially t, t-MA, differed according to the distance groups of residents within the affected area of CFPP (p<0.05).

The UGT1A9*22 genotype identifies a high-risk group for irinotecan toxicity among gastric cancer patients

  • Several studies have shown associations between irinotecan toxicity and UGT1A genetic variations in colorectal and lung cancer, but only limited data are available for gastric cancer patients. We evaluated the frequencies of UGT1A polymorphisms and their relationship with clinicopathologic parameters in 382 Korean gastric cancer patients. Polymorphisms of UGT1A1*6, UGT1A1*27, UGT1A1*28, UGT1A1*60, UGT1A7*2, UGT1A7*3, and UGT1A9*22 were genotyped by direct sequencing. In 98 patients treated with irinotecan-containing regimens, toxicity and response were compared according to the genotype. The UGT1A1*6 and UGT1A9*22 genotypes showed a higher prevalence in Korean gastric cancer patients, while the prevalence of the UG1A1*28 polymorphism was lower than in normal Koreans, as has been found in other studies of Asian populations. The incidence of severe diarrhea after irinotecan-containing treatment was more common in patients with the UGT1A1*6, UGT1A7*3 and UGT1A9*22 polymorphisms than in controls. The presence of the UGT1A1*6 allele also showed a significant association with grade III-IV neutropenia. Upon haplotype and diplotype analyses, almost every patient bearing the UGT1A1*6 or UGT1A7*3 variant also had the UGT1A9*22 polymorphism, and all severe manifestations of UGT1A polymorphism-associated toxicity were related to the UGT1A9*22 polymorphism. By genotyping UGT1A9*22 polymorphisms, we could identify high-risk gastric cancer patients receiving irinotecan-containing chemotherapy, who would experience severe toxicity. When treating high-risk patients with the UGT1A9*22 polymorphism, clinicians should closely monitor them for signs of toxicity such as severe diarrhea or neutropenia.

오훼골과 견갑골이 골절된 참매의 비침습적 재활 치료 성공 사례 (Noninvasive Treatment and Rehabilitation of a Northern Goshawk (Accipiter gentilis) with Coracoid and Scapular Fracture)

  • This is a case report of a northern goshawk (Accipiter gentilis), admitted to the wildlife rescue center with right coracoid and scapular fracture which received conservative treatment and was soft-released successfully. At the admission, the goshawk had callus formed on the fractured bones, scars on eyebrows and severely damaged tail feathers with inability to fly. Cage rest was indicated to prevent further occurrence of fracture and to promote reunion of the fracture surface. The bird went through a rehabilitation process with appropriate physical training in flying cage to recover its fitness and flying ability. Complete molting of flight feathers with damaged rectrices was achieved during the rehabilitation period and its flight ability was recovered favorably. It was released into a proper habitat for northern goshawks, but the bird was found near the aviary where its last phase of rehabilitation was performed. A soft-release program was applied and finally the bird returned to near the first rescue location 235 km away from the release site.

대장균(大腸菌)의 항균제내성(抗菌劑耐性) 및 전달성(傳達性) Plasmid (Antimicrobial Drug Resistance and Transferable Resistance Plasmid in Escherichia coli)

