COVID-19 이환자의 브루스 프로토콜을 이용한 유산소 운동이 심박수, 산소포화도, 혈압에 미치는 영향 (Effect of Aerobic Exercise using Bruce Protocol on Heart Rate, Oxygen Saturation, and Blood Pressure after Recovery from COVID-19 Infection)

  • Purpose : This study aimed to find out the effect of aerobic exercise using Bruce protocol on heart rate, oxygen saturation, and blood pressure after recovery from COVID-19 infection. Methods : In this study, 34 students from D University located in J city were targeted, COVID-19 infected group (17 people) and non-COVID-19 non-infected group (17 people). The Bruce protocol using a treadmill was applied to the aerobic exercise of this study. The Bruce protocol has in the first stage of METs 4 (slope 10 %, speed 2.7 km/h). The second stage was METs 5 (slope 12 %, speed 4 km/h), and the third stage was METs 6 (slope 14 %, speed 5.4 km/h). All measurements were measured 3 times and the average value was used. Results : As a result of this study, as a result of comparing heart rate changes according to aerobic exercise using the Bruce protocol. EG group and the CG group increased significantly according to the progressive exercise load (METs 4~5), and in the third stage of the Bruce protocol between groups, The EG group showed a significantly lower heart rate. As a result of comparing changes in oxygen saturation and blood pressure, there was no significant difference between the EG and C groups according to the gradual exercise load. Conclusion : In conclusion, there was no difference between normal and pressure when MET of moderate intensity exercise (4 to 6) was applied to the effect on heart rate, oxygen saturation, and blood pressure in healthy adults who were fully recovered from COVID-19. Secondary side effects may occur when high intensity exercise with a MET of 6 or higher, so it is strongly recommended that hospitals or specialized institutions measure exercise and physical ability according to individual exercise intensity.

Can Data-Driven Analysis Demonstrate the Plausibility of Traditional Medical Typology?

  • Objectives: Although medical typologies based on indigenous biopsychological ideas have been described, their integrity has been questioned due to its theory-driven nature in categorization. Therefore, studies on the Sasang typology, a temperament-based traditional Korean medicine, are needed to examine whether it is possible to classify types of specific biopsychological profiles using data-driven analysis. Methods: Psychological measures of the Eastern Sasang Personality Questionnaire (SPQ) and Western NEO-Personality Inventory (NEO-PI) along with physical measures and Sasang types were acquired from 2,049 participants. Latent groups based on the SPQ and NEO-PI subscale scores were extracted using Latent Profile Analysis. Their psychosomatic features were then compared with those of Sasang types. Results: Three SPQ-based latent groups showed distinctive psychological and physical features consistent with those of Sasang types. However, four NEOPI-based latent groups presented only psychological features. Furthermore, SPQ-High and SPQ-Low latent groups demonstrated similar psychosomatic profiles to those of So-Yang and So-Eum Sasang types, respectively. Conclusions: This study illustrates that biopsychological profiles of Sasang types are supported by psychosomatic features of latent groups based on SPQ of Eastern psychology, signifying that the categorization of Sasang typology have acceptable validity and reliability.

Acupuncture for Attenuating Frontal Lobe α Band Asymmetry Induced by Anger: a pilot study

