비대칭 비강 내 공기유동에 관한 실험 및 수치해석적 연구 (Experimental and Numerical Research on the Airflow Inside Asymmetric Nasal Cavities)

  • 비강의 3 가지 생리학적 주요기능은 공기조절, filtering, 후각기능이다. 비강 내 공기유동특성에 대한 지식은 비강 호흡의 생,병리학적 측면을 기반으로 한다. 본 연구실에서는 정상 및 변형된 비강 모델 내 유동에 관하여 일정유량 및 주기유동 하에서 다양한 PIV 실험들을 진행해 왔다. 비대칭 비강을 가진 환자들 중에 일부는 고통을 느끼거나 불편함을 호소하는 반면 그렇지 않은 환자들도 있다. 이 원인을 밝히기 위하여 비중격이 휘어진 비대칭 모델에 대하여 PIV 및 수치해석적인 연구를 진행하였다. 이를 위하여 이비인후과 의사로부터 모델에 관한 CT 데이터를 제공받아 PIV 및 수치해석을 통해 호기 및 흡기시의 RMS 값 및 속도 분포를 얻었다. 모델에 따라 좌우 유량이 크게 다른 것을 확인할 수 있었고 이것이 고통을 느끼는 원인중에 하나가 될 수 있다.

A Study on Aggressive Behavior Among Nursing Home Residents with Cognitive Impairment

  • Purpose. With a sample of cognitively impaired nursing home residents and nursing staff, the following were examined 1) the proportion and nature of aggressive behavior, 2) the frequency and types of aggressive behavior, 3) the difference between the residents who demonstrate aggressive behavior and those who do not demonstrate aggressive behavior (age, mental status, functional status, and pain, length of nursing home stay), and 4) nursing staff responses to aggressive behavior by residents. Methods. A cross-sectional descriptive study design was used. Data were collected from cognitively impaired nursing home residents (N=205) and nursing staff (N=60) at two nursing homes using Ryden Aggression Scale I and II, Mini-Mental State Exam, Modified Barthel Index, Verbal Descriptor Scale, and aggressive behavior management questionnaire. Data were analyzed using descriptive statistics including t-test. Results. About $62.9\%$ residents were found to be aggressive and $38.5\%$ were both physically and verbally aggressive. Pushing, making threatening gestures, hitting, slapping, cursing/obscene/vulgar languages, making verbal threats were occurred frequently. Aggressive residents were significantly older, had more cognitive impairment, had more pain, and stayed longer in the nursing home when compared with non-aggressive residents. Considerable proportion of nursing staff responded to aggressive behaviors inadequately. Conclusion. Aggressive behavior among cognitively impaired nursing home residents is prevalent thus needs to be prevented and reduced. Along with environmental modification, educational programs for nursing staff and family caregivers need to be developed and implemented so that they can have extensive knowledge and skills to manage aggressive behaviors.

응급의료센터에 내원한 비외상성 흉통환자의 임상 양상 (Clinical Presentation of the Patients with Non-traumatic Chest Pain in Emergency Department)

