The Effect of Hand Moxibustion Therapy on Knee Joint Pain, Joint Range of Motion and Discomfort during ADL in Elderly People (수지뜸요법이 노인의 슬관절통증, 관절가동범위 및 일상생활활동 불편감에 미치는 영향)

  • Purpose: This study was done for the purpose of testing the effects of hand moxibustion on pain in the knee joint, range of motion of the knee, and discomfort during ADL in elderly persons with knee joint pain. Method: Nonequivalent control group pre-post test research design was used. The participants were 35 elders who had knee joint pain. Sixteen were assigned to the experimental group and 19 to the control group. The instruments used for this study were the CRS (Graphic rating scale) for knee joint pain, goniometer for knee joint ROM, and modified ADL questionnaire developed by Lee. Analysis of data was done by percents, means and standard deviation, $x^2$-test, t-test, and ANCOVA using SPSS WIN 10.0. Result: The pain score for the right knee joint after hand moxibustion was significantly different between the experimental group and the control group after hand moxibustion (p=.035). The pain score for the left knee joint was not significantly different between the experimental group and the control group after hand moxibustion (p=.075). Right and left knee ROM scores were significantly different between the experimental group and the control group after hand moxibustion (Right p=.000, Left p=.034). Discomfort of ADL score was not significantly different between the experimental group and the control group after hand moxibustion (p=.053). Conclusion: In summary, knee joint pain in elders after hand moxibustion decreased and knee ROM in elders after hand moxibustion increased. So it would be useful for nurses to provide hand moxibustion as an alternative therapy to elders with knee joint pain in the community and thus reduce joint pain and increase knee ROM.

  • It is general knowledge that knee joint pain can be attributed to trauma and degenerative change around the knee joint. However most patients who have suffered from pain or limited range of motion of the knee joint show no definite pathology on X-ray or laboratory examination. We examined 242 patients with knee joint pain and found compression or entrapment of the articular nerve fiber by the tissue around the knee joint resulted in pain in almost all cases. Conclusion: by relieving the compression of the articular nerve fiber with just physical therapy and LASER stimulation on the identified trigger points, in conjunction with NSAIDs, muscle relaxants, were found to be very effective in the treatment of knee joint pain.

Effects of a Taping Method on Pain and ROM of the Knee Joint in the Elderly (테이핑 요법이 노인의 무릎 통증과 관절가동범위에 미치는 영향)

  • Purpose: This study was to investigate the effects of a taping method on pain and ROM of the knee joint in the elderly. Method: The subjects were seniors registered in a senior welfare center in D city. An experimental group of 30 persons and acontrol group of 33 persons were chosen and according to the proper conditions and a knee pain score was marked from 1-5 on a Numerical Pain Rating Scale. For the experimental group, the taping method was conducted three times a week for four weeks (twelve times in all) but the control group did not receive taping.. The measuring instrument of knee pain was a numerical pain rating scale from 0 to 10 and the ROM score was the average value of three measured values with a goniometer. The data was analyzed with SPSS WIN 10.0 using an $x^2-test$, t-test, repeated measures ANOVA, and time contrast. Result: Knee joint pain was significantly decreased in the experimental group over that of the control group (p=.001). In addition, knee joint ROM of the experimental group was significantly improved over that of the control group (p=.001). Conclusion: It was proven that the taping method was effective for pain relief and increasing ROM of the knee joint in the elderly.

  • Purpose: The purpose of this study was to identify the effects of hand acupuncture therapy on pain, ROM, ADL, and depression among older people with low back pain and knee joint pain. Methods: The research was a quasi-experimental design using a non-equivalent control group pre-post test. The participants were 40 patients, 18 in the experimental group and 22 in the control group. A pretest and 2 posttest were conducted to measure the main variables. For the experimental group, hand acupuncture therapy, consisting of hand acupuncture and press-pellets based on corresponding points, was given. Results: There were statistically significant differences in pain, ROM in knee joint, and ADL in the experimental group but not in depression compared to the control group over two different times. Conclusion: The hand acupuncture therapy was effective for low back pain, knee joint pain, ROM in knee joint and ADL among the elders in this study. Therefore, the hand acupuncture therapy can be utilized in the field of geriatric nursing as a nursing intervention for older people with low back pain and knee joint pain.

Effect of Joint Mobilization and Kinesio Taping on Pain, Range of Motion, and Knee Function in Patients with Knee Osteoarthritis

  • Purpose: This study was conducted to investigate the effects of joint mobilization and kinesio taping on pain, range of motion and knee function in patients with knee osteoarthritis. Methods: Thirty subjects were selected and divided into three groups: group 1 was treated with joint mobilization, group 2 was treated with kinesio taping and group 3 was treated with joint mobilization and kinesio taping. Joint mobilization was performed for 20 minutes three times a week for a period of 4 weeks, after which tape was applied for the same period of time and it was not exceeded 24 hours. Pain, range of motion and knee function were then assessed to identify the effectiveness. A visual analog scale (VAS) was used for pain assessment, while active and passive range of motion (AROM, PROM) were assessed using smart phones application, and knee injury and osteoarthritis outcome score (KOOS) was used to assess knee function. Results: After intervention, the joint mobilization group and kinesio taping group showed significant improvements in VAS, AROM, PROM and KOOS (Symptom, Pain, ADL, QOL), whereas no significant difference was found in sport/recreation. The joint mobilization with kinesio taping group showed significant improvements in all items, and a significant increase was found in AROM and PROM compared to the other two groups. Conclusion: We found that joint mobilization and kinesio taping effectively improved pain, range of motion and knee function in patients with knee osteoarthritis, but that application of joint mobilization with kinesio taping was most effective.

