Study on Absorbed Dose Determination of Electron Beam Quality for Cross-calibration with Plane-parallel Ionization Chamber (평행평판형이온함의 교차교정 시 전자선 선질에 따른 흡수선량 결정에 대한 연구)

  • Absorbed dose to water based protocols recommended that plane-parallel chambers be calibrated against calibrated cylindrical chambers in a high energy electron beam with $R_{50}$>7 $g/cm^2$ (E${\gtrsim}$16 MeV). However, such high-energy electron beams are not available at all radiotherapy centers. In this study, we are compared the absorbed dose to water determined according to cross-calibration method in a high energy electron beam of 16 MeV and in electron beam energies of 12 MeV below the cross-calibration quality remark. Absorbed dose were performed for PTW 30013, Wellhofer FC65G Farmer type cylindrical chamber and for PTW 34001, Wellhofer PPC40 Roos type plane-parallel chamber. The cylindrical and the plane-parallel chamber to be calibrated are compared by alternately positioning each at reference depth, $Z_{ret}=0.6R_{50}-0.1$ in water phantom. The $D_W$ of plane-parallel chamber are derived using across-calibration method at high-energy electron beams of 16, 20 MeV. Then a good agreement is obtained the $D_W$ of plane-parallel chamber in 12 MeV. The agreement between 20 MeV and 12 MeV are within 0.2% for IAEA TRS-398.

Studies on INtestinal TRematodes in Korea XVII. Development and Egg Laying Capacity of Echinostoma hortense in Albino Rats ans Human Experimental Infection (한국의 장흡충에 관한 연구 XVII. 호르텐스극구흡충의 흰쥐내 발육, 충란산출양상 및 인체실험감염)

  • The worm development and egg laying pattern of Echinostoma hortense (Trematoda; Echinosto-matidae) were studied in albino rats and the brief clinical course was observed in human volunteers. A total of 21 rats were infected with 20~69 metacercariae each and two humans were with 7 and 27 metacercariae, which were collected from the loaches. For recovery of worms, the rats were sacrificed at irregular intervals from the 6th to 150th day after infection and the human volunteers were treated with praziquantel and purged with magnesium salt on the 26~27th day. The stools of the rats and humans were examined for the eggs. The results were as follows: 1. The worm recovery rate from the rats was not affected by the increase of infection time but varied individually; 9.1~50.0% (31.1 % in average). From humans, 14.3% and 37.0% (32.4% in average) of challenged were recovered. 2. In the rats, it was revealed that the worms rapidly grew for the first 14 days to become 7.59mm in average length and 1.17mm in average width but the growth became much slower thereafter until the 150th day; 7.95mm in length on the 21 th day, 9.04mm on the 28th day, 10.21mm on the 49th day and 12. 62mm on the 150th day. During the early stage of infection, the growth of genital organs (male or female) was expressed as sigmoid curves whereas non-genital organs (such as suckers) was simply as straight lines. 3. The prepatent period of this fluke was 10~12 days in the rats and 16~17 days in men. After the start of oviposition, the egg production by the worms remarkably increased, reached maximum on the 32~33th day, followed by decrease thereafter. The maximum value of E.P.G./worm was 390. 4. The major subjective symptoms in human volunteers were abdominal pain and diarrhea during the early stage of infection. The results show that human is as susceptible as the rats to E. hortense infection and the amount of egg production in the rats is greatly affected by the age of worms.

Seasonal Prevalence of the Vector Mosquitoes of Japanese Encephalitis Virus in Kyungpook Province, Korea (경북지역에서의 일본뇌염 매개모기에 출현 소장)

  • In order to determine the seasonal prevalence and population density of vector mosquitoes in Kyungpook Province, a survey based on average numbers of female mosquito per trap-night, were carried out during the period from May to November in 1984. Among the 34, 571 mosquitoes collected in Kyungsan county in animal shelters and human dwellings by light traps, approximately 45.0 per cent were Culex tritaeniorhynchus, 34.0 per cent Culex pipiens pallens, and 19.0 per cent Anopheles sinensis. By comparison, distribution of mosquito species in Ankang town, in which Japanese encephalitis had been reported in the past, were quite different and A. sinensis was the most abundant species, being constituted in 75.8 per cent, followed by C. tritaeniorhynchus(23.2 per cent), and C. pipiens pallens(O.6 per cent). In the general pattern of seasonal prevalence, C. tritaeniorhynchus first appeared in mid-June, and trapped in large numbers during the periods from mid-August to early September, showing a simple sharply pointed one-peaked curve, while C. pipiens pallens was found to be active through almost the entire season showing irregular curves with several peaks. The trend of seasonal distribution of A. sinensis was similar to that of C. tritaeniorhynchus, but the earliest dates of appearance and disappearance differed from that of C. tritaeniorhynchus. The results of hourly catches of mosquitoes by human baited traps have clearly shown the differences in the biting rhythm among the vector mosquitoes of this Province, such as the facts that C. tritaeniorhynchus and A. sinensis are rather constantly active all through a night with more or less inconspicuous peaks twice a night, while C. pipiens pallens exhibits an irregular curve, and the numbers of A. vexans nipponii, C. vagans and Armigeres subalbatus are very small and are not sufficient for estimating the biting rhythm. Four genera and fourteen species of larval mosquitoes collected from eighteen kinds of habitats which classified according to the categories followed by Bates (1949). The main breeding sites contributing to peak C. tritaeniorhynchus adult densities in this surveyed areas were evidently ricefields, ground pools, puddles and swamps and marshes, etc. Summarizing the results, this study indicated that the highest population density of C. tritaeniorhynchus was encountered in late August and early September, showing a simple sharply pointed one-peaked curve.