  • Objectives: Previous studies have shown that anger can lead to frontal lobe α (8-13 Hz) band asymmetry (FAA) in electroencephalogram (EEG), in accordance with motivational direction. This pilot study aimed to investigate the impact of acupuncture on FAA elicited by anger. Methods: Thirty-four right-handed participants scoring above 75 points on the Novaco Anger Scale were included. Baseline EEG signals were recorded for eight minutes using a 32-channel cap under comfortable conditions. Anger was induced through a nine-minute sequence of Articulated Thoughts in Simulated Situations (ATSS) task. Following that, participants received acupuncture at GB20 and GB21 for 10 minutes. Fast Fourier transform was employed for frequency analysis, and repeated measure ANOVA was conducted for statistical analysis. Results: The results revealed that participants exhibited significantly higher FAA (p = 0.026), particularly in the left hemisphere, after the ATSS task sequence compared to the baseline. During acupuncture treatment, the greater left-sided FAA was significantly reduced (p = 0.027) and reversed. Upon the cessation of acupuncture, FAA returned to a value between the baseline and the anger-evoked stage (p = 0.046). Conclusion: The EEG results of this study revealed that anger stimulation induced an increase in left-sided FAA, which was effectively alleviated by acupuncture. This led to an immediate restoration of FAA asymmetry induced by anger. These findings suggest the potential of acupuncture as a treatment option for reducing FAA associated with anger.

폐경 후 고혈압 전단계 및 1기 고혈압에 대한 침 치료 효과: 무작위 대조 예비연구 (Acupuncture for Prehypertension and Stage 1 Hypertension in Postmenopausal Women: A Randomized Controlled Pilot Trial)

  • Objectives : The aim of this study was to assess the effectiveness of acupuncture in treating prehypertension and stage 1 hypertension in postmenopausal women. Methods : The study was a multi-center, four-arm, non-blinded, randomized clinical trial. Sixty participants were randomly assigned to experimental or control groups. The experimental groups received 10 acupuncture sessions over 4 weeks(Group A, n=20) or 20 sessions over 8 weeks(Group B, n=20) along with usual care. The acupoints were GB20, LI11, ST36, and SP6, bilaterally. The acupuncture groups were followed-up for an additional 12 weeks after acupuncture treatment. The control groups received usual care for 16 weeks(Group C, n=10) or 20 weeks(Group D, n=10). The outcomes were blood pressure, blood pressure control rates, lipid profile, and high-sensitivity C-reactive protein(hs-CRP). Results : After 4 weeks, DBP in the acupuncture groups(A+B) showed no significant decrease compared to the control group(C+D). However, after 8 weeks of acupuncture treatment, group B showed a significant decrease in DBP after acupuncture treatment and follow-up period compared to control group. Although there was no difference between the acupuncture and control groups in SBP after acupuncture treatment, group B showed a significant decrease in SBP compared to control group after follow-up period. Lipid profiles and hs-CRP did not differ significantly between acupuncture and control groups. Conclusions : Acupuncture treatment for 8 weeks showed the effect on prehypertension and mild hypertension. To verify the effect of acupuncture on blood pressure, rigorous trials including more participants are required.

Management of asymptomatic to mild COVID-19 patients with Cheongpebaedok-tang on the telemedical basis: A retrospective observational case series

  • Objectives: This retrospective observational study aimed to investigate the efficacy and safety of Cheongpebaedok-tang, a traditional Korean herbal medicine, provided via telemedicine to patients with asymptomatic to mild COVID-19 in Korea. Methods: From February to April 2020, a retrospective analysis investigated COVID-19 patients treated via Korean telemedicine. The study involved asymptomatic to mild cases receiving Cheongpebaedok-tang more than three times, along with continuous Korean medicine care in convalescence. Diagnoses and treatment adhered to the telemedicine guidelines of the Association of Korean Medicine, with varied Cheongpebaedok-tang prescriptions based on symptom severity. Symptom evaluation involved a detailed assessment using a 15-item tool at initial and final sessions. Results: The study included 27 patients, with a mean age of 48.7 ± 2.3 years (mean ± standard error). Patients began self-administering oral Cheongpebaedok-tang for an average of 19.4 ± 1.8 days after the date of COVID-19 diagnosis confirmation and continued the medication for 15.8 ± 1.2 days. The reported side effects of the Cheongpebaedok-tang included palpitations (11.1%), insomnia (7.4%), dizziness (3.7%), and diarrhea (3.7%). All side effects disappeared after adjusting the prescription according to standard treatment guidelines. The occurrence of all COVID-19-related adverse symptoms, except fatigue and myalgia, decreased. Fatigue was the most common chronic symptom persisting after 6 months (51.9%), followed by ocular symptoms (37.0%) and sore throat (22.2%). Conclusions: This study implies Cheongpebaedok-tang may offer a potentially safe, symptom-alleviating approach for managing mild COVID-19 cases via telemedicine, although further comprehensive research is warranted.

