한국 간호지식체의 생산과정과 담론분석적 전망 (A Producing Process for Korean Nursing Knowledge and Discourse on Analytic Prospects)

  • The purpose of this study was to identify the producing process for Korean nursing knowledge as applied by Foucault's discourse analytic method. By Foucault's discourse analytic method, the problem is not what is knowledge but what sort of knowledge is made up through specific level of practice. A Korean body of nursing knowledge has been discussed since 1980. At the end of 19th century, missionaries transplanted western nursing knowledge and method to Korea. Western nursing knowledge and methods have been developed continuously with both merits and demerits to Korean society. Recently our world has be come a global community via advances in transportation and correspondence. Although each person is different in skin color and shape. there is a clear line between Oriental people and Western people. Nursing science is only one in our world. but western humanistic nursing practice based on western worldview and human life has limits. It is natural that the Koreans as Oriental people have a systemic nursing science to reveal the specific experiential and concrete body of nursing knowledge rooted in the Korean worldview and human life. Nursing science is to understand human beings, to promote health. to prevent illness. to restore health. to alleviate suffering and to search for principles needed throughout all of human life. In Korea, now is the quickening period to shape a Korean body of nursing knowledge because of a shortage of nursing language in matters of intellectual recognition, and unfamiliar practical nursing field where there is no familiarity in the system of nursing research methodology. In reviewing articles from the Journal of the Nurses' Academic Society on Korean body of nursing knowledge, it was found that there are two common features. The first, human body and mind are inseperable that is one unit in this world and health is keeping a harmonious relationship between human body and mind. The second, Korean nursing practice is based on human nature and family ties. Accordingly discourse analysis has a good future prospect to produce a Korean body of nursing knowledge for analytic research on body and mind monism and family centered care based on human nature and family ties.

간호대학생의 임상실습중 혈액 및 체액 노출현황과 영향 요인 - 감염관리 지식 및 수행도 중심으로- (The Status of Blood and Body Fluid Exposure and Affecting Factors among Nursing Students including Knowledge, Performance regarding Standard Precautions)

  • Objectives: The purpose of this study was to investigate the status of exposure to blood and body fluid and factors affecting on blood and body fluid exposure of nursing students including knowledge, performance regarding standard precautions. Methods: 280 nursing students in D-city and S-city participated in this study. Data collection was conducted from January to March 2012. Students' knowledge and performance on standard precautions of infection control in hospitals were measured using the measurement developed by Hong(2010). Data analysis was performed using descriptive statistics, t-test, one way-ANOVA, ${\chi}^2-test$, Pearson correlation coeficient, logistic regression by the SPSS WIN 18.0 program. Results: 58.6 percent of subjects experienced a exposure to blood and body fluid. The level of nursing students' knowledge level regarding standard precautions was $22.44{\pm}2.11$ and performance level was $4.31{\pm}0.50$. The factors affecting on blood and body fluid exposure of nursing students were knowledge level and education type about infection control. Conclusions: Based on the findings of this study, to decrease the exposure to blood and body fluid of nursing students, a proper education need to be stressed to improve nursing students' infection control knowledge and awareness.

간호대학생의 체질량지수와 영양지식 및 건강증진행위와의 관계 (The Relationships between Body Mass Index, Nutrition Knowledge and the Health Promotion Behavior of Nursing Students)

  • Purpose: This study was done to identify the relationships between body mass index, nutritional knowledge and the health promotion behavior of nursing students. Methods: Participants consisted of the entire 330 members of the student body of a Korean nursing college. Variables included body mass index, nutritional knowledge and health promotion behavior as well as demographic variables. Data was analyzed by frequencies, ANOVA and Pearson's correlation with the SPSS/Win 21.0 program. Results: The mean score for body mass index was $20.23kg/m^2$ for nutritional knowledge, $14.27{\pm}3.07$ on a total 20-point scale, and for health promotion behavior, $3.03{\pm}0.34$ on a 4-point scale. Nutrition knowledge positively correlated with health promotion behavior (r=.368, p<.001). Health promotion behavior positively correlated with underweightness (r=.120, p=.005), normal weight (r=.212, p<.001), negatively correlated to overweightness (r=-.351, p<.001), and obesity (r=-.413, p<.001). Nutrition knowledge positively correlated to underweightness (r=.155, p=.005), and normal weight (r=.346, p<.001), negatively correlated to overweightness (r=-.548, p<.001), and obesity (r=-.342, p<.001). Conclusion: In order to enhance the effective health promotion behavior of nursing students, it is important to actively intervene to improve body weight control behavior and nutritional knowledge. Therefore, developing an educational program for improving nutritional knowledge and body weight control behavior of nursing students is recommended.