Study on Metagonimus yokogawai(Katsurada, 1912) in Korea Vll. Electron Microscopic Observation on the Tegumental Structure (요꼬가와흡충에 관한 연구 VII. 표피 미세구조의 전자현미경적 관찰)

  • This study was performed to observe the chronological changes in the tegumental structure of M. yokogawai using scanning electron microscope. The subjected worms were encysted metacercariae obtained from the sweetish, and 2-day, 1-week and 4-week old worms experimentally reared in albino rats. The results are as follows: 1. The tegument of encysted metacercariae showed many transverse shallow rugae, which were more remarkable in posterior half body, i.e., posterior to ventral sucker. The whole surface was armed with many scale-like spines; 7~8 pointed ones on anterior body and 2~3 pointed on posterior body. The ciliated knob-like papillae (Type I) were abundant around oral and ventral suckers, which grouped 2, 3 or 4 in number in most cases. A few round swellings of tegument(Type lI were observed only on oral sucker. 2. The tegumental surface of 2-day old worms showed deeper rugae, and the anterior half covered with knob-like processes of distal cytoplasm and the posterior half with cobblestone-like ones. Interspinous space became more wide and 9 pointed spines appeared on anterior dorsal surface. The sensory papillae enlarged but not changed in their distribution. 3. The tegument of 1-week old worms revealed knob-like cytoplasmic processes in posterior half body and velvety ones around oral sucker. The scale-like spines of anterior half body changed remarkably to the slender ones of posterior body at the level of ventral sucker. In dorsal surface, the arrangement of the Type I papillae were bilaterally symmetrical. 4. The tegument of 4-week old worms were finely differentiated and the posterior tegument covered with velvety cytoplasmic processes. The spines had remarkably grown in length and width but the density remained nearly unchanged. The papillae also became larger but their morphology and distribution were not different from younger worms. However, the round elevation of cytoplasmic ridges (Type III papilla) appeared bilaterally on inner wall of oral sucker, approximately 8 in number. From the above results, it is considered that the tegument of juvenile iH. yokogawai continued to differentiate until 4 weeks after infection.

The Healing Effect of 'Self-archetype' Manifested in the Analysis of 'Hunger' and 'Compulsive Overeating' : Investigation Focused on the 'Serpent' Imago ('배고픔'과 '폭식충동'을 주소로 하는 내담자의 분석과정에서 발현된 '자기원형'의 치유적 기능 : '뱀'의 상징을 중심으로 한 고찰)

  • Objectives : In this study I made investigations how 'strange hunger' and 'compulsive overeating' threatening the ego could be resolved and healed. And I aim to present a healing model of psychotherapy and analysis as one of methods of treatment for 'eating disorder'. Methods : The analysands of this study were outpatients who visited the department of psychiatry of Yong-In Mental Hospital from March 2008 to February 2017 with 'hunger' and 'compulsive overeating' as their chief complaints. This study is based on the detailed records of the process of analysis including dreams and visions. Results : 1) Throughout the process of analysis that explore both consciousness and unconsciousness(dream, vision), hunger and compulsive overeating is improved and healed in all analysands. 2) The Imago of 'Snake' appeared in dreams and visions of all analysands. 3) By suffering impulse rather than acting it out, impulse transformes itself into 'Imago'. As impulse transforms into 'Imago' and reveals the 'meaning' of it, ego-threatening power of impulse weakens and mood is calmed. And as a result, synthesis of consciousness and unconsciousness and creative transformation of personality can be possible. Conclusions : In some people, 'hunger' and 'compulsive overeating' are 'creative impulses' that aim 'Self-realization' which can be fruited as creative transformation of personality and as creative transformation in the relation with the world. 'Creative impulses', which often can be experienced as instinctive impulse or emotional suffering unless ego realizes the meaning, reveal the meaning in dreams or visions through 'Imago' and 'Symbol'.

