EARLY ORTHOPEDIC TREATMENT IN CLEFT LIP AND PALATE PATIENT: A CASE REPORT (구순구개열 환아의 조기 악정형치료에 관한 증례)

  • Cleft lip and palate is the most common malformation in the craniofacial region. The patients with cleft lip and palate have functional problem such as, speech, feeding and respiratory as well as esthetic problem. So, treatment should be done by multidiciplinary team approach. The role of pediatric dentist in the team is advicement for feeding method, guidance of normal growth, caries control and preventive orthodontics. In cleft lip and palate patients, maxillary arch after cheiloplasty is usually collapsed by excessive tension of the scar. This collapse increase the difficulty of later orthodontic treatment. Therefore, the maxillary arch segments should be moved and retaind to normal position as soon as cheiloplasty is done to reduce the need and difficulty of orthodontic treatment. This concept is called by the early orthopedic treatment in cleft lip and palate. Also, this orthopedic appliance works as feeding applince to normal feeding and weight gain We reported two cases of early orthopedic treatment with favorable result in complete bilateral cleft lip and palate patients after cheiloplasty. Patients showed normal weight and their maxillary arch widths were increased.

  • In dental roentgenograph it is of mutual benefit to the patient and the dentist to use the minimum amount of radiation capable of producing roentgenograms with maximum interprtive informations. Recent increases in the number of diagnostic x-ray examinations made in this country have caused attention to be paid to the quantity of radiation delivered to the population and operator. The purposes of this study was to assess the quality of dental x-ray units, the amount of films, the average processing procedures and the radiation protection methods in korea. The results were as following: 1. Most of radiation generating system were used in low voltage such as 60Kvp, 10㎃. 2. High sensitivity films such as 'D' group of Kodak or Rinn were mainly used and average 14.7 films used per weeks. 3. Some dentists practiced visual processing technic in simple dark room, and others used instant films. 4. 68.26% of patient held the film themselves, but 27.30% were assisted by dentists film holding. 5. In radiation protection method, 7.85% of dentists had protection equipments such as protective barrier, 2.73% wore protective apron, 27.9% made it a rules to avoid beam, and 7.51% used to maintain a distance from the radiation source.

Infection control awareness and performance of dental Hygienist (치과위생사의 감염관리 인식과 수행)

  • Objectives: The purpose of this study is to investigate awareness and performance of infection control by dental hygienist. Methods: A self-reported questionnaire was answered by 239 dental hygienists working in dental clinics from September 7 to 18, 2016 in Gwangju city & Jeonnam region after explaining the purpose of the study and receiving the informed consent. The questionnaire consisted of sociodemographic characteristics and awareness and performance of infection control using Likert 4 point scale. Results: Average of Infection control awareness was $3.33{\pm}0.38$, average of infection control Performance was $3.04{\pm}0.37$ (p<0.05). The awareness and practice of dental equipment management showed the largest difference. The awareness of infection control by general characteristics varied according to position, number of dental units, number of employees, Infection control needs, Received infection control training experience, infection experience, infection control guideline (p<0.05). The relationship between infection control awareness and Infection control performance was r = 0.624 (p<0.001), indicating positive correlation. Conclusions: To improve awareness and performance of dental hygienist's infection control, regular received infection control training experience is needed. It is considered that the dentist's active cooperation is needed in shortening the life span of the dental equipment due to the Infection control performance and securing the quantity.

A study of hospital determinants and treatment satisfaction of patients in an orthodontic clinic, specifically targeting 20-year-old patients (교정치과 내원 환자들의 치료병원 결정요인 및 치료 만족도에 관한 연구-20대 연령층을 대상으로)

  • Objectives: The purpose of this study was to identify factors affecting choice of treatment hospital (i.e., determinants) and satisfaction with the treatment hospital, and to provide a more satisfactory and high-quality medical service for orthodontic patients. Methods: A questionnaire survey was conducted for approximately 1 month beginning in August 2017, involving patients who visited orthodontic dentistry clinics in Seoul and Gyeonggi-do. Multiple regression analysis was performed on the results to examine factors that affected satisfaction with orthodontic treatment. Results: The determinant of dental clinics was 3.90 points overall; hospital environment and facilities were highest at 4.05, followed by dental hygienist at 3.99, and dentist at 3.97. Factors influencing satisfaction with orthodontic treatment were positively influenced by dentists (p<0.01), medical procedures (p<0.01) and medical expenses (p<0.01). Conclusions: It is important to provide comfort and convenience to patients by simplifying hospital facility management, reception, and reservation procedures.