  • Purpose : The purpose of this study is to analyze the history of nursing and medicine as phenomena of knowledge/power from the viewpoint of Foucault. Method : The present study adopts the genealogical method by Foucault, which tries to unite languages and knowledge. Foucault who attempted to show that the basic ideas which people normally take to be permanent truths about human nature and society change in the course of history. Results : It has been emphasized that the development of modern health care has been due to the progressive efforts of medicine and to medical discoveries. Foucault examines the institutionalization of knowledge and the power exerted thereby, with special reference to the devices of social regulation and their function over the madness, the disease, the crime, and the sexuality. The concept of power is that it is exerted spontaneously in verbal behaviors of individuals through knowledge of everyday life such as definition of body or mind. Therefore as to the problem of knowledge/power, this study tries to understand the meaning of history of nursing and medicine through an analysis of the formation of text of history. In order to have authority in a power relation, the medical professional asserts that medical discourse is the most scientific knowledge. Conclusion : According to the above findings, it can be concluded that the body is also directly invested in a political field: power relations have an immediate hold upon it; they invest it, mark it, train it, torture it, force it to carry out tasks, to perform ceremonies, to emit signs. Therefore the devaluation of nursing care is reinforced by the medicine which has the legitimate authority through use of the political skills about the body.

페미니즘과 질적간호연구 (Feminism and Qualitative Nursing Research)

  • Purpose: The purpose of this article was to describe feminism and to propose the integration of a feminist method into qualitative nursing methodology in order to expand the body of nursing knowledge. Method: The world view of feminism including philosophy, epistemology and methodology was outlined, and a feminist grounded theory and feminist ethnography were suggested as a way of strengthening nursing research methodology using literature review. Results: Four different philosophical perspectives of feminism, that is, liberal feminism, radical feminism, Marxist feminism, and social feminism were described. Also epistemological perspectives including feminist empiricism, feminist standpoint, and postmodem feminism, were explained and were related to the methodology and methods of feminism. To enhance the strengths of nursing research within the feminist perspectives, feminist grounded theory and feminist ethnography were exemplified in the paradigm of qualitative nursing research. Conclusion: This paper suggested that incorporation of feminist approaches within nursing is a valuable attempt to expand the body of nursing knowledge and to enhance the quality of nursing care services by rectifying male-oriented knowledge and by empowering women in the care of other people as well as themselves.

임상실무에서 필요로 하는 생리학 내용에 대한 연구 (An Analysis of Requisite Knowledge Body of Physiology for Clinical Nursing Practices)

  • The purpose of this study was to define the content of physiological knowledge needed for clinical nursing practices. Subjects of physiology were classified into 15 areas, and each areas was further classified into subareas, resulting in a total of 194 subareas. The degree of importance of each subarea was measured with a 4-point scale. The subjects of this study were 179 nurses of two university hospitals located in Seoul and Inchon. The results were as follows : 1. The areas of physiology necessary for clinical nursing practice as a basic knowledge in the order of importance were : blood, respiratory system and renal physiology, function of the immune system, body fluid and cardiovascular system, body temperature, endocrine physiology and gastrointestinal physiology However, the degree of importance for reproductive physiology, neuro-physiology, energy and metabolism, cell and cell membrane physiology, muscular physiology and special sense was relatively low. 2. The most important content of physiology for all clinical areas in nursing was blood physiology. However, the degree of importance for each physiology area was different depending on clinical areas. 3. Subareas of physiology as a basic knowledge for clinical practice and education in nursing were blood transfusion, blood type, function of red blood cell, white blood cell and platelet, characteristics and function of hemoglobin, composition and function of plasma protein, and mechanism of blood coagulation and anticoagulation. In conclusion, areas of physiology necessary for clinical nursing practice were blood, respiratory system and renal physiology, function of immune, body fluid and cardiovascular system, body temperature, endocrine physiology and gastrointestinal physiology. However, the degree of importance for each physiology area was different depending on clinical areas In nursing.