The Development of a Cryotherapy System (한냉물리치료기의 개발)

  • A cryotherapy system using cold air was developed. The developed system had superior low-temperature characteristics with various flow rates and nozzle sizes, and used R-404A, as a coolant, which has no destructive effects of Ozone layers. Flow rates and the treatment time can be easily altered during the operation. In addition, and alarm system was designed for the overload, overheat, and over-charge of the machine. For clinical applications, skin temperatures, intra-articular temperatures of the knee joint and intra-muscluar temperatures of the gluteal muscles were measured during and after the cryotherapy. After a 5-minute therapy, skin and intra-articular temperatures decreased by $23.3{\pm}4.7 and 4.1 {\pm}1.0^{circ}C$, respectively. A 5-minute cryotherapy was good enough to maintain low intra-articular temperatures for 2-3 hours. Resting intra-muscular temperatures in 2, 4, and 6cm deep in the gluteal muscle were $36.5{\pm}1.2, 36.9{\pm}0.2, 37.1{\pm}0.2^{circ}C$, respectively (p<0.05). Lowest temperatures in 2, 4, and 6cm depth were $35.1{\pm}0.7, 36.2{\pm}0.4, 36.9{\pm}0.3^{circ}C$, respectively (p<0.05). Temperatures after a 2-hour cold air application on the skin and in the muscle in dept도 of 2, 4, and 6cm were $32.2{\pm}1.1, 36.2{\pm}0.5, 36.6{\pm}0.3, 36.9{\pm}0.3^{circ}C$respectively (p<0.05). Temperatures on the skin and in the muscle significantly decreased after 2 hours, compared with before cold air application (p<0.05). The intra-muscular temperature was changed more slowly than the skin temperature, and the deeper the muscle, the lesser temperature changes. The effect of a 5-minute cold air application lasts up to 2 hours, and it seems that the rebound-rise of the temperature dut to the reactive vasodilatation does not occur in the gluteal muscle.

Differences in Patients' and Family Caregivers' Ratings of Cancer Pain (암환자와 그 가족간호자가 지각하는 환자의 통증강도 차이)

  • Purpose: Undertreatment of canter pain, especially due to the differences in the perception of pain between the patients and caregivers, is a well recognized problem. The purpose of this study were to determine if there exist differences in communication about pain intensity scores between patients and their family caregivers in Korea. Methods: A total of 127 patient-family caregiver dyads who have experienced canter pain participated in this study at a hospital in Seoul for six months. The data were obtained by fare to face interview with a structured questionnaire based on Brief Pain Inventory-Korean version and other previous researches. The clinical information for all patients was compiled by reviewing their medical records. Results: Patients' 'worst-pain for 24-hour' and 'right-now-pain' scores estimated by family caregivers were significantly higher than those by patient themselves. The degree of agreement between patients and family caregivers in the estimate of patients' 'worst-pain for 24-hour' intensity categories was 78.7% for 'severe pain', 40% for 'no pain', 27.5% for 'mild pain' and 22.9% for 'moderate pain'. In case of 'right-now-pain' intensity categories, the agreement was 50% for 'severe pain', 47.2% for mild pain, 46.3% for 'no pain', and 26.3% for 'moderate pain'. Conclusion: This study demonstrates that the degree of agreement between patients and family caregivers in the estimate of patients 'pain intensity categories was less than 50% except for 'severe pain'. The results indicate that Korean family caregivers tend to overestimate the canter pain intensity of their caring patients, especially, when a lancer patient has 'moderate' or 'mild pain'. Health Providers are advised to educate patient-family caregiver dyads to use a pain measurement scale to promote their agreement in pain Intensity stores. Further analyses and studies are needed to identify the factors and differences that influence their communication about pain intensity scores between patients and their family caregivers.

Test-Retest Reliability of Attention Network Test Scores in Schizophrenia (조현병 환자가 시행한 주의력 네트워크 검사 점수의 검사-재검사 신뢰도)

  • Objectives : Although the Attention Network Test(ANT) has been widely used to assess selective attention including alerting, orienting, and conflict processing, data on its test-retest reliability are lacking for clinical population. The objective of the current study was to investigate test-retest reliability of the ANT in healthy controls and patients with schizophrenia. Methods : Fourteen patients with schizophrenia and 23 healthy controls participated in the study. They are tested with ANT twice with 1 week interval. Test-retest reliability was analyzed with Pearson and Intra-class correlations. Results : Patients with schizophrenia showed high test-retest correlations for mean reaction time, orienting effect, and conflict effect. Also, they showed moderate to high test-retest correlations for mean accuracy and moderate test-retest correlations for alerting effect and conflict error rate. On the other hand, healthy controls revealed high test-retest correlations for mean reaction time and moderate to high test-retest correlations for conflict error rate. In addition, they revealed moderate test-retest correlations for alert effect, orienting effect, and conflict effect. Conclusions : The mean reaction time, alerting effect, orienting effect, conflict effect, and conflict error rate of ANT showed acceptable test-retest reliabilities in healthy controls as well as patient with schizophrenia. Therefore, the analyses of these reliable measures of ANT are recommended for case-control studies in patients with schizophrenia.