  • Fat pad syndrome is a knee joint condition/disease where acute or chronic inflammation occurs in the fat pad of the knee joint, and it is a cause of anterior knee pain. Fat pad syndrome usually occurs concurrently with other conditions/diseases to collectively cause anterior knee pain. No study to date has reported the treatment of anterior knee pain solely due to fat pad syndrome. Here, we report a case of fat pad syndrome of the knee joint as the sole cause of anterior knee pain in a 49-year-old woman who received integrated Korean medicine treatments (pharmacopuncture, acupuncture, herbal medicine, deep fascial meridian therapy, and chuna). Using patient-reported pain scale scores, the level of the patient's pain was relieved, and her mobility improved. Integrated Korean medicine treatments could be effective for patients who have fat pad syndrome of the knee joint.

The Level of Pain, Gait Speed, and ROM in Community-dwelling Older Adults with Knee Joint Pain (슬관절통이 있는 재가노인의 통증과 보행속도 및 관절가동범위)

  • Purpose: The purpose of this study is to investigate the level of pain, gait speed, and ROM (Range of motion) in community-dwelling older adults with knee joint pain. Methods: A convenience sample of 160 participants living in G city were recruited. Elderly people who suffer from knee joint pain with a pain level ${\geq}5$ during the last one year on a visual analogue scale were included in the study. The data were collected from June to August 2016. All statistical analyses were performed using IBM SPSS ver. 22.0. Results: Participants' pain was 6.81 by VAS. The mean gait speed was 13.9 sec/10m. The ROM of left knee and right knee were $54.06^{\circ}$ and $56.50^{\circ}$, respectively. The level of pain was different according to gender (t=-4.87, p<.001), economic status (t=3.51, p=.032), diseases (t=5.23, p<.001), diagnosis of arthritis (t=3.86, p<.001), region of knee joint pain (F=9.30, p<.001), duration of pain occurrence (F=13.82, p<.001), and limp (F=3.58, p<.001). The level of knee joint pain and gait speed had significant correlations. Conclusion: The gait speed in older adults is an important variable that should be taken into account in the assessment of their level of knee joint pain.

  • Objectives : This study was done for reporting effect of moxibustion therapy on the degenerative arthritis of knee joint with osteochondroma. Methods : Two patient with degenerative arthritis of knee joint with osteochondroma was treated by moxibustion therapy. We applied moxibustion therapy three times a day for 12 days(three days per week). To investigate effectiveness of treatment we used visual analogue scale, Korean Western Ontario and McMaster Universities. Results : 1. The moxibustion therapy deceased knee joint pain. 2. As a result of evaluation by visual analogue scale, Korean Western Ontario and McMaster Universities the score marked lower than before treatment and after treatment. Conclusion : Moxibustion therapy decreased knee joint pain that patient have degenerative arthritis of knee joint with osteochondroma.

A Case Study Of Patients With Knee Joint Pain Treated With Rubiae Radix Pharmacopuncture (천초약침 치료를 병행한 슬관절 통증 환자에 대한 증례 보고)

  • Objective : This study was to investigate the clinical effect of Rubiae Radix pharmacopuncture on knee joint pain. Methods : The patients were treated by Rubiae Radix pharmacopuncture, Acupuncture, herbal medicine. To evaluate the effect of the treatment, The patient's symptoms were assessed by Visual Analogue Scale(VAS) Score, Knee injury and osteoarthritis outcome score(KOOS) and physical examinations. Results : In all cases, the pain was reduced significantly according to improvement of VAS score, KOOS and active Range Of Motion(ROM) of knee joint. VAS score and KOOS of all cases were decreased. In two cases, ROM of knee joint has improved. But in one case, ROM of knee joint has still been restricted. But there are no noticeable changes in physical examinations. Conclusion : These results suggest that Rubiae Radix pharmacopuncture might be an effective method to treat patients with knee joint pain. But, It's necessary to have more observations and experiments with Rubiae Radix pharmacopuncture.

  • Purpose: The purpose of this research was to test the effects of ST35 & Sulan moxibustion on knee joint pain, range of motion, and discomfort during Activies of Daily Living (ADL) in elderly with knee joint pain. Method: A nonequivalent control group pre-post test research design was used. Thirty four elderly who had knee joint pain were studied. Of them, sixteen were in the experimental group and eighteen the control group. The instruments used for this study were NRS, goniometer, and a modified ADL questionnaire developed by Lee. Result: The pain scores of right & left knee joint after moxibustion were significantly different between the experimental group and the control group (Right: F=26.27, p=0.000, Left: F=20.77, p=0.000). Right and left knee ROM scores were significantly different between the experimental group and the control group after moxibustion (Right: F=10.74, p=0.003, Left: F=9.239, p=0.005). Discomfort during ADL scores were significantly different between the experimental group and the control group after moxibustion (F= 32.31, p=0.000). Conclusion: It is necessary that nurses provide the elderly with knee joint pain with moxibustion to reduce joint pain and to increase knee ROM as an alternative therapy.