간호대학생의 표준주의에 대한 지식, 이행 자신감, 이행도 (Nursing Students' Knowledge, Self-Confidence in Compliance, and Compliance to Standard Precautions)

  • Purpose: The purpose of this study was to identify nursing students' knowledge, self-confidence in compliance, and compliance with standard precautions, and to identify predicting factors of compliance with standard precautions. Method: From May 18 to June 15, 2012, data were collected via questionnaire surveys from 222 nursing students of 2 colleges in G-province. Descriptive statistics, t-tests, Pearson correlation coefficients, and multiple regression analyses were used with SPSS 22.0 to analyze the data. Results: The average knowledge score was $0.89{\pm}0.08/1.0$, $3.59{\pm}0.57/4.0$ for self-confidence in compliance, and $0.91{\pm}0.11/1.0$ for compliance. Significant positive correlations were observed between knowledge and self-confidence in compliance(r=.14, p=.040), and between self-confidence in compliance and compliance(r=.25, p=.008). Self-confidence in compliance and blood and body fluid exposure were significant factors of compliance(F=5.46, p<.001). Conclusion: To increase nursing students' compliance with standard precautions, continuous efforts must be made to enhance self-confidence in compliance and decrease blood and body fluid exposure.

기초간호자연과학의 인체구조와 기능 내용별 필요도에 대한 연구 (A Study on the Degree of Need of Human Structure and Function Knowledge in Clinical Nurses)

  • The purpose of this study was to define the content of requisite human structure and function knowledge needed for clinical knowledge of nursing practice. Subjects of human structure and function were divided into 10 units, and each unit was further divided into 21 subunits, resulting in a total of 90 items. Contents of knowledge of human structure and function were constructed from syllabus of basic nursing subjects in 4 college of nursing, and textbooks published by nurse scholars prepared with basic nursing sciences. The degree of need of 90 items was measured with a 4 point scale. The subjects of this study were college graduated 136 nurses from seven university hospitals in Seoul and three university hospitals located in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, hospice ward, and their working period was mostly under 5 years. The results were as follows: 1. The highest scored items of human structure and function knowledge necessary for nursing practice were electrolyte balance, blood clotting mechanism and anticoagulation mechanism, hematopoietic function, body fluid balance, function of plasma, and anatomical terminology in the order of importance. The lowest scored items of human structure and function knowledge necessary for nursing practice was sexual factors of genetic mutation. 2. The highest order of need according to unit was membrane transport in the living unit, anatomical terminology in movement and exercise unit, mechanism of hormone function in regulation and integration unit, component and function of blood in oxygenation function unit, structure and function of digestive system in digestive and energy metabolism unit, temperature regulation in temperature regulation unit electrolyte balance in body fluid and electrolyte unit, concept of immunity in body resistance unit, and genetics terminology in genetics unit. The highest order of importance according to subunit was membrane transportation in cell subunit, classification of tissues in tissue unit, function of skin and skin in skin subunit, anatomical derivatives of the skeleton subunit, classification of joints in joint subunit, an effect of exercise on muscles in muscle subunit, function of brain in nervous system subunit, special sense in sensory subunit mechanism of hormone function in endocrine subunit, structure and function of female reproductive system in reproductive system unit, structure and function of blood in blood unit, structure of heart, electrical and mechanical function in cardiovascular system unit, structure of respiratory system in respiratory system subunit, structure and function of digestive system in digestive system subunit, hormonal regulation of metabolism in nutrition and metabolism subunit, function of kidney in urologic system subunit, electolyte balance in body fluid, electolyte and acid-base balance subunit. 3. The common content of human structure and function knowledge need for all clinical areas in nursing was structure and function of blood, hematopoietic function, function of plasm, coagulation mechanism and anticoagulation mechanism, body fluid, electrolyte balance, and acid-base balance. However, the degree of need of each human structure and function knowledge was different depending on clinical areas. 4. Significant differences in human structure and function knowledge necessary for nursing practice such as skin and derivatives of the skin, growth and development of bone, classification of joint, classification of muscle, structure of muscle, function of muscle, function of spinal cord, peripheral nerve, structure and function of pancrease, component and function of blood, function of plasma, structure and function of blood, hemodynamics, respiratory dynamics, gas transport, regulation of respiration, chemical digestion of foods, absorption of foods, characteristics of nutrients, metabolism and hormonal regulation, body energy balance were demonstrated according to the duration of work. 5. Significant differences in human structure and function knowledge necessary for nursing practice such as classification of tissue, classification of muscles, function of muscles, muscle metabolism, classification of skeletal muscles, classification of nervous system, neurotransmitters, mechanism of hormone function, pituitary and pituitary hormone, structure and function of male reproductive organ, structure and function of female reproductive organ, component and function of blood, function of plasma, coagulation mechanism and anticoagulation mechanism, gas exchange, gas transport, regulation of respiration, characteristics of nutrients, energy balance, function of kidney, concept of immunity, classification and function of immunity were shown according to the work area. Based on these findings, all the 90 items constructed by Korean Academic Society of Basic Nursing Science should be included as contents of human structure and function knowledge.