Occurrence of Papaya ringspot virus Infecting Cucurbit Crops in Korea (박과작물에 발생하는 파파야원형반점바이러스의 발생 보고)

  • A flexuous rod-shaped virus was isolated from Cucurbita pepo leaves showing as green mosaic and puckering symptoms at Anseong, Korea. Based on the biological analysis, electron microscopy, and reverse transcription-polymerase chain reaction (RT-PCR), the virus isolate was identified as Papaya ringspot virus type watermelon (PRSV-W). From biological analysis, the host range of PRSV-W was limited to the families Cucurbitaceae and Chenopodiaceae. Most susceptible cucurbit species, such as Cucumis lanatus, Cucumis sativus, Cucurbita pepo, and Citrullus lanatus, showed symptoms of green mosaic, malformation, puckering, and narrow laminae by infection with PRSV-W. The local lesion were showed on the inoculated leaves of both Chenopodium amaranticolor and C. quinoa. Field survey of PRSV, Watermelon mosaic virus (WMV) and Zucchini yellow mosaic virus (ZYMV), three major viruses infecting cucurbit, was done during 2001 to 2003 on 173 commercial cucurbit cultivating fields distributed over the three regions of Gyeonggi, Gyeongbuk and Jeonnam Provinces where cucurbits are grown in different environmental conditions and cropping patterns. Typical viral symptoms were observed from 107 cultivating fields, and all three kinds of potyviruses were detected from 206 samples out of the 235 samples using RT-PCR. Watermelon mosaic virus (WMV) and Zucchini yellow mosaic virus (ZYMV) are the most widely distributed viruses in outdoor and retarding-culture fields, at an infection rating of 48 and 33 percents, respectively. PRSV was detected from 12 percent of 235 samples. The nucleotide and amino acid sequences of coat proteins (CP) of eight PRSV isolates, collected from several areas including Anseong, were determined and sequenced heterogeneity among the isolates was performed. The CP gene of PRSV showed 88.6~97.3 percent homology in nucleotide sequences and 95.1~99.3 percent homology in amino acid sequences with other PRSV isolates worldwide. The phylogenetic analysis indicated that the Korean PRSV isolates belong to the southern-east Asian cluster.

Eating patterns and use of nutritional information in breast cancer survivors treated with radiation therapy in South Korea (일반인과 유방암 환자간의 식행동 및 영양정보에 관한 인식조사)

  • The purposes of this study were 1) to investigate eating behaviors and patterns in breast cancer patients using a newly developed food frequency questionnaire and 2) to examine perception and use of nutritional information about breast cancer treatment among cancer patients treated with radiation therapy. Sixty breast cancer patients (case group) undergoing radiation therapy in Ajou University Hospital, Suwon, South Korea and 79 healthy women (control group) participated in this study. Mean age of subjects in the control group was $46.00{\pm}7.88$ years and BMI was $23.12{\pm}2.85kg/m^2$, and that of the case group was $50.06{\pm}11.64$ years and $22.32{\pm}3.24kg/m^2$. The results of eating behaviors showed several significant differences between control and case groups. Breast cancer patients ate meals on a more regular basis, on time, and more frequently compared to control subjects. In addition, they preferred more salty or spicy and bland food compared to healthy women. According to answers from the food frequency questionnaire, breast cancer patients consumed significantly lower amounts of boiled white rice, meats and processed food, fish and shellfish, coffee, milk, and cheese, whereas they consumed a significantly large amount of boiled multigrain rice, vegetable, seaweeds, soybean and processed food, and yoghurt compared to healthy women. This study also observed the way in which cancer patients and healthy control subjects obtain information about breast cancer treatment and its reliabilities. Results showed that healthy women did not hesitate to obtain information from mass media, while breast cancer patients would obtain nutritional information from specialists rather than mass media. Results of this survey confirmed that breast cancer patients avoided intake of red meat protein, even though they already recognized the importance of dietary protein intake for recuperation and treatment of the disease. These results could be used for future diet and nutrition guidelines for breast cancer patients.