A study on the recognition of dental implant of dental patient in the city of Jin-ju (J시 일부 치과 내원환자의 치과 임플랜트 인식도 조사)

  • The purpose of this study was to provide basic data of education program and consult for dental implant patient. The 210 of dental patients were selected at five dental clinic located in Jin-ju city. Data were collected by self-administrated questionnaire. A SPSS 10.10 for Windows was used for statistical analysis. The results of the study are summarized as follows: 1. The female responser was 56.2%, the rate of age was thirty years old 26.7%, education level was more than university and college 45.7%, job is company unit 15.2%, income was 100~200man won of 48.6%. 2. Recognition of dental implant was that incase of 'yes' is more than sixty years old 77.3%. 3. The information of implant was from neighborhood 29.0%, and when dental consult 24.3%. 4. The responsor who is everyone can take the dental implant operation is 39.5%, can not take is 32.9%. 5. The responsor who is everyone wants implant operation was 45.2%, 'do not' is 17.1%. 6. Using period of implant tooth is 'forever' 40.0%. In limited of twenty years' is 23.8%. 7. When everyone select to dental clinic was because of dentist medical technique is 64.3%, dental treatment cost is 15.9%. In conclusion, it is necessary for the dental implant patient to make dental consult and education program. So, try to study for make basic data with analysis recognition of dental implant patients.

Related factors to satisfaction of patients visiting dental hospital and clinics (치과 병·의원 이용 환자의 만족도 관련요인)

  • Objectives : The quality of service is considered as the difference between services that customers perceive and expect by using SERVQUAL model and the basic data for the efficiency of management of dental hospital and clinics, the differentiation strategies of dental medical institutions and the improvements on quality of service are provided. Methods : Subjects were 469 patients who visited six dental hospitals and clinics in Daegu-Gyeongbuk regions. Questionnaire consisted of five items such as type, reliability, responsiveness, certainty and empathy properties. Data were analyzed using SPSS 12.0 program in this study. Results : Women had high satisfaction scores of medical services in certainty and empathy properties. 40s and higher had the highest score in the type property. 30s had high score in the certainty property. For expected service satisfaction, 30s had the highest score in dental practitioners and administrative staffs. For reuse of current medical institution and intent for recommendation, certainty property and factor of dentist and dental practitioners had high scores. Additionally, reliability and responsiveness properties were statistically significant. Conclusions : Medical institutions should make every effort to get the dentists or dental practitioners have the medical knowledge at a high level, a kindness for patients and trust from patients.

  • Medical emergencies are not rare in dental practice but when an emergency does occur it can be life-threatening. Medical emergency such as hypertensive crisis can be encountered in dental practice and the dentist should be competent in emergency management by taking life support courses. This report shows 3 cases of hypertensive crisis during dental treatment and we discuss proper management of the patients. The medical and dental management for the hypertensive crisis included a correct diagnosis, pharmacological usages for pain control and stress relieving environment making.

  • Inferior alveolar nerve block anesthesia is one of the most common procedures in dental clinic. Although it is well known as safe procedure, complications always can be occurred. Ocular complications such as diplopia, loss of vision, opthalmoplegia are very rare, but once it happens, dentist and patient can be embarrassed and rapport will be decreased between them. We experienced one diplopia case after inferior alveolar nerve block anesthesia and treated without any further complication. We report this case and describe the cause, diagnosis, and treatment objectives of diplopia caused by inferior alveolar nerve block anesthesia.

COMPARATIVE STUDY OF THE MACROSCOPIC FINDING, CONVENTIONAL TOMOGRAPHIC IMAGING, AND COMPUTED TOMOGRAPHIC IMAGING IN LOCATING THE MANDIBULAR CANAL (하악관 위치 분석시 일반단층사진과 전산화단층사진의 비교연구)

  • The purpose of this study was comparision of conventional tomography with reformatted computed tomography for dental implant in locating the mandibular canal. Five dogs were used and after conventional tomographs and reformatted computed tomographs were taken, four dentist traced all films. Mandibles were sectioned with 2mm slice thickness and the sections were then radiographed(contact radiography). Each radiographic image was traced and linear measurements were made from mandibular canal to alveolar crest, buccal cortex, lingual cortex, and inferior border. Following results were acquired : 1. Reformatted computed tomographs were exacter than conventional tomography by alveolar crest to canal length of -0.6mm difference between real values and radiographs. 2. The average measurements of buccal cortex to mandibular canal width and lingual cortex to mandibular canal width of conventional tomographs were exacter than reformatted computed tomographs, but standard deviations were higher than reformatted computed tomographs. 3. Standard deviations of reformatted computed tomographs were lower than conventional tomographs at all comparing sites 4. At reformatted computed tomography 62.5% of the measurements performed were within. : filmm of the true value, and at conventional tomography 24.1% were. 5. Mandibular canal invisibiity was 0.8% at reformatted computed tomography and 9.2% at conventional tomography. Reformatted computed tomography has been shown to be more useful radiographic technique for assessment of the mandibular canal than conventional tomography.

  • The purpose of this study is to inquire about the effects of individual tray design and impression method on the border extension and contour of denture. 10 students at the dentistry college who have normal occlusion and symmetric facial form and normal facial muscle activity were selected. The tray was designed three types (large, medium, small tray). After border molding was done by dentist and operator, final impression was taken. The length of impression body and width of border were measured by a vernier caliper on the cast. The obtained results were as follows : 1. As the tray border was shortened, the length of impression body and width of border was short (P<0.01). 2. When the impression was taken with the small tray, the length of impression body was short in passive border molding than active border molding. 3. When the medium tray was used, the width of impression body was short in passive border molding than active border molding.
