Research on Immune Enhancing Effect and Safety of Wasong (Orostachys japonicus) Extract: Study Protocol for a Single Center, Randomized, Double-blind, Placebo-controlled, Clinical Trial (와송 추출물의 면역기능 개선 효과 및 안전성 연구: 단일기관, 무작위배정, 이중눈가림, 위약대조 비교, 임상연구 프로토콜)

  • Objectives : This trial aimed to determine if Wasong (Orostachys japonicus) extract can enhance immune system and is safe enough to be approved as a health functional food. Methods : Total 62 people, aged 45 and older, will be recruited to participate in a randomized, double-blind, placebo-controlled clinical trial. This study will compare Wasong extract and placebo. Wasong group will take 1g of Wasong extract, once a day, for 8 weeks. Placebo group will take 1g of crystalline cellulose as placebo, once a day, for 8 weeks. Outcomes will be measured at the baseline, the end of 4th week, and 8th week. Primary outcomes are the ratio of NK cells/total lymphocytes and the ratio of T-helper cells/T-suppressor cells. Secondary outcomes are total white blood cell count, the ratio of neutrophils, lymphocytes, and monocytes in total leukocytes, the ratio of total T cells, T-helper cells, T-suppressor cells, and B cells to lymphocytes, the amount of blood IgM, IgG, IgA, and cytomegalovirus (CMV) IgG, and blood metabolite target &global analysis. Results : This trial was approved by institutional review board of Pusan National University Korean Medicine Hospital (registry number: 2016006), and registered in Clinical Research information Service, one of WHO International Clinical Trials Registry Platform (registry number: PRE20161006-002). Recruitment opened in February 2017 and is supposed to be completed by August 2017. The result is expected to be published by June 2018. Conclusion : This trial will provide clinical information to determine the efficacy and safety of Wasong in enhancing immune system of middle-aged and older people.

  • Objectives : The somatization is various physical symptoms that have no pathologic manifestation. It is very common to Korean children and adolescents as well as adults, because cultural background and immature verbal expression. DSM-Ⅳ(Diagnostic and Statistical Manual of Mental Disorders Fourth Edition) presented diagnostic criteria for Somatization disorder by grouping some symptoms out of somatization. This report is on a case of somatization disorder patient. The patient is 13-year-old female. She has been suffered from various physical symptoms those are coincided with diagnostic criteria for Somatization disorder by DSM-Ⅳ. Methods : The authors treated her by acupuncture and 3 kinds of herb medicine-Bunsimgium, Hyungbangdojuksan, Yangguksanwhatang. Results : Bunsimgium was showed a little improvement but could not solved chief complaint. Hyungbangdojuksan was not given a help. After her taking Yangguksanwhatang, most symptoms were subsided. Conclusions : A patient with the somatization disorder was improved by herbal medication and acupuncture. But the Somatization disorder repeats to take a good turn and a bad turn and continues for long time, so we need to follow up her condition.

  • Crohn' disease is an auto-immune disease characterized by intermittent chronic diarrhea, high fever, weight loss, abdominal spastic pain or abdominal discomfort which is followed by granulomatous necrosis and cicatrical inflammation. It is also called segmental enteritis or granulomatous enteritis. In western medicine the exact cause is undefined, however it is presumed as an immunological unbalance in alimentary tract commoonly occured in ileum portion of small intestitine or ascending colon and therefore immuno suppressive agents(usually steroids) and anti-inflammatory drugs are prescribed. In case of emergency such as ileus, perforation of intestinal wall surgical methods are considered. In oriental medicine this falls under the category of diarrhea(泄瀉), dysentery(痢疾), splenic diarrhea(脾泄). As to the pathological mechanism the abnormal ascending and descending circulation of stomach and splenic energy(脾不升淸, 胃不下降) the hepatic stagnation(肝鬱氣滯) and dysfunction of small intestine in expelling urine and feces(小陽淸獨不利) all together causes such condition. Main treatments are inducing diuresis(利小便), warming kindey to reinforce yang(溫賢助陽), nourishing the middle energy to invigorate spleen(補中健脾), elimination of the dampness by cooling(淸熱燥濕). In this case the patient was diagnosed as soyangyin(少陽人) constitution and herb medicine soyangyin Hyongbangjihwan-tang(少陽人 荊防地黃湯), Sa-am acupuncture Sojangjeonggyeok(小腸政格) was applied. There was an significant improve in chief complaints and general conditions.