  • 1997년 1월 1일부터 12월 31일까지 영남대학교 의과대학 부속병원 응급의료센터에 내원한 비외상성 흉통 환자에 대한 임상적 연구를 시행하여 다음과 같은 결과를 얻었다. 응급의료센터를 내원한 총환자 25,583명 중 비외상성 흉통을 주소로 내원한 경우는 488례였으며 남녀비는 1.9:1로 남자에게서 많이 발생하는 것으로 조사되었다. 흉통의 원인별 분류를 보면 심인성인 경우가 320례(65.6%)로 가장 많이 발생하였으며 심인성의 경우를 다시 나누어 보면 협심증이 140례(28.7%), 심근경색증이 128례(26.2%)였다. 흉통의 발생시간별 비교에서 심인성군이 비심인성군에 비해 오전에 더 많이 발생하는 것으로 나타났다(p<0.05). 내원 시간별 비교에서는 6시간내 도착한 경우가 심인성에서 60.0%, 비심인성에서 45.2%로 가장 많았으며, 6시간 이후에 도착한 경우는 심인성 40.0%, 비심인성 54.8%였다. 평균 소요시간은 비심인성군에서 $1,848.9{\pm}4,384.6$분, 심인성군에서 $1,454.5{\pm}3,219.1$분, 심근경색증군에서 $1,230{\pm}2780.6$분으로 심인성군, 심근경색증 환자군에서 소요시간이 짧은 것으로 나타났다. 흉통의 양상은 쥐어짜는 듯한 양상이 256(52.5%)례로 가장 많았고, 다음이 둔한 양상의 순으로 나타났으며, 심인성의 경우 쥐어짜는 듯한 양상이, 비심인성의 경우 둔한 양상이 가장 많이 나타났다. 연관된 증상의 경우, 호흡 곤란이 가장 많았으며, 증상의 평균개수는 비심인성군에서 $1.1{\pm}0.9$개, 심인성군에서 $1.4{\pm}1.1$개, 심근경색증군에서 $1.7{\pm}1.1$개로 나타나 심근경색증의 연관 개수가 의의있게 많았다(p<0.05). 심효소 검사에서 troponin-T rapid assay의 심근경색증 진단에 대한 민감도(sensitivity)는 59.2%, 특이도(specificity)가 95.0%, 양성 예측도86.0%, 음성 예측도 81.9%였으며. CK-MB 검사는 민감도 46.4%, 특이도 95.4%, 양성 예측도 84.1%, 음성 예측도 77.1%였으며 myoglobin 검사는 민감도 45.1%, 특이도 92.1%, 양성 예측도 74.3%, 음성 예측도 76.7%로 나타났다. 급성심근경색증군을 대상으로 한 경우 모두 90% 이상에서 시행되었으나 민감도는 45-59%에 불과했지만 심전도 검사의 경우 민감도가 96.1%로 나타났다. 입원율은 심인성군에서 229례(71.6%)로 비심인성군의 85례(50.6%)에 비해 높게 나타났으며 전체 응급의료센터의 입원율 35.2%에 비해서도 높았다(p<0.01). 사망률은 심인성군에서 13.8%로 전체 흉통환자의 8.8%보다 더 높았으며 비심인성군의 0.6%에 비해 20배 이상의 높은 수치를 보였다(p<0.01). 심인성군중에서도 특히 심근경색증이 원인인 경우에는 사망률이 28.1%에 달했다. 결론적으로 비외상성 흉통을 호소하는 환자의 경우 심인성 질환에 의한 경우가 가장 많고 이중 허혈성 심질환이나 심근경색증이 그 원인인 경우가 많은 것으로 나타났고, 임상양상을 비교해 볼 때 심인성질환 특히 심근경색증의 경우 연관 증상이 더 많은 것으로 나타났으며 쥐어짜는 듯한 양상이 가장 많았고 사망률 또한 가장 높았다. 그러나 정확한 진단이 되지 못한 상태에서 퇴원되는 경우도 있어 초기에 자세한 병력 청취를 통해 연관증상이 많은 경우 심질환을 의심하고, 심전도 검사를 시행하여 이를 통한 환자의 분류를 통해 조기에 심인성 질환을 감별하고 처치할 수 있는 방법이 모색되어야 할 것이며 특히 흉통진료실과 같은 특수한 제도를 도입함으로써 심근경색증 환자에 대한 집중적인 관리와 아울러 저위험군의 진료에도 좀 더 개선된 결과를 가져올 수 있도록 하는 것이 필요할 것으로 사료된다.

간호개념에 대한 기초조사 (The Empirical Exploration of the Conception on Nursing)