Proteomic change by Korean Red Ginseng in the substantia nigra of a Parkinson's disease mouse model

  • Background: Recent studies have shown that Korean Red Ginseng (KRG) successfully protects against dopaminergic neuronal death in the nigrostriatal pathway of a Parkinson's disease (PD) mouse model induced by 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) administration; however, the mechanism has yet to be identified. Therefore, in this study we used two-dimensional electrophoresis to investigate the effects of KRG on the changes in protein expression in the substantia nigra (SN) of MPTP-treated mice. Methods: Male C57BL/6 mice (9 wk old) were intraperitoneally administered MPTP (20 mg/kg) four times at 2-h intervals, after which KRG (100 mg/kg) was orally administered once a day for 5 d. Two hours after the fifth KRG administration, a pole test was conducted to evaluate motor function, after which the brains were immediately collected. Survival of dopaminergic neurons was measured by immunohistochemistry, and protein expression was measured by two-dimensional electrophoresis and Western blotting. Results: KRG alleviated MPTP-induced behavioral dysfunction and neuronal toxicity in the SN. Additionally, the expression of eight proteins related to neuronal formation and energy metabolism for survival were shown to have changed significantly in response to MPTP treatment or KRG administration. KRG alleviated the downregulated protein expression following MPTP administration, indicating that it may enhance neuronal development and survival in the SN of MPTP-treated mice. Conclusion: These findings indicate that KRG may have therapeutic potential for the treatment of patients with PD.

한약 정보 표준화를 위한 의약품 식별 표준 (IDMP) 분석 및 고찰 (Review of Identification of Medicinal Products (IDMP) Standards for Standardization of Herbal Medicine Information)

  • Objectives : The purpose of this study was to apply informations related to herbal medicines to IDMP (Identification of Medicinal Products), an ISO standards related to medicinal products substances, for systematic collection of data through the integration of informations on distribution, manufacturing, and management of herbal medicines. Methods : By analyzing ISO 11238 and ISO/TS 19844, elements that can be used in the information model of herbal medicine were derived from the identification of medicinal products information model on substances. The labeling specified in the safety and quality control regulations for herbal medicines was mapped to the IDMP information model, and ginseng was applied as an example. Results : Herbal medicine corresponded to substance in IDMP. Among the five types of substances specified by IDMP, herbal medicines were expressed as structurally diverse. Scientific name was used as an invariant property of herbal medicine, and the substance level included information about source material and modification, and specifically included information about the scientific name, medicinal part, fraction, and processing. In addition, the specified substance level had information on the constituents, characteristic attributes, manufacturing, and grade of the herbal medicine. Conclusions : It is necessary to establish a code system for identifying herbal medicines. In order to apply the IDMP standards, research on the development of standard terms is required to express the characteristics of herbal medicines. In addition, information for identification of herbal medicines is also required, and information from production to consumption should be systematically accumulated and managed for actual application.

Transport of Tetraethylammonium in Renal Cortical Endosomes of Cadmium-Intoxicated Rats