  • 각종질환(各種疾患)으로 입원(入院)하여 항균제투여(抗菌劑投與)를 받은 환자(患者)와 최근(最近) 1개월간(個月間) 항균제(抗菌劑)를 투여(投與)받지 않은 학생(學生) 각(各) 60명(名)의 대변(大便)을 배양(培養)하여 각각(各各) 121주(株), 합계(合計) 242주(株)의 대장균(大腸菌)을 얻어 12종(種)의 항균제(抗菌제劑)에 대한 내성양상(耐性樣相)을 조사(調査)하였다. 내성빈도(耐性頻度)를 보면 tetracycline에 대(對)한 내성균(耐性菌)이 68.2%로 가장 많았고 streptomycin(Sm) 61.2%, sulfisomidine(Su) 50%, chtoramphenicol(Cm) 40.9%, ampicillin(Ap) 40.1%, carbenicillin(Cb), 39.3%의 순(順)으로 내성균(耐性菌)의 비율(比率)이 낮아졌고 kanafnycin(Km) cephaleridine(Cr) 및 trimethoprim(Tp)에 내성(耐性)인 균(菌)은 약(約) 1/4이었고 nalidixicacid, gentamicin 및 amikacin에는 4주(株)씩 만이 내성(耐性)이고 rifampicin에 내성(耐性)인 균(菌)은 없었다. 환자유래균(患者由來菌)과 학생유래균(學生由來菌)을 비교(比較)하여 보면 Cm, Ap, Km, Cr 또는 Cb 내성(耐性)은 환자유래균(患者由來菌)에서 학생유래균(學生由來菌)보다 월등(越等)히 많았으며 Sm, Su 또는 Tp 내성균(耐性菌)도 다소(多少) 많았으나 기타(其他) 항균제(抗菌劑)에 대(對)한 내성균(耐性菌)에서는 차이(差異)를 볼 수 없었다. 내성균(耐性菌)과 감수성균(感受性菌)에 대(對)한 각(各) 항균제(抗菌劑)의 최소(最少) 발육저지농도(發育阻止濃度)에는 큰 차이(差異)가 있어서 이들 내성(耐性)이 주(主)로 R plasmid에 유래(由來)함을 시사(示唆)하였다. 공시균(供試菌)의 76%가 $1{\sim}10$제(劑) 이상(以上)의 약제(藥劑)에 내성(耐性)이었는데 4제(劑) 이상(以上)의 항균제(抗菌劑)에 내성(耐性)인 균(菌)은 환자유래균(患者由來菌)에서 학생유래균(學生由來菌)에서 보다 많았으며 다약제내성(多藥劑耐性)일수록 환자유래균(患者由來菌)에서 많았다. 2제(劑) 이상(以上)의 항균제(抗菌劑)에 내성(耐性)인 균(菌)에 접합(接合)에 의한 내성전달(耐性傳達)을 145주중(株中) 98주(株)(67.5%)가 내성(耐性)을 전달(傳達)하였으며 7제(劑) 이상(以上)에 내성(耐性)인 균(菌)은 1주(株)를 제외(除外)한 전부(全部)(73주(株))가 그 내성(耐性)을 피전달균(被博達菌)에 전달(傳達)시켰고 다약제내성(多藥劑耐性)의 약제수(藥劑數)가 적을수록 전달성내성(傳達性耐性)을 가진 균(菌)이 적었다. 전달(傳達)되는 내성양상(耐性樣相)은 내성균주(耐性菌株)에 따라 다르며 피전달균(被博達菌)에 따라서도 달랐다. 임의(任意)로 선택(選擇)한 P13균주(菌株)의 R plasmid의 비적합성군(非適合性群)을 보았던바, 이 plasmid는 F II군(群)에 속(屬)함이 판명(判明)되었다. Gel electrophoresis에 의하여 대장균(大腸菌) R plasmid DNA의 형광대(螢光帶)를 증명(證明)할 수 있었다.

류마티스성 승모판막질환과 동반된 만성 심방세동 치료에 대한 변형 Maze 술식의 결과 (Outcomes of the Modified Maze Procedure for Chronic Atrial Fibrillation Combined with Rheumatic Mitral Valve Disease)