  • The study is aimed at exploring concept held by clinical nurses of nursing. The data were collected from 225 nurses conviniently selected from the population of nurses working in Kang Won province. Findings include. 1) Nurse's Qualification. The respondents view that specialized knowledge is more important qualification of the nurse. Than warm personality. Specifically, 92.9% of the respondents indicated specialized knowledge as the most important qualification while only 43.1% indicated warm personality. 2) On Nursing Profession. The respondents view that nursing profession as health service oriented rather than independent profession specifically. This suggests that nursing profession is not consistentic present health care delivery system nor support nurses working independently. 3) On Clients of Nursing Care The respondents include patients, family and the community residents in the category of nursing care. Specifically, 92.0% of the respondents view that patient is the client, while only 67.1% of nursing student and 74.7% of herself. This indicates the lack of the nurse's recognition toward their clients. 4) On the Priority of Nursing care. Most of the respondents view the clients physical psychological respects as important component of nursing care but not the spiritual ones. Specially, 96.0% of the respondents indicated the physical respects, 93% psychological ones, while 64.1% indicated the spiritual ones. This means the lack of comprehensive conception on nursing aimension. 5) On Nursing Care. 91.6% of the respondents indicated that nursing care is the activity decreasing pain or helping to recover illness, while only 66.2% indicated earring out the physicians medical orders. 6) On Purpose of Nursing Care. 89.8% of the respondents indicated preventing illness and than 76.6% of them decreasing 1;ai of clients. On the other hand, maintaining health has the lowest selection at the degree of 13.8%. This means the lack of nurses' recognition for maintaining health as the most important point. 7) On Knowledge Needed in Nursing Care. Most of the respondents view that the knowledge faced with the spot of nursing care is needed. Specially, 81.3% of the respondents indicated simple curing method and 75.1%, 73.3%, 71.6% each indicated child nursing, maternal nursing and controlling for the communicable disease. On the other hand, knowledge w hick has been neglected in the specialized courses of nursing education, that is, thinking line among com-w unity members, overcoming style against between stress and personal relation in each home, and administration, management have a low selection at the depree of 48.9%,41.875 and 41.3%. 8) On Nursing Idea. The highest degree of selection is that they know themselves rightly, (The mean score measuring distribution was 4.205/5) In the lowest degree,3.016/5 is that devotion is the essential element of nursing, 2.860/5 the religious problems that human beings can not settle, such as a fatal ones, 2,810/5 the nursing profession is worth trying in one's life. This means that the peculiarly essential ideas on the professional sense of value. 9) On Nursing Services. The mean score measuring distribution for the nursing services showed that the inserting of machine air way is 2.132/5, the technique and knowledge for surviving heart-lung resuscitating is 2.892/s, and the preventing air pollution 3.021/5. Specially, 41.1% of the respondents indicated the lack of the replied ratio. 10) On Nurses' Qualifications. The respondents were selected five items as the most important qualifications. Specially, 17.4% of the respondents indicated specialized knowledge, 15.3% the nurses' health, 10.6% satisfaction for nursing profession, 9.8% the experience need, 9.2% comprehension and cooperation, while warm personality as nursing qualifications have a tendency of being lighted. 11) On the Priority of Nursing Care The respondents were selected three items as the most important component. Most of the respondents view the client's physical, spiritual: economic points as important components of nursing care. They showed each 36.8%, 27.6%, 13.8% while educational ones showed 1.8%. 12) On Purpose of Nursing Care. The respondents were selected four items as the most important purpose. Specially,29.3% of the respondents indicated curing illness for clients, 21.3% preventing illness for client 17.4% decreasing pain, 15.3% surviving. 13) On the Analysis of Important Nursing Care Ranging from 5 point to 25 point, the nurses' qualification are concentrated at the degree of 95.1%. Ranging from 3 point to 25, the priorities of nursing care are concentrated at the degree of 96.4%. Ranging from 4 point to 16, the purpose of nursing care is concentrated at the degree of 84.0%. 14) The Analysis, of General Characteristics and Facts of Nursing Concept. The correlation between the educational high level and nursing care showed significance. (P < 0.0262). The correction between the educational low level and purpose of nursing care showed significance. (P < 0.002) The correlation between nurses' working yeras and the degree of importance for the purpose of nursing care showed significance (P < 0.0155) Specially, the most affirmative answers were showed from two years to four ones. 15) On Nunes' qualification and its Degree of Importance The correlation between nurses' qualification and its degree of importance showed significance. (r = 0.2172, p< 0.001) 0.005) B. General characteristics of the subjects The mean age of the subject was 39 ; with 38.6% with in the age range of 20-29 ; 52.6% were male; 57.9% were Schizophrenia; 35.1% were graduated from high school or high school dropouts; 56.l% were not have any religion; 52.6% were unmarried; 47.4% were first admission; 91.2% were involuntary admission patients. C. Measurement of anxiety variables. 1. Measurement tools of affective anxiety in this study demonstrated high reliability (.854). 2. Measurement tools of somatic anxiety in this study demonstrated high reliability (.920). D. Relationship between the anxiety variables and the general characteristics. 1. Relationship between affective anxiety and general characteristics. 1) The level of female patients were higher than that of the male patient (t = 5.41, p < 0.05). 2) Frequencies of admission were related to affective anxiety, so in the first admission the anxiety level was the highest. (F = 5.50, p < 0.005). 2, Relationship between somatic anxiety and general characteristics. 1) The age range of 30-39 was found to have the highest level of the somatic anxiety. (F = 3.95, p < 0.005). 2) Frequencies of admission were related to the somatic anxiety, so .in first admission the anxiety level was the highest. (F = 9.12, p < 0.005) 0. Analysis of significant anxiety symptoms for nursing intervention. 1. Seven items such as dizziness, mental integration, sweating, restlessness, anxiousness, urinary frequency and insomnia, init. accounted for 96% of the variation within the first 24 hours after admission. 2. Seven items such as fear, paresthesias, restlessness, sweating insomnia, init., tremors and body aches and pains accounted for 84% of the variation on the 10th day after admission.