  • Effects of cadmium (Cd) intoxication on renal endosomal accumulation of organic cations $(OC^+)$ were studied in rats using $^{14}C-tetraethylammnium$ (TEA) as a substrate. Cd intoxication was induced by s.c. injections of 2 mg Cd/kg/day for $2{\sim}3$ weeks. Renal cortical endosomes were isolated and the endosomal acidification (acridine orange fluorescence change) and TEA uptake (Millipore filtration technique) were assessed. The TEA uptake was an uphill transport mediated by $H^+/OC^+$ antiporter driven by the pH gradient established by $H^+-ATPase.$ In endosomes of Cd-intoxicated rats, the ATP-dependent TEA uptake was markedly attenuated due to inhibition of endosomal acidification as well as $H^+/TEA$ antiport. In kinetic analysis of $H^+/TEA$ antiport, Vmax was reduced and Km was increased in the Cd group. Inhibition of $H^+/TEA$ antiport was also observed in normal endosomes directly exposed to free Cd (but not Cd-metallothionein complex, CdMt) in vitro. These data suggest that during chronic Cd exposure, free Cd ions liberated by lysosomal degradation of CdMt in proximal tubule cells may impair the endosomal accumulation of $OC^+$ by directly inhibiting the $H^+/OC^+$ antiporter activity and indirectly by reducing the intravesicular acidification, the driving force for $H^+/OC^+$ exchange.

  • Slices of rat corpora lutea(CL) incubated with. prostaglandin $F_{{2{\alpha}}}(PGF_{2{\alpha}})$ in Krebs-Hensenleit (K-H) Ringer solution showed a decrease in $Na^+-K^+$-ATPase activity after 60 min of incubation. However, $PGF_{2{\alpha}}$ in vitro did not alter $Na^+-K^+$-ATPase activity of isolated luteal membrane fractions. Following $PGF_{2{\alpha}}$ induced in vivo luteal regression, reduction of Vmax an elevation of the activation energy above transition temperature of the lipid phase of the membrane occurred without changes of Km, optimum pH and transition temperature. These results suggest that reduction of $Na^+-K^+$-ATPase activity after $PGF_{2{\alpha}}$ treatment may be due to the reduction of the number of enzyme molecules or to masking of the active site of the enzyme without any change in enzyme characteristics. In addition, a change in membrane bound enzyme activity may be an early step in $PGF_{2{\alpha}}$ induced luleolysis.

한약의 암 환자에 대한 면역기능 개선 효과 : 체계적 문헌고찰과 메타분석 프로토콜 (Herbal Medicines for the Improvement of Immune Function in Patients with Cancer: A Protocol for Systematic Review and Meta-Analysis)

  • 목적: 암 환자 치료에 대한 반응률은 종양 세포가 면역 회피 반응을 일으키면서 떨어지게 된다. 방사선 치료나 항암 치료 같은 표준 치료들은 병을 고치거나 환자들의 생명을 연장하는 목적으로 쓰이나 그 독성이나 부작용으로 인해 효과가 제한되고 있다. 한약은 아시아 지역에서 활용되고 있는 대표적인 보완 의학이며 면역 작용을 증강시키기 위해 널리 사용되고 있다. 이 프로토콜은 향후 암 환자들의 면역 기능을 개선시키는 데 있어서 한약의 객관적인 유효성을 체계적 문헌 고찰로 평가하고자 한다. 방법: 검색할 데이터베이스는 다음과 같다: MEDLINE via PubMed, EMBASE via Elsevier, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure(CNKI), Korean databases including Regional Information Sharing Systems(RISS), National Digital Science Library(NDSL), Oriental Medicine Advanced Searching Integrated System(OASIS). 한약을 이용해 암 환자들의 면역 기능 개선을 다룬 전향적 무작위 대조 연구들을 포함시키고, 면역 기능과 관련된 모든 지표들(CD3, CD4, CD8, CD4/CD8 비율, CD19(B세포), CD11(수지상세포), CD56(NK세포), 대식세포 등)을 분석한다. 결과: 암 환자의 면역 기능과 관련된 지표들을 포함시켜 체계적 문헌 고찰과 메타 분석을 수행할 예정이다. 결론: 한약의 암 환자 면역 기능 개선 유효성을 객관적으로 알리고, 본 연구를 통해 환자와 의료진에게 암 치료에 있어서 새로운 선택지를 넓힐 수 있도록 방향을 제시하고자 한다.