The Statistical Study of Dermatoses Outpatients in Oriental Hospitals Located in Gwangwon Province (강원지역 소재 한방병원 피부과 외래환자의 임상적 분석)

  • Objective : The purpose of this study is to expand our treatment field in oriental dermatology by understanding the patterns of outpatients who had visited the oriental hospitals located in Gangwon province. Methods : We studied 510 outpatients out of 2240 people from Sangji University Oriental Hospital(SUOH) and 333 outpatients out of 2030 people from Chuncheon Oriental Hospital(COH) from January 1, 2007 to December 31, 2009. We categorized the results by gender, age, disease types, residences, months and number of visiting times. Results : The results were as follows 1. Comparing the number of outpatients that had visited the hospital by gender in SUOH shows that female's visiting rate is 57% and male is 43%. In COH shows that female's visiting rate is 54% and male is 46%. 2. Most frequent visiting age group were 21-30 year-old group(30%) in SUOH and 11-20 year-old group(23%) in COH. 3. In regard to the monthly visiting period, January(13%) is the highest in SUOH and July(10%) is the highest in COH. 4. Regional distributions of the outpatients that had visited SUOH and COH show Wonju(73%) and Chuncheon(74%). 5. In terms of treatment methods, SUOH showed the external application(65%), herb medication(54%), and acupuncture(52%) while COH showed acupuncture(74%), the external application(69%), and herb medication(66%). 6. Visiting frequency of outpatients in SUOH showed that 1 time visiting cases were 51%. In COH case, 1 time visiting cases were 19%. 7. The most prevalent diseases were urticaria(15%), eczema(14%) acne(12%), and seborrheic dermatitis(11%) in SUOH and atopic dermatitis(27%), urticaria(20%), acne(16%), and psoriasis(10%) in COH. 8. Most outpatients had visited SUOH for 1 time(51%) and COH for over 11 times(32%) with the major 16 diseases. Conclusions : This study suggests that outpatients who had visited the department of oriental dermatology preferred to get a treatment for urticaria, eczema, atopic dermatitis and acne. Considering this aspect, it needs to improve current research to expand the oriental medical treatments further in dermatological field.

The follow-up study on patients of Amyotrophic lateral sclerosis after 1 year (근위축성 측삭경화증 환자 1년 후 추적 조사)

  • Objectives : This study was to investigate the effects of Oriental medical treatment on ALS. Methods : We investigated 12 ALS patients which were admitted to Gwang-Ju O.M.hospital for 3 months and follow-up at 8 months after discharge. All patients were treated by SAAM-acupuncture, herb medication, Bee venom Pharmacopuncture therapy, Needle-embedding therapy, etc and after discharge self-therapy at home. We evaluated patients using the Amyotrophic lateral sclerosis Functional Rating Scale-Revised(ALSFRS-R), Medical Research Council (MRC) Scale. Results : After 1 month, mean ALSFRS-R score of patients was $29.08{\pm}7.99$, 2 months $28.70{\pm}7.17$, 3 months $28.16{\pm}8.23$, 1 year $21.33{\pm}9.93$ and mean MRC Scale of patients was $25.34{\pm}8.45$, 2 months $25.34{\pm}8.45$, 3months $21.56{\pm}9.20$. But in both cases, the variation was not statistically significant. Conclusions : We think that the results of this case be a pilot study that proves the effect of Oriental Medical treatment on ALS.

Retrospective Study about the Effectiveness of Topical Preparations Containing 12 Herbal Extracts on Atopic Dermatitis Patients (선태, 백지 외 10종 한약재 추출물 함유 외용한약의 아토피 피부염 치료효과에 대한 후향적 연구)

  • Objectives : The purpose of this study is to examine whether the topical preparations containing 12 herbal extracts is effective for patients with atopic dermatitis. Methods : We retrospectively reviewed the medical records of 11 patients who were prescribed topical preparations containing 12 herbal extracts on the outpatient clinic of 4 hospitals; Noble Korean Medicine Clinic, Amar Korean Medicine Clinic, Chung-Yeon Korean Medical Hospital, Donggwangju Chung-Yeon Korean Medical Hospital. Results and Conclusions : A total of 11 patients were included in the selection and exclusion criteria. The OSI mean value before treatment and at the last treatment day decreased significantly from $37.03{\pm}16.75$ to $13.32{\pm}9.83$, and the mean value of EASI decreased from $13.33{\pm}12.85$ to $2.78{\pm}5.00$, which was statistically significant. No safety concerns were reported. The results of this study confirmed the possibility that the topical preparations can be effective in moderate to severe infants, adolescents, and adult atopic dermatitis patients.