Awareness of Breast Cancer Warning Signs and Screening Methods among Female Residents of Pokhara Valley, Nepal

  • Background: Breast cancer is the second most common cancer in the world and by far the most frequent cancer among women. Objective: The present study was undertaken to assess the awareness of breast cancer warning signs and screening methods among the women of Pokhara valley, Nepal. Materials and Methods: A cross-sectional questionnaire survey was carried out in a community setting with the female population. The questionnaire was administered in face-to-face interviews by trained research assistants. Results: Nepalese women demonstrated poor awareness of warning signs like a breast lump, lump under the armpit, bleeding or discharge from the nipple, pulling of the nipple, changes in the position of the nipple, nipple rash, redness of the breast skin, changes in the size of the breast or nipple, changes in the shape of the breast or nipple, pain in the breast or armpit, and dimpling of the breast skin. While 100% of nurses were aware about breast self-examination(BSE), mammography and warning signs of breast cancer. Levels of knowledge were significantly poorer in women with other occupations. Graduates were more aware about BSE, mammogram and warning signs of breast cancer compared to those with other educational levels. Conclusions: The findings indicated that the level of awareness of breast cancer, including knowledge of warning signs and BSE, is sub-optimal among Nepalese women.

농촌주부(農村主婦)들의 의료(醫療)와 항생제(抗生劑)에 대(對)한 지식(知識)과 태도(態度)에 관(關)한 조사(調査) (A Study on Knowledge and Attitude of Housewives toward Health Care and Antibiotics in a Rural Area)

  • A study was conducted during the period of August 13 to August 18, 1974 to obtain information on knowledge and attitude of the rural area housewife toward health care and antibiotics using. Interviewed 242 housewives dwelling in Soodong and Hwado Myun, Yangju Gun, Kyunggi Do, a typical rural area in Korea and the following results are obtained: 1. Of 242 housewives interviewed, 20.2% were illiteracy, 68.2% was graduated from primary school, 9.1% from middle school and 2.5% from high school. 2. Of those interviewed, 8.7% were Christian, 5.0% Bueldist, 2.9% Confucianism, and 83.4% of those were no religious preference. 3. Utility rate according with the kind of mass media in home was 85.1% of respondants possessed radio, 16.1% of magazine, 12.8% of newspaper, and 4.1% of television. 4. In the case of patients occure in a family, 13.0% out of 242 respondants had chosen physician's clinics for inicial medical care place, 58.4% drug stores, 0.9% herb medicine and 27.7% of those had chosen folk medicine at home. 5. Antibiotics effective complaints listed by the respondants were skin diseases with 43.8%, suppurated wound 30.0%, URI like symptoms 18.2%, diarrhea 14.5%, low back pain 12.9%, fever 6.2%, loss of appetite 3.3%, all kind of diseases 2.5%, urethral discharge 2.1% and tuberculosis 0.8% respectively. 6. Only 14.7% of respondants had obtained antibiotics for medical care from physician's clinics and 85.3% of the respondants had obtained antibioties from drug store (70.7%), village shop (10.4%), and salesmen in street market without any physician's prescription. 7. Eighty-nine percent of the respondants were understanding on patient care activity as the local health subcenter but only 11.0% of those on M.C.H., 29.0% of those on family planning, 21% on vaccination, and only 6.6% on tuberculosis control activity. 8. Utility rate of the local health subcenter was 71.9% out of the patients indicated medical care of medical facilities.

Breast Cancer Screening Barriers from the Womans Perspective: a Meta-synthesis

  • Background: The principal aim of health service providers in the field of breast cancer is to detect and treat lesions at an appropriate time. Therefore, identification of barriers to screening can be very helpful. The present study aimed to systematically review the qualitative studies for extracting and reporting the barriers of screening for breast cancer from the womans perspective. Materials and Methods: In this systematic review; Pubmed, Google Scholar, Ovid Scopus, Cochrane Library, Iranmedex, and SID were searched using the keywords: screening barriers, cancer, qualitative studies, breast and their Persian equivalents, and the needed data were extracted and analyzed using an extraction table. To assess the quality of the studies, the Critical Appraisal Skills Programme (CASP) tool was used. Results: From 2,134 related articles that were found, 21 articles were eventually included in the study. The most important barriers from the point of view of 1,084 women were lack of knowledge, access barriers (financial, geographical, cultural), fear (of results and pain), performance of service providers, women's beliefs, procrastination of screening, embarrassment, long wait for getting an appointment, language problems, and previous negative experiences. Articles' assessment score was 68.9. Conclusions: Increasing women's knowledge, reducing the costs of screening services, cultural promotion for screening, presenting less painful methods, changing beliefs of health service providers, provision of privacy for giving service, decreasing the waiting time, and providing high quality services in a respectful manner can be effective ways to increase breast cancer screening.