간호대학생들의 환경친화적 태도, 노출저감화 행동, 내분비계 장애물질에 대한 지식과 비만의 관련성 연구 (Relationship among Pro-environmental Attitude, Behavior to Decrease Exposure, Knowledge of Endocrine Disruptors, and Obesity-related Profiles in Nursing Students)

  • Purpose: This study was conducted to examine the pro-environmental attitude (actual commitment domain, verbal commitment domain, affect domain), behavior to decreased exposure and knowledge of endocrine disruptors by obesity -related profiles (BMI, body fat percentage, visceral fat percentage, skeletal muscle mass percentage, waist circumference, waist-hip ratio). Methods: A cross-sectional study was conducted with 102 nursing students. Data were collected from November to December, 2015 using self-report questionnaires and physical measurements. Data were analyzed using t-test, Pearson correlation and coefficients with SPSS 18.0. Results: The study results showed that actual commitment domain of pro-environmental attitude and behavior to decreased exposure level on endocrine disruptors were significantly related to visceral fat percentage. Actual commitment domain of a pro-environmental attitude was significantly related to body fat percentage. Pro-environmental attitude was significantly related to the behavior to decreased exposure level on endocrine disruptors and knowledge thereof. Conclusion: These findings suggest that visceral fat and body fat percentages were significantly related to the actual commitment domain of a pro-environmental attitude. Therefore, a replication study is recommended to understand the connection between endocrine disruptors and obesity. In addition, developing an education program about endocrine disruptors for nursing students is recommended. In particular, a pro-environmental attitude, especially on actual commitment domain, could be involved as an education program.

비만관리프로그램이 초등학교 비만아동의 비만도, 신체조성, 체력, 지식 및 식행동에 미치는 효과 (The Effects of Obese Program on the BMI, Body Composition, Physical Fitness, Knowledge, Eating Behaviors in Obese Elementary School)

  • Purpose: The purpose of this study was to identify the effects of the obese program on BMI, body composition, physical fitness, knowledge, eating behaviors among the elementary school obese children. Method: They ranged from the third to sixth grades with over 30% body fat ratio. The subjects consisted of 31 obese children who participated in the obese program and 34 obese who did not participate in the program. The contents of the programs for nine weeks from contained obese education, counseling and exercises once a week. The program lasted from Oct. 8 to Dec. 22, 2001. The Data were analyzed using SPSS program. Result: Flexibility was significantly higher for the program participants than the nonparticipants at the post-test. Agility was significantly higher for the program participants than the nonparticipants at the post-test. Knowledge were significantly higher for the program participants than the nonparticipants at the post-test. Conclusion: This study provides evidence that obese program is effective in changing the flexibility, agility, knowledge in obese elementary children over 9 weeks.