A Clinical Case Report of Bilateral Facial Nerve Palsy (양측 동시형 안면신경 마비 환아 1례 보고)

  • Objectives The purpose of this study is to report a pediatric patient with bilateral facial nerve palsy. Methods The patient was treated by acupuncture, moxa, herb(Cheongungkyeoji-tang, Palmulgunja-tang) for 1 month. We described his progression and measured the degree by House-Brackmann scale and Yanagihara's scale. Results He started getting better from 7th day of the onset. During admission, the facial palsy had improved from Grade V/V(Rt/Lt) to Grade II/II(Rt/Lt) by House-Brackmann scale and from 0/0(Rt/Lt)score to 36/36(Rt/Lt)score by Yanagihara's scale. Conclusions Compared with unilateral facial nerve palsy cases, this case was not different from the time when it got better, and how much it improved. However, biased degree had increased and then decreased because of the time gap between the right facial improvements and left facial one.

A Case of Hepatotoxicity by Salvia Plebeia R. Br.

  • Objective: To investigate the possibility of hepatotoxicity by supplemental foods or folk herbs such as Salvia Plebeia R. Br. Methods: A hospitalized male patient with alcoholic liver disease and electrolyte imbalance had recovered, and then followed by rapid hepatic serum enzymes after taking S. Plebeia. This study monitored the clinical outcome and biochemical parameters. Result: A 58-year male had drunk frequently, which led to alcoholic steatohepatitis and hospitalization. Two weeks after his discharge from hospital, he felt nausea, dizziness, and mild difficulties in speech and walking, resulting in re-hospitalization at the Korean Medical Hospital. The symptoms disappeared on correction of the electrolyte imbalance suspected to have been caused by severe sweating while working in the outdoors, and the patient was discharged. During treatment and monitoring of his health as an outpatient, the serum hepatic enzyme rapidly elevated approximately 10-fold in hepatic enzymes; the enzyme levels fluctuated according to whether or not he was taking the boiled water of S. Plebeia. The RUCAM score was 12, which met the criteria for toxic hepatitis by S. Plebeia. His general condition and abnormal hepatic enzymes recovered with cessation of S. Plebeia and administration of Chungganplus syrup (CGX). Conclusion: This study reports the hepatotoxic risk of Salvia Plebeia, which is commonly used as a folk remedy in Korea.

  • Objective & Methods : Irritable Bowel Syndrome is occurred frequently in daily life. Nevertheless. medical treatment of the Irritable Bowel Syndrome is almost dependent on western cure, but that cure is not effective enough. So we chose the oriental medicine textbook that were dealing with the oriental concept and the treatment of Irritable Bowel Syndrome. we got these results. Results were as follows : 1. Irritable Bowel Syndrome is characterised by recurrent or chronic abdominal pain, with distension, disturbed defecation and psychic problems without organic lesions. 2. The etiology of Irritable Bowel Syndrome include stress. disharmony of liver and spleen, the coldness of spleen and kidney and the lack of qi or yin, etc. 3. The oriental treatments of Irritable Bowel Syndrome are warming spleen and kidney, easing liver and stopping diarrhea. 4. The internal medication of Irritable Bowel Syndrome was the most used 11 times Tongsayobang(痛瀉要方) and 10 times Yijungtang(理中湯). 5. The most many used herb were Atractylodjs macrocephalae rhjzoma(白朮), Gjnseng Radix (人蔘), Cjnnamomj Cortex(肉桂), Polyporus(猪笭), etc.

A Case Report of Urinary Infection in Hyperemesis Gravidarum (요로감염을 동반한 임신악조(姙娠惡阻) 환자의 치험 1례(例)에 대한 임상보고)

  • Purpose: This study is to report of the effect of oriental treatments to urinary infection in hyperemesis gravidarum. Methods: Hyperemesis gravidarum is a severe and intractable from of nausea and vomiting in pregnancy. And urinary infection is frequent medical cause of hospitalization, during the course of pregnancy. Hyperemesis gravidarum and urinary Infection is associated with risk to the mother and fetus. We treated a patient who had urinary infection in hyperemesis gravidarum at Se- Myung university Oriental Hospital in affiliation by herbal medication, acupuncture and moxibustion. After the treatment, we got good result. Results: After the treatment, symptoms are remarkably alleviated. Conclusion: It is effective to cure the urinary infection in hyperemesis gravidarum through the herb-medication and acupuncture treatment.