Use of Ergonomic Measures Related to Musculoskeletal Complaints among Construction Workers: A 2-year Follow-up Study

  • Background: The physical work demands of construction work can be reduced using ergonomic measures. The aim of this study was to evaluate the use of ergonomic measures related to musculoskeletal disorders (MSDs) among construction workers. Methods: A questionnaire was sent at baseline and 2 years later to 1,130 construction workers. We established (1) the proportion of workers reporting an increase in their use of ergonomic measures, (2) the proportion of workers reporting a decrease in MSDs, (3) the relative risk for an increase in the use of ergonomic measures and a decrease in MSDs, and (4) workers' knowledge and opinions about the use of ergonomic measures. Results: At follow-up, response rate was 63% (713/1,130). The proportion of workers using ergonomic measures for vertical transport increased (34%, 144/419, p < 0.01); for measures regarding horizontal transport and the positioning of materials, no change was reported. The proportion of workers reporting shoulder complaints decreased (28%, 176/638, p = 0.02). A relationship between the use of ergonomic measures and MSDs was not found; 83% (581/704) of the workers indicated having sufficient knowledge about ergonomic measures. Lightening the physical load was reported to be the main reason for using them. Conclusion: Only the use of ergonomic measures for vertical transport increased over a 2-year period. No relationship between the use of ergonomic measures and MSDs was found. Strategies aimed at improving the availability of ergonomic equipment complemented with individualized advice and training in using them might be the required next steps to increase the use of ergonomic measures.

간호전문대학생의 재학중 간호전문직관의 변화에 관한 연구 (A study on the change of the views on nursing profession of the nursing college students)

  • The aims of this study are to identify the perceptions and learning needs related to nursing profession of nursing students and to provide the basic data for development of curriculum. The questionnaire used for this study includes 103 items categorized as the fellowing 8 areas : the perception of nursing profession, competency as a nurse, the perception about clients, priorities in nursing activity, the knowledge required for, an ideology about nursing. Data was collected from the same students through their first grade in 1993 to third grade in 1995. The results are as follows : 1) In terms of the views on nursing profession, most of the students responded it as helping treatment of disease. Higher the grade, more significant with regard to health counseling, utilization of nursing and skills, nursing for community people. 2) Asked about competency as a nurse, sufficient professional knowledge on the nursing was responded as the highest. 3) Asked about the clients of nursing, 'patients' was the highest and as the grade was high, the items on 'family', 'community people', 'community' were high. 4) With regard to the points to be considered in the performance of nursing, while 'physical respect', 'psychological respect' were commonly responded, 'spiritual respect' highly responded in the second and third grades. 5) Asked about what nursing activity is, 'to relieve the pain' was high in the first and second grades, 'the promotion of health' was high in the third grade. 6) With regard to the ideology on nursing, the humanity and belief and faith were commonly responded. These results suggest, even though the expanded role of nursing was recognized, there is the tendency of illness oriented, to be need of reinforcement on the curriculum management in nursing education.

A comparative review of epinephrine and phenylephrine as vasoconstrictors in dental anesthesia: exploring the factors behind epinephrine's prevalence in the US

  • This review paper delves into the comparative study of epinephrine and phenylephrine as vasoconstrictors in dental anesthesia, exploring their histories, pharmacological properties, and clinical applications. The study involved a comprehensive literature search, focusing on articles that directly compared the two agents in terms of efficacy, safety, and prevalence in dental anesthesia. Epinephrine, with its broad receptor profile, has been a predominant choice, slightly outperforming in the context of prolonging dental anesthesia and providing superior hemostasis, which is crucial for various dental procedures. However, the stimulation of beta-adrenergic receptors caused by epinephrine poses risks, especially to patients with cardiovascular conditions. Phenylephrine, a selective alpha-1 adrenergic agonist, emerges as a safer alternative for such patients, avoiding the cardiovascular risks associated with epinephrine. Moreover, its vasoconstrictive effect may not be as deleterious as that of epinephrine, due to its selective action. This review reveals that despite the potential benefits of phenylephrine, epinephrine continues to dominate in clinical settings, due to its historical familiarity, availability, and cost-effectiveness. The lack of commercially available pre-made phenylephrine dental carpules in most countries, except Brazil, and a knowledge gap within dental academia regarding phenylephrine, contribute to its limited use. This review concludes that while both agents are effective, the choice between them should be based on individual patient conditions, availability, and the practitioner's knowledge and familiarity with the agents. The underuse of other vasoconstrictors like levonordefrin and the unavailability of phenylephrine in pre-mixed dental cartridges in many countries highlights the need for further exploration and research in this field.