  • 배경: 본 연구에서는 류마티스성 승모판막질환과 동반된 만성 심방세동의 치료에서 냉동절제술을 이용한 변형 maze 술식들의 동율동 회복률 및 중기 결과에 대해 알아보고자 하였다. 대상 및 방법: 2000년 3월부터 2004년 2월까지 류마티스성 승모판막질환과 동반된 만성 심방세동 환자에서 승모판막수술과 동시에 냉동절제술을 이용하여 변형 maze술을 받은 177명의 환자를 대상으로 Cox-maze III (CM군, n=88), Kosakai-maze (KM군, n=63), 그리고 Left atrial maze (LA군, n=26) 세 군으로 분류하여 수술 전, 후 결과 및 동율동 회복률과 중기 결과에 대해 후향조사하였다. 결과: 술 후 병원 사망은 CM군 2명(2.3%)과 KM군 1명(1.6%)을 포함한 3명(1.7%)에서 있었으며 술 후 합병증 발생률에서 세군 간에 차이는 없었다. 총 체외순환 및 대동맥차단 시간은 CM군에서 다른 두 군에 비해 유의하게 길었다(p<0.0001). 생존자 174명에서 평균 $22.4{\pm}15.1$개월($1\sim52.6$개월)을 추적조사한 결과 만기 사망은 CM군 1명(0.6%)에서 있었다. 마지막 추적조사에서 동율동 회복률은 전체환자의 79.9%, CM군 77.9%, KM군 80.7% 그리고 LA군에서는 84.6%였다(p=0.743). 술 후 4년 동안 뇌졸중으로부터의 자유도는 CM군 $84.6{\pm}9.4%$, KM군 $95.0{\pm}:4.9%$ 그리고 LA군에서는 $92.9{\pm}6.9%$였다(p=0.916). 결론: 류마티스성 승모판막질환과 동반된 만성 심방세동 치료에서 승모판막수술과 동반하여 시행하는 냉동절제술을 이용한 변형 maze 술식들은 비교적 안전하게 시행될 수 있으며 심방세동 치료에 효과적이다.

알코올가용성이 음주행태에 미치는 영향: 도시지역을 대상으로 한 다수준 분석 (The Effect of Alcohol Availability on Drinking Behavior : A Multilevel Analysis on Urban Regions)

  • 음주와 관련한 사회적, 보건학적 문제가 심각하다. 음주행태는 개인적 요인뿐만 아니라 환경적 요인에도 영향을 받는다. 이 연구는 개인수준의 변인들을 보정 후에도 지역사회 내 주류 판매점들이 개인의 음주행태에 어떠한 영향을 미치는지 파악하여 알코올 관련 규제정책의 기초자료로 제공하고자 시행하였다. 음주행태에 영향을 미치는 지역요인을 확인하기 위해 현재 음주여부와 고위험 음주여부를 종속변인으로 하는 다수준 로지스틱 회귀분석을 실시하였다. 개인수준의 데이터는 2015년 지역사회건강조사 자료 중 도시지역에 거주하는 사람들의 응답 자료를 활용했으며, 지역수준의 데이터는 통계청에서 제공하는 자료를 활용하였다. 개인수준의 변인으로는 연령, 학력, 소득 수준 등을, 지역수준의 변인으로는 기초생활수급자수, 조 이혼율, $1km^2$당 주점 수, $1km^2$당 소매점 수 등을 활용하였다. 연구결과, 현재 음주 여부에는 상대적으로 높은 학력, 높은 월 가구소득, 자신의 건강상태에 대한 긍정적인 평가가, 고위험 음주 여부에는 상대적으로 낮은 학력, 우울감과 같은 개인적 요인들이 영향을 미치는 것으로 나타났다. 개인수준의 변인들을 보정 후 지역수준의 변인들을 분석한 결과, $1km^2$당 주점 수, 소매점 수, 전체 알코올가용성 수가 높은 지역에 거주하는 사람에게서 현재 음주를 할 가능성이 높은 것으로 나타났다. 반면, 고위험 음주 여부에 있어서는 지역변인으로 조 이혼율만이 음주행태에 영향을 미치는 것으로 나타났다. 개인적 특성(성, 연령, 경제활동 유무)에 따라 층화 분석한 결과에 따르면, 현재 음주 여부에서는 차이가 없었지만, 60세 이상 집단에서 소매점과 전체 알코올가용성 수가 많은 지역에 거주할수록 고위험 음주의 가능성이 높아지는 것으로 나타났다. 이 연구의 결과는 개인을 대상으로 하는 예방정책 뿐만 아니라 지역 수준에서 알코올 가용성 환경을 고려한 알코올 규제 정책이 필요함을 시사한다.
